Primary Traumatic Anterior Shoulder Dislocation in the Middle-aged And Elderly Patients (중장년층에서의 외상성 견관절 탈구)

  • Purpose: The purpose of this study was to review primary traumatic anterior shoulder dislocation in the 40 years and older to determine the incidence of recurrence, rotator cuff injury and associated fractures. Materials and Methods: From May, 1991 to October, 1998, seventeen patients were available to participate in the study. The average age was 55 years(range, 40 to 75 years). Follow-up averaged 27 months(range, 12 to 48 months). There were 12 men and 5 women. 10 right and 7 left shoulders were involved. Any participant with a history of shoulder pain and limitation of motion was excluded from the study. Each shoulder was evaluated with UCLA shoulder functional assessment. Results: The overall results were excellent in 9 patients(52.9%), good in 3 patients(17 .6%), fair in 3 patients(17.6%), and poor in 2 patients(11.7%). Surgical treatment of the associated rotator cuff and greater tuberosity fracture injuries resulted in 5 patients with excellent(55.6%), 3 patients with good(33.3%) and I patient with poor(11.1 %) and Nonsurgical treatment of the associated injuries resulted in 2 patients with excellent(33.3%), 3 patients with fair(50%), 1 patient with poor(16.7%)(P=0.025). Conclusion : Our findings on small number of patients suggest that the diagnosis of a rotator cuff and other pathologies should be approached aggressively by an MRI, CT arthrogram if significant pain and weakness are still present and early surgical repair of rotator cuff and greater tuberosity can lead to restoration of shoulder function and better outcomes in selected patients.

Functional Immunity to Cross-Reactive Serotype 6A Induced by Serotype 6B in Pneumococcal Polysaccharide Vaccine (폐구균 다당질 백신 내 혈청형 6B에 의해 유도되는 교차 반응 혈청형 6A에 대한 기능적 면역)

  • Purpose : Streptococcus pneumoniae serotype 6B and 6A are important pathogens in pneumococcal infections. It is commonly assumed that the 6B vaccines elicit antibodies cross-reacting with the 6A serotype and the cross-reactive antibodies protect against infections of 6A. To examine this assumption, we measured the opsonophagocytic capacity to serotype 6A and 6B in adults. Methods : Twenty-four adults were immunized with pneumococcal PS vaccine that contains 6B PS. Their preimmune and postimmune sera were studied for the capacity to opsonize 6B and 6A serotypes with opsonophagocytic killing assay. Results : Opsonization titers to 6B were significantly higher than those to 6A in preimmune and postimmune sera. Because significant increasesof opsonization titers were observed in adults with polysaccharide vaccines for 6A(cross-reactive) serotype as well as for 6B(vaccine) serotype, 6B PS in vaccine elicited cross-protective antibodies to 6A, but not in all cases. One adult did not have detectable levels of opsonization titers to 6A after immunization. Conclusion : Although 6B PS in pneumococcal PS vaccine elicits antibodies cross-reacting with 6A serotype in some adults, it may not occur always. This study should be extended to other age groups such as children and elderly people. The presence of the cross-protection should be directly determined in clinical trials of the pneumococcal vaccines as well as during the postlicensure monitoring surveys by serotyping the clinical isolates of pneumococci.

A Study on the Aging Knowledge, Sexual Perception and Sexual Attitude about the Aged People in a Local Adults (일 지역 성인의 노화지식과 노인에 대한 성인식 및 성태도에 관한 연구)

  • This study was conducted to: (a) explore correlations between aging knowledge, sexual perception and sexual attitude of aged people among adults, and (b) provide a database to develop an educational program to instill the right perception and sexual attitude of aged people in our society. The subjects were 340 adults aged 20-59 years living in K do, B and U city. The data were collected from June 9, 2015 to August 23, 2015 and analyzed based on the means, percentages, t-tests, ANOVA and Pearson's correlation coefficient using IBM SPSS 21.0. The subjects' score of aging knowledge was 12.12, sexual perception of aged people was 2.99, sexual attitude of aged people was 4.04. There were significant correlations between aging knowledge and sexual perception of aged people (r=0.227, p<0.001), aging knowledge and sexual attitude of aged people (r=0.369, p<0.001), sexual perception of aged people and sexual attitude of aged people (r=0.299, p<0.001). Therefore, it is necessary to provide systematic education about aging knowledge and sex of aged people by to change the perception and attitude of the elderly in our society.

Effects of Occupational Therapeutic Craft Activity on improvement cognitive function and hand function in acute patients with stroke (수공예 작업치료 프로그램 활동이 급성기 뇌졸중 환자의 손 기능에 미치는 효과)

  • This study investigatedwhether occupational therapeutic craft activity improves cognitive function and hand function in patients with acute stroke, with the hope of providing clinically useful results for further studies. From March 2016 to November 2016, thirty acute patients with stroke at A general hospital and B rehabilitation hospital in Seoul and Gyeonggi-do, Korea were enrolled and randomly divided into two groups. One group was treated with occupational therapeutic craft activity,and the other general exercise. The subjects performed each activity for 30 minutestwice weekly, for four weeks. To test the effect of intervention, hand function was tested with the Box & Block Test and the Jebsen-Taylor Hand Function Test.Cognitive function measures were the NCSE and Trail-Making Test. Over the four weeks of treatment activities, in the occupational therapeutic craft activity group, hand function measures(p<.05) increased significantly,as did cognitive function measures(p<.05). This study shows that occupational therapeutic craft activity improves hand function and cognitive function effectively in patients with acute stroke. Occupational therapeutic craft activity programscan be based on this study to prepare intervention programs for further studies with acute stroke patients.

An Analysis of Age Estimation Cases in Korea from the View of Social Aspects (사회적 측면에서 본 한국 연령 감정 대상자 사례의 분석)

  • Age estimations by forensic odontology play a great role in personal identification. The purpose of this study was to analyze the characteristics of age estimation cases in Korea. Surveying clients who requested age estimation at the Department of Oral Medicine, Kyungpook National University Dental Hospital from March 2012 to March 2013. The results were as follows: 1. On gender distribution, females(57.14%) were more than males(42.86%). The elderly with 50's and over 60's were majorities(89.28%) of clients, and no clients were below 40's. Most of clients were equal to and lower than elementary school graduate(69.64%). 2. The most frequent reason for age discrepancy between registered age and alleged age was mistakes by family or relatives(80.36%). The purposes of age estimation were welfare(62.50%), social relationship problem(12.50%), to find right age(10.71%), and occupation(8.93%). 3. In order of route to visit at Department of Oral Medicine, they were via government office(48.21%), acquaintances(21.43%), mass media(14.29%), and clinic(10.71%). Clients had high degree of comprehension on age estimation with forensic odontology (scored 7.03 out of 10). The 2/3 of clients were satisfied with present fee for age estimation. 4. The percentage on the proximity of estimated age to alleged age was 69.81%. 4(11.43%) clients were approved on age correction by court.

Effects of the Long-Term Caregivers' Job Stress on Psychological Burnout and Organizational Effectiveness and the Moderating Role of Social Support (직무스트레스가 조직유효성에 미치는 영향 : 서울소재 재가노인방문요양시설의 요양보호사를 중심으로)

  • The purpose of this study was to investigate the effects of the elderly long-term care workers' job stress on psychological burnout and organizational effectiveness and the moderating role of social support. For this study, data were collected from 290 long-term caregivers from 50 in-home visitation care facilities in Seoul using structured questionnaires. Collected data were analyzed using hierarchical regression techniques. Results showed that role three job stress variables had positive effects on caregivers' psychological burnout; that two job stress variables(role ambiguity and role conflict) and psycholigcal burnout had negaive effects on caregivers' job satisfaction whereas they had positive effects on caregivers' intent to leave; that workload negatively impacted caregivers' job satisfaction but no effect on their intent to leave; and that two social support variables played moderating roles in the relationship between job stress and psychological burnout and the relationship between job stress and organizational effectiveness.

  • Recently due to the enhancement of education and lifestyle, the trend of healthcare services are changed to a more active and differentiated service in which a continuous self health care is possible. The Smart-Telemedicine system offers medical services by merging Blue-tooth and telecommunication modules to former blood pressure, blood sugar, heartbeat and temperature measuring devices. Moreover, it could analyze one's health pattern which would be helpful for the patient to prevent potential future illness. In addition, the easier accesses to various remote controllable medical check-up programs are offered to public as a number of available smart phone are rapidly escalating. The Smart-Telemedicine system provides the most ideal interactive medical service via accessible smart phones and mobile medical check-up devices at anywhere and anytime. It is very beneficial since it can save patients' time and money because people can reach to the service right at their home and be allowed to take charge of their health care process via longitudinal health data. Therefore, not only social costs that occur in elderly community would be saved, but also business in various forms of medical service field transactions could be possible. This paper will suggest the Smart-Telemedicine System for preventive medicine, its design and analysis of business models and the evaluation of those model.

Effect of Horseback Riding on Bone Density of the Senior Citizen (승마가 노인의 골밀도에 미치는 영향)

  • This study is intended to identify how horseback riding affect the bone density of the elderly. A total of 31 normal senior citizens participated in the research, who were randomly divided into two groups: 15 in horseback riding exercise group and 16 in horseback riding machine exercise group. A horseback riding exercise program was carried out for 25 minutes twice a week for 12 weeks. A pre-test was conducted before starting the program and two post-tests were conducted 6 weeks and 12 weeks after exercises in the same way as the pre-test. Measured items were bone density of lumbar vertebrae and femur. In order to identify the changes in measurement variables before exercises and 6 weeks and 12 weeks after exercises, two-way repeated analysis of variance were used. The comparison of changes in the bone density in both groups revealed that there were significant differences in lumbar vertebrae L3, L4, femur trochanter in both groups over time. This study suggests that three-dimensional movements of horses through horseback riding exercises induce normal movements of senior citizens, providing positive effects on their bone density. If appropriate horse riding exercise programs are used considering the health status, time and economic conditions, and preferences of senior citizens, it would improve their quality of life.

Antioxidative Capacity and Quality Characteristics of Yanggaeng using Fermented Red Ginseng for the Elderly (발효홍삼 농축액을 이용한 노인용 기능성 간식(양갱)의 항산화성 및 품질특성)

  • In this study red ginseng was extracted with ethanol and then fermented by yeasts including Lactobacillus casei and Bifidobacterium longum. Fermented red ginseng extracts(FRGE) were found to be more effective antioxidants in vitro with regards to 1,1-diphenyl-2-picrylhydrazyl(DPPH) radical scavenging activity than red ginseng extracts(RGE). In FRGE, the contents of ginsenosides $Rb_1$, $-Rb_2$ and -Rc were much lower than in RGE, however, the contents of ginsenosides 20(S)$-Rg_3$, 20(R)$-Rg_3$ and compound K were higher than RGE. FRGE was added to Yanggaeng(0, 5, 10, 15, 20%), and physicochemical and sensory evaluations of the Yanggaeng were conducted. The L and b values of Yanggaeng with added FRGE were decreased by increasing the ratio of FRGE, while the a value was increased. Sensory evaluations for, taste, color, flavor, texture and overall acceptability of Yanggaeng with addition of FRGE (10%) were applicable for improving the Yanggaeng product.

Characteristics of Pain Threshold and Pain Experience in Elderly Patients with Dementia (노인 치매 환자의 통증 역치 및 통증 경험의 특성)

  • Objectives: We compared the characteristics of the pain threshold and pain experience between demented group and non-demented group. Methods: This study was part of Gangwon projects for early detection of dementia in 2010. We recruited 8302 local resident ages over 65 years old. Of theses, 1259 people who scored low MMSE were selected and 365 of them completed CERAD-K(Consortium to Establish a Registry for Alzheimer's disease). Finally, 90 in non-demented group and 57 in demented group(mild to moderate Alzheimer's disease) were analyzed. Pain threshold was experimentally measured by pressure algometer and we investigated the pain experience, by Brief pain inventory (BPI), a self-report test. Results: In the demographic characteristics, there are more female, higher ages, lower education in the demented group. There was no significant difference between the two groups in the pain threshold. On the BPI results, 'shoulder pain', 'the number of pain' and 'interference of working' were significantly more prevalent in non-demented group. However, there are no significant differences between the groups in the 'pain severity', 'prevalence of pain' and 'pain treatment'. Conclusions: Demented group report less pain experience but, still perceived pain. It support previous studies that patient with dementia have increased pain tolerance but preserved pain threshold. Thus, active pain assessment and treatment for patients with dementia is needed.
