A Rare Cause of Acute Scrotal Pain: Spontaneous Thrombosis of Testicular Vein (급성 음낭 통증의 드문 원인: 고환정맥의 자발적 혈전증)

  • Acute scrotal pain is a common clinical symptom of urinary system and there are various causes. Among them, thrombosis of testicular vein is rare, but one of the causes of acute scrotal pain. Thrombosis is a common disease that can occur anywhere in our body. But thrombosis of testicular vein has not been reported in Korea. We report a case of thrombosis of testicular vein in a 26-years-old man with acute scrotal pain.

  • Objectives: This study aimed to determine whether Korean medicine could improve unilateral testicular pain caused by recurrent varicocele without unfavorable side effects. Methods: A 46-year-old man diagnosed with varicocele, identified with a scrotal ultrasound scan, had right-side testicular pain. We administered Banchong-san and Eunhoebanchong-san to observe changes in the degree of pain. Results: After one month of taking Banchong-san and Eunhoebanchong-san, testicular pain improved. Conclusion: This study suggests that Korean medicine might be effective for managing testicular pain caused by varicocele.

  • Unstable pelvis ring trauma can accompany urologic injuries such as bladder, ureter, or urethral injuries. In very rare cases, testicular dislocation can accompany a pelvic ring injury. Testicular dislocation causes pain, but can be regarded as the pain caused by pelvic ring injury. If not detected early, testicular dislocation may result in gangrenous testes. The authors report a case of pelvic ring injury with testicular dislocation.

  • Testicular lymphoma is an uncommon testicular tumor that usually presents as a painless mass. It usually shows hypervascularity on color Doppler ultrasound (US) and a mild enhancement on enhanced CT or MRI. We present an unusual case of a testicular lymphoma mimicking a missed testicular torsion in a 67-year-old male patient with right scrotal swelling and intermittent pain for 2 months. Color Doppler US demonstrated the absence of vascularity in the enlarged right testis, and the initial diagnosis was a missed testicular torsion. CT demonstrated a poorly enhancing mass rather than a missed testicular torsion with enhanced small nodular foci at the periphery. The final pathological diagnosis was testicular lymphoma.

  • Tuberculous epididymo-orchitis, a rare form of extrapulmonary tuberculosis, results from hematogenous dissemination or retrograde extension from the lower urinary tract. Herein, we studied the case of a 22-year-old male patient who presented with refractory left scrotal pain and inflammation. The patient also complained of multifocal pain involving the right buttock, posterior thigh, and right wrist, without trauma history. The patient was diagnosed with multifocal tuberculosis by sputum AFB study and right sacroiliac joint biopsy.

  • Scrotal involvement has been reported from 2% to 38% of males with Henoch-Sch$\ddot{o}$nlein purpura (HSP). It may present before the rash occurs or even long time after it has disappeared. Scrotal involvement of HSP usually results in pain, tenderness, swelling or discoloration of scrotum. Scrotal pain sometimes mimics testicular torsion to various degrees, which can be followed by unnecessary operation. In our 2 cases, one was a 5-year and 11-month-old boy who came to our emergency department due to scrotal pain before the diagnosis of HSP, and the other was a 5-year and 1-month-old boy who came to the outpatient clinic due to scrotal pain after the resolution of HSP about 1 month before the visit. We performed Doppler ultrasonography (USG) to evaluate the acute scrotal pain in the two boys. On Doppler USG, epididymis showed increased blood flow, and testis showed normal blood flow. These findings enabled the diagnosis of acute epididymitis and excluded testicular torsion. Epididymitis was improved by conservative management including shortterm steroid therapy within 5 days. It is important to perform adequate evaluation with tools such as Doppler USG in the early course of acute scrotum of HSP, to avoid unnecessary scrotal exploration.

  • The management of varicoceles was performed on 24 patients with testicular dragging pain and/or left lower abdominal pain under local anesthesia. The surgery was completed within 20 to 45 minites, postoperative complications were severe cord edema 3 cases, wound infection 1 case, epididymitis 1 case, and postoperative anesthetic duration was 90 to 150 minites. Therefore, these procedures is cost effective and safe surgical method.

  • The prostatitis is characterized as relatively incurable, chronic and recurrent illness. Because of this property, we have a difficulty in treating this illness. The purpose of this study is evaluating the efficiency of herb medicine, acupuncture, and herbal acupuncture therapy to the prostatitis. A clinical observation was made on two patients with chronic prostatitis who had perineal pain, pubic pain, testicular pain, painful urination, frequency and dysuria. They were treated with herb medicine, acupuncture, and herbal acupuncture therapy for four weeks and symptoms were considerably decreased. This result suggests that oriental medical therapy is useful enough to treat the prostatic diseases. Therefore the further study of the approach and application of this treatment on prostatitis would be necessary.

  • The increased application of the da Vinci robotic platform (Intuitive Surgical Inc.) for microsurgery has led to the development of new adjunctive surgical instrumentation. In microsurgery, the robotic platform can provide high definition $12{\times}-15{\times}$ digital magnification, broader range of motion, fine instrument handling with decreased tremor, reduced surgeon fatigue, and improved surgical productivity. This paper presents novel adjunctive tools that provide enhanced optical magnification, micro-Doppler sensing of vessels down to a 1-mm size, vein mapping capabilities, hydro-dissection, micro-ablation technology (with minimal thermal spread-$CO_2$ laser technology), and confocal microscopy to provide imaging at a cellular level. Microsurgical outcomes from the use of these tools in the management of patients with infertility and chronic groin and testicular pain are reviewed. All these instruments have been adapted for the robotic console and enhance the robot-assisted microsurgery experience. As the popularity of robot-assisted microsurgery grows, so will its breadth of instrumentation.

  • In 5-year male m811ese, was showed aggressive bebavior, $1.5{\times}1.0 cm$ mobile mass that was located in right iinguinal area was unilaterally palpated, and the dog was appeared no pain. In the ultrasonography, there was a 41.26{\times}119 cm$ oval hypo-echogenic mass. In the histopathohgic examination, testicular hypoplasia was observed but there was not any evidence of development of neoplasia. In bilateral cryptorchidectomy, descended testis near the left side of prepuce was incised and the retained testis directly incised and eliminated by closed method. After operation, there was a good prognosis without hemorrage and inflammation.
