Relations between Testicular Feminization and Seminiferous Tubule (Testicular Feminization과 세정관과의 관계)

  • Teter and Boczkowski (1966) classified testicular feminization syndrome into two groups; One with no pubic hair and normal clitoris (A group) and the other with pubic hair and enlarged clitoris (B group). However, the patient discussed in this paper shows a mid-form of these two groups (C group). To find out differences among A, B and C groups, we have observed the micro-structure of testes removed from her inguinal canal and analyzed the chromosomes obtained from testicular and blood culture. Her seminiferous tubules show some differences in the maturation and distribution of Leydig and Sertoli cells and in the thickening of basement membrane as compared with group A and B. We suppose that such differences are responsible for the morphological changes of clitoris and pubic hair.

  • Unstable pelvis ring trauma can accompany urologic injuries such as bladder, ureter, or urethral injuries. In very rare cases, testicular dislocation can accompany a pelvic ring injury. Testicular dislocation causes pain, but can be regarded as the pain caused by pelvic ring injury. If not detected early, testicular dislocation may result in gangrenous testes. The authors report a case of pelvic ring injury with testicular dislocation.

  • Sonography with a high-frequency transducer is the modality of choice for imaging the scrotum. Most intratesticular lesions are hypoechoic. Differentiation of intratesticular hypoechoic lesions as either malignant or benign is important because the treatment of these lesions vary. In this paper, we review the sonographic features of different types of intratesticular hypoechoic lesions, such as testicular cysts, testicular tumors, testicular inflammatory lesions, segmental testicular infarction, and testicular trauma.

Comparison of ICSI Outcomes between Fresh and Cryopreserved-Thawed Testicular Spermatozoa (신선 고환조직 정자와 냉동보존-융해 고환조직 정자를 이용한 난자세포질내 정자주입술 결과의 비교 연구)

  • Objective: To compare the outcomes of intracytoplasmic sperm injection (ICSI) with fresh and cryopreserved-thawed testicular spermatozoa in patients with azoospermia. Methods: One hundred and nine cycles (66 couples) where ICSI was planned with fresh or cryopreserved-thawed testicular spermatozoa were included in this study; Ninety two cycles (61 couples) with fresh testicular spermatozoa (fresh group) and seventeen cycles (13 couples) with cryopreserved-thawed testicular spermatozoa (cryopreserved-thawed group). We compared ICSI outcomes such as fertilization rate, implantation rate, pregnancy rate and miscarriage rate, which were statistically analyzed using Mann-Whitney U test or Fisher's exact test, where appropriate. Results: In 9 out of the 92 cycles where ICSI was planned with fresh testicular spermatozoa, testicular spermatozoa could not be retrieved. Fertilization rate tended to be higher in the fresh group than in the cryopreserved-thawed group ($58.0{\pm}27.8%$ vs. $45.9{\pm}25.0%$, p=0.076). The number of high quality embryos was significantly higher in the fresh group ($0.9{\pm}1.2$ vs. $0.2{\pm}0.5$, p=0.002). However, there were no significant differences in clinical pregnancy rate, implantation rate and miscarriage rate between the two groups. Conclusion: The results of this study suggest that although the use of cryopreserved-thawed testicular sperm for ICSI in patients with azoospermia may reduce fertilization capacity and embryo quality, it may not affect pregnancy rate, implantation rate and miscarriage rate. If testicular sperm can be obtained before ICSI procedure, the use of cryopreserved-thawed testicular sperm may also avoid unnecessary controlled ovarian hyperstimulation and cancellation of oocyte retrieval when spermatozoa cannot be retrieved as well as damage on testicular function by repeated TESE.

  • After infection of male mice with the plerocercoids (spargana) of Spirometra mansoni, serum levels of estrogen and testicular weight were analyzed by enzyme-linked immunosorbent assay (ELISA) and weighing machine, respectively. The serum level of estrogen increased progressively in infected mice compared with normal controls, whereas the testicular weight of infected mice decreased significantly (P < 0.05). These results suggest that certain substances from spargana change the steroid hormone metabolisms in the host by unknown pathways, and chronic infection may contribute to change of the function of steroid hormone target organ, i.e., testis, in male mice.

  • This is a report of successful management for pulmonary metastasis following chemotherapy in patient with testicular germ cell tumor. Postchemotherapy PET-CT was not uptake FDG in metastatic lesion. Pulmonary metastasectomy was performed, which is important to manage a residual postchemotherapy lung mass in testicular germ cell tumor for histological correlation with primary testicular lesion to select the patients who require subsequent chemotherapy. Our patient was well 6 months after operation, not carried out chemotherapy because of no viable tumor.

The Effect of Xylene and Formaldehyde Inhalation on Testicular Tissue in Rats

  • In this study, changes in testicular tissues of rats subjected to xylene and formaldehyde inhalation were evaluated. Three experimental groups were included in the study. Each group of rats was exposed to formaldehyde (6 ppm), technical xylene (300 ppm) or a combination of these two agents (150 ppm+3 ppm) for 8 weeks (8 h/d). Control groups were maintained for a period of eight weeks under the same conditions. Staining methods (triple staining, strep ABC method) were applied to examine histometric changes and relaxin like factor (RLF) expression in the testicular tissue. Immunostaining for RLF showed that density of staining for RLF decreased in rats exposed to formaldehyde. Formaldehyde or a combination of formaldehyde and xylene led to a decrease in seminiferous epithelial height. In conclusion, exposure of rats to formaldehyde and xylene-formaldehyde combinations adversely affects Leydig cells (RLF) and seminiferous epithelium of testicular tissue.

  • Testicular adrenal rest tumors are a well-known complication in male patients with congenital adrenal hyperplasia. Corticosteroid suppressive therapy usually results in the regression of these tumors. We describe a patient with 21-hydroxylase deficiency who developed bilateral testicular masses. Despite steroid suppressive therapy, the tumors did not regress and hormonal control was poor. Consequently, bilateral partial orchiectomies were performed.

  • The effect of ccocaine on reproductive system were studied in male ICR mice. daily injection of cocaine HCL (10, and 40 mg/kg s.c for 15 days) disrupted epididymal sperm levels and testicular morphology. These disruptions were manifested in a decreased number of sperum, an increased number of dead sperm as compared with control vehicle, and wre evidenced by vacuolations, multinucleated giant cell formations and coagulation necrosis in the testicular seminiferous tubules. Responses in the current study were trended in dose dependent pattem. These results suggest that thronic cocaine intoxication can perturb on male reproductive organ functioning.

  • Acute scrotal pain is a common clinical symptom of urinary system and there are various causes. Among them, thrombosis of testicular vein is rare, but one of the causes of acute scrotal pain. Thrombosis is a common disease that can occur anywhere in our body. But thrombosis of testicular vein has not been reported in Korea. We report a case of thrombosis of testicular vein in a 26-years-old man with acute scrotal pain.
