  • This study was intended to review comprehensively the articles related to temporomandibular joint disorder patients and to analyze psychoanalytically the case with temporomandibular dysfunction and bruxism as a symptom of hysterical conversion disorder. As a result, the author found that , in the aspect of psychoanalysis, the temporomandibular problem and bruxism was a kind of symptom of emotional conflict being converted to the face. I would like to emphasize that the referral process and the ability to work as a therapeutic team including dentist and psychiatrist can enhance the probability of successful treatment for several types of patients.

The Effects of Spiral Taping Treatment on Temporomandibular Disorder (악관절장애(顎關節障碍)의 개구장애(開口障碍)와 통증(痛症)에 대한 첩대요법(貼帶療法)의 효과(效果))

  • Objective : The purpose of this study is to estimate the effects of spiral taping treatment on temporomandibular disorder. Methods : 22 temporomandibular disorder patients were treated with spiral taping, and no other treatments such as herbal medicine, acupuncture, herbal acupuncture, and chiropractic therapy were added. We evaluated the improvement by temporomandibular pain and mouth opening range. Results : 11 patients felt no pain or inconvenience of daily life and 4 patients showed improvement of pain or symptom after 1 month of spiral taping theraphy. 3 patients had normal opening size of mouth and convenience of daily life, and 2 patient showed improvement of opening size of mouth as 35mm-40mm after 1 month of spiral taping theraphy. Conclusion : These results suggest spiral taping treatments contribute to the improvement of temporomandibular disorders. Further study is needed for the confirmation of this effect of spiral taping treatments on temporomandibular disorders.

Two Clinical Case Studies on Temporomandibular Disorder with Upper Cervical Manipulation (상부경추 교정기법을 사용하여 치료한 턱관절 장애 치험 2례)

  • Objective : We had good effects on two patients who had problem with temporomandibular joint by using upper cervical manipulation combined with acupuncture therapy. Methods : Two temporomandibular disorder cases were managed by using upper cervical manipulation, combined with acupuncture therapy. We used Toggle-recoil technique for the purpose of upper cervical manipulation. Results :Even though symptoms of two patients were not same, each cases showed clinical changes by using upper cervical manipulation. Two cases had effects on decreasing pain of temporomandibular joint, and at the first case, the range of motion of temporomandibular joint was increased. Conclusion : We could treat temporomandibular disorder patients by using upper cervical manipulation combined with acupuncture therapy.

Nasopharyngeal Cancer with Temporomandibular Disorder and Neurologic Symptom: A Case Report

  • Nasopharyngeal cancer is malignant tumor of nasopharyngeal area that is characterized of lymphadenopathy, pain, otitis media, hearing loss and cranial nerve palsy and may present symptoms similar to temporomandibular disorder such as facial pain and trismus. In this case, the patient with symptoms similar to temporomandibular disorder after surgery for otitis media presented with facial paresthesia and masticatory muscle weakness. Examinaion of trigemimal nerve was shown sensory and motor abnormaility. The patient was referred to a neurologist. Nasopharyngeal cancer was suspected on computed tomography and magnetic resonance imaging and was confirmed by biopsy. If the patient presenting with paresthesia and muscle weakness the cranial nerve examination should be performed regardless of typical temporomandibular disorder symptom. The neurologic symptom can be caused by neoplasm such as brain tumor and nasopharyngeal cancer. Nasopharyngeal cancer on rosenmuller fossa can develop otitis media. Therefore, the patient with otitis media history should be consulted to otorhinolaryngologist to examin the nasopharyngeal area.

  • Skull base osteomyelitis is a very rare disease that affects the bone marrow of the temporal bone, sphenoid bone, occipital bone. It occurs typically chronic ear canal infections by malignant otitis externa, but some of atypical osteomyelitis have been reported. It most commonly presents old diabetic patients, and have high morbidity and mortality rate if diagnosis and treatment are delayed. However with respect to pain or dysfunction, it appeared similar to the initial symptoms of temporomandibular joint disorder. So frequently, definitive diagnosis is tend to delayed. We have clinical experience that a patient who presented with symptom similar to temporomandibular disorder, and differential diagnosised by skull base osteomyelitis. We will report this case with literature review.

Relationship between Stress, Temporomandibular Joint Disorders and Oral Health-Related Quality of Life in College Students (대학생의 스트레스와 턱관절 장애 및 구강건강관련 삶의 질의 관련성)

  • This study was conducted to investigate the stress and temporomandibular joint disorders that can affect the oral health level of college students and to identify the relationship between oral health-related quality of life. Data survey was conducted from June 3, 2021 to July 8, 2021, targeting 227 college students in some areas of Daejeon, and was analyzed using independent sample t-test, one-way ANOVA, Pearson correlation analysis, and multiple regression analysis. As a result, temporomandibular joint disorder showed a positive correlation with all stress sub-factors, and oral health-related quality of life showed a negative correlation with all stress sub-factors and temporomandibular joint disorder. Also, it was found that temporomandibular joint disorder, interpersonal stress, and values stress had an effect on oral health-related quality of life. Based on the above results, it is required to find a systematic way to improve the quality of life related to oral health by preventing stress and temporomandibular joint disorder among college students.

A Case Study of the Tic Disorder Patient with Nausea and Vomiting Treated by Korean Medical Treatment with Temporomandibular Joint Balancing Therapy (오심 구토를 동반한 틱 장애 환자에 대해 턱관절균형요법을 병행한 한방 치험례)

  • Objectives: The purpose of this study is to report the effect of Korean medical treatment with Temporomandibular Joint Balancing Therapy (TBT) on Tic disorder patient with nausea and vomiting. Methods: In this study, Tic disorder patient with nausea and vomiting was treated by Korean medical treatment (acupuncture, herb medicine, etc.), including Temporomandibular Joint Therapy from Feb 28th, 2023 to Jul 10th, 2023. Yale Global Tic Severity Scale (YGTSS) and visual analogue scale (VAS) were used for measuring the Tic disorder and accompanying symptoms. Results: After 32 sessions treatment, the YGTSS decreased from 67 to 0, and VAS associated with Tic disorder, nausea and vomiting also decreased 6 to 0 and 8 to 0 respectively. Conclusions: These results showed that Korean medical treatment with Temporomandibular Joint Balancing Therapy could improve Tic symptoms with nausea and vomiting. But further studies will be needed.

  • Temporomandibular joint disorder is quite common dental problem. Research has shown that 5 to 15$\%$ of the population have a TMJ disorder requiring some type of treatment. TMJ disorder's symptoms are pains in the jaw and muscles of your face, limitation of opening or closing jaw and noises in jaw joint such as clicking or popping sounds. Two patients with TMJ symptoms visited our clinic. They complained pain, limitation of jaw opening and clicking sound. We treated them by stimulating trigger points on the TMJ muscle with acupuncture and had a good effects. The symptoms is reduced or disappeared.

Analysis on the Effect of Oral habits and Stress on Temporomandibular disorder in High school students (고등학생의 구강 악습관과 스트레스가 턱관절 장애에 미치는 영향 분석)

  • This study used a structured questionnaire to analyze the effects of oral habits and stress on temporomandibular disorders in high school students. A total of 339 students attending high schools in Ulsan agreed to participate in the study. Data was analyzed using the SPSS Statistics ver. 18.0 for Windows program, t-test, one-way ANOVA, correlation, and regression analysis. Our results reveal that the temporomandibular disorder index is statistically correlated with the oral habit index, school life stress index, family life stress index, interpersonal stress index, and egosphere stress index (p≤0.01). The oral habit index had maximum influence on the temporomandibular disorder index. Moreover, higher the stress level in egosphere stress, greater significant differences were observed in the temporomandibular disorder index (p≤0.01). In other words, persistence of oral habits and non-addressal of stress increased the likelihood of experiencing temporomandibular disorders in adulthood. This was determined by the regression formula Y (temporomandibular disorder index) = 2.563 + 0.550X1 (oral habit index) + 0.134X2 (egosphere stress index). Thus, bad oral habits and stress in adolescence can be the potential cause of jaw joint disorders in adulthood. We believe that measures are needed to actively recognize and improve oral habits, and properly cope with stress during adolescence.

A Study on the Symptom of Temporomandibular disorder(TMD) and the Relationship with the Psychological Character using Symptom Check List-90-Revision(SCL-90-R) (간이정신진단검사(SCL-90-R)를 이용한 측두하악장애 증상과 심리특성과의 관계 연구)

  • This study examined the symptoms of temporomandibular disorder and the relationship with the psychological character using Symptom Check List-90-Revision (SCL-90-R). The self-administered questionnaire survey was implemented from March 1, 2014 to June 30, 2014 targeting 294 University Students in Daejeon and Gangwon area aged 20-31 years old (Men 140 persons, Women 54 persons). The data showed that the teeth grinding and the unilateral chewing out of corrupt practices in the mouth in relation to the use of the lower jaw showed a significant correlation (p<0.01) with the depth of symptoms of temporomandibular disorder, as the result of the simple psychodiagnosis inspection (SCL-90-R), and a significant correlation (p<0.05) with the depth of symptoms of temporomandibular disorder in the item excluding phobic anxiety (PHOB). The somatization (SOM) criteria out of individual psychological characteristic appeared to have the significant plus (+) effect on the depth of symptoms of temporomandibular disorder as a result of implementing multiple regression analysis by controlling the individual characteristic variables to check the influence of the psychological character of the study object on the symptom of temporomandibular disorder.
