Long-term Results of Stellate Ganglion Block in Patients with Olfactory Dysfunction

  • Background: Olfactory dysfunction, including anosmia and hyposmia is difficult to treat. Although the mechanism is not well known, stellate ganglion block (SGB) is used to treat olfactory dysfunction. There are no prior studies on the long-term effects of SGB on olfactory dysfunction. The purpose of this study was to evaluate the continuity of therapeutic effects and patient satisfaction with SGB treatment. Methods: This was a follow-up study carried out via a telephonic survey. The olfactory function of the patient was evaluated using a visual analog scale (VAS). We checked VAS three times: VAS-I (pre-treatment VAS), VAS-A (post-treatment VAS), and VAS-C (VAS at follow up telephone survey). We divided the subjects into 2 groups according to their responsiveness to SGB: the responsive (R group) and the unresponsive groups (UR group). Patient satisfaction was evaluated using a Likert scale. Results: Out of the 40 subjects, 37 responded to the telephone survey. In the UR group, there was difference in the olfactory function. However, in the R group, there were significant VAS differences; VAS-I was $9.6{\pm}0.7$, VAS-A was $5.1{\pm}4.2$, and VAS-C was $2.7{\pm}$2.7 (P < 0.05). On the Likert scale, patient satisfaction was as follows: grade 1, 17 patients (45.9%); grade 2, 6 patients (16.2%); grade 3, 6 patients (16.2%); and grade 4, 8 patients (21.6%). Conclusions: SGB is a safe, long-lasting, and effective therapeutic modality for olfactory dysfunction treatment.

Development of an Efficacy Expectation Promoting Program for the Management of Menopause and its Effect (폐경관리를 위한 효능기대증진 프로그램 개발과 그 효과에 관한 연구)

  • The two objectives of this study were 1) to develop an efficacy expectation promoting program for the management of menopause based on the self-efficacy theory of Bandura(1977), and 2) to investigate the effects of the efficacy expectation promoting program for the management of menopause. The first phase was to development an efficacy expectation promoting program through video tape. A telephone coaching program served as a follow-up to the efficacy expectation promoting program which provided encouragement and support to the subjects. The second phase was the experimental stage. This experimentation was conducted to determine whether an efficacy expectation promoting program based on theory of Bandura would increase the self-efficacy and the management of menopause. Results : The video which we developed consisted of 2 parts. Part one discussed the symptoms and the management of menopause. Part two reflected the vicarious step-by-step success of middle-aged women's menopause management. The telephone coaching program served as a follow up to the efficacy expectation promoting program and provided encouragement and support to the subjects. The total length of the video is 50 minutes. The hypotheses stated in phase two were supported by following results : In the two experimental groups consisting of the natural and artificial menopause groups both demonstrated significant increase in their general and concrete self-efficacy scores. In the experimental groups, both the objective and subjective menopause management scores increased n the natural and artificial menopause experimental groups. In conclusion, it was found that an efficacy expectation promoting program was an effective nursing intervention for increasing self-efficacy and management of menopause.

Effects of Catgut-embedding Acupuncture Therapy on Plantar Fasciitis: Case Report (족저근막염으로 진단된 족저부 통증 환자에 대한 매선침법 증례 보고)

  • Objectives : The purpose of this study was to report the clinical effects of catgut-embedding acupuncture therapy on plantar fasciitis. Methods : Two patients were treated mainly with catgut-embedding acupuncture therapy and partly with other Korean medicine modalities. Catgut-embedding acupuncture therapy was applied once a week for three weeks, and follow-up observation was conducted by telephone survey two months after the first treatment. Results : In these two cases, the pain numerical rating scale(NRS) gradually decreased from 8 to 1, and the pain relief as well as the improvement in ambulatory function was maintained at two-month follow-up. Conclusions : Catgut-embedding acupuncture therapy could be effective for pain-reduction and functional improvement in plantar fasciitis.

Developing a Computerized Reminder System and Evaluating the Effects on the Improvemtent of Cancer Screening (지역사회 주민의 정기적 암검진 수검율 향상을 위한 정보시스템 구축 및 효과평가)

  • Purpose: This study was aimed to develop a computerized reminder system and evaluate it's effect in terms of percent age change of screening, and satisfaction. Method: It was conducted through 6 phases: Analyzing the job and defining the basic input data, developing the information system, collecting and inputting data, testing the system, working with the system, and evaluating it's effect. Participants were 787 people (female 30-69 years, and males 40-49 years) in 2 dong of Suyoung gu, Busan, who haven't had cancer screening for the stomach, breast, or cervix since Dec. 2000. There were three experimental groups: a letter; calling and calling after the letter reminder, and a non-equivalent control group. To determine whether services were obtained, a telephone survey was done after two months of follow-up. Result: A cancer screening information system with five DB modules was developed. Overall compliance with screening was not statistically significantly changedbefore and after applying computerized reminders for all three screening sites. Only 16% were satisfied with the reminder. Conclusion: This data didn't show that a reminder effort was effective of screening. However, because the evaluation interval was too short to find a difference in screening rate, we recommend additional longer prospective follow up studies.

Solitaire AB Stent-Assisted Coiling of Wide-Neck Micro Aneurysms

  • Objective : Solitaire AB stent-assisted coiling facilitates the endovascular treatment of wide-necked intracranial aneurysms. We present our experience of coiling the micro-aneurysms of wide-neck with Solitaire AB stent assisting in a single center. Methods : Thirty-one Solitaire AB stents were used to treat via endovascular approach patients with 31 wide-neck micro aneurysms in a single center in China. Technical and clinical complications were recorded. Modified Rankin Scale was used to evaluate the patients' conditions via clinic and telephone follow-up. Results : The mean width of aneurysm sac was $2.30{\pm}0.42mm$, and the mean diameter of aneurysm neck was $2.83{\pm}.48mm$. Complete occlusion was achieved in 28 aneurysms (90.32%); neck remnant was seen in 3 aneurysms (9.68%). Technical and clinical complications related to the procedure were encountered in four patients (12.5%). Two patients died (6.25%). No patient had a permanent deficit. Conclusion : Solitaire AB stent was a safe and efficiency tool in assisting coiling of micro aneurysms with wide neck, but may be not suitable for a blaster-like one. Mid- and long-term follow-up will be required to elucidate the impact of the Solitaire AB stent on recanalization rate.

Intradural Transpetrosectomy for Petrous Apex Meningiomas

  • Objective : This study aimed to assess the surgical results of the intradural transpetrosectomy for petrous apex meningiomas (PAMs). In addition, we describe the methods and techniques used to expose and manage superior petrous vein and greater superficial petrosal nerve. Methods : The authors conducted a retrospective study of 16 patients with PAMs operated by the senior author via the intradural transpetrosectomy between February 2012 to May 2017. We reviewed patient data regarding the general characteristics, surgical technique and surgery-related outcomes and adopted a combined follow-up strategy of clinic and telephone contacts to evaluate postoperative complications. Results : Simpson grade I and II resection was performed in 10 out of 16 cases (62.5%), and grade III resection were reported in the remaining six cases (37.5%) with no resultant mortality. The mean Karnofsky Performance Status score was 85.6 preoperatively and improved to 91.9 postoperatively, with a mean follow-up period of 34.4 months (range, 6-66 months). Tumor recurrence was found in two patients and they underwent the second surgical operation. Conclusion : PAMs could be completely resected by the intradural transpetrosectomy with an improved survival rate and postoperative life quality. Superior petrous vein and greater superficial petrosal nerve should be managed properly in avoidance of postoperative complications. Finally, most meningioma inside cavernous sinus or adhered to brainstem could be totally removed without postoperative complications.

PROGNOSIS OF TREATED INPATIENTS WITH CONDUCT DISORDERS USING A STRUCTURED TELEPHONE FOLLOW-UP INTERVIEW (구조화 전화추적 면담을 이용한 품행장애 입원 치료 환자의 예후)

  • Objectives:The major goal of this study was to investigate the treatment outcome of psychiatric treatment in inpatients with conduct disorder and to elucidate factors affecting its prognosis. Methods:We reviewed the medical records of 300 inpatients with conduct disorder who had been treated with a specialized adolescent treatment program. Follow-up structured telephone interview had been performed in 96 patients. Results:1) At the point of follow-up, 90% of the patients were improved in behavioral patterns, 2% of the patients were worse, and 8% of the patients were unchanged. 2) Intrafamilial relationship was improved in 70% of the patients, worse in 2%, and unchanged in 28%. 3) Fifty-seven percent of families thought to be helped by psychiatric inpatient treatment, 6% replied to be harmed, and 37% thought not to be helpful. 4) Comparing the good prognosis group who were all better in behavioral patterns, intrafamilial relationship, and efficacy of treatment with the rest of subjects, the good prognosis group was significantly younger and had more history of problems in familial structure. Conclusion:Although the present study had some meterological limits, the promising positive results in the outcome of inpatients with conduct disorder encourages further more sophisticated investigations in this problematic psychiatric conditions.

The investigation process of this study was done in two phases. The first phase was to development an educational program through video tape. A telephone coaching program served as a follow-up to the educational video program and provided encouragement and support to the subjects. The second phase was the experimental stage. This experimentation was conducted to determine whether an educational program based on theory of Bandura would increase self-efficacy and management of menopause. Results : The developed video consisted of 2 parts. Part one discussed symtoms and management of menopause. Telephone coaching program served as a followed to the educational video program and provided encouragement and support to the subjects. Total length of video is 32 minutes. Hypothesis stated in phase two were supported by the following result. Experimental group increased general-self-efficacy scores. Experimental group increased concrete-self-efficacy scores. Experimental group increased objective and subjective management of menopause scale.

Clinical characteristics and outcomes in patients with lesion-positive transient ischemic attack

  • Transient ischemic attack (TIA) indicates high risk for major stroke and is considered a medical emergency. Diffusion-weighted imaging (DWI) enables detection of acute ischemic lesions. The clinical significance of DWI positive lesions in TIA is obscure and its prevalence, clinical features are not established. Therefore, we performed a clinical, etiological and prognostic analysis through a cross-sectional analysis of 235 TIA patients, grouped according to presence of DWI lesion. Clinical features, underlying risk factors for stroke, outcome and rate of recurrence were analyzed. 3 months follow-up of modified Rankin Scales (mRS) were done with telephone survey. DWI positive lesions were present in 14.0% of patients. Etiological factors significantly associated with DWI lesions in TIA patients were male sex (p = 0.038), stroke history (p = 0.012) and atrial fibrillation (p < 0.001). Presence of at least one medium or high risk of cardioembolism from TOAST classification were not associated with lesions when excluding association to atrial fibrillation (p = 0.108). Clinical features showed no significant difference. Whether the patients had lesion-positive DWI was not related to an increase in mRS score during the hospital stay or at the 3-month follow-up after discharge. Future studies should include multi-center samples with large numbers, considering each unique medical environment. Routine acquisition of follow-up DWI for proper evaluation of the tissue-based definition of TIA should also be considered.

  • Purpose: Cancer patients experience a range of physical and psychological sequelae. Consistent nursing support should be provided along the cancer treatment path. This study aimed to i)examine the effects of a telephone counseling program after discharge on perceived health, psychological well-being, and satisfaction with nursing services, and ii)describe symptom distress and their coping methods. Method: The study was a quasi-experimental design with a non-equivalent pre-post test. The sample included 20 women with gynecologic cancer in the experimental group and the same in the control group from a university hospital in Seoul. The telephone intervention was given once from 5 to 7 days after the chemotherapy. The General Well-Being Schedule and Symptom Distress Scale were used. Result: An effect from telephone counseling was found only in the vitality subscale of psychological well-being. Other subscores, perceived health, or satisfaction with nursing services did not differ between the two groups. Pain, skin change, decreased appetite, and constipation were the major symptoms and a relatively few coping strategies were utilized. Conclusion: Protocol of telephone counseling led by a nurse needs to be further developed in regard to best timing, amount, and target effects for follow-up care of gynecologic cancer patients.
