Change of Cerebral Blood Flow Distribution and Vascular Reserve according to Age in Koreans Measured by Tc-99m HMPAO Brain SPECT (한국 정상인에서 연령에 따른 뇌혈류분포와 혈류예비능의 변화: Tc-99m HMPAO SPECT에 의한 연구)

  • Purpose: The aim of this study was to evaluate the normal values of regional cerebral blood flow (rCBF) and cerebrovascular reserve (CVR) in normal children to aged volunteers using Tc-99m HMPAO, Materials and Methods: Thirty four right-handed normal volunteers (20 males, 14 females, mean age $40.3{\pm}24.9$ years, range 4 to 82 years) were underwent rost/acetazolamide (ACZ) brain SPECT using Tc-99m HMPAO and the sequential injection and subtraction method. rCBF was estimated on the basis of a semiquantitative approach by means of right/left ratio, region/cerebellum and region to whole brain ratios in (rental, parietal, temporal, and occipital lobes, basal ganglia, thalami, and cerebellum. CVR was measured by means of % perfusion increase calculated as % mean count change compared to rest rCBF in each regions. Results: Mean values of right to left ratios range from 1.004 to 1.018, rCBF was highest in cerebellum and lowest in basal ganglia and thalami. Frontal and temporal rCBF decreased while occipital and thalamic rCBF increased according to age. No sexual difference of rCBF was noted. Mean CVR was $29.9{\pm}12.9%$. Mean CVR significantly increased to late teens, and declined thereafter. After 6th decade, CVR in both frontal lobes, left parietal lobe and right basal ganglia decreased significantly with advancing age. There was no sexual difference of CVR. Conclusion: Quantitative assessment of CVR was possible by ACZ Tc-99m HMPAO brain SPECT. It revealed that rCBF and CVR changed according to age in normal Korean volunteers. There was no sexual difference.

PSYCHOPATHOLOGY IN ADOLESCENTS WITH PARENT ABUSE (부모학대 청소년의 정신병리)

  • Aim:We think that the most important etiology in parent abuse is the psychodynamic and psychopathology in the family. So, we investigated the adolescents being admitted in SNMH, whose chief complaints were parent abuse. We were trying to explore families psychodynamic and psychopathology, especially mother-child interaction and to differentiate them in according to developmental psychopathology. Method:Our objects were the adolescent patients admitted in SNMH from 1987 to 1997 because they attacked parents verbally and physically. We examined 21 adolescents except those with psychosis, organic mental disorder, autism and mental retardation by means of interview or chart review. Result and Conclusion:The number of male patients was 14 and the number of female patients was 7. The most common diagnosis was conduct disorder and borderline personality disorder. The mean age was in the mid-teens. We observed 4 subgroups that were divided developmentally in object relation. 1) Symbiotic group with mother:(1) They did not separate and remain in symbiotic relationships with their mothers based on insecure attachment. Fathers were abscent emotionally and physically, and their mothers were prominent in close relationships with the patients in their family , where as the patients were the only man in the family. Adolescents entered the second separation-individuation. They expressed anger and internal tension involved with the close attachment with their mothers and also attempted separation from their mothers through physically attacking them. (2) These patients had suffered from physical illness and developmental delay since birth. Therefore the parents overprotected their children. The children had persistent infantile omnipotence and fantasies of power, so they could not deal with unrealistic states, adapt to reality, and depended on their parents overtly. They easily acted out unless their demands were fulfilled. 2) Borderline personality disorder:We observed deficiencies in care taking. Their parents had personality problems and immaturity. They coulden’t help their children to be separated in the rapproachment phase. Their conflict about dependence-independence was revived in the second separation-individuation adolescent period. We understand parent abuse as an attempt to overcome the conflict. 3) Conduct disorder:They did not build up basic attachment with their parents. They think of their parents as only a means of fulfilling their needs. When patients’ need were not fulfilled and remained in a conflicted state, they attacked their parents, unable to control their aggressions and impulses.

  • Dong-Eui-Bo-Gham Oriental medicine clinic on the purpose of losing their weight, from March 1, 2001 to March 1, 2002. The characteristics studied were gender, average weight, average height, distribution of body fat rate, base body metabolism rate, distribution of bmi, purpose of losing weight, treatment history, cause of gaining weight, clinical symptoms, distribution of Sahsang constitution, and change of body constituent. The conclusions drawn from this study were: 1. Among patients who came to the clinic for weight loss, there were 16 females (80%), and 4 males (20%). The ratio of gener was 1 to 4 (male to female) The distribution of their ages were 1 in one's teens (5%), 6 in their 20s (30%), 7 in their 30s (35%), and 6 in their 40s (30%). 2. The average weight of the patients was 72.155kg, and their average height was 163.2 cm. 3. The body fat rates of patients were distributed as: 2 were below 25 (10%), 3 were 25 to 30 (15%), 8 were 30 to 35 (40%), 5 were 35 to 40 (25%), and 2 were 40 to 45 (10%). 4. The base body metabolism rates of the patients were distributed as: 1 was below 1000Kcal (5%), 6 were 1100 to 1200Kcal (30%), 5 were 1200 to 1300Kcal (25%), 6 were 1300 to 1400Kcal (30%), and 2 were 1400 to 1500Kcal (10%). 5. The distribution of the patients bmi was: 4 were below 20 (20%), 5 were 20 to 25 (5%), 11 were 25 to 30 (55%), and 4 were 30 to 35 (20%). 6. The patients visited the clinic to lose their weight for the follwoing purposes: 5 were related to health concerns (25%), and 15 were to imprve their physical apprearnce (75%). Among patients who wanted to lose their weight becase of health concerns, 4 were married (20%) and 1 was single (5%). Among patiens who wanted to lose their weight to improve their physical appearance, 6 were married (30%) and 9 were single (45%). 7. As for the past treatment methods, physical exercises were most frequently used (19 patiens, 43%). 14 tried diet (32%), 8 used food substitues (19%), 1 was treated through Western medicine, and 1 was not related to any of these (2%). 8. Main reasons for weight gains were: 12 were related to overeating (40%), 10 were stress (33%), and 8 were lack of physical exercise (27%). 9. Physical symptoms included: 15 were fatigue (29%), 14 were constipation (26%), 13 were body swelling (25%), 3 were headache (5%), menstruation pain (4%), one was stomach upset (2%), 2 were related to physical structure, and 2 were related to other (4%). 10. As for the distribution of Sahsang constitution, 10 were Taeum (50%), 9 were Sohyang (45%), and 1 was Sohum (5%). 11. The average muscle weight was 44.87kg, average abdomen fat rate was 0.8999, and average base body metabolism rate was1369.2Kcal. 12. For the 5-week period, The body fat rate changes were 35.93 1.56% to 30.40 1.98% for A group, and 34.27 1.19% to 31.73 1.38% for B group. The abomen fat rate changes were 0.90 0.02% to 0.86 0.03 for A group, and 0.89 0.02% to 0.83 0.02% for B group. The body fat weight changes were 26.92 2.04kg to 20.74 1.98kg for A group, and 27.86 3.37kg to 24.58 3.02kg for B group. The body weight chages were 72.36 3.18kg to 64.54 2.55kg for A group, and 72.48 3.14kg to 67.74 3.11kg for B group. The muscle rate changes were 43.86 1.63kg to 43.10 1.76kg for A group, and 42.72 1.22kg to 41.60 1.86kg for B group. Overall, A group was superior to B group. 13. After being treated for obesity, 13 patients used exercise treatment (65%), 5 continued to receive obesity treatment (25%), and 2 used diet (10%). Based on these results, we could conclude that the importande of setting a criterion of obesity and weight control have changed according to changes in social and cultural values, and that treatment of obesity through Oriental medicine and research should keep up with changes in esthetic and psychological values.

THE COMPARATIVE ANALYSIS OF THE DENTITION AND MORPHOLOGIC MALOCCLUSION OF THE TEMPOROMANDIBULAR DYSFUNCTION PATIENT (측두하악장애를 가진 교정환자 교합의 형태학적 특성에 관한 연구)

  • In order In analyze the occlusion of TMD patient, 73 casts and post eroanterior cephalometric X-rays of patients showing TMD symptoms at first interview were measured and compared that of 30 non-patients . Horizental overjet, vertical overbite, openbite, crossbite, depth of curve of Spee, midline deviation, facial asymmetry, attrition, and TPI were measured and processed statistically The results could be summarized as follows, 1. TMD group showed the highest prevalence in twenties, teens, and before 9 years old group in order, and more prevalent in female than male. 2. There were no statistically significance between two groups of overjet and overbite measurements, but showed significance of 6 classification of anterior tooth relationship between two groups TMD groups. 3. n group had more anterior openbite than normal group but there were no statistically significance between two groups. 4. Anterior crossbite was more prevalent in normal group brit posterior crossbite was more prevalent in TMD group. 5. TMD group showed deeper curve of Spee and there were statistically significance between two groups. 6. TMD group had more attrition than normal group and there were statistically significance beween two groups. 7 TMD group showed more facial asymmetry than normal group. 8. TPI did not showed statistical significance beween two groups. According to the above results, TMD group showed severe and complex mode of malocclusion and this should be carefully regarded when treatment planning and during the treatment of malocclusion.

A Study on the State of Smoking and Smoking-Related Oral Health Knowledge Level among Some Adolescents (일부 청소년의 흡연 실태와 흡연 관련 구강건강지식수준에 관한 연구)

  • The purpose of this study was to examine the state of smoking among adolescents and their smoking-related oral health knowledge. The subjects in this study were the teens who were selected by convenience sampling from among the students who were in their first, second and third years in four different middle schools in the city of Iksan, North Jeolla Province. A self-administered survey was conducted from June 14 to July 12, 2011, and the answer sheets from 1,219 respondents were selected for analysis. The collected data were analyzed by the statistical package 12.0, and the findings of the study were as follows: 1. The smoker students accounted for 10.6 percent. The boys and girls who smoked respectively represented 16.0 and 5.4 percent. 2. As for the period of smoking, the largest group that accounted for 51.6 percent smoked for less than a year. Concerning the amount of daily smoking, the greatest group that accounted for 91.9 percent smoked less than 10 cigarettes. As to the motivation of smoking, the biggest group that represented 52.8 percent started to smoke out of curiosity. 3. Regarding smoking-related oral health knowledge, they got a mean of 0.85. Their scores in knowledge of oral malodor(0.95), oral cavity cancer(0.94) and periodontal diseases(0.93) were above the average, and their scores in knowledge of dental caries(0.70) and blunting of taste(0.77) were below the average. 4. The seventh graders got the best score of 5.28 in smoking-related oral health knowledge(p<0.01), and the students who were more satisfied with their life had a better oral health knowledge(p<0.05). 5. The senior students were more dependent on nicotine(p<0.01), and the number of smoking days(${\beta}=0.407$, p<0.001) and the period of smoking(${\beta}=0.235$, p<0.01) affected their nicotine dependence.

A STUDY OF THE MAXIMUM MOUTH OPENING IN CHILDREN (소아의 최대개구량에 관한 연구)

  • Recently, tempormandibular disorder(TMD) shows an tendency to increase every year. TMD is a collective term embracing a number of clinical problems that involve the masticatory musculature, temporomandibular joint(TMJ) and associated structures, or both. TMD, viewed in distribution of age, often occurred from late teens to late twenties. But recently, the age of occurrence tends to be lower. Accordingly, early diagnosis of tempormandibular disorder is very important. In this study, we measured the maximum mouth opening which is simple and easy to carry out as a way of TMD diagnosis. In this study, the maximum mouth opening was examined for 1,775 children from 4 to 12 years of age. We compared the relationship between the maximum mouth opening with the age, height, and weight. The results obtained were summarized as follows; 1. The mean maximum mouth openings of 4, 8, and 12 year of age were respectively 40.16mm, 47.32mm, 50.54mm for male, 39.79mm, 44.85mm, 48.09mm for female. 2. The maximum mouth opening increased with age, and the values were greater in male than in female. 3. The maximum mouth opening increased with height, and the values were eater in male than in female except between 105cm and 115cm 4. The maximum mouth opening increased with weight, and the values were eater in male than in female. 5. The correlation between the maximum mouth opening with the age, height, and weight was all significant, and height showed the highest correlation.

Prevalence of Noroviruses Detected from Outbreaks of Acute Gastroenteritis in Busan, Korea (부산지역 집단 식중독 발생에서 검출된 노로바이러스의 유행양상)

  • Norovirus is the most common causative agent of acute gastroenteritis. This study was carried out to investigate molecular epidemiology of norovirus infections from outbreaks in Busan from 2012 to 2015. Total of 581 stool specimens were collected from diarrhea patients in outbreaks in Busan, 71 samples were resulted in positive to norovirus. The data were analyzed according to seasonality, patient, age and gender. Noroviruses were detected most frequently during the winter season from November (25.4%) to February (28.2%). The age group from teens was the most susceptible to norovirus infections. To obtain the molecular genetic information of norovirus, we performed sequencing analyses of the strains detected. Norovirus genotypes have been reported to show high genetic diversity. Four kinds of GI genotypes (GI-1, GI-2, GI-3, GI-5) and five kinds of GII genotypes (GII-1, GII-4, GII-5, GII-6, GII-17) were indentified in outbreaks in Busan. Other previous studies have shown that GII-4 is the most predominant circulating in Korea and worldwide. The most prevalent norovirus genotypes of each year were GII-6 in 2012, GII-6 in 2013, GII-4 in 2014 and GII-6 in 2015. Except for 2014, GII-6 genotype was the most prevalent and predominant in Busan. We described the epidemiological analysis of the noroviruses in outbreaks in Busan. The result of this study will contribute to update the epidemiological data and improve hygiene and public health via sustainable surveillance.

The Present State of Food Serviee by the Covered Wagon Bars (포장마차 영업실태조사(營業實態調査))

  • In accordance with the rapidly growing number of street food service without a registration, a study was undertaken to determine the present state of food service by the covered wagon bar, through an investigation in Jamwondong, around the south gate market and Kangnam subway station, in Seoul, between July 25th and August 25th of 1987. The survey was comprised of three parts: 1) foodservice operation in covered wagon, 2) personal and food handling hygiene, 3) food behaviors of customers. A total of 54 covered wagon bars, consisting of 51.8% mobile bars and 48.2% non-mobile bars, operating in the above three locations, were investigated. Survey results show non-mobile covered wagon bars to be more popular among persons in their thrities and fourties than among teens or the elderly; also among males than females; among company employees and college students than others. Seventy five percent of the mobile covered wagon bars served snack type foods and others served wine and foods for wine, in contrast to hundred percent of the non-mobile covered wagon bars served wine and foods for wine. The survey found many problems of hygiene, in method of food purchasing, menu planning, food preparation, dish washing treatment of leftovers and water supply, as well as personal hygiene. However, customers prefer the casual and popular atmosphere at the counter of the covered wagon bar. Finally, the study emphasizes a need for better operation of covered wagon bar, improvement of food stuff handling and the way of food services and personal hygiene. A change of the registration system from the illegal operation are urgently needed for better quality food services of covered wagon bars.

The Physiologic change associated with aging, essential nutrients and their diseases in senior or geriatric dogs (노령견의 생리적 변화에 따른 필요 영양소 및 질병에 관한 연구)

  • This article discusses the nutritional requirements, reviews senior or geriatric dog nutritional evaluation, and then addresses some common nutrition-related problems in older dogs. The purpose of this study was to investigate the Physiologic change associated with aging, essential nutrients and their diseases in senior or geriatric dog subjects. According to a 2002 market research, 30% to 40% of dogs raisedin the United States are 7 years of age. In Europe the number of dogs considered to be "senior or geriatric" (>7 years of age) increased by approximately 50% between 1983 and 1995. A 2012 e-mail survey of 50,347 respondents revealed that 33.2% of dogs were 6 to 10 years of age and 14.7% were older than 11 years in the United States. The average life expectancy of dogs raised in the home is affected by health care, aging and nutrition.And, the aging process is influenced by breed size, genetics, nutrition, environment, and other factors. Although many pets remain active and youthful well into their teens, most dogs start to slow down and may show signs of aging beginning as early as 5 or 6 years of age. Improvements in the control of various diseases and in the nutrition of dogs have resulted in a gradual increase in the average lifespan of companion dogs. Nutritional goals for aging dogs include supporting health and vitality, preventing the onset or slowing the progression of age-related health disorders, and enhancing the dog's quality of life and, if possible, life expectancy. Aging brings with its physiologicchanges. Some changes are obvious, such as whitening of hair, a general decline in body and coat condition, and failing senses including sight and hearing. Other changes are less obvious, however, and these include alterations in the physiology of the digestive tract, immune system, kidneys, and other organs. Nutritional requirements can change with age. In addition, many diseases common in older dogs may be nutrient-sensitive, meaning that diet can play an important role in the management of the condition.

A Study on the 18th Joseon Dynasty Sculturers Choi Cheon-Yak (조선 18세기 조각가 최천약(崔天若) 연구)

  • Choi Cheon-yak(about 1684~1755) is an artist who left the various artistic works such as sculptures of royal tombs, architectures. In addition, he was not only a skilled caster but also an able artisan carving jade in the early Joseon Dynasty period. Starting from making royal seals of King Suk-jong, he had made more than about 40 Royal seals until 1755. Choi Cheon-yak was well known as a skilled jade craftsman. Another of his great abilities was to carve subjects into ideal figures. In virtue of his greater abilities, he could take part in the process of constructing Royal tombs and sculpting the stone statues of military officials' which were erected at aristocrats' tombs. With these accumulated skills, when he was in charge of designing the folding screen stones for King In-jo's Jang Neong, he could even replace 12 animals system and clouds with peonies and lotus. Among his various abilities, his skill in carving a sculpture can stand comparison with any other contemporaries. His sculpture skill was at its zenith in 1752, the stone statues of military officials' at the Ui soseson's tomb count his showpiece that describes a model at the age of his late teens and is a realistic and portrayal sculpture, which met the royal family's dignity. In the same year, the stone statues of military officials' constructed by Choi Cheon-yak was elected in front of the Jo Hyen-myeong's tomb(1690~1752). This masterpiece referred to the armor of those of King Gong-min Neong and newly added a helmet and the patterns of a tortoise shell. These patterns of a tortoise shell were passed down to Park Moon-su's tomb in 1756 and Queen Jeong-sung's Hong Neung by his colleagues : Kim Ha-jeong and Byeon Yi-jin etc. He was one of the greatest sculptors in the $18^{th}$ century. People in Joseon praised him highly for his imaginative work from an amorphous object. Especially, these stone statues of military at Jo Hyeonmyeong's tomb shows the proofs of his supreme artwork.
