  • It is necessary to control and evaluate human factors to reduce economic loss by major accident in toxic gas facilities. Conventional works to evaluate hazards have been focused on mechanical and systematic failure, while only a little works have been studied on managing human errors. In this work, a classification system of performance shaping factor (PSF) was suggested to consist human error in managing accident in the toxic gas facilities. Four types of PSFs (human, system, task characteristics, and task environment) were collected, reviewed, and analyzed to be categorized selected according their characteristics of situational, task, and environmental parameters. The PSFs were further modified to set up PSF systems adequate to evaluate human error, and the proposed system to consist PSFs to evaluate human error was further studied through accident analysis in toxic gas facilities.

  • The assessment of slope stability is an essential task in geotechnical engineering. In this paper, a three-dimensional (3D) finite element analysis (FEA) was employed to investigate the performance of different shear pin arrangements to increase the stability of a soil block resting on an inclined plane with a low-interface friction plane. In the numerical models, the soil block was modeled by volume elements with linear elastic perfectly plastic material in a drained condition, while the shear pins were modeled by volume elements with linear elastic material. Interface elements were used along the bedding plane (bedding interface element) and around the shear pins (shear pin interface element) to simulate the soil-structure interaction. Bedding interface elements were used to capture the shear sliding of the soil on the low-interface friction plane while shear pin interface elements were used to model the shear bonding of the soil around the pins. A failure analysis was performed by means of the gravity loading method. The results of the 3D FEA with the numerical models were compared to those with the physical models for all cases. The effects of the number of shear pins, the shear pin locations, the different shear pin arrangements, the thickness and the width of the soil block and the associated failure mechanisms were discussed.

  • The role of maintenance in railway is going to be extended to improve the reliability of railway system in the aspect of Asset management gradually. In this paper, the meaning of reliability and safety in RCM which has been applied in order to improve the efficiency of maintenance is deduced. And the analysis task of reliability and safely which has been recommended in railway standards such as EN50126 and IEC62278 is reviewed in the aspect of RCM. Finally, the several ways are proposed to apply RCM to railway system through the comparison between the RCM procedure and the analysis procedure for the reliability and safety in railway standards. Hereafter, if the analysis of reliability and safety is performed with the concept of RCM in the beginning of railway business, it will be more efficient to improve the reliability and manage the railway asset.

  • In the present study, we proposed a numerical method for simulating thermally induced fracture slip using a grain-based distinct element model (GBDEM). As a part of DECOVALEX-2023, the thermo-mechanical loading test on a saw-cut rock fracture conducted at the Korea Institute of Civil Engineering and Building Technology was simulated. In the numerical model, the rock sample including a saw-cut fracture was represented as a group of random Voronoi polyhedra. Then, the coupled thermo-mechanical behavior of grains and their interfaces was calculated using 3DEC. The key concerns focused on the temperature evolution, thermally induced principal stress increment, and fracture normal and shear displacements under thermo-mechanical loading. The comparisons between laboratory experimental results and the numerical results revealed that the numerical model reasonably captured the heat transfer and heat loss characteristics of the rock specimen, the horizontal stress increment due to constrained displacement, and the progressive shear failure of the fracture. However, the onset of the fracture slip and the magnitudes of stress increment and fracture displacement showed discrepancies between the numerical and experimental results. We expect the numerical model to be enhanced by continuing collaboration and interaction with other research teams of DECOVALEX-2023 Task G and validated in further study.

  • In the software development process, software reliability evaluation is a very important issue. In particular, finding the optimal development model that satisfies high reliability is the more important task for software developers. For this, in this study, Pareto and Erlang life distributions were applied to the finite failure NHPP model to evaluate the reliability attributes. For this purpose, parametric estimation is applied to the maximum likelihood estimation method, and nonlinear equations are calculated using the bisection method. As a result, the Erlang model showed better performance than the Pareto model in the evaluation of the strength function and the mean value function. Also, as a result of inputting future mission time and evaluating reliability, the Erlang model showed an effectively high trend together with the Pareto model, while the Goel-Okumoto basic model showed a decreasing trend. In conclusion, the Erlang model is the best model among the proposed models. Through this study, it is expected that software developers will be able to use it as a basic guideline for exploring and evaluating the optimal software reliability model.

  • This paper describes the performance of a full-authority neural network-based fault tolerant system within a flight control system. This fault tolerant flight control system integrates sensor and actuator failure detection, identification, and accommodation (SFDIA and AFDIA), The first task is achieved by incorporating a main neural network (MNN) and a set of n decentralized neural networks (DNNs) to create a system for achieving fault tolerant capabilities for a system with n sensors assumed to be without physical redundancy The second scheme implements the same main neural network integrated with three neural network controllers (NNCs). The function of NNCs is to regain equilibrium and to compensate for the pitching, rolling. and yawing moments induced by the failure. Particular emphasis is placed in this study toward achieving an efficient integration between SFDIA and AFDIA without degradation of performance in terms of false alarm rates and incorrect failure identification. The results of the simulation with different actuator and sensor failures are presented and discussed.

  • Many real world cases in material failure analysis do not follow perfectly the normal distribution. Forcing of the normality assumption may lead to inaccurate predictions and poor product quality. We examine the failure process of the internal bond (IB or tensile strength) of medium density fiberboard (MDF). We propose a forced censoring technique that closer fits the lower tails of strength distributions and better estimates extremely smaller percentiles, which may be valuable to continuous quality improvement initiatives. Further analyses are performed to build an accelerated common-shaped Weibull model for different product types using the $JMP^{(R)}$ Survival and Reliability platform. In this paper, a forced censoring technique is implemented for the first time as a software module, using $JMP^{(R)}$ Scripting Language (JSL) to expedite data processing, which is crucial for real-time manufacturing settings. Also, we use JSL to automate the task of fitting an accelerated Weibull model and testing model homogeneity in the shape parameter. Finally, a package script is written to readily provide field engineers customized reporting for model visualization, parameter estimation, and percentile forecasting. Our approach may be more accurate for product conformance evaluation, plus help reduce the cost of destructive testing and data management due to reduced frequency of testing. It may also be valuable for preventing field failure and improved product safety even when destructive testing is not reduced by yielding higher precision intervals at the same confidence level.

  • The seismic performance evaluation of the existing non-seismic detailed RC piers has risen as urgent task for rational and cost-effective seismic retrofitting works as well as development of new seismic design concept. The scale model test has been conducted to investigate nonlinear behavior characteristics and the seismic performance of existing piers with lap-spliced longitudinal reinforcements in the plastic hinge zone which are of the solid circular and the hollow rectangular section. The lap splice in this zone is found to cause premature bond failure. The experimental results show very poor seismic performance of circular section pier but relatively large ductility of the rectangular one.
