MRI and Arthroscopy of Osteochondral Lesion of the Talus which was not visible on Plasin Radiography (단순 방사선 상에서 발견할 수 없었던 거골 골연골 병변의 MRI 소견과 관절경 소견)

  • Purpose: To investigate the MRI and arthroscopic findings of osteochondral lesion of the talus which looked normal on plane radiography. Materials and methods: We investigated the MRI and arthroscopic findings of seven osteochondral lesions in which there were no abnormal finding on plane radiography and no cystic changes on MRI. Average age was 31 years(range, 19-43 years). Arthroscopic findings were classified according to the Ferkel's criteria. Results: History of injury was reported in all cases and the average duration from injury to presentation was 4 years and 4 months. Low signal change in T1WI was found in 6 of 7 lesions, no signal change in 1 case. Low signal change in T2WI was found in 4, no signal change in 3. 6 STIR images were obtained. High signal change was found in 3, no signal change in 2 and intermediate signal change was in 1. Arthroscopic grading was A in 1, C in 1, D in 2 , E in 1 and F in 2. We could not find any correlation between the findings on MRI and arthroscopic examination. Conclusion: We suggest arthroscopic examination is needed for accurate diagnosis of the osteochondral lesions of the talus which looked normal on plane radiography, because they have various MRl findings and high likelihood of existence of unstable cartilage lesions.

Prediction and Verification of Distribution Potential of the Debris Landforms in the Southwest Region of the Korean Peninsula (한반도 서남부 암설사면지형의 분포가능성 예측 및 검증)

  • This study evaluated a debris landform distribution potential area map in the southwest region of the Korean peninsula. A GIS spatial integration technique and logistic regression method were used to produce a distribution potential area map. Seven topographic and environmental factors were considered for analysis and 28 different data set were combined and used to get most effective results. Moreover, in an accuracy assessment, the extracted results of the Distribution Potential area were evaluated by conducting a cross-validation module. Block stream showed the highest accuracy in the combination No. 6, and that DEM (digital elevation model) and TWI (topographic wetness index) have relatively high influences on the production of the Block stream Distribution Potential area map. Talus showed the highest accuracy in the combination No. 13. We also found that slope, TWI and geology have relatively high influences on the production of the Talus Distribution Potential area map. In addition, fieldwork confirmed the accuracy of the input data that were used in this study, and the slope and geology were also similar. It was also determined that these input data were relatively accurate. In the case of angularity, the block stream was composed of sub-rounded and sub-angular systems and Talus showed differences according to the terrain formation. Although the results of the rebound strain measurement using a Schmidt's hammer did not shown any difference in topographic conditions, it is determined that the rebound strain results reflected the underlying geological setting.

The Measurement of Normal Talus in Korean Cadaver (한국인 사체에서의 정상 거골의 실측)

  • Purpose: To investigate the measured values of the talus in Koreans. Materials and Methods: We measured 88 tali from 44 cadavers that have been donated between December 2012 and December 2015. Of the cadavers, 27 were male and 17 were female. Their mean age was 73 years. The length and width of the talus were measured using a digital goniometer and vernier caliper. Results: The values of cadaveric measurement, mean maximal width and length, width and length of the dome anterior, width and length of the posterior facet, height and length of the trochlear medial facet, and height and length of the trochlear lateral facet were $43.6{\pm}2.6mm$, $56.5{\pm}3.3mm$, $32.5{\pm}2.0mm$, $42.2{\pm}2.7mm$, $22.2{\pm}2.2mm$, $34.7{\pm}2.0mm$, $15.3{\pm}1.3mm$, $33.3{\pm}2.9mm$, $25.3{\pm}3.3mm$, and $30.8{\pm}2.4mm$ for men and $38.9{\pm}1.6mm$, $53.6{\pm}2.4mm$, $27.9{\pm}2.1mm$, $37.4{\pm}3.2mm$, $20.6{\pm}0.8mm$, $31.9{\pm}1.2mm$, $13.6{\pm}2.6mm$, $28.4{\pm}2.5mm$, $24.9{\pm}2.1mm$, and $28.9{\pm}1.4mm$ for women, respectively. The size of the talus showed an accuracy of 86% when anteroposterior diameter was greater than 59 mm. A difference in the size of the right and left talus was not observed. The mean inclination and declination angles were $24.4^{\circ}{\pm}4.2^{\circ}$ and $28.2^{\circ}{\pm}5.4^{\circ}$ for men, and $24.6^{\circ}{\pm}3.6^{\circ}$ and $24.7^{\circ}{\pm}6.7^{\circ}$ for women (p=0.980, p=0.018), respectively, at least $15^{\circ}$, which showed a big difference for every object up to $37^{\circ}$. Conclusion: This paper, to the best of our knowledge, is the first study to measure the talus in Koreans. There were differences by gender and ethnicity in the in measured talus values. The measurements were smaller than European-Americans and greater than Japanese.

  • Osteochondral lesion of the talus (OLT) is a broad term used to describe an injury or abnormality of the talar articular cartilage and adjacent bone. Various terms are used to describe this clinical entity, including osteochondritis dissecans, osteochondral fractures, and osteochondral defects. Several treatment options are available; the choice of treatment is based on the type and size of the defect and the treating clinician's preference. Arthroscopic microfracture (a bone marrow stimulation technique) is a common and effective surgical strategy in patients with small lesions or in those in whom non-operative treatment has failed. This study had the following aims: 1) to review the historical background, etiology, and classification systems of OLT; 2) to describe a systematic approach to arthroscopic bone marrow stimulation for OLT; and 3) to determine the characteristics that are useful for assessing osteochondral lesions, including age, size, type (chondral, subchondral, cystic), stability, displacement, location, and containment of the lesion.

  • Talus tuberculosis is a rare disease, even in an endemic tuberculosis area. In reviews of the worldwide literature, only 18 cases of talus tuberculosis have been reported. Recently, we experienced a case of a 70-year-old polycythemia vera patient with right metatarsopharyngeal joint pain for 2 months duration who was diagnosed with talus tuberculosis with prostate involvement. Tuberculosis should be considered as one of the causes of monoarticulitis, especially in countries, where the disease is endemic. Additionally, we highly recommend taking a biopsy of the site of suspected infection because an early diagnosis is the key to successful treatment.

  • A medial opening wedge supramalleolar osteotomy (SMO) introduced by Takakura et al. is a useful realignment procedure for patients with ankle joint arthritis and varus malalignment by shifting the weight-bearing axis laterally and redistributing the loads on the ankle joint. When pain persists after arthroscopic microfracture in patients with medial osteochondral lesion of the talus (OLT), redo arthroscopy, osteochondral autograft transplantation, autologous chondrocyte implantation, or matrix-induced chondrogenesis might be indicated. On the other hand, there is insufficient scientific evidence for realignment surgery through SMO, while the effect of realignment surgery has been studied consecutively for osteochondral lesions of the knee. Therefore, this paper reports a patient with medial OLT who underwent redo arthroscopy combined with SMO for persistent pain after primary arthroscopic microfracture.

Extraction of Potential Area for Block Stream and Talus Using Spatial Integration Model (공간통합 모델을 적용한 암괴류 및 애추 지형 분포가능지 추출)

  • This study analyzed the relativity between block stream and talus distributions by employing a likelihood ratio approach. Possible distribution sites for each debris slope landform were extracted by applying a spatial integration model, in which we combined fuzzy set model, Bayesian predictive model, and logistic regression model. Moreover, to verify model performance, a success rate curve was prepared by cross-validation. The results showed that elevation, slope, curvature, topographic wetness index, geology, soil drainage, and soil depth were closely related to the debris slope landform sites. In addition, all spatial integration models displayed an accuracy of over 90%. The accuracy of the distribution potential area map of the block stream was highest in the logistic regression model (93.79%). Eventually, the accuracy of the distribution potential area map of the talus was also highest in the logistic regression model (97.02%). We expect that the present results will provide essential data and propose methodologies to improve the performance of efficient and systematic micro-landform studies. Moreover, our research will potentially help to enhance field research and topographic resource management.

Medial Ankle Impingement Syndrome due to Talar Osteochondroma and Gout Attack: A Case Report (거골 내측 결절에 발생한 골연골종과 통풍 발작에 의한 발목 내측 충돌 증후군: 증례 보고)

  • Osteochondromas are benign bone tumors typically found in the metaphyseal region of long bones. These tumors are often asymptomatic and detected incidentally. However, their occurrence in atypical sites such as the talus can pose significant diagnostic and treatment challenges. This report describes a rare case of osteochondroma of the medial tubercle of the talus, which is an unprecedented location based on a review of relevant literature. A 28-year-old male presented with worsening medial ankle pain and limping. Imaging revealed a lesion consistent with osteochondroma contributing to medial ankle impingement syndrome. Uniquely, this case also featured a coinciding gout attack in the ankle joint. Surgical removal of the lesion resulted in significant symptom relief and functional improvement. This case underscores the need to consider rare diagnoses, such as talar osteochondroma, when presented with persistent medial ankle pain and highlights the potential presence of concurrent conditions, such as gout.

Arthroscopic Treatment of Osteochondritis Dissecans of the Talus (거골 박리성 골 연골염의 관절경적 치료)

  • Purpose : To investigate arthroscopic simple excision for the treatment of osteochondritis dissecans of the talus and to examine gross change and histological evaluation of the defect through second look arthroscopy. Materials and Methods : This study included twenty-two patients who had osteochondritis dissecans of the talus that was treated with excision of arthroscopic loose body and necrotic bone tissue, leaving bleeding bed. In 7 cases, we performed second look arthroscopic examination for the evaluation of gross change of defects and histologic findings. Final results were evaluated with two clinical and functional protocols. Average follow up period was 42 months from 14 months to 8 years. Results : Ankle-hindfoot score (100 point) was improved significantly (p<0.003). Subjective and functional scores (100 points) averaged 82 points. The defect of lesion had a tendency of filling with fibrous tissue and fibrocartilage. Although the defect was filled with fibrocartilage, specific secondary lesion was not detected through second look arthroscopic examination. Conclusion : Arthroscopic debridement was an effective method for the treatment of osteochondritis dissecans of the talus without progression of secondary lesion even though the defect was filled with fibrocartilage.

Clinical Observation and Treatment of Fracture-Dislocation of Talus (거골 골절 및 탈구의 임상적 고찰)

  • Talus is a important structure of the ankle joint and its function is critical for ambulation and weight bearing. The talus fracture is rare, but the complications of fracture and dislocation are serious and resulting in avascular necrosis, osteoarthritis. So its treatment is carefully considered at initial status. Authors reviewed 11 cases of fracture and dislocation of the talus treated at Yeungnam university hospital from 1984 to 1991. The longest follow up was 8 years and shortest, 1 year. The results were as follows. 1. There were all males, the average age was 30 years old. 2. The most common cause was fall down(8 cases), and next traffic accident(2cases), sports injury(1 case). 3. According to Marti-Weber classification, 1 cases was type I, 1 in type II, 4 in type III and 5 in type IV. 4. The method of treatment were open reduction and internal fixation in 6 cases, the others were closed reduction in 5 cases. 5. Final results(by Hawkins grading system) were as follows, 3 cases were excellent, 4 cases were good, 1 case was fair and 3 cases were poor. 6. Complications were AVN in 2 cases, degenerative arthritis in 8 cases, malunion in 1 case. 7. The range of motion of the ankle joint was relatively preserved(74%), but in the subtalar joint it was decreased(43%).
