구개측 매복된 상악 견치의 교정적 치험례 (ORTHODONTIC TREATMENT OF THE PALATALLY IMPACTED MAXILLARY CANINE)

  • An impacted tooth is defined pathologically as a tooth that remains under the mucosa of inside bone without eruption of the crown after a specific period of eruption. Clinically, the term includes those teeth, even before eruption period, that are not expected to erupt due to shape, position and alignment of tooth and lack of space. Canine is prone to impaction more than other teeth because it has the longest time to develop and a complex route from the place of formation to the site of eruption. The impaction incidence of maxillary canine is repoted 0.92$\sim$3.3% (Ferguson, 1990). In 1995 Orton reported that the incidence was 0.92$\sim$2.2% and palatal impaction was more frequent than labial impaction(85%:15%). In 1969 Johnston presented it was more common to woman than to man(3:1). The etiology includes systemic disease such as endocrine disorder, cleidocranial dysostosis, irradiation, Crouzon syndrome, ricketts, facial hemihypertrophy and hereditary and local problems such as ectopic position of the tooth, distance of tooth from its place of eruption, malformation of the tooth, presence of supernumerary teeth, trauma of tooth germ, infection of tooth germ, displacement of tooth germ or tooth by a neoplasm, ankylosis, overretention of deciduous predecessor, lack of space for the tooth in the dental arch and mucosal barrier due to gingival fibrosis. The maxillary canine is especially important as it has the longest root, provides guidance for lateral movement of the mandible and masticatory function and assumes an important role esthetically as it is located at mouth angle. If left untreated, it may cause migration and external, internal resorption of adjacent teeth, loss of arch length, formation of dentigerous cyst or tumors, infection and referred pain as well as malposition of the tooth. Therefore, periodic examination of the development and eruption of the maxillary canine is especially important in a growing child. This case study presents the results of treatment of palatally impacted maxillary canine utilizing surgical exposure and orthodontic tooth movement on patients visiting SNUDH dept. of pediatric dentistry.

18~19세기 정원 예술에서 현대적 시각성의 등장과 반영 - 픽처레스크 미학과 험프리 렙턴의 시각 매체를 중심으로 - (Modern Vision in the 18~19th Century Garden Arts - The Picturesque Aesthetics and Humphry Repton's Visual Representation -)

  • 픽처레스크 미학과 풍경화식 정원은 자연을 그림과 같이 정태적으로 감상하게 만드는 이른바 회화적 자연관의 전범으로 인식되어 왔다. 이러한 해석은 당시의 시각 문화를 선원근법에 의존하여 설명한 데에서 연유한다. 실상 당시는 새로운 시각 매체의 발명에 따라 종래의 원근법에 기반했던 시각성이 '유동적인 시각'과 '시점의 상대성'으로 요약되는 '움직이는 시각성'으로 변모해가는 바로 직전의 시기였다. 이 연구는 그러한 시각 문화의 변동에 주목하여 픽처레스크 미학과 험프리렙턴의 시각 매체에서 나타나는 '움직이는 시각성'의 징후를 읽어내고자 한다. 18세기 독일의 정원 이론가 히르시펠트는 풍경화식 정원은 구불구불한 선을 이용하여 움직임의 환영을 만들어낼 수 있으므로 다른 예술 형식보다 우월하다고 분석했다. 18세기 말 야기된 픽처레스크 논쟁에서 아마추어 이론가들은 회화에서 복잡하고 다채로운 시각의 변주를 중요하게 여기고, 정원도 그러한 시각적 자극이 만들어지도록 설계해야 한다고 주장했다. 렙턴의 "레드북"에 수록된 스케치에서는 움직이는 시각성이 반영된 다양한 재현 전략들이 발견된다. 렙턴은 속도의 차이를 고려하여 여러 유형의 길을 체계적으로 설계했고, 인간의 시각장을 표현하기 위해 회화적 프레임이 아닌 파노라믹 뷰를 활용하였으며, 현실에서의 움직임과 유사한 시각적 경험을 만들어내기 위해 여러 스케치들을 전략적으로 배열하였다. 이와 같이 픽처레스크 미학과 렙턴의 시각 매체는 당대 시각 문화에 새롭게 등장하기 시작한 움직이는 시각성을 반영하고 있었다.

발달성협응장애 아동의 인지기반 작업수행(Cognitive Orientation to daily Occupational Performance; CO-OP) 중재에 대한 체계적 고찰 (A Systematic Review of Cognitive Orientation to Daily Occupational Performance for Children with Developmental Coordination Disorder)

  • 목적 : 본 연구는 발달성협응장애 아동을 대상으로 인지기반 작업수행 중재의 목표 활동, 중재 방법 및 중재 효과, 평가도구에 관한 정보를 수집하고 분석하여 중재 효과에 대한 근거를 제시하고자 하였다. 연구방법 : 본 연구는 2012년 1월부터 2022년 10월까지 게재된 논문을 대상으로, PubMed, Embase, ScienceDirect, Cochrance Library 데이터베이스를 이용하여 검색하였다. 검색어는 ('developmental coordination disorder' OR 'DCD') AND ('Cognitive Orientation to daily Occupational Performance' OR 'Cognitive Orientation to Occupational Performance' OR 'CO-OP')이었다. 검색된 211편 중 선정기준과 배제기준에 따라 총 7편의 논문을 선정하여 분석하였다. 연구결과 : 대상 문헌은 무작위 대조군 연구가 2편, 비무작위 두 집단 연구가 1편, 비무작위 한 집단 연구가 3편, 단일 대상 연구가 1편으로 전반적으로 근거 수준이 높게 나타났다. 중재의 목표 활동은 놀이, 교육, 일상생활활동 영역 순으로 선호도가 높았다. 인지기반 작업수행 중재는 대부분 한 회기에 1시간씩, 10회기로 진행하였으며, 작업수행과 운동기술에 긍정적인 효과가 있는 것으로 나타났다. 중재의 효과는 주로 Canadian Occupational Performance Measure(COPM)과 Performance Quality Rating Scale(PQRS)을 이용하여 작업수행 향상을, Movement Assessment Battery for Children(MABC)를 이용하여 운동 기술 향상을 평가하였다. 결론 : 본 연구는 발달성협응장애 아동의 인지기반 작업수행 중재 적용에 있어 임상적 기초자료로 활용될 수 있을 것으로 보인다.

  • 스쿼시와 스트레치 원칙은 애니메이션 동작을 위한 필수적인 원칙으로 작용한다. 이 원칙의 적용은 캐릭터에게 무게와 부피의 허상을 제공하며, 애니메이션 동작의 뻣뻣함과 딱딱함을 벗어나, 부드럽고 유연한 동작이 되게 하는 것을 가능하게 한다. 애니메이션에서 인물이나 사물의 동작표현을 실제 세계와 같이 표현하게 되면, 부자연스럽게 보인다. 스쿼시와 스트레치가 없는 어떤 동작이든 딱딱하고 흥미가 없고, 살아있지 않은 것으로 보일 것이다. 이것은 질량, 부피, 중력의 기본 법칙과 함께, 모든 사물의 움직임, 캐릭터의 동작, 대사, 얼굴표정 등에 모두 적용될 수 있다. 이 원칙이 없이는 어느 동작도 잘 표현되지 않을 것이다. 좋은 애니메이션 동작이 되려면, 이것은 2D 애니메이션에서 만이 아니라, 3D애니메이션에서도 깊이 적용되어야 한다. 따라서, 스쿼시와 스트레치 원칙의 체계적인 분석이 요구된다.

Characterization and Sequence Analysis of a Lily Isolate of Cucumber mosaic virus from Lithium tsingtauense

  • A new isolate of Cucumber mosaic virus (CMV), identified as Li-CMV was isolated from a diseased Korean native lily (Lithium tsingtauense Gilg). Biological and serological properties of Li-CMV were characterized, and reverse transcription-polymerase chain reaction (RT-PCR) analysis, restriction enzyme profiling of RT-PCR products, and nucleotide sequence analysis of RNA3 of the virus were performed in this study. Remarkable differences in symptoms between Li-CMV and ordinary CMV strains were found in tobacco plants and Datura stramonium. Li-CMV-infected tobacco plants (cv. Xanthi-nc and cv. Samsun) induced chlorotic ringspots on uninoculated upper leaves, and the symptom expression was delayed or faint whereas, ordinary CMV strains induced green mosaic symptoms on the plant. Systemic infections were observed on Nicotiana benthamiana with severe mosaic symptom. Restriction mapping analysis of RT-PCR products using MspI showed that Li-CMV belonged to CMV subgroup I. A full-length CDNA copy of RNA3 for the virus was amplified by RT-PCR, cloned, and its complete nucleotide sequence was determined. The RNA3 of Li-CMV was 2, 232 nucleotides long, and consisted of two open reading frames of 843 and 657 bases encoding 3a protein (movement protein) and coat protein, respectively. Results of this study indicate that Li-CMV is a novel strain and belongs to subgroup I of CMV in the genus Cucumovirus.

악관절 강직증의 치험례 (THE CASES REPORT OF ANKYLOSIS)

  • Temporomandibular ankylosis is defined as a situation in which the condyle is fused to the fossa by bone or fibrous tissue. Conditons such as trauma, infection, or systemic disease may predispose to various types of ankylosis, bringing about different levels of limitation in mandibular movement. Most patients with temporomandibular ankylosis are associated with limitation of maximal mouth opening, deviation of the chin toward the affected side, impaired occlusion, chronic pain, compromised oral hygiene, severe facial asymmetry & impeded mandibular molar eruption occurring in childhood. Several techniques to release ankylosis have been described in the literature, showing variable and often unsatisfactory results. The most frequently used operations are gap arthroplasty, interpositional arthroplasty, and exicision and joint reconstruction with autogenous or alloplastic materials. We have managed the two patients of TMJ ankylosis. They had previously TMJ surgery and we treated with gap arthroplasty & active physial therapy. We have obtained favorable results and report these cases with literatures review.

사회인지이론을 적용한 근감소증 남성 노인의 가족지지기반 신체활동증진프로그램 개발 (Development of Family Support Physical Activity Program for the Male Elderly with Sarcopenia based on Social Cognitive Theory)

  • Purpose: The purpose of this study was to develop family support oriented physical activity program for the male elderly with sarcopenia based on social cognitive theory. Methods: This program was developed through an analysis of 18 related intervention literatures, results of a focus group interview with 5 elderly men with sarcopenia and the content validity index of the program content adequacy and applicability by 6 experts. The combined exercise with resistance exercise and aerobic exercise was constructed in accordance with the recommendations of the American College of Sports Medicine (ACSM) and experts' opinions. Results: The program consists group education sessions (5 times, 60 minutes for each) for 12 weeks and family support oriented physical activity program composed of individual intervention (sending alarm for physical activity for 10 times and telephone monitoring for 2 times). The program also reflects the concept of self-efficacy and self-regulation, which are important factors for continuing physical activity through family support. The progressive resistance exercise was developed by composing 5-6 systemic movement forms that repeat 2-3 days a week and 2-5 sets at least. Conclusion: It is proposed to standardize the family support oriented physical activity program through the further studies so that the program can be utilized for the various groups of people who need increased level of physical activities.

An epidemiological study on the prevalence of temporomandibular disorder and associated history and problems in Nepalese subjects

  • Background: Temporomandibular disorder (TMD) represents a subgroup of painful orofacial disorders involving pain in the temporomandibular joint (TMJ) region, fatigue of the cranio-cervico-facial muscles (especially masticatory muscles), limitation of mandible movement, and the presence of a clicking sound in the TMJ. TMD is associated with multiple factors and systemic diseases. This study aimed to assess the prevalence of TMD in Nepalese subjects for the first time. Methods: A total of 500 medical and dental students (127 men and 373 women) participated in this study from May 2016 to September 2016. The Fonseca questionnaire was used as a tool to evaluate the prevalence of TMD, and Fonseca's Anamnestic Index (FAI) was used to classify the severity of TMD. Results: The majority of the participants with TMD had a history of head trauma, psychological stress, and dental treatment or dental problems. The prevalence of TMD in Nepalese students was mild to moderate. Conclusions: The prevalence of TMD in Nepalese subjects was mild to moderate. The majority of the study subjects had eyesight problems, history of head trauma, psychological stress, and drinking alcohol and had received dental treatments.

Cucumber Mosaic Virus 1a Protein Interacts with the Tobacco SHE1 Transcription Factor and Partitions between the Nucleus and the Tonoplast Membrane

  • The transcription factor SHE1 was identified as an interacting partner with the cucumber mosaic virus (CMV) 1a protein in the yeast two-hybrid system, by a pull-down assay, and via bimolecular fluorescent complementation. Using fluorescent-tagged proteins and confocal microscopy, the CMV 1a protein itself was found distributed predominantly between the nucleus and the tonoplast membrane, although it was also found in speckles in the cytoplasm. The SHE1 protein was localized in the nucleus, but in the presence of the CMV 1a protein was partitioned between the nucleus and the tonoplast membrane. SHE1 expression was induced by infection of tobacco with four tested viruses: CMV, tobacco mosaic virus, potato virus X and potato virus Y. Transgenic tobacco expressing the CMV 1a protein showed constitutive expression of SHE1, indicating that the CMV 1a protein may be responsible for its induction. However, previously, such plants also were shown to have less resistance to local and systemic movement of tobacco mosaic virus (TMV) expressing the green fluorescent protein, suggesting that the CMV 1a protein may act to prevent the function of the SHE1 protein. SHE1 is a member of the AP2/ERF class of transcription factors and is conserved in sequence in several Nicotiana species, although two clades of SHE1 could be discerned, including both different Nicotiana species and cultivars of tobacco, varying by the presence of particular insertions or deletions.

강직성 척추염의 치료 (Treatment of Ankylosing Spondylitis)

  • 강직성 척추염은 천장 관절의 강직뿐만 아니라 척추체 주변의 관절막과 인대의 골화로 인해 척추 강직을 일으킨다. 이는 척추 움직임 제한을 일으키고 많은 경우에서 통증을 동반한 경추, 흉추, 요추 분절의 척추 변형을 유발한다. 방치할 경우 최종적으로는 전신 관절의 강직과 골화를 유발하게 되는 심각한 질환 중 하나이다. 일반적으로 30세 이전에 증상이 발현되며 질병의 점진적인 진행으로 신체적 기능, 환자의 직업적 능력과 삶의 질에 악영향을 끼치게 된다. 이는 우울증 같은 정신과적 질환으로 발전할 가능성을 높인다. 저자들은 이러한 심각성을 인지하고 전신 골화로 악화되기 이전에 다양한 약물 치료로 수술적 치료를 방지하고자 하는 최근의 경향과 연구를 소개하고자 하였으며, 또한 진행을 예방하지 못하여 수술적 치료에 이르게 된 환자들에서 시행되는 다양한 수술 방법과 그에 따른 합병증을 제시하고자 한다.
