외상성 악관절 손상 (TRAUMATIC TMJ INJURY)

  • Mandibular trauma is developed due to traffic accident, fall down, industrial injury, and others. TMJ disorder is usually also developed after facial traumatic injury. Many authors suggested that disc displacement or tearing, acute synovitis, TMJ ankylosis, traumatic arthritis, or effusion are developed after facial trauma. It is still very controversible what is the best treatment of TMJ injury such as condylar fracture and meniscal injury. In TMJ injury, synovial inflammation is developed and pain mediators such as prostaglandin E2 or leukotriene B4 are released from the synovial membrane. This can be a cause of TMJ disorder. I present a variety of experimental study about the condylar fracture and meniscal injury and enzyme-immunoassay of synovial fluid after mandibular trauma that have been studied since 1992 and establish the treatment criteria of traumatic TMJ injury. I think that the treatment option of condylar fracture depends upon the surgeon's criteria exclusively. There are no significant differences between conservative and surgical treatment. If the aggressive functional physical therapy and long-term followup be performed, the favorable functional recovery of TMJ can be obtained. And I think that the initial surgical management of meniscus of TMJ is unnecessary in condylar fracture. And also arthrocentesis can be available to release the patient's subjective symptoms and improve the healing of injured TMJ.

인체 무릎관절 윤활포식세포 cluster designation 표지에 관한 면역전자현미경적 연구 (An Immune-Electron Microscopic Study for Cluster Designation on the Phagocytic Synovial Cells in the Knee Joint of the Human)

  • 사람 무릎관절 윤활막을 구성하는 윤활세포 중 윤활포식세포(phagocytic synovial cell, type A cell)의 기원에 대한 논의는 형태적으로 큰포식세포의 모습을 하고 있는 단핵포식체 계 (mononuclear phagocyte system)의 한 일부로서 아마도 골수(bone marrow)에서 기원되어졌을 것이라고 알려져 있다. 기능적으로도 LCA, HLA-DR과 Ia 항원에 양성반응을 보여 큰포식세포의 일부로 알려졌으나 아직 연구가 부족한 실정이다. 본 연구는 CD14와 활성화된 큰포식세포의 표지물로 알려진 CD105(endoglin)를 이용하여 윤활포식세포의 세포 내 발현부위를 규명하고, 기능적으로 활성화된 큰포식세포와 포식작용의 역할을 수행하는지 여부를 확인하기 위해 사람의 무릎관절에서 윤활세포들을 냉동초미세박절법을 이용한 면역조직화학 기법으로 CD14와 CB105에 대한 금표지를 하여 투과전자현미경으로 관찰한 결과 다음과 같은 결론을 얻었다. 1. CD14는 윤활포식세포의 과립세포질세망과 세포질및 가장자리, 공포 주변 부위에서 표지 되었으며 공포내에서는 표지 되지 많았다. 2. CD105(endoglin)는 윤활포식세포의 세포막 가장자리와 세포질 돌기를 따라 표지 되었으며 공포 주변 부위에서도 표지 되었으나, 공포 내에서는 표지 되지 많았다. 이상의 결과로 보아 사람 무릎관절 윤활세포층에 위치하는 윤활포식세포는 CD14와 CD105의 항원에 대한 표지를 보이므로 활성화된 큰포식세포나 포식작용의 역할을 수행하는 것으로 생각된다.

Can herbal drug(s) meet the challenges of genomewide screen results on rheumatoid arthritis

  • Rheumatoid arthritis (RA) is an autoimmune/inflammatory disorder with a complex genetic component. RA is characterized by chronic inflammation of the synovial membrane in the joint, which leads to the progressive destruction of articular cartilage, ligament and bone. Several cytokines such as tumor necrosis $factor-{\alpha}\;TNF-{\alpha}\;and\;interleukin-1{\beta}\;(IL-1{\beta})$ and interleukin-6 (IL-6) have been implicated in the pathological mechanisms of synovial tissue proliferation, joint destruction and programmed cell death in rheumatoid joint. Genome wide screening of subjects suffering from autoimmune diseases especially arthritis revealed linkage to inflammatory molecules like $TNF-{\alpha},\;IL-1{\beta}$ and IL-6, inducible nitric oxide synthase (iNOS), cyclooxygenase-2 (COX-2), nuclear factor-kappaB $(NF-{\kappa}B)$ and human leucocyte antigen/major histocompatibility complex (HLA/MHC) locus. The status of the pharmacological mechanism of herbal drugs in the light of genome wide screening results has been discussed to reinforce the therapeutic potential and the pharmacological basis of the herbal drugs.

말의 활액 및 혈청 분석을 통한 관절질환의 생화학적 예측 인자 (The Investigation of Synovial Fluid and Serum for Biochemical Markers of Joint Diseases in the Horse)

  • The joint disease including osteoarthris (OA) and rheumatoid arthritis (RA) are common in the horse. Many studies have been performed to develop biochemical markers reflecting the abnormalities of cartilage and synovial membrane. However, no specific, sensitive and clinically well established assay systems have been yet available to characterize the severity of joint diseases. Indeed, radiography is still doctor's best choice of assessing joint damage in OA/RA. This review focuses on biochemical molecules such as proteoglycan, collagen, matrix metalloproteinases (MMPs), lectin and cytokine to assess their potential value for not only predicting stage of joint disease but also monitoring treatment efficacy.

Synovial Chondromatosis of the Temporomandibular Joint: A Case Report

  • Synovial chondromatosis (SC) is a rare, benign disease found in metaplastic cartilaginous nodules within the synovial membrane. The etiology of SC is not well-known, but it may be associated with trauma, chronic abnormal loading, or inflammatory joint disease. It is often found in knee, elbow, ankle, and shoulder joint but rarely in the temporomandibular joint (TMJ) area. SC of TMJ frequently appears in middle age and more often in females. Clinical symptoms include periarticular swelling, pain, crepitus, and limitation of joint motion. The most common feature is a radiographic finding of loose bodies in the joint. Irregularity of joint space and condylar head is also observed. Treatment involves the surgical removal of synovium and loose bodies. The prognosis is good, with low postsurgical recurrence rate. Functional improvement of the joint and pain relief are noted in many patients. Malignant transformation of SC has not been recorded. In this study, we report a 54-year-old female patient who experienced discomfort on the right TMJ with SC on the area but made a satisfactory recovery after surgery.

접골목(接骨木)추출물 및 약침액에 의한 MIF 활성 조절능이 생쥐의 제2형 Collagen 유발 관절염에 미치는 영향 (Effect of Suppressing the Activation of Macrophage Migration Inhibitory Factor by $Sambucus$ $williamsii$ $H_{ANCE}$ Extract & Pharmacopuncture Solution on Type II Collagen-induced Arthritis)

  • Objectives : The purpose of this study is to evaluate effect of suppressing the expression of cyclo-oxygenase-type-2 (COX-2) as a consequence of inhibition macrophage migration inhibitory factor (MIF) activation by $Sambucus$ $williamsii$ $Hance$ (SWH) pharmacopunctureon rheumatoid arthritis (RA). Methods : In vitro test, synoviocytes extracted from type II collagen-induced arthritis (CIA) mouse's knee joint were cultivated After that, each well of synoviocytes was mixed with the extract of SWH at the dosage of $0.4mg/m{\ell}$, $0.6mg/m{\ell}$, $0.8mg/m{\ell}$, and $1.0mg/m{\ell}$ respectively, and cultivated for 24 hours after the addition. Reverse transcriptase - polymerase chain reaction (RT-PCR) is used to investigate the expression of MIF, Tumor necrosis factor (TNF)-${\alpha}$, COX-2 mRNA. $In$ $vivo$ test, thirty DBA female mice were used, and each ten mice were allocated into three group; normal group, CIA-elicitated group, and group treated with SWH pharmacopuncture on it's the point of $ST_{35}$ after CIA elicitation. It is investigated that change of mice foot thickness, histologic change of sliced synovial joint of mouse, and extent change of MIF, TNF-${\alpha}$, COX-2 in synovial membrane. Results : $In$ $vitro$ test, the expressions of cytokine(MIF, TNF-${\alpha}$, COX-2) mRNAs related to RA were dose-dependent decreased. In the SWH pharmacopuncture group, foot thickness and histologic change of sliced synovial joint were decreased comparing with CIA-elicitated group's change. In the SWH pharmacopuncture group, the suppression of MIF, TNF-${\alpha}$, COX-2 in synovial membrane was clearly shown comparing with CIA-elicitated group's change. Conclusions : It might be suggested that SWH pharmacopuncture mitigate tissue damage originated from rheumatoid arthritis by suppressing the expression of COX-2 as a consequence of inhibition MIF activation.

The comparison of two different intraarticular injections using a sonographic anterolateral approach in patients with osteoarthritic knee

  • Background: The intraarticular (IA) injection has become popular for the management of the osteoarthritic knee without an effusion. The success rate of IA injection would be better if it was able to be visually confirmed. We hypothesized that an anterolateral approach, which targets the synovial membrane of the lateral condyle using ultrasound, would provide an equivalent alternative to the anterolateral approach, targeting the synovial membrane of the medial condyle for IA injection of the knee. Methods: A total of 96 knees with osteoarthritis were randomized placed into the two groups, which were group I (anterolateral approach to the medial condyle) and group II (anterolateral approach to the lateral condyle). The primary outcome was to compare the success rate of the two methods of IA injection. The required length of the needle for injection was also measured and compared. Pain intensity was assessed using the Numeric Rating Scale in order to evaluate the success of injection. Results: There were no significant differences in the success rate between both groups. The success rate of group I and group II were 87.8% (95%, CI 78.7-97.0) and 91.5% (95%, CI 83.6-99.5), respectively (P = 0.549). The needle depth was $5.0{\pm}0.8$ (3.0 to 6.1 cm) in group I, and $3.0{\pm}0.8$ (1.5 to 5 cm) in group II (P < 0.001). Conclusions: The anterolateral approach to the lateral femoral condyle, using ultrasound, is an alternative method to the approach targeting the medial femoral condyle, using shorter needle.

홍화자유약침이 CIA 모델 생쥐의 윤활관절막 손상 억제에 미치는 영향 (The Inhibitory Effects of $Chrthami$ Semen Oil Pharmacopuncture (CSOP) on Synovial Membranes in Type II Collagen-Induced Arthritis Mice)

  • Objectives : The purpose of this study is to observe the inhibitory effects of $Chrthami$ semen oil pharmacopuncture(CSOP) on CIA (collagen-induced arthritis) mice. Materials and Methods : Two types of experiments were conducted: $in$ $vitro$ assay, inhibition of MIF mRNA and TNF-${\alpha}$ mRNA expressions in synovial membranes was observed, and $in$ $vivo$ assay, $1{\mu}{\ell}/kg$ CSOP was injected every day to the left $Weizhong$ ($BL_{40}$) from day 3 to 21 after induction of CIA, and changes in paw edema, apical surface morphology, neovascularization in synovial membranes, fibrosis, pro-inflammatory cytokines production, Th-1 differentiation, and anti-inflammatory effect were investigated. Results : 1. In synoviocytes of the CIA mice treated with CSOP, MIF mRNA and TNF-${\alpha}$ mRNA expressions were down-regulated in a dose-dependent manner. 2. Paw edema of the CIA mice treated with CSOP was diminished. 3. Tissue injury in the synovial membranes, capillary distribution and fibrosis were reduced in CSOP-treated mice. 4. MIF, TNF-${\alpha}$, IL-6, MMP-9 expressions were repressed in CSOP-treated mice during the experiment to observe the inhibitory effect on cytokines production in early stage RA. 5. IL-12 and CD28 were reduced in CSOP-treated mice during the observation of inhibitory effect on Th 1 differentiation. 6. PPAR-${\gamma}$ was increased during the experiment to observe the anti-inflammatory effect of CSOP. Conclusions : The results may suggest that administration treatment using $Chrthami$ semen oil pharmacopuncture decreases the inflammatory response on an Animal Model with CIA.

전갈약침이 adjuvant 유발 흰쥐의 관절활액막내 cytokine과 prostaglandin E2에 미치는 영향 (The Effect of Buthus martensi Karsch Herbal-acupuncture on Immune Responses to Adjuvant Induced Arthritis in Rats)

  • Objective : The purpose of this study is to investigate the immunohistological effect of Buthus martensi Karsch Herbal-acupuncture in treating the arthritis, performed several experimental items : those are paw edema, $IL-1{\beta}$, IL-6, IL-8, $TNF-{\alpha}$ and $PGE_2$. Methods : All the male Sprague Dawley rats used in this study were bred and maintaned in our pathogen-free rat colony and were 8 weeks of age at the start of to experiment. The experimental model of arthritis was induced by injection of $50{\mu}g/{\mu}{\ell}$ adjuvant(mineral oil mixed Mycobacterium butyricum). Buthus martensi Karsch Herbal-acupuncture was injected into ST36(足三里) of rats daily for 21 days. Immunohistological analysis was carried out to assess paw edema, $IL-1{\beta}$, IL-6, IL-8, $TNF-{\alpha}$ and $PGE_2$ expression in synovial membrane and sera Buthus martensi Karsch Herbal-acupuncture injected. Results : Buthus martensi Karsch Herbal-acupuncture group showed a decrease with statistical significance, in paw edema, $IL-1{\beta}$, IL-6, IL-8, $TNF-{\alpha}$ and $PGE_2$ in synovial membrane and sera compared with control group. Conclusion : Buthus martensi Karsch Herbal-acupuncture stimulation inhibited the development of immunity to adjuvant induced arthritis in rats. Thus, Buthus martensi Karsch Herbal-acupuncture may have preventive effects on autoimmune inflammatory joint diseases as arthritis. The effect of Buthus martensi Karsch Herbal-acupuncture on the immune function and the disease activity in patients with arthritis warrants further investigation.

청폐사간탕(淸肺瀉肝湯)이 생쥐의 Lipopolysaccharide 유발 관절염(關節炎)에 미치는 영향(影響) (The Effect of Chungpyesagantang on Lipopolysaccharide induced Arthritis in Mice)

  • 1. PURPOSE : The purpose of this study is to investigate the effect of Chungpyesagantang on LPS induced Arthritis in Mice. 2. METHOD : All the BALB/C Mice used in this study were 4wks of age at the start of the experiment. The experimental model of Arthritis was induced by injectection of $300{\mu}g/kg$ LPS in mice knee joint. The experiment was compare daily CS treatment group after Arthritis elicitation with Arthritis elicitated group at day 4, 7, 14 after Arthritis elicitation. 3. RESULTS 1) The hyperplasia of synoviocytes of CS treatment group after Arthritis elicitation is soften than Arthritis elicitated group. 2) The aggregation of collagen fibers CS treatment group after Arthritis elicitation is decreased than Arthritis elicitated group. 3) The distribution of TUNEL positive cells(apoptotic cell) of CS treatment group was remarkably increased than Arthritis elicitated group. 4) The distribution of $TNF-{\alpha}$, $NF-{\kappa}B\;p50$, COX-2 positive cells of CS treatment group after Arthritis elicitation in synovial membrane was decreased than Arthritis elicitated group. 5) The distribution of $IL-2R-{\alpha}$, ICAM-1 positive cells of CS treatment group after Arthritis elicitation in apical surface of synovial membrane was decreased than Arthritis elicitated group. 6) The distribution of $NF-{\kappa}B\;p50$, $IL-2R-{\alpha}$ in common iliac lymph node of CS treatment group after Arthritis elicitation positive cells was decreased than Arthritis elicitated group. 4. CONCLUSION : As a result of these experimental results, it may be concluded that Chungpyesagantang used for treatment of LPS induced Arthritis in Mice. Inflamation activity in CS treatment group after Arthritis elicitation was decreased than Arthritis elicitated group.
