Preliminary Study to Develop the Instrument of Korean Medical Pattern Identification and Functional Evaluation for Five Organ (한의 오장변증진단평가 도구 개발을 위한 기초 연구)

  • This study was aimed to develop the instrument of Korean medical pattern Identification and Functional Evaluation for Five Organ. This study followed seven steps, consisted of first composition of research members, second set-up of the aim, third review for advanced research, fourth finding an important index, fifth puting on the weight for index, sixth advice from linguist and last development of draft of questionnaire. Twenty two in and out researchers joined and put the weight on each question. We developed a draft version of questionnaire of Korean medical pattern identification and functional evaluation for five organ. Most questions between pattern identification and functional evaluation of five organ have similar weight, but several ones have difference in order. Generally, specific symptom represented specific location or related with pain have relatively higher weight on pattern identification, and weakness question gained weight on functional evaluation. We develop the questionnaire of Korean medical pattern identification and functional evaluation for five organ. Following study, which will evaluate the reliability and validity, is needed.

  • The aim of this study was to investigate the predictive validity of the five organ pattern identification questionnaire(FOPIQ). Data collection was conducted from 190 people who were randomly selected from the general population living in D city from October 2016 to June 2017, and the collected data were analyzed by SPSS 23.0 Statistics Program. Pearson correlation coefficient was used to know the relation between the expert's score and FOPIQ's one. The cut-off value, sensitivity and specificity were analyzed through ROC-curve. Significant p was <.05. The pearson correlation coefficient was .735, .756, .762, .736, and .513 between individual score of FOPIQ and that of the experts in liver, heart, spleen, lung, and kidney, respectively. The cut-off value of the FOPIQ was 46.209, 47.276, 45.336, 48.823, and 42.508 in liver, heart, spleen, lung, and kidney respectively. The AUC derived from the cut-off value of the FOPIQ was .907, .854, .888, .902, and .781 respectively. This study suggests that the FOPIQ could be valid to apply for general population in clinics as well as health checkups.

Report on the Korean Standard Pattern Identifications for Stroke-III (한의 중풍변증표준안-III에 대한 보고)

  • Objectives : The purpose of this study was to develop the Korean standard pattern identifications for stroke-III (KSPIS-III). KSPIS-III includes 4 major pattern identifications (PIs) and clinical indicators for each. Methods : To extract the indicators for 4 major PIs, we analyzed 1548 clinical data from 15 traditional Korean medicine hospitals. Patients got acute stroke within 30 days from onset. Two physicians independently checked 65 indicators and performed pattern diagnosis. If the PI were diagnosed the same, PI would be confirmed. First we built an assumption model that set up the relationship among pattern identifications. Second, we extracted the indicators for fire-heat pattern and qi deficiency pattern by comparison between excessive and deficiency group, heat and non-heat group. By comparing yin deficiency pattern and 3 other patterns respectively, we extracted the indicators for yin deficiency pattern. Dampness-phlegm pattern indicators were extracted by the same method. Results : After cross tabulation with 65 indicators on the basis of our assumption model, we finally extracted 19 indicators for fire-heat pattern, 11 for qi deficiency pattern, 7 for yin deficiency pattern, and 7 for dampness-phlegm pattern. Conclusions : KSPIS-III was more improved than KSPIS-II because it was based on more clinical data. Further study to establish the PI diagnostic model would be required for practical use in the clinical field.

A Study on "Dongchunohji(洞天奧旨)" ("동천오지(洞天奧旨)"에 관한 소고(小考))

  • This book was completed in the 33rd year of Gang-hui-gapsul(康熙 甲戌) in the Cheong(淸) dynasty(1694), and was first inscribed in the 55th year of Geonryung(乾隆). In this first edition named 'Daechudangbon(大雅堂本)', his descendant Jinbonghui(陳鳳輝) wrote the postscript. There are also other editions such as 'Ga-gyeong-ganchwihyeondangbon(嘉慶間聚賢堂本)', 'Wimundanggeonsangbon(緯文堂巾箱本)' and 'Guangseo-ganseonseongdangbon(光緒間善成堂本)' Second, this book was called "Oe-gwabirok(外科秘錄)" and used the pen name 'Gibaekchunsasojeon(岐伯天師所傳)'. There are 16 volumes in total. The beginning of volume one, is a drawing of the 14 meridians. Volumes 1$\sim$4 are on the symptoms and treatment of abscesses and sores and ulcers[癰疽瘡瘍]. Volumes 5$\sim$13 are on surgery, dermatology and 156 diseases such as wounds by contusion, sharp objects and insects and beasts[跌撲, 金刃, 蟲獸傷]. Volumes 14$\sim$16 list the internal treatments, external medicine, acupuncture and moxibustion and surgery of sores and ulcers. Third, the book stressed early detection and treatment of diseases, emphasized inner resolving[內消] being cautious about using medicine, and further deepened the syndrome differentiation and treatment[辨證施治] of sores and ulcers[瘡瘍] by dividing fire toxin(火毒) into Eumhwa and Yanghwa(陰火/陽火). Also, it established the cause of sores and ulcers development as the deficiency of Gi(氣) and blood[血] and focused especially on the liver and kidney's involvement in the process. It also asserted that the pathogen[邪] is the tip[標], so one should eliminate and reinforce[攻補] to balance it out. The sore and ulcer surgery[瘡瘍外科] part is mostly based on detoxifying[解毒] and resolving[消散]. Although the meridians were mentioned, they were not emphasized, while moxibustion treatment of sores and ulcers[瘡瘍] were thoroughly analyzed. Also, of all 550 formulas in this book, 90% are experience-based which frequently use Geum-eunhwa(金銀花), Pogong-yeong(蒲公英) and Jahwajijeong(紫花地丁). The usages are unique, and the formulas have good adaptability. The symptoms of the diseases are explained first, followed by according main and sub treatments.

Survey for Standardization of Medical Examination Items in Oriental Medicine (한의 진찰항목 표준화를 위한 실태 및 수요조사)

  • Objective : To poll oriental medical doctors on their opinions about the necessity of standard medical charts and of detailed items in their development. Currently, oriental medical institutions use their own medical charts, but a standard medical chart is necessary for medical information sharing. Methods : Report by 912 of the 10,490 oriental medical doctors surveyed on their general aspects, actual conditions, and requirements. Results : The oriental medical doctors surveyed who worked in oriental medicine clinics, oriental medicine hospitals, and public health centers said medical examination programs should consist of chief complaints (said by 814 respondents or 89.25%), history (792 or 86.84%), ordinal symptoms (753 or 82.57%), diet (727 or 79.71%), emotions and overstrain (654 or 71.71%), side effects and allergies (622 or 68.20%), improvement of symptoms (605 or 66.34%), a questionnaire on particular diseases (558 or 61.18%), social aspects (523 or 57.35%), a physical examination (520 or 57.02%), a questionnaire on syndrome differentiation (514 or 56.36%), diagnosis using medical devices and laboratory tests (471 or 51.64%), and Sasang constitution (357 or 39.14%). Ninety-one percent of the respondents said they intended to use a standard chart, and 82.19% agreed to share patient information with medical institutions. Conclusions : Over 90 percent of the oriental medical doctors surveyed said they need a standard medical examination program. Oriental medical examination items that correspond with the opinions of the oriental medical doctors surveyed and of experts will thus be developed, and the draft chart will be distributed to oriental medical institutions with the developed medical forms and electronic medical chart.

  • Objectives: The purpose of this study was to explore some new therapies to control clinical symptoms of patients with terminal cancer by using external applications. Methods: We investigated some literatures on the external applications for cancer patients and made diagrams. Results: The results are summarized as follows. External applications are one of the traditional oriental medical methods and these are effective for pain control, ascites & pleural effusion and palpitable mass. It has some characteristics which are simple, safe and popular, but we must pay attention carefully to allergic reaction and toxicity in using external applications. The therapeutic portion of external applications are decided by discipline of syndrome and disease differentiation, and the prescriptions are composed of antitumor herb medicines. Conclusions: From the above results, it is expected that external applications are useful to improve clinical symptoms and quality of life(QOL) for patients with terminal cancer who cannot intake foods or medicines.

Study of Clinical Application of Pathology of Blood Stasis, Focused on 33 Prescriptions in 『Yilingaicuo』 (『醫林改錯』 처방의 현대 질병 범위에 관한 연구)

  • This paper researches the features of blood stasis theory of Wangqingren, who wrote 『Yilingaicuo』 that greatly contributed in the development of blood stasis theory at Qing dynasty period. And the disease cause, disease mechanism of blood stasis and scope of modern diseases related with blood stasis are studied by research on clinical papers which used 33 prescriptions in 『Yilingaicuo』 in modern times. Research on the features of blood stasis theory of Wangqingren is proceeded by referring to the annotations of 『Yilingaicuopingyi』 and the papers which related with blood stasis from Korea and China. And clinical papers are searched in China Academic Journals(CAJ) of China National Knowledge Infrastructure(CNKI) to analyse the scope of modern diseases related with blood stasis. The features of blood stasis theory in 『Yilingaicuo』 expanded the range of existing theory. Wangqingren thought that chronic disease, weird disease, the disease of no effect from normal treatments were related with blood stasis. And he attached great importance to qi and blood and thought that the main pathogenesis of blood stasis was qi deficiency. And a lot of Astragalus membranaceus Bunge were combined in many prescriptions to reinforce qi. He also used different herbs according to the location of the disease. Musk and Allium fistulosum were used for the disease located at head or upper part of the patient's trunk. Bupleurum falcatum L., Aurantii Fructus Pericarpium and Platycodon grandiflorum A. De Candolle were used for the disease located at thorax. Cyperus rotundus L., Linderae Radix and Aurantii Fructus Pericarpium were used for the disease located at the stomach or below the costal angle. Foeniculi Fructus and Corydalis remota were used for the disease located at belly or lower part of the patient's trunk. Trogopterorum Faeces, myrrha, Cyperus rotundus L. and Cnidium officinale were used for the disease located at extremity or joint.

  • Langerhans cell histiocytosis(LCH) appears to arise from Langerhans cell and comprises a spectrum of clinical disease previously described in the literature by a variety of eponyms including histiocytosis X, eosinophilic granuloma, Hand-Schuller-Christian disease, and Letterer-Siwe syndrome. This rare disorder occurs in all groups, predominently affecting children & young adults. LCH has a wide spectrum of clinical features. The differentiation of several forms of this disease is primarily a clinical and not a histologic one. The radiographic characteristics include the appearance of solitary "intraosseous" lesions, the multiplicity of "alveolar bone" lesions, the bone lesions, periosteal new bone formation, and slight root resorption. Prognosis of a single bone lesion, is known to be excellent. In contrast, disseminated disease has seen associated with a chronic course, a high rate of morbidity and late consequences, and possible mortality. Treatment of LCH remains problematic. Treatment of multisystem disease, where organ function is being compromised has generally been with high-dose systemic corticosteroids or multiple chemotherapy.

  • Objective : This study was carried out with focus on written by Joh, Taek-seung (曺澤承) and Joh, Byeong-who(曺秉矦) in the relation of father and son in 1933. This book is a medical book including rare data, which has never been reported to academic circles all this while. Method : First, this study looked into the authors of this book and its history of publication. Further, this study analyzed the composition and contents of this book. Lastly, this study summed up the meaning of this book from the standpoint of medical history. Result : The authors were Confucian doctors who were active in the latter era of the Joseon Dynasty and also in the period of Japanese colonial rule. They lived in Haenam district of Jeonlanam-do, and cured its neighboring local residents while studying. They published the book of by putting together their own medical experiences. The authors suggested their remedial prescription according to gender and age whereas Zhang Zhongjing(張仲景) suggested the remedial prescription according to Six-Meridian Pattern Identification & Syndrome Differentiation(六經辨證). In addition, the authors of gave weight to the relationship with internal damage. Additionally, the authors not only thought much of the relationship between internal damage and external damage but also thought of the weakness and strength of the healthy qi, and the new and the old of a disease as an important clue to medical treatment. It seems that such contents was influenced by (東醫寶鑑). Conclusion : shows the results of the research on which was spontaneously conducted in Joseon.

A Case Report of Post-herpetic neuralgia uncontroled after Stellate Ganglion Block (성상신경절차단술로 제어되지 않은 대상포진후 신경통환자 치험 1예)

  • Post-herpetic neuralgia(PHN) is a chronic pain syndrome associated with the reactivation of a primary infection with varicella zoster virus(chinken pox), which leads to a chronic infection of dorsal root ganglia. The most common risk factor for shingles and its potential sequela, PHN, is advanced age. For a significant number of patients, the pain following healing of shingles can persist for months to years. If this pain, classified as PHN, persists longer than one month. PHN often leads to depression, disrupted sleep, decreased productivity, and utilization of health care. We treated a 60 year-old female patient who suffered pain and headache after Stellate Ganglion Blocks(SGB). In identifying points for differentiation of syndrom(辨證), this subject was diagnosed as Yangmyeong Merdian wind-heat syndrom(陽明經風熱證) and was administered Seungmagalgeuntanggamibang(revised Shengmagalgen-tang, 升麻葛根湯加味方). To ease pain, Western medication was administered as well. After fourteen days of treatment, pain and other symptoms improved.
