Radical surgery for stage IV gallbladder cancers: Treatment strategies in patients with limited metastatic burden

  • Backgrounds/Aims: The present study looked at the role of radical surgery in gallbladder cancers (GBC) with limited metastatic disease. Methods: The retrospective observational study was conducted to screen the database from 1st January 2010 to 31st December 2019. Patients of GBC found to have low-volume metastatic disease upon surgical exploration were included. Results: Of the 1,040 patients operated for GBC, 234 patients had low-volume metastatic disease (microscopic disease in station 16b1 node or N2 disease isolated port-site metastases, or low burden peritoneal disease with deposits less than 1 cm, in adjacent omentum or adjacent diaphragm or Morrison's pouch or a solitary discontinuous liver metastasis in adjacent liver parenchyma) detected intraoperative. Of these, 62 patients underwent radical surgery for R-0 metastatic disease followed by systemic therapy, while the remaining 172 patients did not undergo radical surgery and were given palliative systemic chemotherapy. Patients who underwent radical surgery had significantly superior overall survival (19 months versus 12 months, p < 0.01) and superior progression-free survival (10 months versus 5 months, p < 0.01) when compared to the rest. This difference in survival was more significant amongst patients when operated on after neoadjuvant chemotherapy. Regression analysis showed that a sub-group of patients with incidental GBC with limited metastases showed more favorable outcomes with radical surgery. Conclusions: Authors suggest a possible role for radical treatment of advanced GBC with a limited metastatic burden. Neoadjuvant chemotherapy can be used for preferentially selecting patients of favorable disease biology for curative treatment.

Differences in Treatment Outcomes According to the Insertion Method Used in Extracorporeal Cardiopulmonary Resuscitation: A Single-Center Experience

  • Background: Venoarterial extracorporeal membrane oxygenation (ECMO) is a key treatment method used with patients in cardiac arrest who do not respond to medical treatment. A critical step in initiating therapy is the insertion of ECMO cannulas. Peripheral ECMO cannulation methods have been preferred for extracorporeal cardiopulmonary resuscitation (ECPR). Methods: Patients who underwent ECPR at Daegu Catholic University Medical Center between January 2017 and May 2023 were included in this study. We analyzed the impact of 2 different peripheral cannulation strategies (surgical cutdown vs. percutaneous cannulation) on various factors, including survival rate. Results: Among the 99 patients included in this study, 66 underwent surgical cutdown, and 33 underwent percutaneous insertion. The survival to discharge rates were 36.4% for the surgical cutdown group and 30.3% for the percutaneous group (p=0.708). The ECMO insertion times were 21.3 minutes for the surgical cutdown group and 10.3 minutes for the percutaneous group (p<0.001). The factors associated with overall mortality included a shorter low-flow time (hazard ratio [HR], 1.045; 95% confidence interval [CI], 1.019-1.071; p=0.001) and whether return of spontaneous circulation was achieved (HR, 0.317; 95% CI, 0.127-0.787; p=0.013). Low-flow time was defined as the time from the start of cardiopulmonary resuscitation to the completion of ECMO cannula insertion. Conclusion: No statistically significant difference in in-hospital mortality was observed between the surgical and percutaneous groups. However, regardless of the chosen cannulation strategy, reducing ECMO cannulation time was beneficial, as a shorter low-flow time was associated with significant benefits in terms of survival.

  • This paper analyzes the contents and the effects of the UNSC 2270, and its implications to South Korea's defense strategy and navy. The UN Security Council passed strong sanctions against North Korea which punish North Korea's 4th nuclear test. The sanctions compared to the previous ones require international society to take practical actions such as comprehensive trade bans as well as diplomatic isolation which will put significant pains on North Korea. Especially, these measures would greatly hamper economic development policy of Kim Joung-un regime. Because Kim Jung-un regime has inherent legitimacy problems which stems from the third family succession of the power, economic difficulties may play an important cause on the regime instability in the long term. In fact, the United States sees this possibility as an option to coerce North Korea in which North Korea choose denuclearization for its regime survival. Nevertheless, the prospects of the UN sanctions are not so optimistic. Considering North Korea's willingness for nuclear development and its level of nuclear technology, North Korea will try to play a gambit with the US and South Korea by exploiting its strategic advantages. North Korea's response will have three following strategies. First, it would actively pursue political and economic survival strategy by using China's support for the regime, strengthening its power grip in the name of countering US hostile policy, and enhancing peace propaganda. Second, North Korea will accelerate efforts to position its status as a nuclear de facto state. For this purpose, it could create nuclear crisis on the peninsula. Third, it would exploit local provocations as an exit strategy to get over the current situation. In order to counter North Korea's actions and punish North Korea's behavior strongly, South Korea needs following strategies and efforts. It should first make all the efforts to implement the UN sanctions. Strong and practical nuclear deterrence strategy and capability with the U.S. should be developed. Effective strategy and capabilities for the prevention and deterrence of North Korea's provocation should be prepared. For this purpose, North Korea's provocation strategy should be thoroughly reviewed. Active international cooperation is needed to punish and coerce North Korea's behavior. Finally, South Korea should prepare for the possible occurrence of North Korea's contingency and make use of the situation as an opportunity to achieve unification. All these strategies and efforts demand the more active roles and missions of South Korea's navy and thus, nullify North Korea's intention militarily.

  • While the survival of extremely premature infants with respiratory distress syndrome has increased due to advanced respiratory care in recent years, necrotizing enterocolitis (NEC) remains the leading cause of neonatal mortality and morbidity. NEC is more prevalent in lower gestational age and lower birth weight groups. It is characterized by various degrees of mucosal or transmural necrosis of the intestine. Its exact pathogenesis remains unclear, but prematurity, enteral feeding, bacterial products, and intestinal ischemia have all been shown to cause activation of the inflammatory cascade, which is known as the final common pathway of intestinal injury. Awareness of the risk factors for NEC; practices to reduce the risk, including early trophic feeding with breast milk and following the established feeding guidelines; and administration of probiotics have been shown to reduce the incidence of NEC. Despite advancements in the knowledge and understanding of the pathophysiology of NEC, there is currently no universal prevention measure for this serious and often fatal disease. Therefore, new potential techniques to detect early biomarkers or factors specific to intestinal inflammation, as well as further strategies to prevent the activation of the inflammatory cascade, which is important for disease progression, should be investigated.

Success Factors of Immigrant-Owned Informal Grocery Shops in South African Townships: Native Shop-Owners' Account

  • Purpose - This paper explores the perceptions held by native grocery shop-owners of their foreign counterparts to ascertain if there are lessons to be shared especially regarding operational/survival strategies. Research design, data and methodology - A questionnaire was used to collect data from 121 subjects who were purposively selected. The data was processed and analysed with the aid of Statistical Package for the Social Sciences(SPSS) software. Results - The study found that native South Africans hold the perception that immigrants have operational advantage over them. The study also found that with bulk buying, immigrants' stores stock a larger variety of groceries while their items are sold cheaper. It is also arguable from the standpoint of the findings that immigrant grocery store owners avoid expensive lifestyles which often is common among natives who own informal grocery shops. Conclusions - It is the position of this paper that adopting the strategies utilized by immigrant entrepreneurs, informal grocery shops owned by native South Africans are likely to see an improvement in their township-based grocery stores. A major merit of this study beyond its novelty is its potential to advance learnership and collaboration between immigrant entrepreneurs and their native counterparts.

Studies on Vegetation for Ecological Restoration of Salt Marshes in Saemangeum Reclaimed Land - Germination Strategies and Character of Halophytes - (새만금 간척지일대 염습지 생태복원을 위한 식생학적 연구 - 염생식물 발아 전략 및 특성 -)

  • A study on vegetation in the Mangyeong River and Dongjin River basins and the surrounding regions of the Saemangeum Reclaimed Land was conducted in a series of efforts to determine the expected ecological changes in the salt marshes, to restore their vegetation, to explore the restoring force of halophyte, to examine the community mechanism and, ultimately, to rehabilitate marshy land vegetation along the lakeside, coastal dune and salt marshes of the Saemangeum Project Area. The findings of the study may be summed up as follows: Five species such as Suaeda japonica, Salicornia herbacea, Atriplex gmelini, Aster tripolium and Suaeda asparagoides that are mostly distributed in the estuary of the Saemangeum Reclaimed Land were analyzed to examine the mechanism of halophyte to maintain their community. To find out the strategies of plants for survival and the cause of forming community structure, a research was made as for appearance ratio of young sapling. From the results of laboratory analysis into dynamics of the saplings of halophyte, it was revealed that the germination ratio of the dry area and submerged area decreases in the order of Suaeda asparagoides, Suaeda japonica, Salicornia herbacea, Atriplex gmelini and Aster tripolium.

Cervical Cancer Trends in Mexico: Incidence, Mortality and Research Output

  • Purpose: To evaluate the recent incidence and mortality of and scientific research trends in cervical cancer in Mexican females. Materials and Methods: Data between 2000 and 2010 from the Department of Epidemiology of the Ministry of Health, and International Agency for Research on Cancer (IARC) of World Health Organization were analyzed, and age-standardized rates (ASRs) were calculated. In addition, scientific research data were retrieved from the Web of Science database from 2003 to 2012, using different terms related to cervical cancer. Results: The incidence rate decreased during last five years, while mortality rates showed an annual decrease of 4.93%. A total of 780 articles were retrieved, and the institutions with the majority of publications were National Autonomous University of Mexico (34.87%), Social Security Mexican Institute (16.02%), and National Institute of Cancerology (15%). The main types of research were treatment, diagnosis, and prevention. Conclusions: The above results show that incidence of cervical cancer decreased over time in Mexico during last five years; similarly, the downturn observed in mortality mainly reflects improved survival as a result of earlier diagnosis and cancer treatment. Also, this article demonstrates the usefulness of bibliometrics to address key evaluation questions and to establish priorities, define future areas of research, and develop cervical cancer control strategies in Mexico.

Problems and Solutions for Public Libraries in Korea (한국 공공도서관의 문제점과 개선방안)

  • The author explored the survival and prosperity strategies for Korean public libraries in the age of information technology. She reviewed that public libraries can perform these strategies or not. On the basis of the review, she identified 5 problems of public libraries : shortage of libraries and collections, staff inefficiency, inadequate services, inefficient design of library facilities. And then she proposed solutions to improve the present state of public libraries in Korea ; campaigning for constructing libraries, increasing the materials budget, turning nonprofessional work to professional work of staff, and constructing libraries for safety and preservation.
