가상 현실 시뮬레이션은 다양한 분야에서의 교육과 훈련에 활용이 되며, 특히 최근에는 의료 분야에서 많이 사용되고 있다. 교육/훈련용 시뮬레이터는 의사가 실제 수술 도구를 이용하여 실제 환자에 대해 처치를 하는 것과 같은 느낌이 나게 하는 촉감/역감 생성 및 영상/음향 출력 하드웨어와 여기에 실감 나는 영상과 촉감을 생성해주는 소프트웨어로 이루어진다. 기존의 시뮬레이터들은 수술 시에 사용되는 다양한 수술 도구들을 모사하기 위해 다양한 형태의 하드웨어들을 사용해야 하므로 복잡하고 비용이 많이 소요되는 문제가 있다. 이 논문에서는 포스 피드백 장치와 변형 가능한 햅틱 컨트롤러를 이용한 치과 수술 시뮬레이션 시스템을 제안한다. 햅틱 하드웨어들은 수술 도구와 수술 부위의 충돌 여부를 파악하고 그에 따른 저항감과 진동감을 제공한다. 특히 길이 변화, 굽힘과 같은 변형이 가능한 햅틱 컨트롤러는 여러 수술 도구들의 형태에 따라 느껴지는 다양한 감각을 표현할 수 있다. 사용자가 햅틱 피드백 장치를 조작하면 햅틱 피드백 장치의 움직임이나 버튼 클릭 등의 이벤트가 시뮬레이션 시스템에 전달되어 치과용 수술 도구와 구강 내부 모델들 사이의 상호작용이 발생하고 이에 따른 햅틱 피드백이 햅틱 피드백 장치로 전달된다. 이러한 기반 기술들을 활용하여 정교한 3차원 모델로 표현된 가상 환경에서 대표적인 치과 수술기법인 매복 사랑니 발치 수술의 현실적인 훈련 경험을 제공한다.
Purpose: The purpose was to investigate nurse staffing levels and patient outcomes(nosocomial infection, patient fall, pressure ulcer). Method: The subjects of this study were 305 nurses from 20 general hospitals who worked at 39 medical and surgical wards. Self-reporting questionnaire which was developed by the writer through preceding study was used. In data analysis, SPSS WIN 10.0. program was utilized for descriptive statistics, ANOVA. Result: The mean of patient-to-nurse ration was 5.2:1. 65% among 20 hospitals was over 300 beds, 90 was located in urban area and 55 was private hospitals. Patient-to-nurse ration of hospitals in under 300 beds or rural area or private ownership was lower than hospitals in 300 beds or urban area or public ownership. 89.9 among 39 wards was medical or surgical wards. The mean of length of stay, 8-14 days got a majority and showed higher patient-to-nurse ration. Of the general characteristics, rural was significantly hight to patient fall(F=3.205, p<.05), medical unit was significantly high to patient fall, pressure ulcer(patient fall: F=8.890, p<.001, pressure ulcer: F=3.399, p<.05) and over 15 days was significantly higher than under 14 days of the mean of length of stay. And there was significant relationship between over 6.0:1 and over 4.0:1 to less than 5.0:1(F=4.817,p<.01). Conclusion: This study has shown a relationship between patient-to-nurse ration and patient fall using not objective research tool but self-reporting questionnaire. Therefore further research is needed to study using objective research tool. Based on this study, the effect of nurse staffing levels on patient outcome also has to be studied.
Objective : Magnetic resonance imaging (MRI) grading systems using sagittal images are useful for evaluation of lumbar foraminal stenosis. We evaluated whether such a grading system is useful as a diagnostic tool for surgery. Methods : Between July 2014 and June 2015, 99 consecutive patients underwent unilateral lumbar foraminotomy for lumbar foraminal stenosis. Surgically confirmed foraminal stenosis and the contralateral, asymptomatic neuroforamen were assessed based on a 4-point MRI grading system. Two experienced researchers independently evaluated the MR sagittal images. Interobserver agreement and intraobserver agreement were analyzed using ${\kappa}$ statistics. Results : The mean age of patients (54 women, 45 men) was 62.5 years. A total of 101 levels (202 neuroforamens) were evaluated. MRI grades for operated neuroforamens were as follows : Grade 0 in 0.99%, Grade 1 in 5.28%, Grade 2 in 14.85%, and Grade 3 in 78.88%. Interobserver agreement was moderate for operated neuroforamens (${\kappa}=0.511$) and good for asymptomatic neuroforamens (${\kappa}=0.696$). Intraobserver agreement by reader 1 for operated neuroforamens was good (${\kappa}=0.776$) and that for asymptomatic neuroforamens was very good (${\kappa}=0.831$). In terms of lumbar level, interobserver agreement for L5-S1 (${\kappa}=0.313$, fair) was relatively lower than the other level (${\kappa}=0.804$, very good). Conclusion : MRI grading system for lumbar foraminal stenosis is thought to be useful as a diagnostic tool for surgery in the lumbar spine; however, it is less reliable for symptomatic L5-S1 foraminal stenosis than for other levels. Thus, various clinical factors as well as the MRI grading system are required for surgical decision-making.
This study was designed to test the possibility of using a standardized lateral facial photographs as a clinical tool which produce the prediction of postsurgical soft-tissue profile changes associated with surgical correction in skeletal CIII patients. The number of the patients involved in this study were 27 in total, including 11 male patients and 16 female patients. A practical method to the utilization of presurgical photo prediction for mandibular prognathic patients has been presented. To predict postoperative facial appearance, montage photographs were superimposed on standard facial reference photos taken preoperatively. Within the limitations of its technology, postoperative predictions generated by this method were of sufficient accuracy, especially mandible and chin area, for clinical use. In addition, they provide valuable communication and diagnostic information which may be used in formulating treatment plan in cases requiring corrective orthognathic surgery. But, the lip changes were somewhat exaggerated by photo prediction. Consequently, the photo prediction seems suitable for planning profile changes in orthognathic surgery that include mobilization of one main mandibular fragment. Futher investigations are needed to determine whether changes of soft-tissues and hard-tissues are sufficiently reproducible so that more meaningful predictive values can be established.
Since. its first description by Albrecht in 1904 that hamartoma (Greek. meaning "error" and "tumor")is tumor-like malformation characterized by abnormal mixture of normal constituent in any organ it arises, hamartoma involving the lung has assumed the following clinical features.: 1) all the pulmonary lesions presenting as "coin lesions", it occupies about 8% according to various reports. It also constitutes the most prevalent lesion of all the benign lung tumors, appearing in 0.25% of the general population. 2) Peak incidence of the lesion occurs in the 5th and 6th decade as the carcinoma patient do, and the occasional growing tendency and periperal location as in this presentation renders the differential diagnosis from the malignant disease difficult and surgical intervention inevitable for definite histological diagnosis and treatment. 3) Nearly all the clinical investigations such as history, physical exam, bronchoscopy, culture, and cytological exam of sputum sre likely to futile:the only tool for detecting the presence of the lesion is X-ray, butthat's short of telling the true nature of it. 4) Because of its age of presentation and growing tendency in middle-aged adult, it still controversial whether it is developmental anomaly as Albrecht described, or true neoplasm denovo of fibrous connective tissue origin. This is a case report of surgical experience of pulmonary hamartoma which had nearly all the typical clinical features above mentioned and was reviewed with related literatures.
A 67-year-old woman presented for evaluation of severe coccygeal pain. The computed tomography scans and magnetic resonance imaging showed an asymmetric midline sacral tumor invading the right lower portion of S2. To preserve both S2 nerve roots and to obtain negative surgical margins, a modified mid-sacrectomy with an aid of a computed navigation system was performed. The sacral tumor was excised en bloc with negative tumor margins. Both S2 nerve roots were preserved and additional reconstruction was not necessary because of minimal resection of the sacroiliac joint. We report a case of a sacral chordoma which was excised en bloc with adequate surgical margins by a computer-assisted modified mid-sacrectomy. The computed navigation system may be a useful tool for tumor targeting and safe osteotomies in sacral tumor surgery via the posterior only approach.

Although FDG PET cannot match the anatomic resolution of conventional imaging techniques in gastrointestinal and abdominal organs, it is particularly useful for identification and characterization of whole body at the same time. FDG PET can show foci of metastatic disease that may not be apparent at conventional anatomic imaging and can aid in the characterization of indeterminate soft-tissue masses. Most gastrointestinal cancer need to surgical management. FDG PET can improve the selection of patients for surgical treatment and thereby reduce the morbidity and mortality associated with inappropriate surgery. FDG PET is also useful for the early detection of recurrence and the monitoring of therapeutic effect. The gastrointestinal cancers, such as gastroeso-phageal cancer, colorectal cancer, liver cancer and pancreatic cancer, are common malignancies in Korea. PET is one of the most promising and useful methodology for the management of gastric cancer as well as other gastrointestinal cancers.

Background: Renal cell carcinoma has shown less response to systemic therapies including chemotherapy, radiation, and immunotherapy than other cancers. Surgery has therefore become an important treatment tool. The protocol for treatment is the same for pulmonary metastasis of renal cell carcinoma. We performed surgery for pulmonary metastatic renal cell carcinomas and analyzed the results. Materials and Methods: We retrospectively analyzed 15 patients who had undergone pulmonary metastasectomy from renal cell carcinoma at our hospital from January 2005 to December 2009. Results: No patients had extrathoracic metastatsis. The mean age was 60.2 years (range 35~73). There were 12 male and 3 female patients. The number of synchronous and metachronous patients were 8 and 7, respectively. The mean survival times of synchronous and metachronous patients were 32.6 and 42.9 months, respectively. 6 patients had single lesions and 9 patients had multiple (more than 3) lesions. The surgical procedures included wedge resection (10), lobectomy (2), wedge resection with segmentectomy (2), and segmentectomy (1). Median observation and survival time were 54.1 and 34.9 months. The 1-year and 3-year survival rates were 80% and 50%, respectively. Conclusion: Pulmonary resection for pulmonary metastatic renal cell carcinoma was found to be a safe and effective treatment modality when complete resection was performed.
The objective of this paper is to propose a new technology of the orthopaedic surgery using the combination of reverse engineering (RE) based on CT data and rapid prototyping (RP). The proposed technology utilizes symmetrical characteristics of the human body and capability of the combination of RE and RP, which rapidly manufactures three-dimensional parts from CT data. The original .stl data of injured extents are generated from the mirror transformation of .stl file fur uninjured extents. The physical shape before injuring is manufactured from RP using the original .stl data. Subsequently, pre-operative planning, such as a selection of proper implants, preforming of the implant, a decision of fixation locations and an insert position for the implant, an estimation of the invasive size, and pre-education of operators are performed using the physical shape. In order to examine the applicability and the efficiency of the proposed surgical technology, various case studies, such as a distal tibia commented fracture, a proximal tibia plateau fracture and an iliac wing fracture of pelvis, are carried out. From the results of case studies, it has been shown that the proposed technology is an effective surgical tool of the orthopaedic surgery reducing the operational time, the operational cost, the radiation exposure of the patient and operators, and morbidity. In addition, the proposed technology could improve the accuracy of operation and the speed of rehabilitation.
