Comparison of accuracy between free-hand and surgical guide implant placement among experienced and non-experienced dental implant practitioners: an in vitro study

  • Purpose: This study investigated the accuracy of free-hand implant surgery performed by an experienced operator compared to static guided implant surgery performed by an inexperienced operator on an anterior maxillary dental model arch. Methods: A maxillary dental model with missing teeth (No. 11, 22, and 23) was used for this in vitro study. An intraoral scan was performed on the model, with the resulting digital impression exported as a stereolithography file. Next, a cone-beam computed tomography (CBCT) scan was performed, with the resulting image exported as a Digital Imaging and Communications in Medicine file. Both files were imported into the RealGUIDE 5.0 dental implant planning software. Active Bio implants were selected to place into the model. A single stereolithographic 3-dimensional surgical guide was printed for all cases. Ten clinicians, divided into 2 groups, placed a total of 60 implants in 20 acrylic resin maxillary models. Due to the small sample size, the Mann-Whitney test was used to analyze mean values in the 2 groups. Statistical analyses were performed using SAS version 9.4. Results: The accuracy of implant placement using a surgical guide was significantly higher than that of free-hand implantation. The mean difference between the planned and actual implant positions at the apex was 0.68 mm for the experienced group using the free-hand technique and 0.14 mm for the non-experienced group using the surgical guide technique (P=0.019). At the top of the implant, the mean difference was 1.04 mm for the experienced group using the free-hand technique and 0.52 mm for the non-experienced group using the surgical guide technique (P=0.044). Conclusions: The data from this study will provide valuable insights for future studies, since in vitro studies should be conducted extensively in advance of retrospective or prospective studies to avoid burdening patients unnecessarily.

A 10-year follow-up study on clinical outcomes of dental implant rehabilitation using surgical guide

  • Objectives: The surgical guide is a static computer-assisted device used for implant surgery planning and guidance. By taking an impression and referring to the patients' three-dimensional computed tomography scan of the desired implant site, a surgical guide can be created. During surgery, the surgical guide aids in achieving the designed implant placement position and direction. We examined and evaluated the long-term clinical outcomes of implant surgery using surgical guides. Materials and Methods: This study investigated a total of 15 patients with 32 implants that were placed using surgical guides from 2009 to 2011 with a mean follow-up period extended beyond 10 years. Patient demographics and implant survival rates were recorded. We analyzed marginal bone loss (MBL) by assessing the radiographs acquired at installation, three months after installation, and one month, one, two, and five years after prosthesis delivery. Results: The mean patient age was 57.33 years at implant placement. Of the 32 implants, five implants were placed in the anterior region and 27 implants were in the posterior region. Six implants failed and three of them were replaced, resulting in an 81.25% survival rate. The mean follow-up period was 10 years and nine months. Mean MBL compared to post-installation was significantly higher than at three months after installation, and one month, one, two, and five years after prosthesis delivery. Mean MBL at three months after installation, and one month, one year, and two years were significantly higher compared to the previous visit (P<0.05). However, MBL at five years after prosthesis delivery did not differ significantly compared to at two years. Conclusion: In this study, implant rehabilitation assisted by surgical guides exhibited favorable survival rates. With the limitation of the sample amount in this study, further research and more samples are required to evaluate the long-term clinical effectiveness of surgical guides.

  • This clinical note introduces a method to assist surgeons in performing single-tooth dento-osseous osteotomy. For use in this method, a surgical guide was manufactured using computer-aided design/computer-aided manufacturing technology and was based on preoperative surgical simulation data. This method was highly conducive to successful single-tooth dento-osseous segmental osteotomy.

Mixed reality visualization in shoulder arthroplasty: is it better than traditional preoperative planning software?

  • Background: Preoperative traditional software planning (TSP) is a method used to assist surgeons with implant selection and glenoid guide-pin insertion in shoulder arthroplasty. Mixed reality (MR) is a new technology that uses digital holograms of the preoperative plan and guide-pin trajectory projected into the operative field. The purpose of this study was to compare TSP to MR in a simulated surgical environment involving insertion of guide-pins into models of severely deformed glenoids. Methods: Eight surgeons inserted guide-pins into eight randomized three-dimensional-printed severely eroded glenoid models in a simulated surgical environment using either TSP or MR. In total, 128 glenoid models were used and statistically compared. The outcomes compared between techniques included procedural time, difference in guide-pin start point, difference in version and inclination, and surgeon confidence via a confidence rating scale. Results: When comparing traditional preoperative software planning to MR visualization as techniques to assist surgeons in glenoid guide pin insertion, there were no statistically significant differences in terms of mean procedure time (P=0.634), glenoid start-point (TSP=2.2±0.2 mm, MR=2.1±0.1 mm; P=0.760), guide-pin orientation (P=0.586), or confidence rating score (P=0.850). Conclusions: The results demonstrate that there were no significant differences between traditional preoperative software planning and MR visualization for guide-pin placement into models of eroded glenoids. A perceived benefit of MR is the real-time intraoperative visibility of the surgical plan and the patient's anatomy; however, this did not translate into decreased procedural time or improved guide-pin position.

성장 장애를 가진 부분 무치악 환자에서 위축된 골에 무피판 임플란트 수술 증례 (Flapless implant surgery on atrophied alveolar ridge in a patient with growth disorder)

  • 성장장애는 두개안면부의 골격성장과 영구치의 조기상실에 영향을 미친다. 본 증례는 성장장애 환자에서 치아를 조기 상실한 부위에 무피판 술식으로 임플란트를 식립 시 컴퓨터 단층촬영(CT)을 통한 인접치아의 확인과 임플란트 가이드의 필요성을 살펴보았다. 특발성 저신장과 3급 정신지체의 20세의 여환에서 구치의 회복을 위해 임플란트 고정성 보철을 계획하였다. CT 영상에서 인접치가 심하게 협측으로 위치되어 있었고, 이를 반영하여 가이드를 제작후 임플란트를 식립하였다. 이상 위치된 인접치를 기준으로 임플란트를 식립할 경우 협측 골의 천공이 발생할 수 있다. 그러므로 성장장애 환자에서 치아 조기 상실부에 임플란트를 식립 시 CT를 통한 치아의 위치 확인과 가이드의 제작이 요구된다.

가상 수술 시뮬레이션과 3차원 프린팅 절골술 가이드를 이용한 교정 수술: 증례 보고 (Corrective Surgery Using Virtual Surgical Simulation and a Three-Dimensional Printed Osteotomy Guide: A Case Report)

  • A 74-year-old female patient, who underwent surgery for a left distal tibiofibular fracture 40 years earlier, visited the hospital with an ankle varus deformity due to malunion. The patient complained of discomfort while walking due to the ankle and hindfoot varus deformity but did not complain of ankle pain. Therefore, correction using supramalleolar osteotomy was planned, and through virtual surgical simulation, it was predicted that a correction angle of 24° and an osteotomy gap open of 12 mm would be necessary. An osteotomy guide and an osteotomy gap block were made using three-dimensional (3D) printing to perform the osteotomy and correct the deformity according to the predicted goal. One year after surgery, it was observed that the ankle varus was corrected according to the surgical simulation, and the patient was able to walk comfortably. Thus, for correction of deformity, virtual surgical simulation and a 3D-printed osteotomy guide can be used to predict the target value for correction. This is useful for increasing the accuracy of correction of the deformity.

  • Cone beam computerized tomography (CBCT)의 발전은 환자의 해부학적 구조를 3차원적으로 분석할 수 있게 하였다. Surgical guide는 CBCT와 CAD/CAM, 임플란트 진단 소프트웨어의 접목을 통해 미래의 보철물을 계획하고 적합한 위치에 임플란트를 식립할 수 있게 한다. Guided surgery를 통해 해부학적 구조물에 대한 침범을 최소한으로 줄일 수 있고 보다 재현성 있는 치료계획의 설정이 가능하다. 본 증례는 전악 무치악 환자에게 surgical guide를 이용하여 다수의 임플란트를 식립한 증례로 수술시간을 단축시킬 수 있었으며 임시 보철물을 미리 제작함으로써 보다 쉽게 immediate loading을 시행할 수 있었다. 환자는 개선된 안모와 저작기능에 만족하였다.

  • 연구 목적: 최근 정확하면서도 외상이 적은 임플란트 식립을 위해 임플란트 수술용 가이드가 많이 쓰이고 있다. 그 중 대표적인 방식이 stereolithography 방식과 positioning device로 제작한 방식이다. 본 연구는 이 두 가지 방식의 정확성을 비교하기 위하여 4 가지 형태의 치아상실 모델에서 실험 후 정확성에 관하여 분석하였다. 연구 재료 및 방법: 4가지 형태 치아 결손 모델 각각에 대하여 stereolithography 방법과 positioning device를 이용한 방법으로 수술 가이드를 제작하였다. 제작된 수술 가이드를 제작에 사용되었던 치아모델에 장착하여 수술 가이드의 내면과 모델의 치아표면 사이 공간크기를 측정하여 가이드의 적합도를 평가하였다. 또한 이 수술 가이드를 이용하여 총 40개 모델에서 임플란트 시술을 진행하고, 시술 후 모델을 다시 cone-beam computed tomography촬영 하여 수술 전후 영상을 중첩시켜 계획 상의 임플란트와 실제 시술한 임플란트의 위치를 비교하였다. 통계학적인 검증을 위하여 PASW Statistics$^{(R)}$ 18.0을 이용하여서 Mann-Whitney U 검정을 사용하였다. 결과: 수술 가이드 내면과 모델 사이 공간크기가 stereolithography는 $1.4{\pm}0.3mm$이고, positioning device는 $0.4{\pm}0.3mm$로 통계학적으로 유의한 차이를 보였다(P<.05). Stereolithography는 오차가 근원심 측에서 $3.9{\pm}1.6^{\circ}$, 협설 측에서 $2.7{\pm}1.5^{\circ}$, 깊이에서 $1.9{\pm}0.9mm$였다. 반면 positioning device는 오차가 근원심 측에서 $0.7{\pm}0.3^{\circ}$였고, 협설측에서 $0.3{\pm}0.2^{\circ}$, 깊이에서 $0.4{\pm}0.2mm$였다. 두 수술 가이드 제작방법은 모든 방향에서의 오차에서 통계학적으로 유의한 차이를 보였다(P<.05). 결론: 본 연구의 결과는stereolithography방법보다 positioning device를 이용하여 수술 가이드를 제작할 때 C.T.와 임플란트 치료 계획 프로그램을 이용하여 미리 계획한 임플란트 식립 위치와 방향으로 더 정확하게 식립 할 수 있음을 보여 주었다.

The application of "bone window technique" using piezoelectric saws and a CAD/CAM-guided surgical stent in endodontic microsurgery on a mandibular molar case

  • Apical surgery for a mandibular molar is still challenging for many reasons. This report describes the applications of computer-guided cortical 'bone-window technique' using piezoelectric saws that prevented any nerve damage in performing endodontic microsurgery of a mandibular molar. A 49-year-old woman presented with gumboil on tooth #36 (previously endodontically treated tooth) and was diagnosed with chronic apical abscess. Periapical lesions were confirmed using cone-beam computed tomography (CBCT). Endodontic microsurgery for the mesial and distal roots of tooth #36 was planned. Following the transfer of data of the CBCT images and the scanned cast to an implant surgical planning program, data from both devices were merged. A surgical stent was designed, on the superimposed three-dimensional model, to guide the preparation of a cortical window on the buccal side of tooth #36. Endodontic microsurgery was performed with a printed surgical template. Minimal osteotomy was required and preservation of the buccal cortical plate rendered this endodontic surgery less traumatic. No postoperative complications such as mental nerve damage were reported. Window technique guided by a computer-aided design/computer-aided manufacture based surgical template can be considerably useful in endodontic microsurgery in complicated cases.

양악 무치악 환자에서 NobelGuideTM 시스템을 이용한 고정성 임플란트 보철수복 (Fixed Prosthetic Restoration in an Edentulous Patient with NobelGuideTM System)

  • 임플란트 술식에서 방사선 영상 기술, 컴퓨터 소프트웨어의 발전으로 정확한 진단 및 surgical guide의 제작이 가능해졌다. 본 증례는 양악 무치악 환자에서 고정성 임플란트 보철을 위해 CAD/CAM technique을 이용하여 수술을 하고 즉시 하중을 가한 증례이다. Planning software program을 이용하여 해부학적 구조물과 단면상을 고려하여 상 하악에 각각 6개씩의 임플란트를 최적의 위치에 계획하였다. 정밀한 surgical guide 이용하여 미리 계획된 위치와 방향으로 무절개 임플란트 식립 수술을 시행하였다. 즉시 사용 가능한 고정성 임시 보철물을 미리 제작해 수술 직후 장착하여 환자의 만족도를 높였으며, 이를 6개월간 평가하여 심미적이며, 기능적으로 안정적인 최종 보철물을 제작할 수 있었다.
