무치악 구개결손 환자를 위한 폐쇄장치의 삼차원 유한요소 분석 (A three-dimensional finite element analysis of obturator prosthesis for edentulous maxilla)

  • 연구 목적: 이 연구의 목적은 삼차원 유한요소 분석법을 이용하여 무치악 구개결손 환자에 사용되는 폐쇄장치의 응력 및 변위를 측정하고 이를 총의치 모형과 비교하는 것이다. 연구 재료 및 방법: 무치악 환자의 컴퓨터단층촬영 영상을 기반으로 구개 중앙에 상악절제술 후 발생한 결손부를 가지는 무치악 상악모형과, 무치악 3차원 유한요소 모형을 제작하였다. 각각의 모형에 적합한 총의치와 폐쇄장치를 제작하고 좌측 소구치와 구치 부위에 200 N의 수직하중을 가하였다. 3차원 유한요소 분석법을 이용하여 두 모형에서 보철물과 잔존 상악골조직의 von Mises 응력값과 변위량을 측정하였다. 결과: 두 모형 모두 하중을 가한 쪽의 피질골에서 최대 von Mises 응력 값이 관찰되었다. 총의치 모형의 피질골에서 가장 높은 값은 2.73 MPa 이었으며 폐쇄장치 모형의 피질골에서 가장 높은 값은 2.69 MPa 이었다. 보철물의 조직면에서도 높은 응력값이 관찰되었다. 최대 변위량은 폐쇄장치에서 총의치보다 더 높은 값이 관찰되었다. 결론: 폐쇄장치는 총의치에 비해 교합력 전달 면에서 불리했고, 더 큰 변위량이 관찰되어 상악절제술을 받은 무치악 환자의 보철치료 시 해부학적 지식에 바탕을 둔 정확한 인상채득과 보철치료 원칙의 적용이 고려되어야 한다.

최신 근관 세척 방법과 기구에 대한 고찰 (Review of root canal irrigant delivery techniques and devices)

  • 서론: 근관 치료의 성공을 위해 근관계 내에 남아있는 조직 잔사와 박테리아를 제거하는 것은 매우 중요하다. 그러나 충분한 근관 형성과 근관 세척을 하더라도 근관의 복잡한 형태로 인하여 근관 내 박테리아의 biofilm이나 괴사된 치수 조직이 남아 있게 된다. 근래 보다 효과적으로 근관 세척을 하기 위한 여러 가지 방법과 근관 세척을 위한 기구들이 개발되었다. 본 종설에서는 근관 치료 영역에서 사용되는 근관 세척 방법과 기구에 대해 고찰하고자 한다. 본론: 아래와 같은 기구와 방법이 논의된다. - syringe-needle irrigation, manual dynamic irrigation, brushes - sonic and ultrasonic irrigation, passive ultrasonic irrigation, rotary brush, RinsEndo, EndoVac, Laser 결론: 최근 근관 세척을 위한 기구와 세정 방법이 개발되었으나, syringe와 needle을 이용한 근관 세척법과 ultrasonic을 이용한 근관 세척법을 제외하고는 새로 개발된 많은 기구들의 임상적 치료 결과의 향상에 대한 근거 중심 연구가 아직은 부족하며, 어떠한 기구나 장치도 근관 내 잔사를 완벽하게 제거할 수는 없다. 효과적인 근관 세척을 위해서는 적절한 근관 세척제의 선택과 함께, 근관 세척제를 근관장까지 충분한 양이 도달할 수 있도록 적절한 근관 세척 기구와 방법을 선택하여야 한다.

백서의 척수강 내로 장기간 투여한 Cannabinoids의 진통 효과 및 독성에 관한 연구 (Study for the Antinociceptive Effect and Toxicity of Chronic Intrathecal Infusion of Cannabinoids in Rats)

  • Background: Cannabinoids have shown antinociceptive action. The aims of this study were to examine the effect of chronic infusion of a cannabinoids receptors agonist (WIN 55,212-2) for thermal nociception at the spinal level, and to also observe the development of toxicity. Methods: Male Sprague-Dawley rats were implanted with lumbar intrathecal catheters with the nociceptive response (withdrawal response latency) determined by exposing the plantar surface of the hindpaw to radiant heat. Initially, the effect of intrathecal WIN 55,212-2 was evaluated followed by the change in the effect at 1, 2, 3 and 4 weeks after repeated infusion. Finally, the histopathological findings were assessed 1 and 4 weeks following the infusion of WIN 55,212-2. Results: Intrathecal WIN 55,212-2 was found to produce a limited antinociception during the thermal test. %MPE of WIN 55,212-2 at 1, 2, 3, and 4 weeks after infusion was not different from each other. No abnormal pathological findings were observed following a chronic intrathecal infusion of WIN 55,212-2. Conclusions: WIN 55,212-2, a cannabinoids receptors agonist, may be useful in the management of thermal nociception, without changing the effectiveness or causing the toxicity following a chronic infusion at the spinal level.

Ganoderma lucidum Pharmacopuncture for the Treatment of Acute Gastric Ulcers in Rats

  • Objectives: The gastric ulcer is a common disorder of the stomach and duodenum. The basic physiopathology of a gastric ulcer results from an imbalance between some endogenous aggressive and cytoprotective factors. This study examined whether Ganoderma lucidum pharmacopuncture (GLP) would provide protection against acute gastric ulcers in rats. Methods: Sprague-Dawley rats were divided randomly into 4 groups of 8 rats each: normal, control, normal saline (NP) and GLP groups. The experimental acute gastric ulcer was induced by using an EtOH/HCl solution and the normal group received the same amount of normal saline instead of ethanol. The NP and the GLP groups were treated once with injections of saline and GLP, respectively. Two local acupoints were used: CV12 (中脘) which is the alarm point of the Stomach Meridian, and ST36 (足三里), which is the sea point of the Stomach Meridian. The stomachs from the rats in each group were collected and analyzed for gross appearance and histology. Also, immunohistochemistry staining for BAX, Bcl-2 and TGF-${\beta}1$ was performed. Results: Histological observations of the gastric lesions in the control group showed comparatively extensive damage of the gastric mucosa and necrotic lesions had penetrated deeply into the mucosa. The lesions were long, hemorrhagic, and confined to the glandular portions. The lesions were measured microscopically by using the clear depth of penetration into the gastric mucosal surface. The length and the width of the ulcer were measured and the inhibition percentage was calculated. Wound healing of the acute gastric ulcer was promoted by using GLP, and significant alterations of indices in gastric mucosa were observed. Such protection was shown by gross appearance, histology and immunohistochemistry staining for BAX, Bcl-2 and TGF-${\beta}1$. Conclusion: These results suggest that GLP administered at CV12 and ST36 can provide significant protection to the gastric mucosa against an ethanol-induced acute gastric ulcer.

  • 구순열 수술에서 인중의 해부, 형태, 기능적 재건에 도움을 받고자 상순 및 하순의 전층표본을 가지고 미세구조를 관찰하였으며, 자연스럽고 미적인 인중을 재건하기 위해서는 정상적 근육 부착의 재건과 구륜근 표층의 진피 부속물들의 회복과 장력 없는 상순의 접합, 진피 부속물들을 포함한 인중융기의 두께 회복 및 비탄력성 흉터 조직의 최소화가 필요하다고 사료되었다.

Morphometric analysis of maxillary alveolar regions for immediate implantation

  • PURPOSE. The purpose of this study was to provide an actual guideline in determining the shape, diameter, and position of the implant in immediate implantation by the measurement of the thickness of facial and palatal plate, the thickness of cortical bone on the facial and palatal plate, the diameter of the root, and the distance between the roots in the cadavers. MATERIALS AND METHODS. The horizontal sections of 20 maxillae were measured and analyzed to obtain the average values. Resin blocks were produced and cut serially at 1 mm intervals from the cervical line to the root apex. Images of each section were obtained and the following measurements were performed: The thickness of the facial and palatal residual bone at each root surface, the thickness of the facial and palatal cortical bone at the interdental region, the diameter of all roots of each section on the faciopalatal and mesiodistal diameter, and the interroot distance. Three specimens with measurements close to the average values were chosen and 3-dimensional images were reconstructed. RESULTS. The thickness of the facial and palatal cortical bone at the interdental region in the maxilla, the buccal cortical bone was thicker in the posterior region compared to the anterior region. The interroot distance of the alveolar bone thickness between the roots increased from anterior to posterior region and from coronal to apical in the maxilla. CONCLUSION. In this study, the limited results of the morphometric analysis of the alveolar ridge using the sections of maxilla in the cadavers may offer the useful information when planning and selecting optimal implant for immediate implantation in the maxilla.

경골 근위부 골채취를 위한 내측 및 외측 접근법시의 삼차원적 길이계측 (THREE DIMENSIONAL LINEAR MEASUREMENT OF PROXIMAL TIBIA IN MEDIAL AND LATERAL APPROACH FOR BONE HARVESTING)

  • Purpose: The aim of this study was simply assessing linear measurements in the lateral and medial approach, respectively, for bone harvesting using anatomic and three-dimensional(3D) computed tomographic(CT) analyses on a dried cadaveric proximal tibia. In addition, the availability of the three-dimensional computed tomographic(3D-CT) analysis was also estimated. Materials and methods: Ten dried proximal tibia were obtained from five Korean cadavers. Four the reference points, the SM(superior-medial), IM(inferior-medial), SL(superior-lateral), and IL(inferior-lateral) were marked around the tibial tuberosity. The PM(posterior-medial) and PL(posterior-lateral) points were randomly marked at points farthest from the lateral and medial reference points, respectively, in the posterior border of the superior articular surface of both condyles. All measurements were obtained on the dried proximal tibia. After computed tomography had been performed, the three dimensional images were reconstructed using V works $4.0^{TM}$(Cybermed Inc., Seoul, Korea), and the length between the reference points were measured three dimensionally using the method described above. The error between the mean actual and mean 3D-CT measurements was calculated in order to determine the availability of the three dimensional computed tomographic analysis. Results: The length between the reference points was greatest at the IL-PM, which averaged $65.39mm{\pm}10.35$. This was followed by the SL-PM with $63.24mm{\pm}8.10$, the IM-PL with $58.09mm{\pm}10.02$, and the SM-PL with $51.99mm{\pm}9.06$. The differences between the IL-PM and SM-PL were 13.4 mm. The mean values were 55.04 mm in the medial approach and 64.32 mm in the lateral approach, and the differences between medial and lateral were 9.28 mm. The error between the mean actual and mean 3D-CT measurements was 0.31% and the standard deviation was 0.28%. Conclusion: The anatomical and three dimensional computed tomographic analysis indicates that there was only a 9.28 mm linear difference between the lateral and medial approach. This is consistent with previous studies, which showed that there was little difference between the two approaches in terms of the bone volume. In addition, the error(0.31%) and the standard deviation(0.28%) were considered low, demonstrating high accuracy of 3D-CT. Therefore it can be used in preoperative treatment planning.

Expression of TRPV1 and iNOS in the Dorsal Root Ganglion Exposed by Autologous Nucleus Pulposus in the Rat

  • Purpose: To determine whether upregulation of inducible nitric oxide synthase (iNOS) transcription and translation is related to radicular pain in a model of lumbar disc herniation. Also, to investigate the temporal changes of mRNA expression of iNOS and the identity of iNOS and transient receptor potential vanilloid (TRPV) 1 channel expression cells in dorsal root ganglion (DRG) of a model of lumbar disc herniation. Methods: A lumbar disc herniated rat model was developed by implantation of the autologous nucleus pulposus, harvested from the coccygeal vertebra of each tail, on the left L5 nerve root just proximal to the DRG. Rats were tested for mechanical allodynia of the plantar surface of both hind paws 2 days before surgery and 1, 5, 10, 20 and 30 days postoperatively. Reverse transcription polymerase chain reaction (RT-PCR) was used to follow iNOS mRNA expression. To stain iNOS and TRPV1 in DRG, an immunohistochemical study was done 10 days after surgery. Results: A significant drop in mechanical withdrawal threshold on the ipsilateral and contralateral hind paws was observed 1 day after surgery and was prolonged to 30 days in rats with lumbar disc herniation. The expression of mRNA for iNOS peaked at postoperative day 10 on both sides of the DRG. iNOS-positive sensory neurons in the DRG varied in size from large to small diameter cells. A majority of small and intermediate sensory neurons were TRPV1-positive cells. Double immunofluorescence staining for TRPV1 and iNOS revealed that most intermediate TRPV1-positive sensory neurons co-localized with iNOS-positive neurons. Conclusion: Nucleus pulposus-induced mechanical allodynia can be generated without mechanical compression. This pain is related to temporal changes in expression of iNOS mRNA in the DRG. Co-localization of TRPV1 and iNOS in intermediate neurons of the DRG is correlated with pain modality and intensity.

저온 상압플라즈마에 의한 Hairless Mouse-2 마우스 조직의 Candida albicans 사멸 효과 (The Killing Effect of Candida albicans on Hairless Mouse-2 Mouse Tissues by Non-Thermal Atmospheric Pressure Plasma)

  • 본 연구는 저온 상압 플라즈마 장치를 이용하여 구강점막질환을 일으키는 C. albicans 균을 효과적으로 사멸하기 위해 시행하였다. 조직에 적합하게 처리될 수 있도록 저온 상압 플라즈마 장치를 고안하고, 먼저 agar plate에 C. albicans 균을 처리하여 플라즈마를 조사한 결과 agar plate표면에 C. albicans 균을 처리 후 저온 상압 플라즈마 장치를 적용한 결과 60초 처리시 1.2 cm, 180초 처리시 1.4 cm, 300초 처리시 1.7 cm의 박테리아 생장 억제 구간이 나타나는 것을 확인하였다. 또한, 조직에서의 구강병원균 사멸 효과를 확인하기 위해 HRM-2 마우스 조직에 C. albicans 균을 처리하여 저온 상압 플라즈마를 조사 시 마우스 조직 표면에 C. albicans 균을 오염시켜 저온 상압 플라즈마 처리 후 CFU 방법으로 측정한 결과 300초간 1회 처리시 2 log CFU/ml, 300초간 2회 처리시 3 log CFU/ml, 300초간 3 회 처리시 6 log CFU/ml의 균 수 감소 효과가 나타나는 것을 확인하였다(p<0.05). 따라서, 저온의 저온 상압 플라즈마 장치는 효과적으로 구강 병원균을 사멸시킬 수 있으며, 구강점막질환 치료 장비로서 사용될 수 있을 것으로 생각된다.

대기오염에 의한 폐장조직 손상 -연구방향의 설정을 위한 논의- (Human Lung Insults due Air Pollutant -A Review for Priority Setting in the Research-)

  • Much progress has been made in understanding the subcellular events of the human lung injuries after acute exposure to environmental air pollutants. Host of those events represent oxidative damages mediated by reactive oxygen species such as superoxide, hydrogen peroxide, and the hydroxy, free radical. Recently, nitric oxide (NO) was found to be endogenously produced by endothelial cells and cells of the reticulo-endothelial system as endothelialderived relaxation factor (EDRF) which is a vasoactive and neurotransmitter substance. Together with superoxide, NO can form another strong oxidant, peroxonitrite. The relative importance of exogenous sources of $N0/N0_2$ and endogenous production of NO by the EDRF producing enzymes in the oxidative stresses to the heman lung has to be elucidated. The exact events leading to chronic irreversible damage are still yet to be known. From chronic exposure to oxidant gases, progressive epithelial and interstitial damages develop. Type I epithelial cells become thicker and cover a smaller average alveolar surface area while thee II cells proliferate instead. Under acute damages, the extent of loss of the alveolar epithelial cell lining, especially type II cells appears to be a good predictor of the ensuing irreversible damage to alveolar compartment. Interstitial matrix undergo remodeling during chronic exposure with increased collagen fibers and interstitial fibroblasts. However, Inany of these changes can be reversed after cessation of exposure. Among chronic lung injuries, genetic damages and repair responses received particular attention in view of the known increased lung cancer risks from exposure to several air pollutants. Heavy metals from foundry emission, automobile traffics, and total suspended particulate, especially polycystic aromatic hydrocarbons have been positively linked with the development of lung cancer. Asbestos in another air pollutant with known risk of lung cancer and mesothelioma, but asbestos fibers are nonauthentic in most bioassays. Studies using the electron spin resonance spin trapping method show that the presence of iron in asbestos accelerates the production of the hydroxy, radical in vitro. Interactions of these reactive oxygen species with particular cellular components and disruption of cell defense mechanisms still await further studies to elucidate the carcinogenic potential of asbestos fibers of different size and chemical composition. The distribution of inhaled pollutants and the magnitude of their eventual effects on the respiratory tract are determined by pollutant-independent physical factors such as anatomy of the respiratory tract and level and pattern of breathing, as well as by pollutant-specific phyco-chemical factors such as the reactivity, solubility, and diffusivity of the foreign gas in mucus, blood and tissue. Many of these individual factors determining dose can be quantified in vitro. However, mathematical models based on these factors should be validated for its integrity by using data from intact human lungs.
