  • Supernumerary teeth are extra teeth or tooth-like structures which may have either erupted or unerupted in addition to the 20 deciduous teeth and the 32 permanent teeth. Clinicians sometimes confront unerupted supernumerary teeth where implants would be placed. Many clinicians consider immediate placement of implant into an extracted socket as an effective technique compared to the conventional method of waiting several months for bone healing, because of the advantages such as reduction of treatment period, preservation of alveolar bone and soft tissue, etc. We could hardly find the case of immediate placement of implant after extraction of impacted supernumerary teeth and permanent teeth together. Therefore we report the case of immediate implantation following extraction of impacted supernumerary teeth and poor periodontal conditioned permanent teeth.

  • In this article, we found the dentigerous cyst due to the impacted supernumerary tooth. Involved teeth were treated with immediat canal filling and root resection technique. The cystic walls and its contents were marsupialized and perfectly enucleated with surgical techniques. After the operations, no clinical signs and symptoms were found, Radiographic findings after I months of the operations reveal the decrease of the radiolucency on the surgical field. The author thinks that it indicate the formation of the osteoid tissue.

  • The prevalence of the supernumerary tooth is $1{\sim}4%$. It usually occurs in the permanent dentition. It frequently occurs in the maxillary mid palatal area. The presence of the supernumerary tooth causes the following problems ; (1) interfering the eruption of successional tooth, (2) displacing the neighboring teeth, (3) resulting large diastema, (4) forming cystic change. So the supernumerary tooth should be removed as soon as possible. To extract the supernumerary tooth, the exact position must be noticed first. Radiographic techniques that were used in the past are tube shift technique, right angle technique, sterioradiography, using radiopaque contrast media and conventional tomography. But these methods include the subjective opinion of the operator. So, a technique eliminating the operator's opinion and showing the position 3-dimentionally can be used. 3-dimentional computed tomography equipped with dental softwares can show the position of the impacted supernumerally tooth in 3-dimentional position. It has an advantage to eliminate the subjective opinion of the operator. With a fast computer this techenique is done in a relatively short period of time. The rationale of this methods is relatively simple. After tacking X-ray and reconstructing the data 3-dimentionally, sequential removal of the soft tissue and hard tissue(bone) CT number leaves the teeth alone in 3-dimentional position. The image can be seen from anywhere, so the operator can see the image in front, rear, upper, and lower positions. In both cases 1 and 2, the position of the impacted supernumerary tooth is viewed by the 3-dimensional computed tomography. And it made the operator easy to figureout the exact position.

  • Supernumerary teeth are teeth added to the normal complement of teeth. They are most often found in the maxillary anterior region. Most supernumerary teeth in the maxillary anterior region have complications, such as impaction or malposition of permanent teeth, formation of diastema, cysts, and eruption into the nasal cavity or into the oral cavity. These complications have influence on deciding the treatment and its prognosis is depends on how these complications are treated. In clinical studies, it has been found that the removal of supernumerary teeth before the age of 5 years resulted in significant fewer eruption problems of the permanent tooth compared to removal at 7 years of age or later. However, such an early removal may leads to serious consequences of tooth development and behavioral management of patients. Therefore clinicians must consider complications of supernumerary tooth when deciding the time of removal. These cases report five-type of complication associated with supernumerary tooth such as diastema, delayed eruption, external root resorption, migration into nasal cavity and cyst formation.

  • A Case of a supernumerary teeth on the buccal side of upper right and left molar teeth was observed from a 21 years young Korean female. The characteristics were as follows : 1. The site of the occurrence were resemble on the right and left sides. 2. The cusps of the supernumerary tooth on the right side were resemble to the premolar, and the cusp of the left one was added by one more cusp than the normal molar tooth. 3. The supernumerary teeth were smaller than the normal teeth size. 4. The right and left teeth had only one root.

  • We decided among extraction, orthodontic traction and autotransplantation such as direction and position of unerupted tooth, degree of developing root apex, eruption space, being of supernumerary tooth or odontoma or cyst when tooth impacted. Autotransplantation is considered when orthodontic traction is unrealistic or when tooth movement can absorb root of neighbor tooth. The prognosis for successful autotransplantation is dependent on a number of factors such as root development, surgical technique, patient's age, endodontic treatment, time and type of splinting, preservation of periodontal ligament and storage medium. Especially when severe osseous defect is being, bone graft considered for reducing of mobility and for assisting recovery. In all cases, chief complaint is unerupted tooth and various causing factors were supernumerary, odontoma, ectopia and so on. Before autotransplantation, space regaining was done if needed and demineralized freezed dried bone and autogenous bone graft was done when there is severe osseous defect by extraction of supernumerary tooth or odontoma. Splinting was removed after 2-3weeks At 3-4weeks after autotransplantation, endodontic treatment was decided. At follow up check, normal recovery was done and there was no inflammatory or replacement root resorption in periapical radiograph.

  • The supernumerary tooth which is extra tooth in comparison to normal dentition is one of the developmental problems. The most common type of supernumerary tooth is mesiodens which may cause several complications like delayed eruption, crowding, spacing et al. Moral Incisor Hypomineralization (MIH) describes the clinical appearance of enamel hypomineralization of systemic origin affecting one or more permanent first molars that associated frequently with affected incisors. We report a case of a 6 - year - old boy who visited our clinic for removal of mesiodentes. The patient was diagnosed by mesiodentes and MIH by clinical examination and radiographic examination. Under local anesthesia, Mesiodentes were removed surgically. The demarcated opacities, a feature of MIH, were observed in the removed mesiodentes. After removal of mesiodentes, the maxillary central incisors erupted normally and in order to manage the teeth affected MIH, follow-up and fluoride varnish application were done every 3 months.

Characterization of Odontoblasts in Supernumerary Tooth-derived Dental Pulp Stem Cells between Passages by Real-Time PCR (과잉치 치수유래 줄기세포의 Real-time PCR에 의한 계대간 상아질모세포 발현 특성)

  • The aim of this study is to compare the properties of odontoblast gene of early passage cells and late passage cells derived from impacted maxillary supernumerary teeth. Impacted supernumerary teeth with maxilla were extracted from 12 patients (8 males, 4 females) between 6 - 9 years old without medical history. Real-time polymerase chain reaction (PCR) was conducted to compare characterization of odontoblast cell in the 3rd and 10th passage, and between with bone inducing additive group and without additive group. Genes for odontoblasts characteristics are osteonectin (ONT), alkaline phosphatase (ALP), osteocalcin (OCN), dentin matrix protein 1 (DMP-1) and dentin sialophosphoprotein (DSPP). The level of gene expression was in a decreasing order of ONT, ALP, OCN, DMP-1 and DSPP in the 3rd passage, and in decreasing order of ONT, DMP-1, OCN, ALP, and DSPP in the 10th passage in the undifferentiation and differentiation group. The order of ONT, DMP-1, and OCN did not changed. ALP and DMP-1 were switched in order. ALP and DMP-1 may be used as important markers for differentiating between the 3rd passage and 10th passage cells. Considering that supernumerary tooth was extracted young age and the time required to cultured 10th passage was short, supernumerary tooth can be considered a useful donor site of dental pulp stem cells.

Common dental anomalies in Korean orthodontic patients: An update

  • Objective: The aim of this study was 1) to investigate the prevalence and pattern of dental anomalies (DAs), 2) to compare DAs according to the type of malocclusion, and 3) to investigate the correlation between tooth impaction and other DAs in the Korean orthodontic population. Methods: A total of 3,240 orthodontic patients were classified as Class I, Class II, or Class III malocclusion groups. The presence and location of common DAs, including impaction, microdontia, agenesis, supernumerary tooth, transposition, and fusion, were identified by examining diagnostic records. Furthermore, samples were classified as Group 1 without impaction or Group 2 with impaction. The prevalence of other DAs concurrent with impaction was investigated and compared to Group 1. Results: Impaction was the most prevalent DA, followed by microdontia, agenesis, and supernumerary. Class I and Class III groups showed the same order of prevalence, but agenesis was more frequent than microdontia in the Class II group. The prevalence of the four DAs was lowest in the Class III group. Overall, 8.6% of patients were classified into Group 2. The incidence of DAs other than impaction and the prevalence of multiple concurrent DAs were significantly higher in Group 2. Impaction showed a significant relationship with supernumerary tooth, transposition, and fusion. Conclusions: The prevalence and pattern of DAs varied depending on the type of malocclusion. As there was a higher risk of other DAs in patients with impacted teeth, early detection of the impacted tooth and a detailed diagnosis of other possible DAs may be essential.

Effects of nanoscale ridge/groovepattern arrayed surface on in vitro differentiation of multi-potent pulp cells derived from human supernumerary teeth

  • Human dental pulp stem cells (DPSCs) are multi-potent mesenchymal stem cells that have several differentiation potentials. An understanding of thetissues that differentiate from these cells can provide insights for future regenerative therapeutics and tissue engineering strategies. The mesiodens is the most frequent form of supernumerary tooth from which DPSCs can differentiate into several lineages similar to cells from normal deciduous teeth. Recently, it has been shown that nanoscale structures can affect stem cell differentiation. In our presentstudy, we investigated the effects of a 250-nm nanoscale ridge/groove pattern array on the osteogenic and adipogenic differentiation of dental pulp cells from mesiodenscontaining human DPSCs. To this end, the expression of lineage specific markers after differentiation induction was analyzed by lineage specific staining and RT-PCR. The nanoscale pattern arrayed surface showed apositive effect on the adipogenic differentiation of DPSCs. There was no difference between nanoscale pattern arrayed surface and conventional surface groups onosteogenic differentiation. In conclusion, the nanoscale ridge/groove pattern arrayed surface can be used to enhance the adipogenic differentiation of DPSCs derived from mesiodens. This finding provides an improved understanding of the effects of topography on cell differentiation as well as the potential use of supernumerary tooth in regenerative dental medicine.
