Evaluation of diagnostic ability of CCD digital radiography in the detection of incipient dental caries (CCD 디지털 방사선사진촬영법의 초기 치아우식증의 진단능 평가에 대한 연구)

  • Purpose : The purpose of this experiment was to evaluate the diagnostic ability of a CCD-based digital system (CDX-2000HQ) in the detection of incipient dental caries. Materials and Methods : 93 extracted human teeth with sound proximal surfaces and interproximal artificial cavities were radiographed using 4 imaging methods. Automatically processed No.2 Insight film (Eastman Kodak Co., U.S.A.) was used for conventional radiography, scanned images of conventional radiograms for indirect digital radiography were used. For the direct digital radiography, the CDX-2000HQ CCD system (Biomedisys Co. Korea) was used. The subtraction images were made from two direct digital images by Sunny program in the CDX-2000HQ system. Two radiologists and three endodontists examined the presence of lesions using a five-point confidence scale and compared the diagnostic ability by ROC (Receiver Operating Characteristic) analysis and one way ANOV A test. Results: The mean ROC areas of conventional radiography, indirect digital radiography, direct digital radiography, and digital subtraction radiography were 0.9093, 0.9102, 0.9184, and 0.9056, respectively. The diagnostic ability of direct digital radiography was better than the other imaging modalities, but there were no statistical differences among these imaging modalities (p > 0.05). Coclusion : These results indicate that new CCD-based digital systems (CDX-2000HQ) have the potential to serve as an alternative to conventional radiography in the detection of incipient dental caries.

Periondontal Disease Detection in Dental Radiography by ROI segment (관심영역을 이용한 치과용 방사선 영상에서의 자연치아 주위 미세변화 검출에 관한 연구)

  • In this paper, we propose a medical image processing method for detection of periodontal disease. The proposed method is the method of an automatic image alignment and detection of minute changes, to overcome defects in the conventional subtraction radiography by digital image processing technique, that is necessary for getting subtraction image and ROI(Region of Interest) focused on a selection method using the structured features in target images. And the method services accuracy, consistency and objective information or data to results. In result, easily and visually we can identify minute differences in the affected parts whether they have problems or not, and using application system.

  • Purpose: The purpose of the experiment was to evaluating the diagnostic ability of dental caries detection using digital subtraction in the artificial caries activity model. Materials and Methods: Digital radiographies of five teeth with 8 proximal surfaces were obtained by CCD sensor (Kodak RVG 6100 using a size #2). The digital radiographic images and subtraction images from artificial proximal caries were examined and interpreted. In this study, we proposed novel caries detection method which could diagnose the dental proximal caries from single digital radiographic image. Results: In artificial caries activity model, the range of lesional depth was $572-1,374{\mu}m$ and the range of lesional area was $36.95-138.52mm^2$. The lesional depth and the area were significantly increased with demineralization time (p<0.001). Furthermore, the proximal caries detection using digital subtraction radiography showed high detection rate compared to the proximal caries examination using simple digital radiograph. Conclusion: The results demonstrated that the digital subtraction radiography from single radiographic image of artificial caries was highly efficient in the detection of dental caries compared to the data from simple digital radiograph.

THE STUDY OF THE INTERPRETATION OF THE TMJ RADIOGRAPHY USING SUBTRACTION TECHNIQUE (Subtraction법을 이용한 악관절 X-선사진 판독에 관한 연구)

  • The purpose of this study is to know the value of the photographic subtraction technic in the transcranial oblique lateral projection of the TMJ. The author examined the transcranial oblique lateral projection radiographs which comprise 50 cases of 32 persons, compared the transcranial oblique lateral projection films and those subtraction films. The following results were obtained. 1) The condyle at closed jaw position had showed a reversed tone image, but the condyle at the opening position had showed a re-reversed ordinary image. Both condyles had showed one subtraction film because radiographic interpretation of TMJ was easy. 2) On 50 cases of subtraction films, 46 cases had showed same radiographic images compared with trascranial oblique lateral projection films. Four cases (3 cases of erosion, 1 cases of sclerosis) had showed additional changes of images so that capability of interpretation was improved.

Effectiveness of digital subtraction radiography in detecting artificially created osteophytes and erosions in the temporomandibular joint

  • Purpose: Erosions and osteophytes are radiographic characteristics that are found in different stages of temporomandibular joint (TMJ) osteoarthritis. This study assessed the effectiveness of digital subtraction radiography (DSR) in diagnosing simulated osteophytes and erosions in the TMJ. Materials and Methods: Five intact, dry human skulls were used to assess the effectiveness of DSR in detecting osteophytes. Four cortical bone chips of varying thicknesses (0.5 mm, 1.0 mm, 1.5 mm, and 2.0 mm) were placed at the medial, central, and lateral aspects of the condyle anterior surface. Two defects of varying depth (1.0 mm and 1.5 mm) were created on the lateral, central, and medial poles of the condyles of 2 skulls to simulate erosions. Panoramic images of the condyles were acquired before and after artificially creating the changes. Digital subtraction was performed with Emago dental image archiving software. Five observers familiar with the interpretation of TMJ radiographs evaluated the images. Receiver operating characteristic (ROC) analysis was used to evaluate the diagnostic accuracy of the imaging methods. Results: The area under the ROC curve (Az) value for the overall diagnostic accuracy of DSR in detecting osteophytic changes was 0.931. The Az value for the overall diagnostic accuracy of panoramic imaging was 0.695. The accuracy of DSR in detecting erosive changes was 0.854 and 0.696 for panoramic imaging. DSR was remarkably more accurate than panoramic imaging in detecting simulated osteophytic and erosive changes. Conclusion: The accuracy of panoramic imaging in detecting degenerative changes was significantly lower than the accuracy of DSR (P<.05). DSR improved the accuracy of detection using panoramic images.

Bone Loss Detection in Dental Radiography by Domain Knowledge Based Multi-template (지식기반 다중 템플릿을 이용한 치과용 디지털 X-ray 영상에서의 미세변화 검출에 관한 연구)

  • This study proposes the algorithm to automate image alignment and detect marginal bone destructions, based on subtraction radiography for dental radiographic images necessary for dental PACS, which hasn't been covered by related literatures. The proposed algorithm enables a quick and precise detection of marginal bone destructions around teeth including implant through the knowlege-based multi-template matching in reference to ROI(Region Of Interest) obtained from applicable teeth using information about their geometric forms to solve problems single-template matching is exposed to. Actually, the test showed that it was possible not only to quickly and precisely detect marginal bone destructions around teeth, but also to get more objective and quantitative results through the algorithm.

  • Various biological approaches to the promotion of periodontal regeneration have been used. These can be divided into the use of growth and differentiation factors, application of extracellular matrix proteins and attachment factors and use of mediators of bone metabolism. The purpose of this study was to evaluate the effect of enamel matrix protein and platelet-rich plasma on the treatment of intrabony defect, with bovine-derived bone powder in humans by digital subtraction radiography. 12 teeth(experimental I group) were treated with enamel matrix protein combined with bovine-derived bone powder and 12 teeth(experimental II group) were treated with platelet-rich plasma combined with bovine-derived bone powder. The change of bone density was assessed by digital subtraction radiography in this study. The change of mineral content was assessed in the method that two radiography were put into computer program to be overlapped and the previous image was subtracted by the later one. Both groups were statistically analyzed by Wilcoxon signed Ranks Test and Mann-whitney Test using SPSS program for windows(5% significance level). The results were as follows: 1. The radiolucency in 3 months after surgery was significantly increased than 1 month after surgery in both groups(experimental I and II groups)(p<0.05). 2. The radiopacity in 6 months after surgery was significantly increased than 3 months after surgery in both groups(experimental I and II groups) (p<0.05). 3. In experimental I group, there was no significant difference between 1 month and 6 months after surgery. 4. In experimental II group. the radiopacity in 6 months after surgery was significantly increased than 1 month after surgery(p<0.05). 5. There was no significant difference between experimental I and II group at 1 month and 3 months after surgery, but the radiopacity in experimental II group was significantly increased at 6 months after surgery(p<0.05). In conclusion, platelet-rich plasma can enhance bone density than enamel matrix protein until 6 months after surgery.
