The purpose of this research is to investigate how obesity and eating behavior are related to the concentration of serum insulin and lipids and blood pressure of middle-aged men The sample for this study utilizes 240 middle-aged men between th4 age of 40 and 60 living in the city of Kumi. Through a process of anthropometry and categorization by relative body weight, the sample is devide into two groups, the obese group (51 men) and the normal group (129 men). The results of the study were as follows : 1111111. The men from the obese group with the exception of height, subs-capular/triceps skinfold girth ratio(STR) had a higher level of body mass index (BMI), waist and hip circumference, waist/hip girth ratio(WHR), subscapular and triceps skinfold thickness than the normal group (P=0.0001). 2. In comparing the serum insulin and lipids between the obese and normal group, the obese group had a higher level of insulin and triglyceride (P=0.016, P=0.050) but a lower concentration of HDL-cholesterol and HDL-cholesterol/total cholesterol (P=0.034, P=0.004). Also, the obese group had a higher level of systolic and diastolic blood pressure (P=0.001, P=0.029). When looking at the relationship between the serum insulin and lipids and anthropometric measurements, the relationship between waist/hip girth ratio(THR), the concentration of fasting serum insulin and lipids were stronger than the obesity index, body mass index(BMI). 3. Men who exercised regularly had a lower total cholesterol and systolic blood pressure 9P=0.049, P=0.041), and a higher level of HDL-cholesterol/total cholesterol (P=0.004). There was no observable relationship between the food habit score, the preference for fiber foods, and the concentration of serum lipids. Also, the concentration of serum lipids had no apparent effect on the preference for salty, sweet, and greasy foods.
Anthropometric and body compositional changes and the outcomes of the pregnancies of 90 healthy Korean women were investigated in a longitudinal study. Their weight increased from 51.3$\pm$5.9kg to 65.1$\pm$7.8kg during their pregnancies. The total weight gain was 13.8$\pm$4.5kg, and therefore, weekly weight gain was 340$\pm$110g during the entire period of the pregnancy. The weight gain was composed of approximately 50-60% fat mass and 40-50% fat-free mass. Skinfold thicknesses, both of triceps and subscapular, increased during the pregnancies. The fat mass calculated from skinfold thickness and that measured with bioelectrical impedance analysis went on increasing during the pregnancies. Although there was a considerable difference with respect to the fat mass observed using the three methods, fat mass gain was 5.0-6.1kg and fat-free mass gain was 4.0-5.3kg from the first trimester to the third trimester of pregnancies. There were significant correlations between maternal anthropometric parameters and indices of pregnancy outcomes. Especially, the infant's birth weight was associated with maternal pre-pregnancy weight and weight gains during the pregnancies. The infant's birth length was related to the maternal weight observed at term(p<0.05) and weight gain during the entire pregnancies (p<0.05) . Neither increase of fat mass nor fat-free mass affected the outcomes of pregnancy. These results show that maternal weight gain during pregnancy is led by increments of approximately above 50% fat mass. The fat mass increase seems to be larger in central areas than in subcutaneous areas. Maternal weight gain during pregnancy, especially during late pregnancy, is a factor affecting the birth weight and length of infant. On the basis of the body compositional changes, it can be predicted that the additional energy requirement for pregnancy in Korean women is more than 200-230 MJ (64,500-76,250kca1). Background Disulfiram implantation is a widely used treatment alternative for alcohol abuse, yet reports on the surgical aspect of disulfiram implantation with respect to patient and drug-related treatment efficacy and wound complications are very limited. We present our clinical experiences with disulfiram implantation and discuss the surgical outcomes obtained with different anatomical planes for implantation. Methods Medical records of all patients referred to our clinic from the psychiatry department between 2007 and 2013 for disulfiram implantation were retrospectively analyzed. Implantation was carried out using 10 sterile Disulfiram tablets (WZF Polfa S.A.), each tablet containing 100 mg of disulfiram. The procedure was carried out by implanting the tablets randomly in either a subcutaneous or an intramuscular plane. The location and the plane of implantation and the complications were recorded for each patient and compared to determine the differences in the outcomes. Results A total of 32 implantation procedures were evaluated for this study. Twenty-five implants were placed in the intramuscular plane (78.2%), while seven implants were placed subcutaneously (21.8%). Exposure was encountered in three of the seven subcutaneous implants (42.9%), while no exposure was seen with the intramuscular implants. Incomplete absorption of the tablets was encountered in one patient with a previous subcutaneous implant who presented 1 year later for re-implantation as part of the continuation of therapy. Conclusions To overcome the issue of treatment continuation in the case of disulfiram therapy, which may be ceased due to frequently encountered wound complications, we believe that implantation in the subscapular intramuscular plane allows both uneventful healing and an out-of-reach implant location.
This study was conducted to investigate the dietary intake, anthropometric data, and association between two factors and main food sources contributing macronutrients for overweight and obese females. Subjects were 85 adult females (overweight : 28, obese 57) where mean age was 38.7y. The results are summarized as follows. Mean fat percent, BMI, obesity rate were 29.3, 23.9 and 15.1%, respectively for overweight women and 32.7, 28.3, 36.4%, respectively for obese women. There were significant differences for most of the anthropometric data between groups. Fat percent for all subjects was significantly correlated with weight, waist circumference, hip circumference, mid arm circumference or skinfold thickness for the triceps, subscapular and suprailiac (p < 0.001). The parameter which showed the highest correlation coefficient (r=0.6156) with fat Percentage was the waist to hip ratio. Any significant differences were not found in dietary intake of nutrients or in diet composition between groups. The mean energy intake was 2090.1kcal (104% of RDA) for overweight women and 2113.0kcal (106% of RDA) for obese women. PFC ratio for overweight was 17 : 24 : 59 and 18 : 23 : 58 for obese subjects, which can be regarded as higher fat and lower carbohydrate percentages compared to recommended PFC ratio (15 20 65) .4) Fat intake was positively correlated (r : 0.2301, p < 0.05) with the triceps skinfold thickness, protein intake was also positively correlated with waist circumference (r=0.2668, p < 0.05) or fat weight(r: 0.2406, p < 0.05) .5) The main food items that contributed to energy intake for overweight or obese subjects were similar (rice, pork, bread, grapes, barley) except com oil in overweight or instant noodle for obese group. The subjects in this study were taking less energy from rice and more energy from pork and bread than women from 98 National Health and Nutrition Survey. Because there were no significant differences of dietary data between overweight and obese group, further investigation considering basal metabolic rate or activity would be needed.
Objective; Childhood obesity has become one of the most common health problems facing children in the world. Although a number of regimens have been developed and distributed for the treatment of adult obesity, few studies have focused on therapeutic programs for obese children. The objective of this study was to develop and measure the effects of Taeumin childhood obesity by a sasang constitutionbased education program. Methods ; A total of 14$(\%RBW\geq20)$ among 18 taeumin with childhood obesity from June to August 2003 participated in the constitution-based education program. It was designed to examine the effects of a constitution-based education program through anthropometric measurements, body composition indicates, biochemical indicates and serum level of taeumin childhood obesity. Results; The results of this study were as follows: 1. The anthropometric measurement of subscapular skinfold thickness was reduced after 4 weeks of the constitution-based education program. 2. The body composition indicates reduction in degree of obesity, percent body fat, and fat distribution. The other side increased in height, total body water, soft lean mass, and lean body mass. 3. For biochemical indicates, HDL-cholesterol was increased significantly after 4 weeks of the constitution-based education program. The other side decreased in AST and ALT. 4. There was a significant reduction in BMI and $\%RBW$ after the constitution-based education program. Conclusion : A constitution-based education program for weight control results in reduced degree of obesity among taeumin children.

In the current study, the relationships between abdominal adipose tissue depots and skinfold thickness, body circumference and composition, and blood lipids and pressure were investigated in adolescent obese man. Total abdominal adipose tissue (TAT) and subcutaneous adipose tissue (SAT) were correlated with skinfold thickness. Especially subscapular skinfold was highly correlated with TAT and SAT, and it was found that chest skinfold was the only predictor for estimating visceral adipose tissue (VAT). Body circumference and body composition were associated with TAT and SAT, not VAT, and correlation coefficient of waist circumference was highest. On the other hand, body fat mass was highly correlated with TAT and SAT than %body fat and fat free mass. It was also shown that blood triglyceride concentration and systolic blood pressure were the only predictor for estimating VAT and SAT respectively.

Anatomy: The rotator cuff comprises four muscles-the subscapularis, the supraspinatus, the infraspinatus and the teres minor-and their musculotendinous attachments. The subscapularis muscle is innervated by the subscapular nerve and originates on the scapula. It inserts on the lesser tuberosity of the humerus. The supraspinatus and infraspinatus are both innervated by the suprascapular nerve, originate in the scapula and insert on the greater tuberosity. The teres minor is innervated by the axillary nerve, originates on the scapula and inserts on the greater tuberosity. The subacromial space lies underneath the acromion, the coracoid process, the acromioclavicular joint and the coracoacromial ligament. A bursa in the subacromial space provides lubrication for the rotator cuff. Etiology: The space between the undersurface of the acromion and the superior aspect of the humeral head is called the impingement interval. This space is normally narrow and is maximally narrow when the arm is abducted. Any condition that further narrows this space can cause impingement. Impingement can result from extrinsic compression or from loss of competency of the rotator cuff. Syndrome: Neer divided impingement syndrome into three stages. Stage I involves edema and/or hemorrhage. This stage generally occurs in patients less than 25 years of age and is frequently associated with an overuse injury. Generally, at this stage the syndrome is reversible. Stage II is more advanced and tends to occur in patients 25 to 40 years of age. The pathologic changes that are now evident show fibrosis as well as irreversible tendon changes. Stage III generally occurs in patients over 50 years of age and frequently involves a tendon rupture or tear. Stage III is largely a process of attrition and the culmination of fibrosis and tendinosis that have been present for many years. Treatment: In patients with stage I impingement, conservative treatment is often sufficient. Conservative treatment involves resting and stopping the offending activity. It may also involve prolonged physical therapy. Sport and job modifications may be beneficial. Nonsteroidal anti - inflammatory drugs(NSAIDS) and ice treatments can relieve pain. Ice packs applied for 20 minutes three times a day may help. A sling is never used, because adhesive capsulitis can result from immobilization.
The purpose of this study was to develop cardiovascular health related percent body-fat standards that may be applied to epidemiologic investigations of the prevalence and incidence of obesity in adolescents, pediatric health screenings, and youth fitness tests. The subjects included 102 males and 80 females aged 19~22years. All subject were Honam University Students Total body fat was derived from body density which was estimated from age and the triceps and subscapular skinfold thickness measured with Lang calipers to the nearst l.0mm. Serum total cholesterol and lipoprotein cholesterol fraction(HDL-CLDL-C) were measured from blood obtained from fore arm vein after blood pressure measurement. In analyses to determine critical fat levels associated with elevated CDD(Chronic Degenerative Disease) risk factors;male and female were grouped by level of percent fat as follows: male, 〈 10%, 10-14.9%, 15-19.9%, 20-24.9%, and $\geq$ 25%;female, 〈 20%, 20-24.9%, 25-29.9%, 30-34.9%, and $\geq$ 35%. As the results of the data, the conclusions were as follows: 1. A dose respones effect was observed between blood pressure and percent body fat in males and females; in contrast, total cholesterol and lipoprotein ratios were relatively independant of percent fat among the lower four fatness group in males and the lower three fatness groups in females. 2. The percentage of subjects in the uppermost quintile for S-Bp, D-Bp, TC, LDL/H was significantly(P〈.05) greater than expected by change alone(20%) in males with $\geq$ 25% fat and in females with $\geq$ 30% fat females with $\geq$35% had even greater representation in the uppermost quintile of all CDD risk factors compared to females with 30-34.9% fat.
Among many kinds of introduced free flaps, scapular freeflap is one of the most popularly using modalities in fasciocutaneous defect coverage with minimal donor defect and easier procedure and constant vascular patterns of the donor. Many surgeons who had experience of this flap pointed out deficit of the reliable sensation of the transplanted flap is the main shortcoming of the scapular free flap. If we can subjugate that point, scapular free flap is the most excellent procedure in such a cases as heel pad reconstruction and hand reconstruction which are relatively important to have skin with protective sensation. Author performed anatomical literature review, 10 cadaveric dissections and 12 clinical dissections. In surgical anatomical aspect, the upper six dorsal rami of the thoracic nerves have medial branches which pierce Longissimus thoracis and Multifidus muscle with small cutaneous twigs which pierce Latissimus dorsi and Trapezius muscle. Among that cutaneous twigs, several twigs distribute to the skin of the back from midline to lateral aspect which territory is identical to scapular free flap. We analysed clinical experiences of that sensory bearing scapular free flap surgical anatomy and one year follow-up studies with several results. 1) Two to three cutaneous twigs which pierced from the Trapezius muscle over the scapular free flap region. 2) Each twigs has two to four nerve fascicles with small artery. 3) The nerve distributed to the ordinary scapular free flap and large enough size and pedicle length to neurorrhapy with various recipient site nerves. 4) The inconvenience of this procedure is the vascular pedicle and nerve pedicle have opposite directions, vascular pedicle of that comes from lateral direction from subscapular vessels, but nerve pedicle comes from medial direction from trapezius muscle. Author can found constant cutaneous nerve branches which come from piercing the Trapezius. This nerves are helpful for protective sensation in transplanted scapular free flap. We can't had enough follow-up and evaluation of the nerve function of this procedure, we need continuous research works to application of this procedure. The in conveniences come from directional differences of pedicle can solve with longer harvest neural pedicle and change direction of the neural pedicle.
The purpose of the study was to develop generalized equations for estimating stature and weight for the nonambulatory elderly persons. Height weight recumbent knee height total ann length, midarm, waist and calf circumferences, triceps and subscapular skinfolds were measured from over 60 years old 315 ambulatory elderly. The equations to predict stature and weight were derived from participants in the validation sample and were applied to the participants in the cross-validation to test the accuracy and validity of equations. Stature and weight were significantly and negatively associated with age of women and similar patterns observed in men but associated to a slight degree. Knee height and total arm length were highly correlated with stature but the majority of the variances in stature was accounted for by knee height for both the men and women. In men, waist circumference was the most significantly correlated with weight and am, calf circumferences and so forth. But in women arm circumference was the highest then waist and calf circumference in order. The possible predictor variables to estimate of stature were knee height total arm length and age for both elderly men and women. Predictor variables to estimate of weight were recumbent measures of waist am, calf circumferences and knee height for both sexes. Inclusion of skinfold thickness measurements did not improve the prediction power of estimation for weight. When both equations developed from the present study and Chumlea's study were applied to cross-valida-tions samples, the equations derived from present study showed better accuracy and validity. The presentation of prediction equations using two, three, or four recommended measurements allows the selection of an equation based upon the measurements that are possible to collect on an individual basis.
