구강 내 접근과 하악하 절개를 통한 몰입성 하마종의 수술적 치료 (Surgical Treatment of a Plunging Ranula using the Intraoral and Submandibular Approach)

  • Purpose: A plunging ranula is relatively uncommon and represents a mucus escape reaction occurring from a disruption of the sublingual salivary gland. It is a common condition found in young adults, even though the reported age range is 2 - 61 years. We report our experience of a complete excision of a plunging ranula via the intraoral and submandibular approach. Methods: A 23-year-old man had a large protruding mass in the right submandibular area. Initially, the protruding mass appeared bilaterally but the left side disappeared spontaneously. The MRI findings revealed a homogenous fluid attenuation mass in the submandibular space, suggesting a ranula. The sublingual gland was extirpated through the intraoral approach and the ranula excised totally via the submandibular approach. Results: The patient had an uneventful postoperative course without infection, paralysis and tongue sensory changes, etc. The pathology findings were characteristic of a pseudocyst without a lining epithelium or endothelium but with a vascular fibro-conective tissue wall filled with mucinous fluid. No recurrence was observed on the submandibular area during the 8 month follow-up period. Conclusion: The combined intraoral approach and submandibular approach is an effective and highly recommended method for sublingual gland extirpation and complete excision of a plunging ranula.

  • Sublingual resection is known as the most effective treatment of choice to prevent recurrence in patients with plunging ranula. In this case report, we present our experience with a 37 year-old man with prolonged upper neck mass diagnosed as plunging ranula. He had persisting mass lesion even after 4 times of sclerotherapy. Due to fibrotic change the pseudocyst could not be drained after removing the sublingual gland. To resolve the mass lesion, ultrasonography guided transoral drainage was performed. Intraoperative ultrasonography may be useful for transoral drainage of plunging ranula difficult to approach after sublingual resection.

  • Ranula is a mucocele caused by extravasation of the sublingual gland on the floor of the mouth. The most common presentation is a cystic mass in the floor of the mouth. A portion of the sublingual gland could herniate through the mylohyoid muscle, and its extravasated mucin can spread along this hiatus into submandibular and submental spaces and cause cervical swelling. This phenomenon is called plunging ranula. A variety of treatments for ranula has been suggested and include aspiration of cystic fluid, sclerotherapy, marsupialization, incision and drainage, ranula excision only, and excision of the sublingual gland with or without ranula. Those various treatments have shown diverse results. Most surgeons agree that removal of the sublingual gland is necessary in oral and plunging ranula. Four patients with ranula were investigated retrospectively, and treatment methods based on literature review were attempted.

  • Granular cell tumor was described in a 7-year-old male Miniature Pinscher. Sublingual mass was surgically removed and submitted for diagnosis. The mass was about 1.8 cm in diameter, firm, tan, and ulcerated. On histopathology, the mass was unencapsulated and infiltrated adjacent tissue and consisted of nest or sheet of round to polygonal, and occasional spindle-shaped cells. The neoplastic cells contain large amount of eosinophilic granular materials in the cytoplasm. Immunohistochemically, the neoplastic cells were positive to neuron-specific enolase but were negative to S-100. The cytoplasmic eosinophilic granules were positive to PAS but were negative to PTAH methods. Based on these results, the mass was diagnosed as sublingual granular cell tumor. No recurrence or metastasis was noted so far since surgery.

피쁠견에서 경추타액점액류의 외과적치료 (Surgical Treatment of Cervical Salivary Mucocele in a Pit Bull Terrier)

  • A 15 months old male pit bull terrier was shown submandibular swelling, which was extended from left submandibular area through mandibular symphysis to right submandibular area and toward around left neck. In history taking, recurrence of swelling was recorded after conservative surgical incision, drainage and dressing. Palpation revealed no pain and heating, partial flutuation and hardness. By paracentesis, it was showed blood-tinged tenacious exudate without bad-smelling. Left submandibular salivary gland was able to be movable freely and the size decreased to half of that of right submandibular salivary gland. It was diagnosed as cervical salivary mucocele. In operation, rostral portion of left sublingual salivary gland was observed to be damaged transversely, showed black color and leaked saliva. Submandibular gland and rostral portion of sublingual salivary gland were resected, after ligation of ducts of submandibular and sublingual salivary glands. At 15 days postoperation, serosanguineous exudate from operation wound was dramatically decreased and stable granulation tissue mass at this area was first palpated. At 39 days after operation, outline of left and right mandibular was appeared normal and skin tenderness of mandibular area was equal to that of the other body wall.

  • Purpose: The thyroglossal duct runs from the base of the tongue to the thyroid. Rarely the thyroid completely fails to migrate and results in ectopic thyroid tissue, which can be demonstrated scintigraphically. A 31-year old female patient was referred for thyroid scintigraphy due to protruding mass at the base of the tongue. She was mildly hypothyroid. Te-99m pertechnetate thyroid scan was performed to rule out ectopic thyroid gland. There showed a focal area of intense tracer uptake in sublingual area, suggesting the sublingual thyroid. In addition there noted diffusely increased tracer uptake in both breasts. The patient delivered a baby 6 months prior to the scan and was on breast-feeding. Free Tc-99m pertechnetate physiologically secrets into the salivary glands, the stomach, the gastrointestinal tract, the genitourinary tract and the mammary glands and sodium-iodide symporter plays a role in the accumulation of free Tc-99m pertechnetate. We report simultaneous visualization of lactating breasts and ectopic thyroid gland in the base of the tongue.

Adenoid cystic carcinoma of the sublingual gland: A case report

  • Adenoid cystic carcinoma (ACC) of the sublingual gland is an extremely rare neoplasm. The clinicopathological characteristics of ACC are slow-growing swelling with or without ulceration, perineural spread, local recurrence, and distant metastasis. This report describes a 58-year-old male who had a slowly growing swelling without ulceration on the right side of the mouth floor that had been present for 1 month. In a radiological examination, the mass showed multilocular cystic features and no bony or tongue muscle invasion. No enlarged cervical lymph nodes were detected. Excisional biopsy and histological analysis showed that the lesion was ACC. In addition to reporting a rare case of ACC, this report also discusses the differential diagnosis and treatment of ACC with a review of the relevant literature.

이소성 갑상선 1례 및 국내 증례의 임상적 고찰 (A Case of Ectopic Thyroid with Clinical Evaluation of Fifteen Cases)

  • The authors recently experienced a case of ectopic thyroid. A 15-year-old female patient visited to ENT department with the complaint of the submental neck mass. On physical examination, the mass was relatively firm, non tender and 3$\times$2cm in size. Oral cavity examination revealed 0.5$\times$0.5cm sized pink colored mass near the foramen cecum area. Suspecting ectopic thyroid, thyroid function test, thyroid scan, neck computed tomogram scan were performed. Thyroid scan revealed a functioning thyroid on the lingual and submental area without normal uptake in the anterior neck area. Thyroid (unction test was normal. Pre-contrast computed tomogram scan revealed an ectopic thyroid in the lingual and submental area. A review of literature concerning ectopic thyroid was discussed.

Neurilemmoma in the floor of the mouth: a case report

  • Neurilemmomas are well-encapsulated, benign, slow-growing tumors originating from Schwann cells of the nerve sheath surrounding cranial, peripheral, or autonomic nerves. Intraoral neurilemmomas are relatively rare and have a wide variety of morphologic and radiologic features. This makes differential diagnosis difficult, and only histopathological features can lead to a definitive neurilemmoma diagnosis. In this report, we present the case of a 30-year-old woman whose chief complaint was a solitary, nodular mass on the right floor of the mouth. After computed tomography and magnetic resonance imaging, we performed an incisional biopsy that showed the typical characteristics of a neurilemmoma. The mass was removed completely through an intraoral surgical approach. Despite losing a portion of the lingual nerve, the patient did not complain of any specific discomfort. Wound healing was uneventful and there were no signs or symptoms of recurrence.

주 타액선 종양에 대한 10년 간의 경험 (Major Salivary Glands Tumors:A 10-Year Experience)

  • Background and Objectives : This study reports our clinicopathological experiences of major salivary gland tumors. Materials and Methods : This study included 302 patients with major salivary gland tumors who had got the diagnosis from January 1995 through December 2004. Medical records were reviewed retrospectively. Results : We found 244 benign and 58 malignant major salivary gland tumors. Among 267 parotid tumors, the most common benign parotid tumor was pleomorphic adenoma. Mucoepidermoid carcinoma, adenoid cystic carcinoma, and acinic cell carcinoma were three most common parotid malignancies. Among 33 submandibular gland tumors, 20 cases were benign and 13 were malignant. There were one benign and one malignant sublingual gland tumors. The duration of symptom of submandibular gland tumors was longer than that of parotid tumors. Most patients presented with asymptomatic mass. Seventy-one percent of salivary gland malignancies underwent postoperative radiation therapy. Five-year disease free survival rate of parotid malignant tumors seemed to be higher than that of submandibular one, although there was no statistical significance. Conclusion : Malignancy rate of each salivary gland followed old axiom that it is inversely related with the size of gland. Submandibular gland tumor tends to be delayed to reach diagnosis. Clinicians must be alert about this finding because submandibular gland tumors are known to have poorer prognosis than parotid tumors.
