Sample Size Requirements in Diagnostic Test Performance Studies (진단검사의 특성 추정을 위한 표본크기)

  • There has been increasing attention on sample size requirements in peer reviewed medical literatures. Accordingly, a statistically-valid sample size determination has been described for a variety of medical situations including diagnostic test accuracy studies. If the sample is too small, the estimate is too inaccurate to be useful. On the other hand, a very large sample size would yield the estimate with more accurate than required but may be costly and inefficient. Choosing the optimal sample size depends on statistical considerations, such as the desired precision, statistical power, confidence level and prevalence of disease, and non-statistical considerations, such as resources, cost and sample availability. In a previous paper (J Vet Clin 2012; 29: 68-77) we briefly described the statistical theory behind sample size calculations and provided practical methods of calculating sample size in different situations for different research purposes. This review describes how to calculate sample sizes when assessing diagnostic test performance such as sensitivity and specificity alone. Also included in this paper are tables and formulae to help researchers for designing diagnostic test studies and calculating sample size in studies evaluating test performance. For complex studies clinicians are encouraged to consult a statistician to help in the design and analysis for an accurate determination of the sample size.

A Review of Studies on Antibiotic Course and Antibiotic Resistance in Nasopharyngeal Pathogens in Primary Care Setting (일차진료 항생제 치료기간과 비인두 항생제 내성률에 대한 연구 고찰)

  • Objectives The purpose of this study is to examine the correlation of antibiotics administration duration and antimicrobial resistance by reviewing domestic and foreign literatures. Methods We searched literatures dated up to 23 February, 2018 in PubMed and Cochrane Library using terms of "Anti-Bacterial Agents", "Carrier State", "Nasopharynx", "Drug Administration Schedule", and also searched via RISS (Research Information Service System), KISS (Koreanstudies Information Service System), DBpia (DataBase Periodical Information Academic) using terms of antibiotics, resistance, and dose. Results In comparison with shortened and standard antibiotic course, longer treatment duration is associated with greater antimicrobial resistance or non-significant difference, but we cannot find literature that shortened antibiotic course increases antimicrobial resistance on human nasopharyngeal flora. Conclusions Currently, there is no evidence that completing the standard antibiotic course reduces antimicrobial resistance. It can be a strategy for reducing antibiotic use to apply Korean medicine treatment, as well as short-course antibiotic therapy or delayed antibiotic prescription. Additional well-designed trials should be conducted in domestic and foreign settings about the appropriate duration of antibiotic therapy.

Literature Review of Manual Therapy for Mononeuropathies of Upper Limb (상지의 단일신경병증에 대한 수기치료의 국내외 동향)

  • Objectives This study aimed to review manual therapies for mononeuropathies of upper limb through domestic and foreign studies designed for human body. Methods We searched databases (KMbase, OASIS, RISS, NDSL, KISS, KoreaMed, MEDLINE/Pubmed, CENTRAL, EMBASE) on the 1st to 31th of July 2017 to find related literatures that published after 2000. Results Twenty-eight studies were finally included. Of these, 13 articles were published after 2010. Twenty-two studies were clinical trials and 6 were observational studies. Carpal tunnel syndrome were the most researched type of diseases (85.7%). Most frequently used method of manual therapies was neurodynamic mobilization (35.7%). Pain scales and questionnaires were generally employed for evaluation. Significantly effective studies were 72.2% in controlled trials and 90% in the studies without control group. Conclusions In this study, we reviewed literatures concerning manual therapies on mononeuropathies of upper limb. Further studies are needed on the various diseases of mononeuropathies of upper limb to retain the evidence for the effectiveness of manual therapies.

Analysis of Status of Researches on Korean National R&D : Research Fields and Their Network of Literatures in Domestic Journals (국가연구개발에 관한 연구현황 분석 - 국내 학술지 문헌의 연구 분야 및 분야 간 네트워크를 중심으로)

  • With the recent slowdown of R&D budget growth rate, efficient resource allocation, operation and management of national R&D is becoming more important. At this point, recognizing the necessity of systematic and scientific research on the national R&D itself, this study analyzed previous researches on national R&D of Korea. In this study, based on the previous researches and laws, national R&D was classified according to the major stages such as planning, selection, management, performance and so on. Secondly, by using the classification criteria forged, previous researches on the national R&D published in domestic journals for the last 5 years were divided into 12 categories, and the status of researches in each field was analyzed. Lastly, through network analysis, linking status and influence of each fields were identified by using co-classification information of research literatures. As a result of this research, the performance related fields were the most active ones in terms of the number of research literatures, and connections with other fields, while the fields such as selection and infrastructure were lacking in the number of research and linkage. This study can find its meaning in identifying research fields that need more studies and connections with other areas by systematic analysis of previous studies on Korean National R&D.

A Literature Review of Clinical Studies on Pharmacopuncture for Cancer Pain through Korean Database Search (국내 데이터베이스 검색을 통한 암성통증의 약침치료 임상연구 동향)

  • Objectives: The effectiveness of acupuncture on cancer pain is relatively well established, while there have been few clinical studies on pharmacopuncture on that. This research was aimed to investigate a trend of pharmacopuncture treatment for cancer pain and to propose information for further research. Methods: Clinical studies on pharmacopuncture for cancer pain were collected from 5 Korean medical databases. And the studies were analyzed by classifications of cancer, chief complaints, interventions, treating skills, additional treatments, treatment session, outcome measures, results and adverse effects. Results: The literatures were 8 case reports and 1 randomized controlled trial. Interventions were pharmacopunctures of soyeom, mountain ginseng, Harpagophyti Radix, snake venom, Trionycis Carapax, bee venom, Aconitum Ciliare Decaisne. Treatment skills, procedures, and indications of each pharmacopuncture were different. An adverse effects of bruising, bleeding, mild chilling, sore throat and pain were mentioned on snake venom and bee venom treatment. Conclusions: An appropriate interventions and treatment skills should be selected according to the type of cancer pain. And treatment procedures to ensure safety must be developed.

A Comparison Study of Acupuncture Points Selection between Classics of Traditional Medicine and Clinical Trials in Dental Disorders (치아 질환의 침 치료 혈위 선택에 대한 고전문헌과 현대 임상연구 비교)

  • Objectives : The aim of this study is to summarize and compare acupuncture points used for dental disorder in the classics with those used in recent clinical trials. Methods : We searched the data for acupuncture points used and rationale of acupuncture in dentistry. Following two sources were searched: 1) seven Classics of traditional medicine and 2) clinical trials through Pubmed from January 2000 to March 2013 with no language restriction. Results : Dental pain was the most common disorder in the dentistry section of the Classics of traditional medicine. We found many similarities of acupuncture points used between literatures and clinical trials. From the meridian perspective, large intestine meridian(LI), stomach meridian(ST), triple energizer meridian(TE), and gallbladder meridian(GB) were frequently used in the Classics of traditional medicine and the clinical trials. From the acupuncture point perspective, acupuncture points were selected according to syndrome. The specific points such as five transport points, source points, and connecting points were also used. In the clinical trials, combination of acupuncture points usually consisted adjacent points and distant points. Among them, LI4 was used in all of the studies included in this review. Conclusions : For well-designed clinical trial, appropriate intervention is essential. To establish appropriate acupuncture treatment, we have to suggest reasoning for treatment based on literature and/or expert consensus. Our review only focused on pain relief in dentistry and had many limitations. Further studies based on the literatures such as the Classics of traditional medicine are required to ensure the rationale of acupuncture treatments in various dental disorders.

Experimental Studies on the Effects of Ferric Oxide (철액수(鐵液水)의 효능(效能)에 관(關)한 실험적(實驗的) 연구(然究))

  • These studies were conducted about the effects of Ferric oxide in the aspect of the basic pharmacology through experimental animals with the investigation of its clinical effects on oriental medical literatures of Ferric oxide. The results were as follows: 1. The analgenic effects induced by acetic acid were significant. 2. The anticonvulsive effects induced by ECT unit were significant. 3. The sedative effects in Rotor rod method were significant. 4. The number of platelets was increased but not significantly. 5. Prothrombin time was decreased significantly. 6. Fibrinogen was increased but not significantly. 7. The degree of concentration of FDP was decreased significantly. According to the above results, it is considered that Ferric oxide retains effectiveness on the treatment of cerebral stroke.

Acupuncture for Parkinson's Disease in China (중국에서의 파킨슨병 침치료 임상연구 동향)

  • Objectives : The purpose of this study was to obtain some knowledge about the acupuncture treatments on Parkinson's disease for the future practice and the research from the clinical studies in China. Methods : The literatures were searched using the data base-China Academic Journals (CAJ) (1994-2011). Clinical studies of acupuncture for Parkinson's disease such as randomized controlled trial (RCT), non-randomized controlled trial (NRCT), case series, case report were included. Results : Of 145 articles, 52 Clinical studies met our inclusion criteria: 20 RCTs, 7 NRCTs, 22 case series and 3 case reports. Various kinds of acupuncture treatments such as scalp acupuncture, body acupuncture, abdominal acupuncture, acupoint injection, eye acupuncture and thick needle acupuncture (粗鍼) were used in the clinical studies. The treatment and control groups take Madopa Tablet (Levodopa + Benserazide HCl) in the same time in many studies, and Madopa treatment was used as a comparative treatment in the most of studies. Acupuncture treatment was done daily or 2~3 times a week for 2~20 weeks. All of studies reported high effective rate in the treatment group compared to control group. Conclusions : The results of this study could be used for the future study about the acupuncture therapy for Parkinson's disease.

What Makes CIOs Successful?: Ex-Ante vs. Ex-Post Appointment Factors (CIO제도의 임용 이전과 임용 이후 요인에 대한 연구)

  • In order to investigate how firms appoint and manage successful CIOs, this study reviews previous literatures and finds major factors affecting the performance of CIOs, Eight factors are found and classified into two groups based on time horizon characteristics: ex ante factors before appointment and ex post factors after appointment. These eight factors are also re-classified into three groups based on the degree of interaction between CIOs and CEOs: CIO factors, CEO factors, and interaction factors between CIOs and CEOs. In order to investigate how these sub-groups affect the performance of CIOs, we have conducted case studies on 6 firms that are considered to have successful CIOs. The results indicate that ex ante factors, for instance, a factor whether the CIO is appointed from inside or outside of the firm, need to be carefully considered for the successful ex post management of CIO positions. This study derives several managerial guidelines and finally proposes a future research model for empirical studies.

Review of the Studies on Acustimulation for Gynecological Surgery induced Nausea and Vomiting (부인과적 수술 후 오심 및 구토에 대한 경혈자극 임상 연구 고찰)

  • Purpose: This study aims to estimate the effects of on acustimulation for Gynecological surgery induced nausea and vomiting through reviewing the tendency of the studies. Methods: Literature searches were made through Pubmed, Koreanstudies Information Service System(KISS) and Korean Oriental medical Society database. Data were extracted according to pre-defined criteria. The methodological quality was assessed using Modified Jadad scale. Results: 8 studies were met the condition among the 43 searched studies. They were designed as double blind or observer blind and randomized controlled trial, and were analyzed statistically. 6 studies out of the 8 reported that acupuncture could prevent PONV effectively. Conclusion: The hypothesis that acustimulation may be effective for the prevention of Gynecological surgery induced PONV is supported by the data in recent literatures.
