Short hairpin RNA targeting of fibroblast activation protein inhibits tumor growth and improves the tumor microenvironment in a mouse model

  • Fibroblast activation protein (FAP) is a specific serine protease expressed in tumor stroma proven to be a stimulatory factor in the progression of some cancers. The purpose of this study was to investigate the effects of FAP knockdown on tumor growth and the tumor microenvironment. Mice bearing 4T1 subcutaneous tumors were treated with liposome-shRNA complexes targeting FAP. Tumor volumes and weights were monitored, and FAP, collagen, microvessel density (MVD), and apoptosis were measured. Our studies showed that shRNA targeting of FAP in murine breast cancer reduces FAP expression, inhibits tumor growth, promotes collagen accumulation (38%), and suppresses angiogenesis (71.7%), as well as promoting apoptosis (by threefold). We suggest that FAP plays a role in tumor growth and in altering the tumor microenvironment. Targeting FAP may therefore represent a supplementary therapy for breast cancer.

Peripheral Giant Cell Granuloma in a Dog (개에서 거대세포 치은종의 증례)

  • A mass was detected in the oral cavity from a 18-year-old female miniature poodle dog. Grossly, the mass was soft to hard, red to purple, and $1.5{\times}1.5{\times}1cm$ in size. Histopathologically, the mass was composed of hyperplastic gingival epithelium, well-vascularized stroma, closely packed pleomorphic cells, and numerous giant cells with multiple nuclei and abundant eosinophilic cytoplasm. Immunohistochemically, tumor cells were positive for alkaline phosphatase and cytokeratin 7, but not positive for CD68 unlike in human. The mass was diagnosed to peripheral giant cell granuloma in oral cavity through typical clinical and histopathological features.

  • The pilomatricoma (calcifying epithelioma of Malherbe) is rare benign hard, spherical and freely movable cutaneous tumor, which was differentiated from hair cells, particulary hair cortex cells. It is usually occured as a single, asymptomatic, 0.5 cm to 3.0 cm sized, deep seated, firm nodule, covered by normal or pink skin. It arises chiefly in young people, including children, and most often in the head, neck and upper extrimites. The authers experienced a case of pilomatricoma which occured in preauricular region. This case was summarized as follows. 1. 10 years old female has suffered from hard subepidermal mass on preauricular area and she visited our out patient clinic. So we performed surgical extirpation and the excised specimen was pathologically examined. 2. Grossly the tumor measures 2.0 cm in diameter and firm, bosselated, spherical shaped which covered by a thin layer of fibrous tissue. On cut section, it shows spicular gritty surfaces, well encapsulation, interwoven and keratotic lamellae. 3. Histopathologically, the epithelial masses of the tumor are composed of two type of cells, basophilic cells and shodow cells. The basophilic cells resemble hair matrix cells which posses round or elogated, deeply basophilic nuclei and scanty cytoplasm. The shadow cells show a central, unstained shadow at the site of the lost nucleus. Gradual development of basophilic cells into shadow cells can be observed. Foci of calcification are present within the lobule of shadow cells. The stroma of the tumor shows a considerable foreign body giant cell reaction adjacent to the shadow cells. 4. No recurrence was observed until post-operative 40 months.

Hormonal Activity of Ovarian Fibroma and Thecoma (난소 섬유종과 포막종의 호르몬 분비능에 관한 연구)

  • Six cases of ovarian fibrous stromal neoplasm were studied clinically, light microscopically and ultrastructurally for the clinico-pathological evidences of hormone production. Of the six cases, two cases were fibroma, three cases were fibrothecoma, and one case was thecoma. Two cases of fibroma and one fibrothecoma were associated with clinical history of menstrual abnormality, however fat staining of the tumor was negative or weakly positive. Two cases of fibrothecoma and one thecoma were negative for the clinical history of hormone imbalance. Fat stain of those cases revealed positive in varying intensity. Ultrastructural examination of fibroma-thecoma group revealed dark and pale cells by their nuclear characteristics. The dark cells had indented nucleus and abundant cytoplasmic organelles of rough ER, Golgi apparatus and mitochondria. Intracytoplasmic cisternal spaces were seen in the dark cell cytoplasm and some lipid droplets were seen around the cisternae. Pale cells had pale swollen nucleus and fine chromatins. Their cytoplasm showed scanty amount of organelles. Fibroma-thecoma spectrum showed varying degree of population of dark cells, light cells and intervening collagenous stroma. Lipid droplet was structurally associated with intracytoplasmic cisterna and they were frequently seen in thecoma and two of the fibrothecoma. But clinical history of hormone imbalance was poorly related to the light microscopic morphology and ultrastructural organization.

E-cadherin and $\beta-catenin$ Expression and Mutation in Gastric Carcinomas (위암에서 E-cadherin과 $\beta-catenin$ 발현과 유전자 돌연변이에 관한 연구)

  • Purpose: When cancer cels invade the stroma, they should be dissociated from the adjacent cells at first. E-cadherin and $\beta-catenin$ constitute an important protein complex associated with cellular adhesion, development, and differentiation, especially in epithelial cells. The role of E-cadherin and $\beta-catenin$ in gastric carcinogenesis were studied. Materials and Methods: The expression of E-cadherin and $\beta-catenin$ in gastric adenocarcinomas by using immunohistochemical staining and the mutation by using polymerase chain reaction- single stranded conformation polymorphism (PCR-SSCP) and sequencing were performed in 40 adenocarcinomas and 5 dysplasia of stomach. Thirteen cases, which had lymph node metastasis, were also included for immunohistochemical staining. Results: Inappropriate cytoplasmic and/or nuclear expression of a E-cadherin-$\beta-catenin$ complex was more frequent in poorly differentiated, diffuse type signet ring cell carcinomas than in well-differentiated, intestinal type adenocarcinomas (P<0.05). However, the expression was not related with clinical stage or lymph node metastasis. Mutation of E-cadherin was detected in 4 cases by using PCR-SSCP, whereas mutation of $\beta-catenin$ was detected in 2 cases. Conclusion: E-cadherin and $\beta-catenin$ seem to be important in gastric carcinogenesis, especially in poorly differentiated diffuse type.

A Case Report of Chondroid Syringoma of Philtrum (인중에 발생한 연골모양 땀샘종의 치험례)

  • Purpose: Chondroid syringoma is a rare mixed tumor of the skin that was first described by Hirsch and Helwig (1961). Characteristically, it is composed of the proliferation of epithelial cells in a myxoid and chondroid matrix. Most lesions occur on the head and neck region, and their size may range from 0.5~3 cm. Since a chondroid syringoma presents similar characteristics to other masses on the head and neck region, it is significantly important to distinguish with other masses by a surgical biopsy. Methods: A 51-year old woman presented with a painless nodular mass ($0.5{\times}0.5{\times}0.5cm$) on the philtrum, which appeared during the previous year. The mass was treated with a laser without a surgical biopsy. However, the size of the mass showed no changes. Surgical excision under local anesthesia was performed and sent for histopathology. Results: Gross examination showed a mass surrounded by a well developed capsule within the dermal layer. After complete excision without injury to the orbicularis oris muscle, the wound was covered with a local flap. The histology examination revealed numerous cuboid epithelial cells with tuboalveolar structures and keratinous cysts within a chondroid stroma. No recurrence or metastasis was observed at the follow-up visits. Conclusion: Chondroid syringoma is a rare mass on the face. An accurate diagnosis is essential for optimal treatment. This paper reports a case of a chondroid syringoma on the philtrum with a brief review of the relevant literature.

ULTRASTRUCTURAL STUDY ON THE EFFECTS OF RETROGRADE INFUSION OF WATER-SOLUBLE CONTRAST MEDIA ON THE RABBIT SUBMANDIBULAR GLAND (수용성조영제의 주입이 가토악하선조직에 미치는 영향에 관한 전자현미경적 연구)

  • The author observed the effects of retrograde infusion of water soluble contrast media (Tele- brix 30) on the rabbit submandibular glands and compared the effects of different degrees of filling. 26 rabbits were divided into 2 groups of 12 each as experimentals and I group of 2 as normal controls. One experimental group was filled with 0.2㎖ and the other with 0.4㎖. Right submandibular gland of each rabbit was infused with contrast media and left one with physiologic saline as a experimental control, at a constant rate of 0.12㎖/min. using an infusion pump via the main excretory duct. Immediately after the infusion of contrast media, oblique lateral radiographs of the glands were made with occlusal film in order to confirm the glandular filling. The rabbits were sacrificed after varying periods (1, 8, 24 hours and 3, 6, 10 days) and the tissues were prepared for light and electron microscopic examination. The results were as follows: 1. In glands filled with 0.2㎖ contrast media, the initial changes were a few vacuole formation in the acini and slight dilation of the intralobular duct. The moderately severe changes such as vacuole formation in the acini, the abnormal substructure within the secretory granule, dilation of acinar and intercalated duct lumen, scalloping of striated duct lumen and inflammatory cell infiltrate were observed at 3 days. The general appearance was successively recovered, so the tissue had a normal appearance at 10 days. 2. In glands filled with 0.4㎖ contrast media, the most prominent alterations such as severe acinar atrophy, decreased number of secretory granules, proliferation of connective tissue stroma and pronounced inflammatory cell infiltrates appeared at 6 days. Although the general appearance returned to be almost normal at 10 days, acinar cells showed some atrophy and decreased secretory granules. 3. In glands subjected to 0.4㎖ infusion, the alterations were more severe and the recovery was slower than those seen in the glands to 0.2㎖ infusion.

The Effect of Fibrillar Collagen on Bony Healing of Calvarial Defect in Rats (골 조직 치유과정에서 Collagen 막의 효과)

  • Many researches have been reported that collagen as cellular stroma, matrix of grafting materials, mediator of agents for the purpose of promoting healing process invivo, but the responses in vivo were seen various. The goal of this experiment is to assess the effect of collagen on bony healing, through histological evaluation of implanted collagen on the calvarial defect in rats. 2-month-old Sprague-Dawley, 24 rats were used and 12 rats assigned to each group of control and test. Defect of 5mm in diameter was made on the calvarial bone with trephine bur. Following thorough saline rinse, defect of control group was left in empty and that of experimental group was filled with fibrillar collagen($COLLATAPE^{(R)}$, COLLA-TEC. INC. U.S.A.) soaked in saline. 3 rats in each group were sacrificed at 3, 7, 14, 21 days after operation respectively, and the tissue blocks were prepared for light microscope with H-E for evaluation of overall healing, with TRAP(tartrate resistant acid phosphatase) for evaluation of osteoclastic activity and with immunohistochemical staining for macrophages. The results were as follows : 1. In the control group, inflammatory responses were disappeared at day 14, but, in the experimental group inflammatory infiltrates were reduced at day 21. Thus, the experimental group showed more severe soft tissue inflammation than control group. 2. Both control and experimental group showed slight appositional growth at day 7 and gradual bony growth to 21th day. But, complete bony healing of the defect was not shown. There was no significant difference in bony healing between control and experimental group 3. Specific response of macrophages for implanted collagen was observed at day 14 in the experimental group. In conclusion, although fibrillar collagen caused inflammation of soft tissue during initial healing period, inflammatory responses by fibrillar collagen didn't inhibit bony regeneration and implanted collagen was biodegradaded by macrophages. Thus, we expect that fibrillar collagen can be used for useful mediator of graft materials or growth factors.

  • Giant cell tumor of the small bones of the hands and feet is rare. Giant cell tumors in these locations develop at young age, are more commonly multifocal, and show the higher risk of recurrence than those at the end of the long bone. It should be differentiated from the other lesions of the hands, such as giant cell reparative granuloma, aneurysmal bone cyst and enchondroma. We experienced a case of giant cell tumor in the proximal phalanx of the left hand with swelling and pain. Curettage and bone graft were performed. Histologically large number of giant cells were distributed diffusely in the highly cellular stroma containing sheets of mononuclear cells. Secondary aneurysmal bone cyst and hemorrhage were associated.

Fine Needle Aspiration Cytology of Carcinoma ex Pleomorphic Adenoma of Salivary Gland - Report of five cases - (타액선의 다형성 선종에서 발견한 암종의 세침흡인 세포학적 소견 -5 예 보고-)

  • Carcinoma ex pleomorphic adenoma(CXPA) is a rare malignant tumor of the salivary gland, fine needle aspiration cytology(FNAC) of which are rarely reported. We retrospectively reviewed FNAC of five cases of histologically proven CXPA of the salivary glands diagnosed in Seoul National University Hospital to evaluate their cytologic characteristics. Histologically, the carcinoma components were low-grade in three cases and high-grade in two cases. The cytologic diagnosis were malignancy in two cases(40%), suspicious for malignancy in one(20%), and benign(pleomorphic adenoma) in two(40%). All of the high-grade carcinomas were correctly diagnosed by FNAC, however, 2 cases of low-grade carcinomas were diagnosed as benign by FNAC. The low sensitivity in cytologic diagnosis is due to interpretative difficulties in low-grade tumors. The characteristic cytologic features of CXPA were high cellularity, necrotic, bloody, and inflammatory background, marked nuclear overlapping, coarsely granular chromatin, prominent macronucleoli, and atypical mitosis in epithelial component in addition to the typical biphasic pattern of pleomorphic adenoma consisting of epithelial cells admired with spindle-shaped cells and chondromyxoid stroma. Even though low-grade tumors show mild pleomorphism, the cytologic findings of necrotic background, moderate to high cellularity, and nuclear overlapping could lead to an accurate cytodiagnosis of CXPA.
