한방병원 뇌졸중환자의 삶의 질과 자기효능감, 가족지지와의 관계 연구 (A Study on the Quality of Life, Self-Efficacy and Family Support of Stroke Patients in Oriental Medicine Hospitals)

  • The purpose of this study was to analyze the relationship between quality of life, self-efficacy and family support of stroke patients in Oriental medicine hospitals. The subjects of this study were 211 hospitalized stroke patients in seven oriental medicine hospitals in the Busan Metropolitan, Gyeongsangnam-do and Gyeongsanbuk-do area. Data was collected through questionnaires from June 1st, 2002 to September 30th, 2002. 1) The relationship between general characteristics and other variables showed significant differences as follows : for general self-efficacy degree, age, occupation, and level of education ; for family support, level of education and occupation ; for quality of life, level of education, occupation, religion, and monthly income. 2) The relationship between disease-related characteristics and other variables showed significant differences as follows ; for general self-efficacy, level of paralysis, and periods of hospitalization ; for specific character of paralysis, level of paralysis, and periods of hospitalization ; for family support, care-giver, character of paralysis, level of paralysis, and periods of hospitalization related; for quality of life, level of paralysis, and periods of hospitalization. 3) There was a significant correlation between the quality of life and general self-efficacy, the quality of life and the specific self-efficacy, the specific self-efficacy and family support.

재가 뇌졸중환자의 일상생활활동, 자기효능감, 삶의 질, 자조관리프로그램요구도와의 관계에 관한 연구 (A Correlational Study on Activities of Daily Living, Self-efficacy, Stroke Specific Qualify of Life and Need for Self-help Management Programs for Patients with Hemiplegia at Home)

  • The purpose of this study was to identify levels of activity of daily living, self-efficacy. stroke specific quality of life and need for self-help management program for patients with hemiplegia in the home. Data were collected from June to November, 2000 and subjects were 88 poststroke patients who lived in Seoul and Kyunggi-do. The questionnaire consisted of 5 scales: activities of daily living, self-efficacy, stroke specific qulaity of life and need for a self-help management program. Data were analyzed using frequencies, percent, paired t-test, and Pearson's correlation coefficient with the SAS(version 6.12) program. The results are as follows ; 1) Most of subjects were Partially independent in ADL, but they needed assist once to do dressing, bathing meal preparation and house keeping work. 2) The mean self-efficacy score was 54.89(range : 1 to 80) and the individual differences were large. 3) Subjects responded that they were satisfied on the stroke specific quality of life scale totaled 65.8%. This value is comparatively low, especially for social role(51.4%), family functioning(58.3%) and mood (62.2%). 4) The highest needs for self-help management programs were for physical therapy, stress management, and range of motion exercise and the lowest needs were for elimination management and training, family counseling, and speech therapy. 5) On the demographic variables, sex showed significant differences for the dependent variables. Females had higher scores than males for IADL, self-efficacy, stroke-specific quality of life, and need for self-help management. 6) Age had high negative correlation with ADL, self-efficacy and stroke specific quality of life. Age was also correlated with need for self-help management. In conclusion, there was a high correlation for ADL, Self-efficacy and Quality of life in poststroke patients of home. The patient with a stroke also had a strong need for self-help management programs especially physical therapy and stress management. Therefore rehabilitation programs based on self-efficacy enhancement need to be developed in order to promote independent living for patients with hemiplegia.

Examining the Quality of Life Related to Fall Experience in Chronic Stroke Patients

  • PURPOSE: The purpose of this study was to investigate the quality of life related to fall experiences in chronic stroke patients. METHODS: This cross-sectional study included 117 patients with stroke from 3 hospitals in D metropolitan city. General characteristics, including fall experiences and quality of life, were assessed through a face-to-face interviews conducted in a quiet place using a questionnaire. Measurement of quality of life in stroke patients was conducted using the Korean Stroke Specific Quality of Life Scale (SS-QOL). To identify the SS-QOL items related to fall experiences, the items of the SS-QOL were considered as independent variables, and the variables that were significantly different according to fall experiences were identified using a univariate analysis. A binary logistic regression was then performed using fall experiences as the independent variable. RESULTS: According to the univariate analysis, self help activities, social role, and upper extremity function were significantly lower in the fall group than that in the non-fall group (p<.05). The findings of the binary logistic regression confirmed that social roles and upper extremity function were the SS-QOL items that were related to fall experience in chronic stroke patients. CONCLUSION: These findings suggest that social roles and upper extremity function may be risk factors for fall experience in patients with chronic stroke.

성격특성이 통원 재활치료를 받는 뇌졸중 환자들의 우울과 삶의 질에 미치는 영향 (Influences of Personality Traits on Depression and Quality of Life in Stroke Outpatients Receiving Rehabilitation Therapy)

  • Purpose : The aim of this study was to investigate the influences of personality traits on depression and quality of life of stroke outpatients receiving rehabilitation therapy. Methods : The current study included 52 outpatients who received occupational therapy in a rehabilitative and general hospital located in Incheon. Their personality traits were assessed using the Big Five Inventory-Korean Version (BFI-K), and depression was measured using the Beck Depression Inventory (BDI). Quality of life was evaluated using the Stroke Specific Quality of Life (SS-QOL) scale. The collected data were analyzed to identify the effects of personality traits on depression and quality of life using a multiple regression analysis. Result : Neuroticism and depression were significantly correlated (p<.05), and neuroticism and agreeableness were significantly correlated with quality of life (p<.05). Additionally, depression and quality of life were significantly correlated (p<.05). Neuroticism affected depression significantly ($R^2=.503$, p<.05). Among the five personality traits, neuroticism, agreeableness, and extraversion had a significant effect on quality of life ($R^2=.474$, p<.05). Conclusion : These findings suggest that personality traits need to be considered in order to improve the quality of life of stroke patients.

거주 환경에 따른 뇌졸중 환자의 시간 사용과 삶의 질 비교 연구 (A Comparative Study of the Time Use and Quality of life in Local Community and Convalescent Hospital of the Stroke Patient)

  • 목적 : 본 연구의 목적은 지역사회와 시설에 거주하는 뇌졸중 환자의 시간 사용과 삶의 질을 비교 분석하고자 한다. 연구방법 : 연구 대상은 요양병원에 입원 중이거나 외래로 재활치료 중인 뇌졸중 환자 74명을 대상으로 하였다. 일반적 특성, 작업 설문지(Occupational Questionnaire: OQ), 뇌졸중 환자 삶의 질(Stroke Specific Quality of Life: SS-QOL) 평가를 통해 조사하였고, 대상자 선정 기준에서 부적합한 8명을 제외한 총 66명을 대상으로 비교 분석하였다. 결론 : 거주 환경에 따른 시간 사용을 비교 분석한 결과 일상생활(p<.05), 휴식(p<.05)에서 유의한 차이가 있는 것으로 분석 되었다. 또한 삶의 질은 가족(p<.05), 자조관리(p<.05), 사회적 역할(p<.05)에서 유의한 차이가 있는 것으로 분석되었다. 결론 : 지역사회 거주 뇌졸중 환자가 시설 거주 뇌졸중 환자보다 일상생활과 휴식에 소비되어지는 시간을 효율적으로 사용을 하였고, 삶의 질 또한 높은 것으로 나탔다.

Effects of respiratory reeducation exercise using a pressure biofeedback unit on the quality of life of persons with stroke

  • Objective: The purpose of this study was to confirm the validity of a respiratory retraining exercise using pressure biofeedback units among individuals with stroke as an effective intervention for improving quality of life. Design: Randomized controlled trial. Methods: Thirty patients with stroke were recruited as subjects. Among them, 15 patients were randomly assigned to an experimental group performing lumbar stabilization exercise and respiratory retraining exercise, and the other 15 patients were randomly assigned to a control group conducting only lumbar stabilization exercises. Exercises were conducted 3 times a week for 6 weeks, and quality of life was evaluated in the pre-test, 3 weeks and 6 weeks periods. The respiratory retraining exercises were performed using a pressure biofeedback unit and the degree of the quality of life was measured using the Stroke- Specific Quality of Life. For data analysis on the study results, a two-way repeated ANOVA was used in order to observe for changes in the measured variables according to time for both groups. If there was a reciprocal action between the groups and the time in the effect test within the entities, a one-way repeated ANOVA was implemented and was statistically processed. Results: There was a significant difference in the main effect test between the 2 populations depending on the duration of the experiment (6 weeks) (p<0.05). Conclusions: The above results showed that respiratory retraining exercises may provide positive effects in the treatment of stroke as the quality of life showed significant differences according to the duration of treatment.

클라이언트 중심 상상훈련이 뇌졸중 환자의 일상생활 수행과 삶의 질에 미치는 영향 (Effects of Client-centered Mental Practice on Stroke Patients' Activities of Daily Living and Quality of Life)

  • 본 연구는 클라이언트 중심 상상훈련이 뇌졸중 환자들의 일상생활활동 및 삶의 질에 미치는 효과를 알아보고자 실시되었다. 경상북도 K병원에 내원한 뇌졸중 16명을 대상으로 하였으며, 클라이언트 중심 상상훈련은 전 후 평가를 제외한 총 4주 동안 주 5회 30분씩 실시하였다. 평가도구는 기능적 독립척도(Functional Independence Measure:FIM)와 뇌졸중 특이 삶의 질 척도(Stroke Specific Quality of Life: SS-QOL)이 사용되었다. 연구 결과 클라이언트 중심 상상훈련 실시 전에 비하여 실시 후 만성기 뇌졸중 환자들의 FIM 점수에서 유의미한 변화가 나타났고, SS-QOL값에서도 긍정적인 변화가 있었다. 본 연구의 결과로 볼 때 클라이언트 중심 상상훈련이 뇌졸중 환자들의 일상생활활동 및 삶의 질 향상에 긍정적인 효과의 가능성을 확인할 수 있었다.

과제 지향적 훈련과 수정된 강제유도 운동치료가 뇌졸중 환자의 삶의 질에 미치는 영향 (The Effects of Task Oriented Activity and Modified Constraint Induced Movement Therapy on Quality of Life for Patients With Stroke)

  • 목적 : 본 연구는 과제 지향적 훈련과 수정된 강제유도 운동치료(modified Constraint Induced Movement Therapy; mCIMT)가 뇌졸중 환자의 삶의 질에 미치는 영향을 알아보고자 하였다. 연구방법 : 대구에 소재한 K병원에서 입원하여 치료를 받는 30명의 뇌졸중 환자를 대상으로 과제 지향적 훈련 그룹과 mCIMT 그룹으로 각각 15명씩 무작위로 배분하였다. 치료 중재 전 후의 삶의 질을 비교하기 위하여 뇌졸중 특이 삶의 질 척도(Stroke Specific Quality of Life; SS-QOL)를 시행하였다. 결과 : 두 그룹 모두 치료 중재 전 후 뇌졸중 환자의 삶의 질은 통계학적으로 유의한 차이를 보였고(p<.05), 치료 중재 후 두 그룹간의 삶의 질은 통계학적으로 유의한 차이가 없었다(p>.05). 결론 : 과제 지향적 훈련과 mCIMT가 뇌졸중 환자의 환측 상지기능 및 움직임 향상을 통해 삶의 질을 향상시킨다는 것을 알 수 있었다. 과제 지향적 훈련과 mCIMT를 작업치료와 함께 임상에 적용함으로써 뇌졸중 환자의 삶의 질에 긍정적인 효과를 보일 것이라 사료된다.

뇌졸중 후 정신건강이 삶의 질에 미치는 영향 (The Effects of Mental Health on the Quality of Life After Stroke)

  • 본 연구는 뇌졸중 환자의 정신건강과 삶의 질 간의 상관성을 파악하고 정신건강이 삶의 질에 미치는 영향을 알아보고자 하였다. 뇌졸중으로 진단받은 224명을 대상으로 삶의 질의 측정을 위해 뇌졸중 환자 삶의 질 평가도구(Stroke Specific Quality of Life: SS-QOL), 정신건강은 간이정신진단검사(Symptom Check-List-90-Revised: SCL-90-R)를 사용하였다. 정신건강과 삶의 질 간의 상관성을 알아보기 위해 피어슨 상관관계를 이용하였고, 정신건강이 삶의 질에 미치는 영향을 확인하기 위해 단순 회기분석과 하위 요인들을 알아보기 위해 단계적 다중 회귀분석을 실시하였다. 정신건강과 삶의 질은 통계적으로 유의한 상관성을 보였으며, 정신건강이 삶의 질 변화에 영향을 미치는 유의한 요인으로 $R^2$ 0.28로 28%의 설명력을 보였으며, 하위영역 중 우울은 $R^2$ 0.42로 42%의 설명력을 보였다. 뇌졸중 환자에서 우울이 낮을수록 환자의 삶의 질이 높아지는 것을 알 수 있었다. 그러므로 뇌졸중 환자의 삶의 질을 향상시키기 위해 재활 초기부터 정신건강 요인을 포함한 전인적 접근과 중재에 초점을 두어야 할 것이다.

클라이언트 중심 수공예 활동이 뇌졸중 환자의 우울과 삶의 질에 미치는 효과 (The Effects of Client-Centered Art and Craft Activities on Depression and Quality of Life in Stroke Clients)

  • Purpose: The purpose of this study was to evaluate the effects of client-centered art and craft activities on depression and quality of life in stroke clients. Through this, we aim to find a way to reduce depression and improve the quality of life in stroke clients. Methods: Clients diagnosed with stroke were selected as participants for the study. Participants in the experimental group (n=13) and control group (n=14) received general occupational therapy. Clients in the experimental group participated in client-centered art and craft activities, whereas clients in the control group participated in general art and craft activities for 8 weeks. Beck Depression Inventory(BDI) and Stroke Specific Quality of Life(SS-QOL) were used to evaluate the depression and quality of life of the clients before and after the intervention. Results: The experimental group and control group presented significant statistical difference in depression before and after intervention (p<.01; p<.05). The experimental group showed a greater decrease in depression (p<.05) than the control group. Additionally, the experimental and control group displayed significant statistical difference in quality of life (p<.01) before and after intervention. The experimental group showed a more statistically significant improvement in quality of life (p<.01) than the control group. Conclusion: These results demonstrate the significance of client-centered art and craft activities in reducing depression and improving quality of life in clients with stroke. Therefore, it is expected to be useful in clinical settings. Occupational therapy should be provided based on the decision of the clients.
