항인지질항체증후군 환자에서 경동맥 부동혈전에 의한 반복적인 허혈뇌졸중 (Recurrent Ischemic Stroke by Carotid Free-Floating Thrombus in Patient with Antiphospholipid Antibody Syndrome)

  • Carotid free-floating thrombus (FFT) is a rare cause of stroke. The FFT is commonly associated with an atherosclerotic plaque rupture, but other causes need to be examined in young patients. We report a case of 31-year-old male patient with recurrent ischemic stroke caused by carotid FFT. Although initial laboratory test was unremarkable, recurrent stroke events led us to perform additional study and antiphospholipid syndrome (APLS) was diagnosed. Repetitive testing for APLS seems considerable when young patients present with recurrent stroke.

  • Background: To determine effects of mobilization with movements (MWM) application on stroke patient with hypomobility on sacroiliac joint. Methods: The subject was 47years old male who have left hemiplegia because of right intra-cerebral hemorrhage. The subject was have hypomobility on sacroiliac joint. MWM using during 4weeks and using Berg balance scale (BBS), Timed up and go (TUG), 10meter walking (10MW) test for evaluation. Results: The results of this study were summarized below; BBS score for evaluating balance ability was increased, and TUG time, 10MW test time was decrease. Conclusion: We consider that MWM application on stroke patient with hypomobility on sacroiliac joint is not only regain mobility on sacroiliac joint but also increase in balance ability and walking speed.

마리화나 흡연으로 인해 발생한 젊은 연령 뇌경색 1례 (A Case of Ischemic Stroke in Young Patient Associated with Marijuana Use)

  • A 21-year-old female patient complaining of hemiparesis was diagnosed with right middle cerebral artery infarction. No risk factor was found, despite an extensive young-age stroke work-up, except her history of marijuana use. The patient had smoked marijuana for treating depression for more than five years. Magnetic resonance angiography showed multifocal intra- and extracranial stenoses, suggesting cannabinoid-induced vasculopathy. Since the use of illicit drugs has increased nationwide, physicians should consider it as a possible cause of a stroke due to an unknown etiology.

재활치료 중단 후 보행 양상이 다시 악화된 뇌졸중 환자 1례에 관한 고찰 (A Clinical Study of a Stroke Patient with a Worsened Gait Pattern after Discontinuing Rehabilitation)

  • Objective: This study describes the case of a patient with a left basal ganglia (BG) infarct who had made almost a complete recovery upon discharge from the hospital in 2014 but who was re-admitted after 31 months with a worsened hemiplegic gait. Methods: The patient had undergone no rehabilitation treatment in the 31 months since his discharge. When the patient was re-admitted to our hospital in 2017, stance and gait data were collected on the patient using a treadmill gait analysis system. In addition, the patient underwent a manual muscle test (MMT) evaluation, and his Motricity index (MI) and modified Barthel index (MBI) scores were recorded. After rehabilitation for one month, the patient was reassessed, and the results were compared to those on the day of re-admission and those recorded in 2014. Results: Compared to the 2014 evaluation results, the patient's stance parameters and gait parameters had worsened at re-admission. However, there was no significant change in the patient's MMT grade or MI and MBI scores in comparison to the results of the 2014 evaluation. After one month of rehabilitation, the patient was re-evaluated again, but there was no significant change in comparison to the evaluation results at re-admission. Conclusion: Some of the stroke patients who have passed six months since the onset of their stroke may require ongoing rehabilitation although the functions of them is almost recovered. Because there is a possibility that recovered functions get worse again without any rehabilitation for a long time. And once the recovered functions get worse, re-recovery of them is not easy.

환자선택적 과제지향훈련이 뇌졸중 환자의 일상생활동작 수행, 삶의 질, 우울감에 미치는 영향 (The Effect of Patient-Selected, Task-Oriented Training on Activities of Daily Living, Quality of Life, and Depression in Stroke Patients)

  • PURPOSE: This study was conducted to determine the effect of patient-selected, task-oriented training on activities of daily living, quality of life, and depression for stroke patients. METHODS: This study included 40 stroke subjects, randomly assigned to either an experimental group or a control group. Both the experimental group and the control group included 20 stroke patients. Patients from the experimental group selected and performed task-oriented training. In the exercise program, the patient chose two categories from six categories and trained for 30 minutes a day, three times a week, for four weeks. Patients from the control group performed conventional task-oriented training. Outcomes were measured using the Korean version of a modified Barthel Index (K-MBI), the Korean Instrumental Activities of Daily Living (K-IADL), the Stroke-specific Quality of Life (SS-QOL), the Short Form 8 (SF-8), and the Beck Depression Inventory (BDI), both before and after training. RESULTS: There were significant differences between the before- and after-intervention data of the K-MBI and the K-IADL (p<.05) for both groups. Also, there were more significant improvements in the experimental group compared to the control group (p<.05) for the SS-QOL, SF-8 and BDI. CONCLUSION: The results of this study showed that patients-selected, task-oriented training had positive effects on stroke patients' activities of daily living, quality of life, and depression.

뇌졸중 환자의 정서반응 유형에 관한 연구 (A Subjectivity Study on Type of Emotional Response of Stroke Patient)

  • The purpose of this study was to identify emotional responses, of stroke patients using Q analysis. From the data analysis, five types of emotional responses in stroke patients were classified as follows. 1. Acceptance and Overcoming : They attached to life strongly and had self-confidence of recovery. They were striving against their situation through using anything else good for their recovery. 2. Anger Type : They recognized a labile feeling and were angered easily. They had a self-confident and self-esteemed personality before they had a stroke. 3. Conflict Type : They wanted interaction with others. This individual recognized that no one knows their agonies. They were thankful to god or their family, while they could not control their minds. Their family supports were low. 4. Deficiency of self-esteem Type : They had a self-confidence of recovery, but they were in fear of interaction with others. Especially, they were ashamed of themselves. They have a high sense of responsibility and nigh self-esteem for themselves before they had a stroke. 5. Despair Type. : They were sorry to trouble their family. They felt depression, anxiety, darkness, uselessness of themselves, and they wanted to die. They recognized financial burden. for their family. The characteristics of the five types of stroke patient identified in this study will be used to assess emotional nursing needs for stroke patients. The findings of this study will provide practical guidelines for developing nursing interventions for stroke patients based on the characteristics of subjectivity types.

뇌졸중 환자의 장애물 보행능력 향상과 하수족 개선을 위한 고유수용성신경근촉진법의 적용: 증례보고 (Approach of Proprioceptive Neuromuscular Facilitation on Foot drop and Crossing Obstacles in stroke patient: Case report)

  • Purpose: The purpose of this study was to describe the effects of proprioceptive neuromuscular facilitation (PNF) on episodes of foot drop and the ability to cross obstacles in stoke patients. Methods: The patient was a 57-year-old man with left hemiplegia. This patient displayed episodes of foot drop while walking and issues with crossing obstacles. The patient underwent therapeutic exercises and occupational therapy twice daily at a hospital five times a week. The patient was treated with PNF outside of these therapeutic sessions. Each session, which was 60 minutes in length, was conducted 3 times a week over 8 weeks. Over the course of treatment, a pre-test and post-test were completed. The patient walked for a test time of 6M. During the testing process, the patient crossed obstacles. In addition, the number of foot drop episodes was recorded. Each test was measured three times to determine the mean value. Results: After the PNF treatments, the number of foot drop episodes decreased from five to one. The 6M walking test time during which the patient crossed obstacles also decreased from 18 seconds to 10 seconds. Conclusion: The use of PNF treatments is beneficial to reduce the number of foot drop episodes while walking and to improve the ability to cross obstacles in stroke patients.

방실차단에 의한 서맥을 보이는 환자 한방 치험 1례 (A Case of Bradycardia with Atrioventricular Block Treated with Korean Medicine)

  • This is a case report of a 57-year-old patient diagnosed with bradycardia and atrioventricular (AV) block. The chief complaint of the patient was palpitation and dizziness. An electrocardiogram (ECG) showed a second degree AV block with 2:1 AV conduction. The patient was treated with Korean medicine, including herbal medicine, for 35 days. After 2 weeks of treatment, the heart rate increased from 33 to 56 and the follow-up electrocardiogram showed normal findings. The symptoms of palpitation and dizziness were also improved. The effect on heart rate, ECG, and symptoms lasted until the end of the treatment. This case report demonstrated that Korean medicine could be effective for treatment of symptomatic bradycardia with AV block.

  • ■ Objectives The purpose of this clinical study is to evaluate the effect of Sosiho-tang(小柴胡湯) on a patient with Central fever due to pontine hemorrhage. ■ Methods A patient with central fever diagnosed with pontine hemorrhage was treated with herbal medication, acupuncture, electro-acupuncture(EA). Then we evaluated the improvement by average and peak of each day's body temperature. ■ Results Decrease of average and peak of each day's body temperature were observed after taking Sosiho-tang(小柴胡湯). ■ Conclusion This study proved the effect of moxibustion treatment on left side paresthesia due to central post-stroke pain.

  • ■ Objectives The purpose of this clinical study is to evaluate the effect of Traditional Korean Medicine(TKM) on a patient with abnormal eye movement. ■ Methods A patient with abnormal eye movement of limbs diagnosed with midbrain infarction was treated with herbal medication, acupuncture, moxa, and herbal medical injection. Then we evaluated the improvement by measuring range of eye movement. ■ Results Increase of range of eye movement and improvement of symptom of diplopia were observed after the TKM treatment. ■ Conclusion This study proved the effect of TKM treatment on abnormal eye movement due to midbrain infarction.
