  • Dr. Lawrence H Jones developed innovative approach for the treatment of neuromuscular and musculo-skeletal disorders such as fibromyalgia, myofacial pain syndrome, any other muscle pain. This technique is called Strain/Counterstrain technique which uses effect of neuromuscular reflex for treatment of strain. First, Relieving spinal or other joint pain by passively putting the joint into its position of greatest comfort; secondly relieving pain by reduction and arrest of the continuing inappropriate proprioceptor activity. This was accomplished by markedly shortening the muscle that contains the malfunctioning muscle spindle by applying. mild strain to its antagonists. In other words, the inappropriate strain reflex is inhibited by application of counterstrain. Many other techniques have been developed for treating of muscle pain due to somatic dysfunction, but we want to introduce you to strain/counterstrain technique because this is basic concept and theory of Chum therapy for controling motor system.

Immediate Effects of Strain-Counterstrain Technique on Pressure Pain Threshold and Muscle Activity in Male Adults With Upper Trapezius Latent Trigger Point

  • The aim of this study was to determine the immediate effects of single treatment of strain-counter strain (SCS) on pressure pain threshold (PPT) and muscle activity during scapular plane abduction with 3% body weight load. Fifteen asymptomatic male adults with upper trapezius latent trigger point (LTrP) (PPT<2.9 $kg/cm^2$) participated in this study. Pressure algometer was used to measure PPT and surface electromyography was used to record upper, middle arid lower trapezius, serratus anterior, infraspinatus and middle deltoid muscle activity and relative ratio during scapular plane abduction between pre- and post-intervention. There was a significant increase in upper trapezius PPT after a 90-second SCS (p<.05). The activity of the upper trapezius and middle deltoid was significantly decreased (p=.014, p=.001), coupled with a decreased muscle activity ratio between the upper and lower trapezius (p<.05). These results indicate that the SCS may effectively deactivate upper trapezius activity, thereby alleviating muscle balance and reducing pain sensitivity.

  • Positional Release Techniques (PRTs) are an umbrella term for manual therapies harnessing spontaneous musculoskeletal balancing mechanism of the body facilitated by finding and maintaining therapeutic position. PRT has its origin in the Strain Counterstrain (SCS) technique by Dr. Jones but encompasses diverse related techniques that stemmed off from the SCS. PRT emphasizes postural balance within the body and innate healing potential of the body including the postural balance of the temporomandibular joint (TMJ). This study briefly reviews concepts, history, and contemporary study reports on PRT with a focus on the yin-yang balance based approach of PRT.

Analysis of Osteopathic Manipulation and Study on Relationship with Chuna Manual Therapy for Meridian Sinew System (정골의학적(Osteopathic) 수기요법 분석 및 경근추나와의 관련성 연구)

  • Objectives : The aim of this study is to analyze the actual investigation and classification of osteopathic manipulation by investigation of the various literature of the inside and outside of the country, and to present the application plan of osteopathy in chuna manual therapy for meridian sinew system. Methods : I referred to the domestic and foreign books about osteopathy and chiropractic. In order to investigate domestic dissertations, I searched 4 Korean medical databases and 4 Korean medical journals of osteopathy. Search terms used were osteopathy, osteopathic, fascia, proprioreceptor, mechanoreceptor, muscle spindle, golgi tendon organ, osteopathic manipulation technics. And I classified all the searched studies into principle and region and etc. In order to investigate foreign dissertations, I search 'NCBI pubmed'. Search terms used were osteopathy, osteopathic technique, osteopathic manipulative technique. Results : 1. Osteopathy do not regard the systems which compose the human body in individual territory, but regard whole. It is diagnosis, prevention and medicine which treats 2. Osteopathic manipulation techniques are classified into direct techniques, indirect techniques, and compound techniques. 3. Osteopathic manipulation techniques are classified into fascia, muscle, ligament-joint in applied region. 4. I could search clinical cases in domestic and foreign study. I found cases about myofascial release technique(MFR), postisometric relaxation(PIR), proprioceptive neuromuscular fascilitation(PNF), muscle energy technique(MET), joint mobilization in domestic studies, and strain-counterstrain technique(SCS), MET, AK in foreign studies. Conclusions : Osteopathic manipulation techniques can be used in diagnosis and treatment of meridian muscle theory, because osteopathy and the oriental medicine have many similarities in theoretical background. So osteopathic manipulation technique can be useful in oriental medicine treatment techniques.
