Purpose: Recently, a wide application of gastrofiberscopy in the pediatric group have revealed that nodular duodenitis is not an uncommon disease in children and is suspected to be associated with H. pylori infection. The aim of this retrospective study was to investigate the clinical and histopathologic features in children with nodular duodenitis, and to assess the correlations beween both. Methods: During a period of 5 years (Jan. 1995~Dec. 1999), we investigated clinical, endoscopic and histopathologic features of 39 consecutive patients diagnosed as having nodular duodenitis at Pediatric department of Seoul Red Cross Hospital. In 35 children with nodular duodenitis endoscopic biopsy specimens were stained with Hematoxylin & Eosin and Giemsa's stain, and were graded according to the criteria outlined by Triadafilopoulos, Whitehead et al., and Prieto et al.. Statistical analyses were performed with Graph PAD InStat. Results: The prevalence rate of nodular duodenitis was 17.1% and the most frequent chief complaint was abdominal pain (69.2%). Endoscopically grade 1 was the most common (45.7%) and nodular gastritis was coexistent in 28.3%. The most common histology of the duodenum was grade 2 (54.3%), and the most common histologic score of the stomach was 2 (42.9%). H. pylori was found in the duodenum in 37.1%, and in the stomach in 31.4%. The correlation coefficient between the endoscopic grade and the histologic grade of nodular duodenitis was 0.3983 (p=0.0178). And the correlation coefficient between the histologic grade and the grade of H. pylori colonization in the duodenum was 0.5154 (p=0.0018). Conclusion: There was significant correlation between the endoscopic grade and the histologic grade of nodular duodenitis, and was also significant correlation between the histologic grade and the grade of H. pylori colonization in the duodenum. Therfore H. pylori infection should be regarded as an etiologic factor of nodular duodenitis.
The endocrine cells of the stomach and duodenum of the pond tortoise (Amyda sinensis) have been studied by electron microscopy. At least six types of endocrine cells could be identified in these regions. Type I cells were characterized by having pleomorphic or medium sized granules ($100{\sim}430nm$ in diameter, mean; 290 nm) which were moderate in electron density. Type II cells were characterized by having pleomorphic or medium sized granules ($190{\sim}430nm$ in diameter, mean; 280 nm) which were high in electron density. Type III cells were characterized by having large oval or irregular granules($170{\sim}650nm$, mean: 430 nm) with wide vesicular halo which were high in electron density. Type IV cells were characterized by having round or medium sized granules($140{\sim}370nm$, mean; 240 nm) which were low to moderate in electron density and showed a very thin halo. Type V cells were characterized by having large round or oval granules($300{\sim}600nm$, mean; 410 nm) which were low to moderate in electron density and showed a dense core and a wide clear halo. Microfilaments were also found in the cytoplasm. Type VI cells were characterized by having round and small granules($100{\sim}220nm$, mean; 160 nm) which were low to moderate in electron density and showed a very thin halo or crystalline structure of the contents. Many microfilament bundles were also found in the cytoplasm.
The regional distributions and relative frequencies of some gastrointestinal endocrine cells in the 8 portions (fundus, pylorus, duodenum, jejunum, ileum, cecum, colon and rectum) of the gastrointestinal tract of ICR mouse (ICR) with immunohistochemical method using 7 types of specific antisera against somatostatin, serotonin, glucagon, cholecystokinin (CCK)-8, secretin, pancreatic polypeptide (PP) and gastrin. In this study, somatostatin-, serotonin-, glucagon-, CCK-8-, secretin- and gastrin-immunoreactive (IR) cells were identified. Most of these IR cells in the intestinal portion were generally spherical or spindle in shape (open-typed cell) while cells showing round in shape (close-typed cell) were found in the stomach regions occasionally. Their relative frequencies were varied according to each portion of gastrointestinal tract. Somatostatin-IR cells were demonstrated throughout whole gastrointestinal tract except for large intestine. Serotonin-IR cells were detected throughout whole gastrointestinal tract and they were most predominant endocrine cell types in this species of mouse. Glucagon-IR cells were restricted to the fundus and rectum with moderate and a few frequencies, respectively. CCK-8-IR cells were observed in the pylorus, duodenum and ileum with numerous, moderate and rare frequencies, respectively. Secretin-IR cells were restricted to the duodenum and ileum with a few and rare frequencies, respectively. Gastrin-IR cells were restricted to the pylorus with numerous frequency. However, no PP-IR cells were found in this study. In conclusion, some peculiar distributional patterns of gastrointestinal endocrine cells were found in the ICR mouse compared to those of other mammals.
Purpose: This study was performed to assess the clinical usefulness of transabdominal ultrasonography (TUS) in detecting peptic ulcer disease (PUD) in children. Methods: Twenty-four patients (19 boys, 5 girls; mean age, $10.6{\pm}4.5years$ [range, 3.0-17.9 years]) who were admitted to the hospital for acute abdomen or gastrointestinal bleeding and diagnosed with PUD by endoscopy and who underwent TUS were included. Clinical data were retrospectively collected by reviewing patient medical records. Gastric ulcer (GU) was suspected when the gastric wall exceeded 8 mm in thickness and had lost its five-layer structure on TUS. Duodenal ulcer (DU) was suspected if the duodenal wall thickness exceeded 5 mm. Results: Sensitivity of TUS in diagnosing PUD was 66.7% for GU and 38.9% for DU. Mean age and body weight of the 11 patients suspected with PUD on TUS were $10.9{\pm}4.4years$ and $38.1{\pm}17.2kg$, respectively. For 13 patients without suspected PUD, they were $12.1{\pm}4.1years$ and $39.6{\pm}17.0kg$, respectively. There was a significant difference in height, weight, and body mass index between patients who were suspected to have PUD and those who were not suspected on TUS (p=0.014, 0.008, and 0.005, respectively). A significant difference in the sensitivity of TUS in diagnosing PUD was found between patients under 30 kg and those over 30 kg (88.9% and 20.0%, respectively; p=0.003). Conclusion: TUS investigation of the stomach and duodenum is an efficient method for PUD detection in children with low body weight. TUS can be used in preliminary diagnostic work-up before further invasive tests.
Purpose: The goal of this study was to review the clinicopathologic characteristics of neuroendocrine tumor (NET) of the stomach. Materials and Methods: We retrospectively reviewed the medical records of 13 patients who were diagnosed with neuroendocrine tumor from January 1999 to August 2007 at Kosin Medical Center; 4,159 gastric cancer patients were treated surgically during the same time. The average follow up period was 14.3 months. Results: The majority of 13 patients were men (male-female ratio: 11:2) and the average age of patients with NET was 59.4 years (range: 42~72 years). The presenting symptoms were mostly epigastric pain and soreness. The tumor was limited to the mucosa or submucosa in two cases, and the tumor extended beyond the muscle layer in 11 cases. The mean size of the tumor was 7.0 cm, ranging from 0.7 cm to 15 cm. The type of the NEC (according to the WHO classification) was type 3 for eight patients, type 4 for four patients and type 1 for one patient. Regional lymph node metastasis was noted in 11 patients. Four cases showed recurrence of disease and the site of recurrence included liver in two patients, multiple organs (including the peritoneum and lung) in one patient and multiple organs (including liver, pancreas and duodenum) in one patient. The recurrent cases were type 3 and type 4 and the average survival period of the recurrent patients was 12.8 months. Conclusion: The majority of neuroendocrine tumors of the stomach were at an advanced stage at the time of diagnosis. These tumors frequently recurred in the liver and they have a poor prognosis.

Numerous glucagon-immunoreactive cells were found in the stomach except the pyloric region, and generally a few in the intestine. Moderate number of BPP-immunoreactive cells were found in the stomach except the pyloric region, and a few in the large intestine. Numerous porcine CG-immunoreactive cells were restricted to the cardiac and fundic region. In the pancreas, four types of pancreatic endocrine cells-somatostatin-, glucagon-, BPP- and insuline-immunoreactive-were identified in the pancreatic islet and exocrine portion. These results suggest that the regional distribution, the relative frequencies and cell types of the GEP endocrine cells in the GI tract and pancreas varies considerably among the species.

BACKGROUND/OBJECTIVES: Helicobacter pylori (H. pylori) colonization of the stomach mucosa and duodenum is the major cause of acute and chronic gastroduodenal pathology in humans. Efforts to find effective anti-bacterial strategies against H. pylori for the non-antibiotic control of H. pylori infection are urgently required. In this study, we used whey to prepare glycomacropeptide (GMP), from which sialic acid (G-SA) was enzymatically isolated. We investigated the anti-bacterial effects of G-SA against H. pylori in vitro and in an H. pylori-infected murine model. MATERIALS/METHODS: The anti-bacterial activity of G-SA was measured in vitro using the macrodilution method, and interleukin-8 (IL-8) production was measured in H. pylori and AGS cell co-cultures by ELISA. For in vivo study, G-SA 5 g/kg body weight (bw)/day and H. pylori were administered to mice three times over one week. After one week, G-SA 5 g/kg bw/day alone was administered every day for one week. Tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$), IL-$1{\beta}$, IL-6, and IL-10 levels were measured by ELISA to determine the anti-inflammatory effects of G-SA. In addition, real-time PCR was performed to measure the genetic expression of cytotoxin-associated gene A (cagA). RESULTS: G-SA inhibited the growth of H. pylori and suppressed IL-8 production in H. pylori and in AGS cell co-cultures in vitro. In the in vivo assay, administration of G-SA reduced levels of IL-$1{\beta}$ and IL-6 pro-inflammatory cytokines whereas IL-10 level increased. Also, G-SA suppressed the expression of cagA in the stomach of H. pylori-infected mice. CONCLUSION: G-SA possesses anti-H. pylori activity as well as an anti-H. pylori-induced gastric inflammatory effect in an experimental H. pylori-infected murine model. G-SA has potential as an alternative to antibiotics for the prevention of H. pylori infection and H. pylori-induced gastric disease prevention.
This study was conducted for the development of recording system of students' health problems, and for the application of International Classification of Health Problem in Primary Care(ICHPPC) as a tool of morbidity classification in school health care. The data were collected from 12th of September to 24th of September in 1988. The objects were composed of health problems written by 10 school nurses who take service in the elementary school. The results were as follows: 1. The features of students' health problmes. The health problems of students were 68 problems from the total numbers of 361 codes of ICHPPC. The 93.4% of health problems was contained in 20 descriptive diagnoses and 97.0% was contained in 30 descriptive diagnoses. According to frequency of main health problems, There were abrasion, scratch and blister(26.7%); disorder of stomach function, other disease of stomach and duodenum (20.4%); headache(10.6%); bruise and contusion (5.3%); acute URI (5.0%); laceration and open wound(4.6%); Insect bite and sting(4.0%); epistaxis(3.4%): abdominal pain(2.6%): superficial tissue(1.7%). Out of all health problems, Category 17(accident, injury and poisoning was 44.7%. and Category 9(digestive system Disease) was 22.2%. 2. Applicability of ICHPPC by the school nurses. School nurses used 68 codes, among the total number of 361 codes from ICHPPC. According to ICHPPC method, school nurses can classified more diverse health problems systematically and objectively than that in other studies on school nurses activities. ICHPPC was found as a useful and applicable tool of morbidity classification in the practice of school nurses.

We'd performed the upper gastrointestinal study for a total of 1,033 insureds-male 630, female 403 persons-who were examined at medical dept. of Dae Han Kyoyuk Insurance Co., Ltd., from August, 1986 to January, 1987. The results on diverticulum were as follows; 1. The incidence rate of duodenal diverticulum is exceptionally higher than esophageal diverticulum. 2. In all of 53 insured who have duodenal diverticulum and esophageal diverticulum, there was little difference between male and female in the incidence rate of diverticulum; 5.08% in male, 5.21% in female. 3. The possessing rate of both diverticulums increased by age regardless of sex. ; 0.71% in 20yrs, 2.12% in 30yrs, 11.11% in 40yrs, 12.75% in 50yrs, 30.43% in 60yrs more. 4. The possessing rate of both diverticulums in male is 0% in 20yrs, 1.97% in 30yrs, 7.21% in 40yrs, 15.09% in 50yrs, 27.27% in 60yrs more and in female, 1.16% in 20yrs, 2.40% in 30yrs, 7.87% in 40yrs, 10.20% in 50yrs, 33.33% in 60yrs more. 5. Those who have duodenal diverticulum 47 insureds felt the following subjective symptoms; uncomfortable 8.51%, heartburn and tingling each 4.26%, sore throat 2.13%and esophageal diverticulum's heartburn 16.67%. 6. There occurred the following complications in 47 insureds with duodenal diverticulum-gastric polyp, erosive gastritis, antral gastritis, cascade stomach, fatty liver, polyp in GB and choledocholithiasis; each 2.13% and cholelithiasis 6.38% and cascade stomach. 7. All duodenal diverticulum occurred in duodenal inlet. 8. The number of lesion was single in all esophageal diverticulum, but there was each one case with 2 lesions and 3 lesions in duodenal diverticulum.
