슬개골 탈구 환견에서 지대 중첩, 활차구 성형 및 경골결절변위술의 복합수술 평가 (Evaluation of Combination of Retinacular Imbrication, Trochleoplasty and Tibia Tuberosity Transposition in Dogs with Patellar Luxation)

  • The purpose of this study is to evaluate the results obtained from three surgical techniques (retinacular imbrication, trochleoplasty and tibia tuberosity transposition), which were simultaneously applied for correction of patellar luxation. Eleven, (6 to 108 months) old dogs were presented. The dogs showed, medial patellar luxation (17 stifle joint) and lateral patellar luxation (2 stifle joint) ranging from grade 1 to 4. By palpation and radiographic view, all 11 dogs were diagnosed as patellar luxation. Bilateral patellar luxation was diagnosed in 8 dogs. After operation, the dogs were monitored daily for 7 days and on the 10th, 15th, 20th, 25th and 30th days. Lameness progressively decreased in all dogs around 30 days after surgery, and the patients were showed normal walking. Normal walking was possible in 2 dogs after 4 days, in a dog after 5 days, a dog after 7 days, 3 dogs after 10 days,2 dogs after 15 days, a dog after 25 days and a dog after 30 days (Mean$\pm$S.E: 12.27$\pm$2.57). The recovery took 14 days in dogs with bilateral patellar luxation, and 9 days in dogs with unilateral patellar luxation. Trend of decrease of lameness grade in bilateral patellar corrected dogs were similar to that in unilateral patellar corrected dogs. Swelling, pain and fever disappeared on 7 days. In conclusion, combination of tibial tuberosity transposition, retinacular imbrication and trochleoplasty is appropriate for dogs with patellar luxation.

실험적으로 전십자 인대를 단열한 개에서 TPLO의 핵의학적 평가 (Scintigraphic Evaluation of Dogs with Experimentally Transected Cranial Cruciate Ligaments Treated Using Tibial Plateau Leveling Osteotomy)

  • This study was performed to assess therapeutic effect of the tibial plateau leveling osteotomy (TPLO) in dogs with experimentally transected cranial cruciate ligaments (CrCL). Nine healthy adult Beagle dogs were transected left CrCL under general anesthesia. The dogs were assigned to TPLO and non-TPLO control groups. The TPLO procedures for correcting the CrCL rupture in the left stifle of dogs were performed under sterile conditions. Before TPLO procedures, all dogs were screened by orthopedic and radiographic examinations. Dogs were lameness free for the previous three months, and when examined at the walk and trot on a hard surface, in a straight line and on a circle. Lateral and craniocaudal radiographs were done to confirm the soundness of the both knee joint in dogs and not detectable lesions were diagnosed. The dogs were intravenously injected with a 10 mci/kg of 99mTechnetium-methylene diphosphonate (99mTc-MDP) under general anesthesia. Scintigraphs were obtained using a large field of view gamma camera equipped a parallel-hole, low-energy about 3 hours after intravenous injection of 99mTc-MDP. Before CrCL transection and 4, 8, and 12 weeks after the procedures, scintigraphy were conducted. Bone uptake of the left stifle joint increased after the procedures in all dogs. When the bone uptake from the TPLO procedure was compared with that of the control, there was a significant difference (p < 0.05). At 12 weeks after the TPLO procedure, the dogs showed normal anatomical posture and gait. It is concluded that TPLO procedure was effective in reconstruct of the stifle joint in dogs with CrCL rupture.

정상 개의 관절액에서 TRAP(Tartrate resistant acid phosphatase)농도 측정 (The Concentration of Tartrate Resistant Acid Phosphatase in Synovial Fluid of Canine Stifle Joint)

  • 본 연구에서는 십자인대 손상견의 무릎관절액의 TRAP의 농도와 비교 할 수 있는 정상 관절액의 TRAP 농도의 지표를 제시 하기 위하여, 정상 무릎 관절액의 TRAP의 농도를 측정하였다. 십자인대 손상이 없는 23두 건강한 잡종견(14두 노령견과 9두 성견)을 실험견으로 선택하고 성별, 나이, 몸무게로 구별하였다. 무릎 관절액을 채취 한뒤, Lang method로 관절액의 TRAP을 측정하였다. 측정된 TRAP의 농도를 성별, 나이, 몸무게 등으로 구별하여 $Mean{\pm}SD$로 표시 하였으며, 유의성 차이를 T-test로 구별하였다. 노령견(14두, 2년이상) TRAP의 농도는 $0.083{\pm}0.039\;IU/ml(mean{\pm}SD$, 성견(9두, 2년 이하) $0.064{\pm}0.023\;IU/ml$, 대형견(15두, >22 kg) $0.075{\pm}0.028\;IU/ml$, 소형견(8두, <22 kg) $0.076{\pm}0.046\;IU/ml$, 중성화견(15두) $0.085{\pm}0.036\;IU/ml$, 중성화 되지 않은견(8두) $0.056{\pm}0.022\;IU/ml\;(Mean{\pm}SD)$로 나타났으며, 중성화 견과 중성화 되지 않은 견에서만 유의성 있는 차이 (P<0.05)가 보였다. 본 연구의 정상견에서 TRAP 농도는 십자인대의 손상견과 비교할수 있는 지표로 활용할 수 있을 것으로 사료된다.

경골조면 전이동술을 이용한 개에서의 부분 전십자인대 단열치료 2례 (Tibial Tuberosity Advancement for Partial Rupture of Cranial Cruciate Ligament in Two Dogs)

  • 두 마리 개가 (증례 1: 27 kg, 증례 2: 42 kg) 뒷다리 파행을 주증으로 내원하였다. 신체검사상, 증례 1은 무릎 검사 시 중증도 통증과 마취 상태에만 경골의 전위 증후을 보였고 증례 2는 과신전시 불편함을 호소하였으나 마취 전후 모두 전위 증후는 관찰되지 않았다. 방사선검사에서 두 증례 모두 관절 내 종창을 확인할 수 있었다. 위의 검사를 바탕으로 전십자인대 단열을 의심하고 관절경을 실시하였다. 관절상에서 증례 1은 불안정한 부분 전십자인대 단열을 증례 2는 안정화된 부분 전십자인대 단열로 진단하였다. 두증례 모두 경골조면 전이동술을 이용하여 치료 하였다. 증례 2에서 술 후 수술 부위에 장액종이 발생하여 수술적으로 제거하였다. 수술 5개월 후 두 증례 모두 임상증상이 개선되었다. 이 증례들을 바탕으로 경골조면 전이동술은 부분 전십자인대 단열에서 유용한 치료법으로 사료된다.

한우의 무릎관절 발생에 관한 조직학적 연구 (Histological studies on the development of the stifle joint in Korean cattle)

  • The morphogenesis of the stifle joint in Korean cattle embryos and fetuses was observed by radiography, alizarin red S stain and light microscopy. Fourty-eight(48) embryos and fetuses, ranging from 11mm to 160mm in crown-rump length (C-R L), were used for this study. The experimental samples were divided into twelve separate groups according to their C-R L. The first to the nineth group ranged from 11-100mm in C-R L, spaced at 10mm intervals. The tenth to the twelfth group ranged in C-R L from 101mm to 160mm, and were spaced at 20mm intervals. The results were as follows; 1. The first appearance of the interzone between the femur and tibia was formed in the second experimental group. The patellar ligament and cruciate ligament appeared in the third group and the patella, menisci and synovial cavity appeared in the fourth group. 2. The lateral and medial menisci were first obseved in the fourth group, and these structures showed crescent shape in the fifth group of the fetuses. 3. The first appearance of the femoro-tibial joint cavity was in the third group. The fifth group contained the first appearance of the patello-femoral cavity. Also, the femoro-tibial cavity was divided into two cavities by the anterior and posterior cruciate ligament.

Single-Plane Fluoroscopic Three-Dimensional Kinematics of Normal Stifle Joint in Beagle Dogs

  • The objective of this study was to establish kinematic reference ranges for the femorotibial (FT) joint and the patellofemoral (PF) joint in healthy small-breed dogs by measuring 3D kinematics at the walk. Single-plane fluoroscopy was used to image the stifle joints of five healthy beagle dogs while the dogs were walking. 3D bone models of the femur, patella, and tibia were reconstructed by computed tomography scanning of the beagle dogs' hind limbs. The shape-matching technique was used to measure kinematic data from the fluoroscopic images and the 3D bone models. The cranial translation of the tibia during walking was inversely proportional to the FT joint flexion. There were significant correlations between the patellar motion and the tibial motion. The FT joint flexion had a strong correlation with the patellar proximodistal translation and flexion. Additionally, the tibial mediolateral translation had a strong correlation with the patellar shift and tilt. In this study, normal in vivo 3D FT joint and PF joint kinematics were demonstrated, and the average kinematic parameters were determined in walking beagle dogs.

소형견에서 외측 무릎 종자뼈의 대칭적 다분화 (Symmetrical Multipartite of Lateral Fabella in a Small Breed Dog)

  • 복부 팽만과 간헐적인 가벼운 좌측후지의 파행을 나타낸 11년령의 암컷 말티즈견이 내원하였다. 신체검사에서 복부 촉신 시 동통을 호소하였고 농성의 질 삼출물을 나타내었다. 혈액화학적 검사결과 독성변화를 동반한 심한 백혈구감소증, 질소혈증, 고인산혈증, 그리고 저나트륨혈증이 관찰되었다. 복부 방사선검사에서 소장을 앞쪽과 등쪽으로 변위시키는 연부조직음영의 관상구조를 가진 매스가 관찰되었고 복부 초음파검사에서 에코성의 물질이 함유된 종괴는 자궁음영으로 판단되어 자궁축농증으로 진단하였다. 후지의 방사선검사결과 양쪽 무릎에서 내측 슬개골탈구와 외측 종자뼈가 4-5 조각으로 분화된 다분화를 관찰하였다. 환자는 곧바로 난소자궁적출술을 받아 잘 회복되었으며 정상적인 혈액화학 수치를 보였다. 좌측 후지의 파행은 간헐적이고 매우 경미하였으므로 특별한 처치를 하지 않았다. 이후 지금까지 3개월동안 뚜렷한 후지파행은 관찰되지 않았다. 여러조각으로 갈라진 후지의 양측성 외측종자뼈는 선천적인 종자골 다분화로 여겨지며 파행과 직접적인 관련성은 없는 것으로 판단된다.

개에서 국소형과 범발성형 조직구육종 2예 (Localized and disseminated histiocytic sarcoma in two dogs)

  • A male mixed breed dog with unknown age (case 1) and 5-year-old female golden retriever (case 2) were admitted to the Department of Veterinary Pathology at College of Veterinary Medicine, Seoul National University after sudden death. At necropsy, stifle region of case 1 was replaced by coalescing firm tan nodules approximately 1-3 cm in diameter. Neoplastic nodules were also presented in the lung. In case 2, the spleen was enlarged 3-5 times its normal size and numerous firm tan nodules were noted in the kidney, pericardium, diaphragm and lymph nodes. Histopathologically, the neoplastic masses of case 1 consisted of plump spindle cells having abundant cytoplasm and indistinct cell borders that formed interlacing bundle. In case 2, the masses were consisted of round to polygonal cells with abundant eosinophilic cytoplasm and eccentric nuclei. Numerous multinucleated giant cells containing nuclei of various sizes were noted. Immunohistochemically, neoplastic cells of all two cases stained intensely with vimentin and lysozyme. Based on the gross and light microscopic findings together with immunohistochemistry, both cases were diagnosed as localized histiocytic sarcoma with pulmonary metastasis and disseminated histiocytic sarcoma, respectively.

Hyperbaric gaseous cryotherapy for postoperative rehabilitation enhances functional recovery of canine stifle joint: a report on a short-term study

  • Background: Hyperbaric gaseous cryotherapy (HGC) is a type of cryotherapy used in human medicine for rehabilitation after orthopedic surgeries. Because HGC is known to reduce acute or chronic pain, research is needed to prove its effectiveness in veterinary medicine. Objectives: To compare the effects of HGC between the HGC treatment group and the nontreatment (NT) group on postoperative swelling, range of motion, lameness score, postoperative pain, and kinetic measurements after stifle joint surgery in dogs. Methods: Dogs were randomized in an HGC group or NT groups. In the HGC group, HGC was applied once a day for a total of 2 days after surgery. All parameters were measured postoperatively and at 1, 2, 10, and 28 days after surgery. Results: Twenty dogs were enrolled: 10 in the HGC group and 10 in the NT group. Soft tissue swelling was not significantly different between groups at any time point. In the HGC group, pain scores decreased significantly 24 h after surgery and 48 h after surgery. Dogs in the HGC group showed a significantly decreased lameness and improvement for all kinetic measurements beginning 48 h after surgery. In addition, the HGC group indicated a significant increase in range of motion as compared with the NT group at 28 days after surgery. Conclusions: HGC plays a powerful role in decreasing initial postoperative pain. Furthermore, the improvement in pain affects the use of the operated limb, and the continued use of the limb eventually assists in the quick recovery of normal function.
