Objectives: Frozen shoulder (FS) is one of the most challenging shoulder disorders for patients and clinicians. Its symptoms mainly include any combination of stiffness, nocturnal pain, and limitation of active and passive glenohumeral joint movement. Conventional treatment options for FS are physical therapy, nonsteroidal anti-inflammatory drugs, injection therapy, and arthroscopic capsular release, but adverse and limited effects continue to present problems. As a result, pharmacoacupuncture (PA) is getting attention as an alternative therapy for patients with FS. PA is a new form of acupuncture treatment in traditional Korean medicine (TKM) that is mainly used for musculoskeletal diseases. It has similarity and specificity compared to corticosteroid injection and hydrodilatation, making it a potential alternative injection therapy for FS. However, no systematic reviews investigating the utilization of PA for FS have been published. Therefore, this review aims to standardize the clinical use of PA for FS and validate its therapeutic effect. Methods: The protocol was registered in Prospero (CRD42023445708) on 18 July 2023. Until Aug. 31, 2023, seven electronic databases will be searched for randomized controlled trials of PA for FS. Authors will be contacted, and manual searches will also be performed. Two reviewers will independently screen and collect data from retrieved articles according to predefined criteria. The primary outcome will be pain intensity, and secondary outcomes will be effective rate, Constant-Murley Score, Shoulder Pain and Disability Index, range of motion, quality of life, and adverse events. Bias and quality of the included trials will be assessed using the Cochrane handbook's risk-of-bias tool for randomized trials. Meta analyses will be conducted using Review Manager V.5.3 software. GRADE will be used to evaluate the level of evidence for each outcome. Results: This systematic review and meta-analysis will be conducted following PRISMA statement. The results will be published in a peer-reviewed journal. Conclusion: This review will provide scientific evidence to support health insurance policy as well as the standardization of PA in clinical practice.

Objectives : Osteoarthritis is characterized by degeneration of articular cartilage, which is characterized by chronic pain, stiffness and decrease range of motion. The present study was designed to compare the therapeutic effect of Dioscoreae Rhizoma water extract (DRW) and Dioscoreae Rhizoma 30% ethanol extract (DRE) on the monosodium iodoacetate (MIA)-induced osteoarthritis rats. Methods : Osteoarthritis was induced by injection of MIA ($50{\mu}{\ell}$ with $80mg/m{\ell}$) into the knee joint cavity of rats. After adaptation period for seven days, rats were divided by 5 groups (n=10/group): normal group, control group, positive control (indomethacin 5 mg/kg), DRW 200 mg/kg treated group, DRE 200 mg/kg treated group (n=10/group). The hind paw weight distribution was measured with the changes of reactive oxygen species (ROS), peroxynitrite ($ONOO^-$) in articulation tissue. Also, the cyclooxygenase-2 (COX-2), inducible nitric oxide synthase (iNOS), tumor necrosis factoralpha ($TNF{\alpha}$), interleukin-6 (IL-6), matrix metalloproteinase-1 (MMP-1), and tissue inhibitor of metalloproteinases-1 (TIMP-1) were investigated by western blot analysis. Results : The administration of DRW and DRE significantly decreased the hind paw weight distribution. The ROS and $ONOO^-$ levels of cartilaginous tissue were significantly decreased in DRW and DRE compared to control group. The results showed that DRE decreased inflammatory cytokines such as iNOS and $TNF{\alpha}$. Also DRE decreased MMP-1 and increased TIMP-1. Conclusions : Based on the above results, Dioscoreae Rhizoma extract seems to have the therapeutic effect on osteoarthritis via suppression of inflammation.
In order to evaluate the bond performance of domestic larch glulam and the glass fiber reinforced plastic (GFRP) rod, the specimen with the GFRP rod bonded-in domestic larch glulam for pull-out test was produced. The test was carried out using various specimens with different gluing depth, width of glue-line and type of adhesive. The cantilever type rahmen structure specimen with bonded-in GFRP rods was produced based on the result of pull-out test, and its moment resistance performance was compared and examined with the moment resistance performance of slotted-in steel plate specimen. As a result of the pull-out test, the most excellent bond performance was found when the insertion depth of GFRP rods was 5 times larger than the diameter of GFRP rods. When the glue-line thickness was 1 mm, the bond performance improved by 17%~29% in comparison to the bond performance in the case of the glue-line thickness of 2 mm. Also, the bonded strength of the specimen used with poly-urethane adhesive was 2.9~4.0 times greater than the bonded strength of specimen used with resorcinol adhesive. The cantilever type rahmen structure specimen with bonded-in GFRP rods showed the moment resistance performance 0.82 times lower in comparison to the slotted-in steel plate specimen used with the drift pin, but the initial stiffness was similar as 0.93 times.

Background: To assess the clinical outcomes of short-term oral corticosteroid therapy for impingement syndrome of the shoulder and determine whether it can be substituted as an alternative to the intra-articular injection. Methods: The clinical outcomes of the 173 patients, the oral steroid group (n=88) and the injection group (n=85), were measured at 3 weeks, 2, 4, and 6 months postoperatively. The clinical outcomes were assessed by measuring the the University of California at Los Angeles (UCLA) score, visual analog scale (VAS) and range of motion (ROM) at every follow-up. Any complications and recurrence rate were noted. A relationship between the treatment outcomes and factors such as demographic factors, clinical symptoms and radiographic findings were determined. Results: No difference was observed in VAS and UCLA scores between the two groups, but forward flexion and internal rotation of ROM were significantly improved in the injection group at the 2nd and 4th postoperative month (p < 0.05). At 6th postoperative month, recurrence rate of symptoms was 26% in the oral steroid group and 22% in the injection group. No major adverse effects were observed. When the clinical outcomes of the oral steroid group were compared to either demographic, clinical symptoms, or radiographic findings, UCLA score was found to be significantly low (p < 0.05) in patients with joint stiffness and UCLA score, whereas VAS score was significantly improved in patients with night pain (p < 0.05). Conclusions: Short-term low-dose oral corticosteroid therapy of impingement syndrome showed comparable clinical outcomes to intra-articular injection without any remarkable adverse effects. Low-dose oral steroids can be regarded as a partial alternative to intra-articular injection for the initial therapy of impingement syndrome of the shoulder.

Cytomegalovirus (CMV) infection is one of the most common congenital infections. The first case of discordant congenital CMV infection in twins occurred in Korea. A 31-year-old woman became pregnant with twins (dichorionic-diamniotic). An elective caesarean section was performed at 37 weeks. The first baby was male, weighing 2,410 g with an Apgar score of 8/9. The second baby was female, weighing 1,380 g with an Apgar score of 5/8. She had experienced intrauterine growth retardation, and presented with microcephaly, micrognathia, and joint stiffness. During the work-up for discordant twins, the second baby's serum test was positive for CMV immunoglobulin M. Her urine, blood, and cerebrospinal fluid (CSF) were CMV polymerase chain reaction positive. The first baby's CMV tests were negative. Ophthalmologic exam and audiometry performed on the second baby showed CMV retinitis and bilateral sensorineural hearing loss. She was treated with intravenous ganciclovir. Currently, she is bed-ridden and has significant developmental delay. Although the causes of discordant congenital CMV infection in twins are unclear, this case shows that discordant congenital CMV infection should be considered in twins with significant differences in intrauterine growth or clinical symptoms after birth.

Purpose: The nonoperative outcome of elbow dislocations with associated radial head and coronoid fractures are often unsatisfactory because of chronic instability and stiffness from proloned immobilization, Therefore we managed these injuries with well programed surgical appproaches. Method: Ten patients with this injury were evaluated retrospectively from May 1998 to June 2004 after a minimum of 12 months. These injuries include elbow dislocation and associated fractures of both the radial head and the coronoid process. All ten patients were treated by one clinic operatively with similar scheduled surgical methods which started on the lateral side and terminated on the medial side of the elbow. Radial head and neck fractures were classified Mason types, as two and three types respectively with six and four cases and six cases were fixated. Coronoid process were fixated with screws anteroposterior directly or anchor suture in all cases, each type was classified one, two and three. where were three type one, four type two, and three type three were according to Regan and Morrey classification. Results: The outcome was three resulting in excellent, four good, two normaland and the remaining case was one poor according to the Mayo Elbow Performance score. At a terminal follow up, the range of motion of the elbow averaged flection contracture, $6^{\circ}(0{\sim}20^{\circ})$ and further flection, $129^{\circ}(115{\sim}140^{\circ})$. Two patients had complications requiring additional care. One, displaced coronoid process which was repaired with capsule and the other patient experienced, palsy of ulnar nerve and contracted elbow joint. Conclusions: Usage of early operation as the minimum injury of medial ligaments complex and the rigid fixation of fractures to prompt motion with our scheduled management for elbow dislocations with associated radial head and coracoid fractures provided excellent results.

This study was performed to evaluate joint behavior of prestressed concrete(PSC)-steel mixed girders through the flexural test of 14 beams according to embedded length, amount of reinforcing steel, stud arrangement, and prestressing force. All test beams were failed by turns of desertion of reinforcing steel, stud, and steel plate. From test results, prestressing force was more effective on performance of connection than stud arrangement and reinforcing steel. And the spacing of stud is also more effective than embedding length. This paper also presented 3D nonlinear analysis considering the slip of composite section as well as the static load tests of PSC-steel mixed girders. According to the slip modulus, the nonlinear analysis showed that the behavior of hybrid girders could be divided into three parts as full-composite, partial-composite and non-composite. However, the experimental results showed that the PSC-steel hybrid girders with shear connectors took the part of partial composite action in ultimate load stage. In addition, it was founded that stud shear connectors and welded reinforcements were contributed to improve the ultimate strength of hybrid girders for about 20%.

Purpose: To evaluate the clinical and radiographic results of the treatment of a proximal humerus fracture with a locking compression plate(LCP). Materials and Methods: This study reviewed the results of 24 cases of a proximal humerus fracture treated with a LCP from January 2005 to April 2007, after a follow up of more than 12 months. There were 8 males and 16 females with a mean age of 68.9(33-90) years. The clinical results were evaluated using the Neer's evaluation criteria, and the radiographic results were evaluated using the bone union time and Paavoleinen method. Results: The mean time for bone union was 11.9 weeks. Using the Neer's functional evaluation, 21 cases of the 24 patients (87%) showed excellent or satisfactory results. Twenty two cases (91%) showed good results according to the Paavoleinen method. The complications encountered were metal failure (1 patient), AVN of the humeral head (1 patient) and joint stiffness (1 patient). Conclusion: The LCP demonstrated good results in the treatment of a proximal humerus fracture and has relatively fewer complications than other internal fixators.

Bridges constructed without any expansion joint or bridge bearing are called integral abutment bridges. They integrate the substructure and the superstructure. Possible deformation of the superstructure, due to changes in temperature for example, is prevented by the bending of the piles placed at the lower part of the abutment. This study examines the behavior of integral abutment bridges through soil-pile interaction modeling method and proposes an appropriate modeling method. Also, it assesses the behavior characteristics of the superstructure and piles of integral abutment bridges through parametric study. Soil condition around the pile, abutment height, and pile length were selected as parameters to be analyzed. Structural analysis was conducted while considering the interactions of soil-pile and temperature change-earth pressure on the abutment. Comparative behavior analysis through soil-pile interaction modeling showed that elastic soil spring method is more appropriate in evaluating the behavior of integral abutment bridges. The parametric study showed the tendency that as the soil stiffness around the pile increases, the moment imposed on the superstructure increases, and the displacement of the piles decreases. In addition, it was observed that as the bridge height increases, the earth pressure on the abutment increases and that in turn affects the behavior of the superstructure and piles. Also, as the length of the pile increased, the integral bridge showed more flexible behavior.

The application of seismic separation joints that can improve the deformation capacity of piping is an effective way to improve seismic performance. Seismic separation joints capable of axial expansion and bending deformation are installed where deformation is expected and used for the purpose of safely protecting the piping. Bellows are flexible and have low stiffness, so they can be used as seismic separation joints because they have excellent ability to respond to relatively large deformation. In this study, the seismic performance and limit state for bending deformation of 2-ply and 3-ply bellows specimens were evaluated. Seismic performance was evaluated by applying an increasing cyclic load to consider low-cycle fatigue due to seismic load. In order to confirm the margin for the limit state of the evaluated seismic performance, an experiment was conducted in which a cyclic loading of constant amplitude was applied. As a result of the experiment, it was confirmed that the bellows specimen was made of stainless steel and had a high elongation, so that the 2-ply bellows specimen had the limit performance of resisting within 3 cycles even at the maximum forced displacement of the 3-ply bellows specimen.
