Two Cases of Sternal "Cold" Lesions on Bone Imaging in the Metastatic Skeletal Disease (전이성골암(轉移性骨癌)에서 골(骨)스캔상 흉골(胸骨)에 냉소(冷所)로 나타난 2예(例))

  • Traditionally, a positive bone scan shows single or multiple areas of increased uptake in the metastatic skeletal disease. The occurence of "cold" lytic-like or photon-deficient lesions in bone imaging is probably uncommon. Photon-deficient focus or cold lesion of the sternum was demonstrated on $^{99m}Tc-MDP$ bone imaging in 2 individuals with acute myloid leukemia and primary hepatoma, respectively.

  • The most common deformity of the sternum is the depression deformity, variously calid pectus Excavatum, funnel chest, schwusterbrust, trichterburst, thorax en entonnoir. During the period 1983 to 1991 a total of 15 cases of funnel chest were treated surgically at Department of Thoracic Surgery, Hanyang University Hospital. The age at the time of operation ranged from 4 to 26 years 73.3 percent of the patients were under 12 years of age They all had symptoms of feeling Inferiority about chest deformity. The concavity on the funnel chest varied in its Extent, and the severity, which was measured by water volume filled into it, varied from 45ml to 100ml. We have Experienced 15 cases of pectus Exc-avatum with several operative method, i.e., Ravitch operation in 2 cases, Taguchi operation in 3 cases[using by Both IMA], Adkins operation in 5 cases, Modified wada operation [sternal costal cartilage Elevation Technique] in 5 cases. After surgery, follow up vertebral Index showed 44%, preoperatively and 29% postoperatively, average decrement of 15%. We Reported 15 cases of funnel chest and the surgical results of thease cases were satisfactory Except one case of Ravitch operation.

  • Synovial sarcoma is an uncommon malignant mesenchymal tumor that occurs in the vicinity of the joints, bursae, and tendon sheaths. Typically the lesions are located in the extremities, especially in the lower extremities. They also occur in the abdominal and thoracic walls, but rarely in the head and neck. We experienced a case of synovial sarcoma of the rib in 17-year -old woman. We performed on bloc chest wall resection including the right second rib tumor, fi st and third ribs. The postoperative course was uneventful. She received chemotherapy because of a relapse of the tumor in sternum and both lung during follow-up.

The Effects of Thoracic Flexibility Exercise on Vital Capacity and Chest Expansion in Patients With Idiopathic Scoliosis (흉부 유연성 운동이 척추측만증 환자의 폐활량과 흉곽 확장에 미치는 영향)

  • The objectives of this study were to investigate the effects of thoracic flexibility exercise on chest function and mobility and to provide the information of physical therapy for patients with idiopathic scoliosis. Forty female subjects who were diagnosed with scoliosis participated in this study and were divided into the experimental and control groups. The experimental group consisted of 20 patients who were treated with thoracic flexibility exercise program during the admission (10 days) ad one month after discharge. The control group consisted of 20 patients who were not treated with thoracic flexibility exercise program. Vital capacity was measured using a respirometer. The chest expansion were calculated using differences of chest circumference between maximum inspiration and maximum expiration measured under armpits, at the junction between the sternum and xyphoid process, and at the waist. All subjects were measured two times: before the admission and at one month after discharge. Data were compared by groups using independent t-test, Vital capacity and chest expansion values (the armpit, chest and waist values) were significantly higher for the experimental group compared to those of the control group (p<.05). The findings of this study show that thoracic flexibility exercise program can lead to an increase in vital capacity and chest expansion and has a positive effects in relieving symptoms and restoring thoracic mobility.

V-shaped Liver Retraction during a Laparoscopic Gastrectomy for Gastric Cancer

  • Purpose: The aim of this study was to evaluate the effectiveness of our retraction method for achieving a good operative field for the adequate lymph node dissection during laparoscopic gastrectomy in view of short term surgical outcome. Materials and Methods: This study prospectively enrolled 19 patients who underwent laparoscopic gastrectomy for early gastric cancer. The procedure was simply performed by putting the laparoscopic sigle suture in the phrenoesophageal ligament, and then the string was pulling and tying over the sternum. Surgical outcomes of these patients were evaluated. Results: Under V-shaped liver retraction, the mean operating time and mean number of retrieved lymph nodes was 166.3 minute and 31.37, respectively. And the results were satisfactory compared to open or conventional laparoscopic gastric surgery. Conclusions: V-shaped liver retraction requires no extra port or assistant's hands, and prevents additional injury to any intra-abdominal organ. And this method can easily, efficiently and safely enable to achieve a good operative field for the lymph node dissection near the lesser curvature of the stomach.

Herbal Toxicological Effects on Rats' Fetus -Focusing on Ojeoksan- (한약이 실험동물의 태자에 미치는 생식독성학적 영향 -오적산을 중심으로-)

  • Purpose : The experiments was undertaken to evaluate the effects of Ojeoksan-herbal medicine, in rats' fetus Methods : Female Sprague-Dawley rats were orally administered with the Ojeoksan at dose of 5mg/kg/day for 20 days. Pregnant rats were sacrificed at 20th day of gestation, and observed internal and reproductive organs. Approximately live fetuses in the 20th day of gestation were randomly selected and fixed in 95% ethanol. Results : Neonatal body weight and number of fetus of Ojeoksan group were increased to that of control group. The fetuses of dams treated with Ojeoksan didn't showed external malformation. Vertebral and sternal skeletal variations were observed in Ojeoksan administered group, but compared to the control, those skeletal variations were insignificant. There were no significant changes in number of ribs, cervical, thoracic, lumber, sacral and caudal Conclusion : From these results, it can be concluded that Ojeoksan showed no toxicity effects on number of live fetuses. There were no significant changes in skeletal variations were showed in vertebrate and sternum, Ojeoksan weren't shown significant changes in bone malformation. We need more precise study to investigate the mechanism of early or late resorption by the herbal medicines such as Ojeoksan.

First report of Harmacloninae (Lepidoptera, Tineidae) in Korea

  • Previously, 33 species, 18 genera, eight subfamilies of the family Tineidae were reported in Korea. In this study detail a new record to Korea, species, Micrerethista denticulata Davis, 1998 in the subfamily Harmacloninae in Korea. This species is the most widespread species of the genus in southern Asia, ranging from Thailand through Indonesia to southern Japan but absent from New Guinea. Globally, Harmacloninae Davis, 1998 includes 22 species of two genera. The subfamily can be distinguished from the other Tineidae subfamilies by an additional wing locking mechanism, loss of pretarsal arolium and pseudempodial seta, tympanic organ in abdominal sternum II, enlargement of sternal apophyses, and aedeagus with basal midventral keel in male genitalia. The morphological characteristics of this subfamily and species are described, and illustrations of examined species and the key characters for each subfamilies of the family Tineidae from Korea are provided.

  • Severe hemodynamic dysfunction may follow after the closure of the median sternotomy in patients with myocardial edema, cardiac dilatation, and severe postoperative bleeding. In extreme cases, myocardial edema may be so severe that cardiac function is compromised merely by the edges of the open sternum touching the heart. Sternotomy remained open and delayed sternal closure is already described adjunct in complicated cardiac operations. We experi nced a case of delayed sternal closure after 16 hours of CABG using synthes DCP wide plate in a 54-year old obese female patient. Our experience conims that delayed sternal closure Is an effective at simple means of dealing with severe myocardial edema so we report this case with literature review.

  • Partial splitting of the upper sternum provides an excellent surgical view in reconstruction of the intrathoracic trachea. However, when deep-seated mediastinitis develops postoperatively, it is difficult to manage especially when combined with sternal osteomyelitis. It also needs an additional consideration compared to the usual treatment modality applied to mediastinitis following a standard median stemotomy because the lower part of the stemum remains intact. We treated a 50 year old female patient with deep-seated mediastinitis and sternal osteomyelitis following resection and end-to-end anastomosis of the trachea through an upper midline sternotomy. The patient underwent extensive stemectomy, omental free grafting, and pectoral myocutaneous flap. Postoperative viability of the free-grafted omentum was evaluated by angiography and CT scan.

Large Focal Extrapleural Hematoma of Chest Wall: A Case Report

  • Although hemothorax and pneumothorax are common complications seen in rib fractures, focal extrapleural hematoma is quite rare. We report a 63-year-old female patient that developed large focal extrapleural hematoma after falling off a second floor veranda. The patient had sustained 3, 4, 5th costal cartilage rib fractures and a sternum fracture. She had developed suspected empyema with loculations with small amount of hemothorax. She underwent a planned early decortication/adhesiolysis by video assisted thoracoscopic surgery at the 12th post-trauma day due to failed drainage. Unexpectedly, she had no adhesions or any significant retained hematoma mimicking a mass, but was found with the focal extrapleural chest wall hematoma. She was discharged on postoperative 46th day for other reasons and is doing fine today.
