Effects of Freezing of Gait and Visual Information on the Static Postural Control Ability in Patients with Parkinson's Disease

  • Objective: The purpose of this study was to analyze the effects of freezing of gait and visual information on the static postural control ability in patients with Parkinson's Disease (PD) during the bipedal stance with feet together. Method: This study included a total of 36 patients with PD; the freezer group included 17 PD patients (age: $69.3{\pm}6.2yrs$, height: $159.6{\pm}9.0cm$, weight: $63.4{\pm}9.78kg$) and the nonfreezer group included 19 PD patients (age: $71.4{\pm}5.6yrs$, height: $155.8{\pm}7.1cm$, weight: $57.7{\pm}8.6kg$). Static postural control ability was analyzed using variables of center of pressure (COP) and dividing by mediolateral, anteroposterior, and integration factors during a bipedal stance with the eyes open and closed. Results: Freezers and nonfreezers showed increases in anteroposterior velocity, mediolateral velocity, averaged velocity, and mediolateral 95% edge frequency when visual information was blocked. Additionally, freezers had greater anteroposterior range, 95% confidence ellipse area, and COP anteroposterior mean position than nonfreezers. Conclusion: Freezers and nonfreezers showed a reduction in static postural control ability when visual information was blocked. Additionally, the results of this study found a significant difference in static postural control ability between freezers and nonfreezers with PD. In particular, anteroposterior range, 95% confidence ellipse area, and COP anteroposterior mean position might be used to distinguish between freezers and nonfreezers with PD.

시·청각리듬자극이 만성 뇌졸중 환자의 정적균형과 동적균형에 미치는 영향 (The Effects of Visual·Auditory Rhythmic Stimulation(VARS) in Static Balance and Dynamic Balance with Chronic Stroke Patients)

  • Purpose : The purpose of this study was to investigate the effect of visual Auditory rhythmic stimulation(VARS) in gait ability and proprioception with chronic stroke patients. Twenty-one persons after six months post stroke participated in pre test-post test control. Method : The subjects were randomly assigned to a visual Auditory rhythmic stimulation(VARS) group (n=10) and control group (n=11). Training process was practiced with exercise on thirty minutes a day, three days a week for four weeks. To find out the effect, inspected the FRT(functional reach test) by static balance and TUG(timed up and go test) by dynamic balance. Results : In static balance, FRT distance was significantly different between two group. In dynamic balance, TUG time was significantly different between two group. This study showed that the VARS training increase a balance by postural adjustment of chronic stroke patients more than control group. And so, the VARS training of hemiplegic patients was very important to successive rehabilitation. Conclusion : A continuous examination of VARS training could practical used of physical therapy with exercise.

동적시력과 대비감도의 상관관계 (A Relationship between Kinetic Visual Acuity and Contrast Sensitivity)

  • 목적: 동적시력(Kinetic visual acuity, KVA), 대비감도(Contrast sensitivity), 정지시력(정지시각 Static visual angle, SVA) 등의 상관관계를 분석해 보고자 하였다. 방법: 안경광학과 재학생 99명(남 51명, 여 48명)을 대상으로 Topcon CV-3000 포롭터를 이용하여 완전교정 후 명소시($100cd/m^2$)상태에서 Vector Vision CSV-1000E를 이용하여 2.5 m 거리에서 대비감도를 측정하였다. 또한 KVA 측정장치(KOWA AS-4A)을 이용하여 동적시력을 측정하였다. KVA 정도에 따라, 0.1~0.3 L그룹, 0.31~0.60 M그룹, 0.61 이상을 H그룹으로 분류하고 대비감도, 정지시각, 굴절이상량 등의 상관관계를 분석하였다. 결과: KVA와 대비감도의 상관성은 3 cpd r=0.26, 6 cpd r = 0.48, 12 cpd r=0.38, 18 cpd r=0.47로 저주파수인 3 cpd를 제외하고, 모두 KVA와 SVA의 상관성 r = -0.37 보다 높았다. 주파수별 대비감도는 3, 6, 12, 18 cpd에서 각각 L그룹은 59.41, 92.22, 38.41, 14.39, M그룹은 66.03, 108.78, 53.51, 19.20, H그룹은 70.90, 146.10, 62.90, 25.33이였다. 결론: KVA와 대비감도의 상관성이 SVA보다 높으며, KVA가 높은 경우 저주파수 이외에서는 대체로 대비감도가 높은 것을 알 수 있었다.

컴퓨터 애니메이션을 이용한 가로경관의 평가기법 연구 -정적 및 동적 시뮬레이션 기법의 비교- (A Study of Streetscape Evaluation Methods Using Computer Animation -A Comparison of Static and Dynamic Simulation Methods-)

  • Previous research for visual assessment of streetscape employed static simulation methods to represent future landscape. However, streetscape is experienced sequentially, and thus dynamic simulations can be more effective. This study tried to adopt computer animation in the evaluation of streetscape, and examined its effects and possibilities. Three development scenarios for the redevelopment districts of Sokong-Ro and Banpo-Ro in Seoul were designed, and simulations were produced by three methods-photo-retouching, computer still image, and animation. A preference questionnaire was asked to 69 university students, and the effects of simulation methods on visual preference were examined. In addition, the frames of the animation were reclassed to identify the visibility of physical elements. The relationships between the visibility and visual preference were analyzed. The results showed that visual preference can be explained by three factors-Amenity, Tidiness, and Variousness-that account for 62.4% of the total variance, and the Amenity showed the highest proportion: 36.0%. Among the three simulation methods, animation showed the largest difference in preference for the most important factor(Amenity), and yielded the highest correlation between visibility of physical elements and Amenity. This result demonstrated that dynamic simulations can provide more accurate observation of visual changes, especially because the simulated landscape is experienced sequentially. The results also revealed that the sequential change in the visibility of physical elements can be examined easily and precisely by animation. This benefit of animation enables analysts to identify the points where the landscape varies the most, and thus visual preference should be evaluated.

Comparison of Effects on Static Balance in Stroke Patients According to Visual Biofeedback Methods

  • Objective: The purpose of this study is to investigate the impact of visual biofeedback methods utilizing pressure sensors on the static balance of stroke patients. Design: Randomized crossover study. Methods: A total of 27 patients with hemiparesis participated in this study. The following three feedback conditions were considered: condition 1 (Knowledge of performance feedback), condition 2 (Knowledge of result feedback), and condition 3 (None feedback). A force plate was used to measure static balance. The total sway length, average sway velocity, x-axis excursion, and y-axis excursion of the center of pressure were measured. One-way repeated-measures analysis of variance was employed for comparisons of variables between each condition. The statistical significance level was set at α = 0.05 for all analyses. Results: There was a significant difference in the static balance results between each feedback condition (p<0.05). In the post-hoc results, it was confirmed that the static balance was significant in the order of knowledge of performance feedback, knowledge of result feedback, and none feedback. Conclusions: When comparing the three conditions, it was observed that knowledge of performance feedback showed the most improved effect on static balance ability. As further research progresses, that this approach could be used as an effective intervention method in clinical settings.

  • The purpose of this study was to assess the effect of visual block (eyes open or closed), mental task type, and participation in an exercise program on static balance in the elderly. The subjects were 34 elderly (>65 years old) residents of a social welfare institute in Gyeonggi-do. We measured the following variables. Berg balance scale, mini mental state examination, balance performance monitor (sway area, path, and maximal sway velocity), age, weight, height and whether the subject participated in an exercise program. Scores for the Berg balance scale and mini mental state examination were evaluated to assess static balance ability either alone (single task paradigm) or while performing a mental task (dual task paradigm). Static balance variables that were measured included sway area, path, and maximal sway velocity. Each test was repeated three times. Multiple regressions analysis was used to examine the effect of each variable on static balance ability. For the dual task paradigm, static balance was affected by whether the subject participated in an exercise program. The Berg balance scale score for subjects with their eyes open was affected by whether they participated in an exercise program, while this variable in addition to the height and weight of subjects were determining variables in subjects with their eyes closed. As a result, whether subjects participated in an exercise program affected their static balance irrespective of whether their eyes open or closed. Therefore, regular exercise is recommended for elderly people and further research is needed to examine the relationship between static and dynamic balance while performing mental tasks such as cognition and attention.

퇴행성관절염 환자들에게 균형훈련이 균형지수에 미치는 영향 (The Change of balance index to balance training in Osteoarthritis)

  • Purpose : In order to investigate the effects of balance training on patients with degenerative arthritis. Methods : 30 participants aged 60 or older participated in balance training for an 8-week period. The effects of the balance training were measured by the visual analog scale(VAS) and static and dynamic balancing. The following are the results of the study. Results : There were no statistically significant differences in measurements of pain when control group participants were at rest and while walking as measured by VAS, but there were statistically significant reductions for the experiment group. Within the control group, there were no statistically significant differences between pretest and posttest results for opened and closed-eye static balance index and visual dynamic balance index. However, within the experiment group, there were statistically significant differences between pretest and posttest results for opened and closed-eye static and dynamic balance indices. Conclusion : The results above provide evidence that balance training effects pain and balance of patients with osteoarthritis and aids in functional movement.

시각적 피드백 스쿼트가 슬개대퇴 통증 증후군이 있는 성인의 대퇴사두근 각에 미치는 영향 (The Effect of Visual Feedback Squat on Q-angle with Patellofemoral Pain Syndrome)

  • Background: The purpose of this study is to examine the effects of visual feedback squat exercise on the young adults with Patellofemoral pain syndrome. Methods: The subjects of this study were 22 female and 8 male who were equally and randomly allocated to a visual feedback exercise group (VSEG), an experimental group, and squat exercise group (SEG), a control group. Both did so for 30 minutes three times per week over a six-week period. Using Dartfish, their static Q-angle (SQA) and dynamic Q-angle (DQA) were evaluated. Results: The static Q-angle was significantly reduced in both groups of VSEG and SEG and in the comparison of difference values before and after exercise between groups, VSEG had more significant effect than SEG but in the comparison of the effects of dynamic Q-angle both VSEG and SEG had significant effects and in the comparison of difference values before and after exercise between groups, VSEG had more significant effect than SEG. Conclusion: Visual feedback squat exercise may be applied as a method to correct the lower extremity alignment with PFPS.

  • Purpose: The purpose of this study was to investigate the effects of a visual feedbackbased balance training, using force platform biofeedback, on the postural balance of elderly faller. Methods: Fifty one community-dwelling older adults (aged 66-88 years) with a recent history of fall participated in the study. Participants were randomized to an experimental group (EG, n=25) and to a control group (CG, n=26). The EG participated in training sessions three times/week for 6 weeks. Visual feedbackbased balance training with the a computerized force platform with visual feedback screen was used in the experimental group. Static balance (center of gravity) and dynamic balance (Functional reach test, Timed "Up & Go" test, Berg balance scale) were assessed before and after end of training. Results: A significant improvement in static balance and dynamic balance were demonstrated within the EG (p<.05), but not in the CG. Conclusion: Visual feedback-based balance training may be an effective intervention to improve postural balance of elderly fallers.

The Effects of Visual Biofeedback Balance Training on Functional Ability in Children with Cerebral Palsy : A Pilot Study

  • PURPOSE: The purpose of this study is to examine the impact of balance training on a three-dimensional balance trainer that provides the up-and-down vertical movement of the knee joint and left-and-right horizontal movement, along with visual feedback on the functional ability of children with spastic cerebral palsy (CPs). METHODS: 8 CPs participated in this study. The experiment was implemented for 40 minutes, three times a week for a total of six weeks. The subjects received general physiotherapy for 15 minutes in each session focused on balance and walking, as based on the neuro-developmental treatment theory. Balance training was performed for 20 minutes on a three-dimensional balance trainer where knee joint movement providing visual feedback is applied. The evaluations were conducted before and after the test, and posture sway was measured using 10 Meter Walking Test (10MWT), Timed Up & Go Test (TUG), and the Good Balance System to evaluate the functional ability and balance of the subjects RESULTS: 10MWT was not statistically significant (p>.05). On the contrary, TUG and postural sway indicate static balance showed a statistically significant decrease (p<.05). In a static balance test using the Good Balance System, the average moving speed statistically significantly decreased in the AP and ML directions (p<.05), and the mean velocity moment also significantly decreased (p<.05). CONCLUSION: These findings suggested that balance training using the three-dimensional balance trainer, with the features of visual feedback and up-and-down knee joint movement effects on increasing dynamic and static balance.
