PURPOSE: Aging causes changes in the postural alignment and gait due to changes in the nervous and musculoskeletal systems. On the other hand, the relationship between the changes in posture alignment and gait is unclear. This study examined the relationship between the postural alignment and spatiotemporal gait parameters in Korean elderly women. METHODS: Thirty-two-healthy elderly women participated in this study. All subjects were assessed for their posture alignment and gait ability. Stepwise multiple linear regression was performed to determine to what extent the postural alignments could explain the spatiotemporal gait parameters. RESULTS: Coronal head angle was moderately correlated with the velocity (r = -.51), normalized velocity (r = -.46) and gait-stability ratio (r = .58) (p < .05). The trunk angle was moderately correlated with the normalized velocity (r = -.32) and gait-stability ratio (r = .32) and weakly correlated with the velocity (r = -.28) (p < .05). The coronal shoulder angle was moderately correlated with the swing phase (r = -.57), stance phase (r = .56), single limb stance (r = -.56) and double limb stance (r = .51) (p < .05). The coronal head angle and trunk angle accounted for 36% of the variance in velocity, 33% variance in normalized velocity and 46% variance in the gait-stability ratio (p < .05). The coronal shoulder angle accounted for 32% variance in the swing phase, 32% variance in the stance phase, 31% variance in the single limb stance and 26% variance in the double limb stance (p < .05). CONCLUSION: Changes in posture alignment in elderly women may serve as a biomarker to predict a decrease in walking ability due to physical aging.
Purpose: This study aimed to investigate the effect of single-leg stance training according to different support surfaces on walking speed and balance in patients with chronic hemiplegia. Methods: Twenty-two patients with chronic stroke were randomly categorized into an experimental group (11 patients) and a control group (11 patients). In the experimental group, single-leg stance training was performed on an unstable surface after 50 min of general physical therapy. In the control group, single-leg stance training was performed on a stable surface after 50 min of general physical therapy. All participants performed five sets of single-leg stance exercises per minute and rested for 3 min. The intervention was performed 5 times a week for 4 weeks, and each patient was evaluated using the Berg Balance Scale (BBS), Fugl-Meyer Assessment Scale (FMA), and difference in walking speed between the first and last day of the intervention. Results: Compared to baseline measurements, both study groups showed significant increases in FMA, BBS, and walking speed (p<.05) after the intervention. However, there was no statistically significant difference (p>.05) between the experimental and control groups. However, in the experimental group, the increases in FMA, BBS, and walking speed were 3.36 %, 9.50 %, and 7.71 %, respectively. In the control group, the increases in FMA, BBS, and walking speed were 2.39 %, 6.65 %, and 7.64 %. Conclusion: Single-leg stance training on different support surfaces could help improve walking ability and balance in patients with chronic hemiplegia.
The purpose of the current experiment was to describe interlimb coordination when swing limb conditions are being manipulated by constraining step length or by adding a 5 or 10 pound weight to the swing limb distally. Subjects were asked to begin walking with the right limb to land on the primary target (normal step length) that is 10 cm in diameter. However, if, during movement, the light was illuminated, then the subject had to step on one of the secondary targets (long and short step length). These three step length conditions were repeated while wearing a 5 pound ankle weight and then when wearing a 10 pound ankle weight. Ground reaction force (GRF) data indicated that there were changes in the forces and slopes of the swing and stance Fx GRFs. Long stepping subjects had to increase the propulsive force required to increase step length. Consequently, swing and stance toe-off greatly increased in the long step length condition. Short step length subjects had to adequately adjust step length, which decreased the speed of gait initiation. Loading the swing limb decreased the force and slope of the swing limb. Swing and stance toe-off was longest for the long step length condition, but there was a small difference of temporal events between no weight and weight condition. It appears that subjects modulated GRFs and temporal events differently to achieve the peak acceleration force of the swing and stance limb in response to different tasks. The findings from the current study provide preliminary data, which can be used to further investigate how we modulate forces during voluntary movement from a quiet stance. This information may be important if we are to use this or a similar task to evaluate gait patterns of the elderly and patient populations.
Purpose : The purpose of this study was to investigate the effect of tensor fasciae latae length on the rotation of pelvis during one leg stance. Methods : 41 healthy adults participated in this study. The movement of the pelvis and trunk was measured using 3-dimensional motion analyzer, during one leg stance. The movement of the pelvis and trunk was collected lateral shift, rotation, side bending, and flexion-extension. Tensor fasciae latae length of subjects was measured in sidelying positon with neutral position of hip joint and flexion $90^{\circ}$ of knee. Also, the range of motion of hip exteral and interal rotaion were measured in prone position wih lexion $90^{\circ}$ of knee. The subjects were separated 2 groups that more pelvic rotation group(n=15) and less pelvic rotation group(n=15) according to the degree of pelvic rotation. Results : The more pelvic rotation group was showed significantly higher in the ROM of hip external rotation than less pelvic rotation group(p<0.05). The difference of tensor fasciae latae length not showed significant difference between groups. During one leg stance, The movement of the shifting and flexion-extension of trunk and pelvis were not showed significant difference. But the side bending and the rotation of pelvis and trunk showed significant difference between groups. Conclusion : The difference of tensor fasciae latae length not showed significantly in more pelvic rotation group and less pelvic rotation group. But, this study suggests that the pelvis instability brings the instability of the trunk during one leg stance.
The purpose of this study was to investigate the effect of aqua exercise program on the static balance of child with mental retardation. The subjects were seven male child and three female child with mental retardation(age ranges from 5 to 6). The subjects were practiced with focusing on the aqua exercise program for 8 weeks, from 7, January 2003 to 28, February 2003. The results of this study were as follows : 1. After the exercise program, the balance of double-stance by BPM was significantly reduced of lateral sway number by $12.2\%$(P<.01) with eyes obstruct. 2. After the exercise program, the balance of double-stance by BPM was significantly reduced of anterior and posterior sway number of left leg by $8.5\%$(P<.05) with eyes obstruct. 3. After the exercise program, the balance of double-stance by BPM was significantly reduced of anterior and posterior sway number of right leg by $8.3\%$(P<.05) with eyes obstruct. 4. After the exercise program, the balance of double-stance by BPM was significantly reduced of lateral sway number by $13.2\%$(P<.01) with eyes open. 5. After the exercise program, the balance of double-stance by BPM was significantly reduced of anterior and posterior sway number of left leg by $10.3\%$(P<.05) with eyes open. 6. After the exercise program, the balance of double-stance by BPM was significantly reduced of anterior and posterior sway number of right leg by $10.3\%$(P<.01) with eyes open. As we can see the results of this study, the aqua exercise program improved the static balance of child with mental retardation. Futhermore, it is reasonable program that can improvability of static balance for the children with mental retardation using aqua exercise program. And then it is necessary to develop much better program than now for the children with mental retardation's health and to provide the adequate pool for their aqua exercise.
Recently among several tennis techniques forehand stroke has been greatly changed in the aspect of spin, grip and stance. The most fundamental factor among the three factors is the stance which consists of open, square and closed stance. The purpose of this study was to investigate the relations between the segments of the body, the three dimensional anatomical angle according to open stance patterns during forehand stroke in tennis. For the movement analysis three dimensional cinematographical method(APAS) was used and for the calculation of the kinematic variables a self developed program was used with the LabVlEW 6.1 graphical programming(Johnson, 1999) program. By using Eular's equations the three dimensional anatomical Cardan angles of the joint and racket head angle were defined 1. In three dimensional maximum linear velocity of racket head the X axis showed $11.41{\pm}5.27m/s$ at impact, not the Y axis(horizontal direction) and the z axis(vertical direction) maximum linear velocity of racket head did not show at impact but after impact this will resulted influence upon hitting ball It could be suggest that Y axis velocity of racket head influence on ball direction and z axis velocity influence on ball spin after impact. the stance distance between right foot and left foot was mean $74.2{\pm}11.2m$. 2. The three dimensional anatomical angular displacement of shoulder joint showed most important role in forehand stroke. and is followed by wrist joints, in addition the movement of elbow joints showed least to the stroke. The three dimensional anatomical angular displacement of racket increased flexion/abduction angle until the impact. after impact, The angular displacement of racket changed motion direction as extension/adduction. 3. The three dimensional anatomical angular displacement of trunk in flexion-extension showed extension all around the forehand stroke. The angular displacement of trunk in adduction-abduction showed abduction at the backswing top and adduction around impact. while there is no significant internal-external rotation 4. The three dimensional anatomical angular displacement of hip joint and knee joint increased extension angle after minimum of knee joint angle in the forehand stroke, The three dimensional anatomical angular displacement of ankle joint showed plantar flexion, internal rotation and eversion in forehand stroke. it could be suggest that the plantar pressure of open stance during forehand stroke would be distributed more largely to the fore foot. and lateral side.
The purposes of this study were to evaluate and compare the limits of stability(LOS) at different body positioning(standing and one leg standing) in normal 20 years of age. Fourty subjects participated in the study. Subjects comprised 20 males and 20 females who without neurologic, orthopaedic impairments and balance performance impaired. The LOS was measured at Two Feet Forceplate and One Leg Forceplate with BPM(Balance Performance Monitor) Dataprint Software Version 5.3. The subjects stood 4 inches between the feet at Two Feet Forceplate and stood one legged at One Leg Forceplate. In this study applied the paired t-test and independent t-test to determine the statistical significance of results at 0.01 and 0.05 level of significance. The results of this study were as follows: 1) The anteroposterior LOS significantly increased with one legged stance(p<0.05). 2) The mediolateral LOS significantly decreased with one legged stance(p<0.01). 3) There were significant difference posterior LOS in standing and anterior LOS in one legged stance according to sexual difference(p<0.05). 4) The mediolateral LOS was not significant difference between standing and one legged stance according to sexual difference(p>0.05).
The purpose of study was to analyze how the visual information affects balance control of individuals during single leg stance. A total of 27 young normal people (20 males and 7 females, age: $13.7{\pm}2.6$, height: $162.3{\pm}13.2$ cm, weight: $53.9{\pm}13.9$ kg) was voluntarily involved in the experiment. The subjects were requested to maintain balance for 20 seconds with eyes both open and closed on a force plate and then foot ground reaction data were collected for that duration. Results showed that mean velocity of COP in closed eyes condition was larger 1.84 times than that of the open-eyes condition and range of vertical angle was increased approximately one degree in the closed eyes condition. To accomplish a balance, the frequency power in mediolateral and anteroposterior components of the foot-ground reaction force was increased by 1.3~1.4 times. Consequently, visual absence during single leg stance can result in critical loss of balance and lead to instability of body control.
Purpose: This study was to identify the anticipatory postural adjustment (APA) mechanism which is represented by the onset time of trunk muscles and the displacement of the center of pressure (COP) according to the different base of support (BOS) during upper extremity movement. Methods: Thirty healthy subjects (14 males, 16 females) participated in this study. The movement was performed for 10 trials during each of various BOS (shoulder - width double leg stance, narrow base double leg stance, tandem stance, non-dominant single leg stance) at the 1.2 Hz frequency. Electromyography was used to measure muscle onset time and biorescue was used to measure characteristics of the displacement of COP. Surface bipolar electrodes were applied over the right deltoid anterior, right latissimus dorsi, both rectus abdominis, both internal oblique and both erector spinae. The data were analyzed by repeated one-way ANOVA and Duncan's post hoc test. Results: The study has revealed following. There were significant differences with muscle onset time in each BOS (p<0.01). There were significant differences in characteristics of the COP in each BOS (p<0.01). Conclusion: The study found that the more narrowed the basis requires the more rapid anticipatory postural control in contralateral postural muscle when the upper extremity movement is performed.
Purpose: We investigated the effects of single-leg stance training on standing balance and mobility in patients with subacute hemiplegia. Methods: Seventeen matched subjects were assigned randomly to the experimental group or the control group. The experimental group comprising of 8 subjects received single-leg stance training and conventional physical therapy interventions 5 times per week for 4 weeks. The control group comprising of 9 subjects received only conventional physical therapy interventions 5 times per week for 4 weeks. Outcome measures were assessed before and after 4 weeks of intervention using the Berg Balance Scale (BBS), gait speed, and weight bearing index of the affected side. Results: Both the exercise groups showed significant improvements in BBS, gait speed, and weight bearing index (p<0.05). After 4 weeks of intervention, there were statistically significant differences in BBS and weight bearing index between the two groups (p<0.05). Conclusion: These findings suggest that conventional physical therapy interventions along with single-leg stance training could be more effective than conventional physical therapy alone for improving standing balance and mobility in patients with subacute hemiplegia.
