노인운동의 변화단계에 따른 의사결정 균형에 대한 연구 (Decisional balance corresponding to the Stage of Change of Exercise in Elderly)

  • Purpose : This study was carried out to identify the factor of decisional balance for exercise. This was composed of a set of variables at the level of decision making when the elderly decide to do exercise. It was used to classify and identify the characteristics of the stages of change for exercise to which the elderly belong, and was used to identify the variables of decisional balance which influence the stage of change for exercise in the elderly. Methods : Convenient samples of 198 subjects over age 60 in Seoul( mean age=70) were selected from community living, mentally competant older adults and the data was collected from April 1, 1999 to May 31, 1999. The research instrument was the Decisional Balance Measure for Exercise(Marcus & Owen., 1992), Stage of Change Measure(Marcus et al,1992). The data was analyzed by SAS Program. Results: 1. According to stage of change measure, without missing data,191 subjects were distributed in each stage of change for exercise : 50 subjects(26.1%), 7 subjects (3.6%), 52 subjects(27.2%), 4 subjects(2%), and 78(40.8%) belonged to the precontemplation stage, contemplation stage, preparation stage, action stage and maintenace stage. 2. Factor analysis identified 3 factors of decisional balance as appropriate factors for exercise of the elderly and named by researchers; 1)'Perceived Physical-psychological benefit', 2)'Perceived Physical-psychological burden', and 3)'Perceived time burden'. 3. The analysis of variance showed that the two components Perceived Physical - psychological benefit(F=45.95, P=.0001), and Perceived Physical-psychological burden (F=26.52, P=.0001) were significantly associated with stage of change. 4. Through the discriminant analysis, it was found that both 'Self Perceived Physical - Psychological benefit' and 'Perceived Physical-Psychological burden' were the influential variables in discriminating the three stages of change(pre-contemplation, preparation, and maintenance). Conclusion : Results are consistent with the application of the Transtheoretical model, which has been used to understand how people change health behaviors. Even though this study is a cross-sectional, not a longitudinal study, the findings of this study give useful information for exercise intervention about especially the factors relating to decision making for exercise of the elderly in the different stages of change of exercise.

범이론적 모형에 기반한 서비스 전환 행동 단계 연구: 숙박공유경제 서비스를 중심으로 (Stage of Service Switching Behavior based on the Transtheoretical Model: Focused on Accommodation Sharing Economy Service)

  • 정보기술이 발전함에 따라 소비자들은 혁신적인 새로운 서비스로의 전환을 보다 빈번하게 고려하게 되었다. 이에 따라 기업에게도 고객의 서비스 전환 행동은 주요 과제 중 하나가 되었다. 이러한 요구에 부응하기 위해 학계에서도 고객의 서비스 전환 행동과 관련한 다양한 연구들이 진행되어 왔다. 그럼에도 불구하고, 쾌락적 서비스(hedonic service)에 대한 고객의 전환 행동을 단계적으로 파악한 연구는 거의 이루어지지 않고 있다. 본 연구는 범이론적 모형(TTM: transtheoretical model)을 기반으로 서비스 전환 행동의 단계를 파악하고, 서비스 제공자 전환 모형(SPSM: service provider switching model)을 기반으로 전환 행동 단계별로 서비스 전환에 영향을 미치는 요인이 어떻게 변화하는지를 파악하고자 한다. 이를 위해 숙박공유경제 서비스에 대한 고객의 전환 행동과 관련한 가설을 제시하고 이를 실증적으로 분석하였다. 분석 결과 서비스 전환 행동 단계에 따라 서비스 전환 행동에 영향을 미치는 요인이 달라짐을 파악하였다. 본 연구는 서비스 전환 행동과 영향 요인이 단계별로 차이가 있음을 밝힘으로써 고객 유지 및 이탈 방지를 위한 가이드라인을 제시할 수 있을 것이다. 또한 고객 유지 역량을 증대시킴으로써 기업성과를 향상시키고자 하는 경영자들에게 실질적인 도움을 줄 수 있을 것이다.

Comparative study on damage identification from Iso-Eigen-Value-Change contours and smeared damage model

  • The paper proposes two methodologies for damage identification from measured natural frequencies of a contiguously damaged reinforced concrete beam, idealised with distributed damage model. The first method identifies damage from Iso-Eigen-Value-Change contours, plotted between pairs of different frequencies. The performance of the method is checked for a wide variation of damage positions and extents. The method is also extended to a discrete structure in the form of a five-storied shear building and the simplicity of the method is demonstrated. The second method is through smeared damage model, where the damage is assumed constant for different segments of the beam and the lengths and centres of these segments are the known inputs. First-order perturbation method is used to derive the relevant expressions. Both these methods are based on distributed damage models and have been checked with experimental program on simply supported reinforced concrete beams, subjected to different stages of symmetric and un-symmetric damages. The results of the experiments are encouraging and show that both the methods can be adopted together in a damage identification scenario.

중년 흡연남성의 금연 변화단계에 따른 의사결정 균형에 대한 연구 (Decisional balance corresponding to the Stage of Change of Smoking Cessation in Middle Aged Men)

  • Purpose : This study was done to identify the factors of decisional balance for smoking cessation among middle aged men in Korea. This was composed of a set of variables at the level of decision making when middle aged men decide to stop smoking. It was used to classify and identify the characteristics of the stages of change for smoking cessation to which the middle aged men belong, and was used to identify the variables of decisional balance which influenced the stages of change for smoking cessation in middle aged men. Methods : A convenience sample of 169 men who had smoked and were smoking, were between age 30 and 60 and lived in Seoul(mean age=44), were selected from the community. The data was collected from December 10, 1999 to February 28, 2000. The research instrument was the Decisional Balance Measure for Smoking Cessation (Velicer et al., 1985). and Stage of Change Measure(DiClemente et al., 1991). The data were analyzed using the SAS Program. Results : 1. According to stage of change measure, the 169 subjects were distributed in each stage of change for smoking cessation: in the pre-contemplation stage 63 subjects(32.7%), contemplation stage 60 subjects (35.5%), preparation stage 17 subjects(10.1%), and maintenance stage 29 subjects(17.2%). 2. Factor analysis identified 4 factors of decisional balance as appropriate factors for smoking cessation of middle aged men. There were named by the researchers; 1)'Perceived Burden to Self', 2)'Perceived Benefit to Others' 3)'Perceived Approval to Others', and 4)'Perceived Disapproval to Self'. 3. Analysis of variance showed that the three components, 1)'Perceived Burden to Self(F=8.50, P=.0001)', 2)'Perceived Benefit to Others(F=3.19, P=.025)' and 3)'Perceived DisApproval to Self(F=2.87, P=.038)were significantly associated with stage of change. 4. Through discriminant analysis, it was found that 'Perceived Burden to self' was the most influential variable in discriminating the four stages of change(pre-contemplation, contemplation, preparation, and maintenance). Conclusion : The results are consistent with the application of the Transtheoretical model, which has been used to understand how people change health behavior. Even though this study is a cross-sectional, not a longitudinal study, the findings of this study give useful information for smoking cessation intervention for the middle aged men.

범이론적 모형을 적용한 대면적 금연상담 프로그램의 효과 (Effectiveness of Public Health Center Smoking Cessation Counseling Program using the Transtheoretical Model)

  • Purpose: The purpose of this study was to identify the effectiveness of public health center smoking cessation counseling program using the transtheoretical model on the process of change, smoking temptation, decisional balance and stage change transition. Methods: A nonequivalent control group non-synchronized design was used for this study. The study population was 115 adult smokers. The counseling program was administered to the experimental group and a smoking cessation program from the Korean Ministry of Health & Welfare was administered to the control group. Descriptive analysis, $\chi^2-test$, t-test, and ANCOVA were used with the SPSS 12.0 program to analyze the data. Results: After treatment with this program, the experimental group showed significantly higher scores for process of change (t=4.148, p<.001), smoking temptation (t=-2.988, p=.003), and stage change transition ($\chi^2=5.871$, p=.031) compared to the control group. Experimental group members also showed significantly lower score for Pros of smoking (t=-3.151, p=.002). Conclusion: The findings indicate that this program could have positive effect on process of change, smoking temptation, decisional balance and stage transition for adult smokers. Based on these findings, the authors suggest additional counseling program focusing on smokers in specific stages.

범이론적모델을 이용한 교대근무 간호사의 운동행위 변화단계에 미치는 융합적인 영향요인 (Converged Influencing Factors on the Stages of Exercise Behavior Change of Nurses in Shift Work Using Transtheoretical Model)

  • 본 연구의 목적은 교대근무 간호사의 운동행위변화단계에 미치는 융합적 영향요인을 분석하는데 있다. 연구대상자는 병원에 근무하는 교대근무 간호사로 연구의 목적을 이해하고 연구에 참여할 것을 동의한 120명의 간호사이다. 연구 자료는 빈도, 백분율, 평균, 표준편차, t-test, $X^2$ test, 다중 로지스틱 회귀분석을 이용하여 분석하였다. 교대근무 간호사의 운동행위변화단계에 유의한 영향을 미치는 요인은 운동 동기(${\beta}=2.480$, p=.022), 건강상태 지각(${\beta}=1.151$, p=.013), 사회적지지(${\beta}=1.819$, p=.002)와 결혼유무(${\beta}=-1.820$, p=.004)로 나타났다. 즉, 운동행위 변화가 있는 대상자가 운동행위 변화가 없는 대상자에 비해 운동 동기가 11.9배 높고, 건강상태 지각은 3.1배 높았으며 사회적 지지는 6.1배, 미혼인 대상자는 기혼인 대상자에 비해 .16배 높았다. 그러므로 교대근무간호사의 운동행위 변화단계에 영향을 미치는 요인이 운동동기와 사회적지지, 건강상태 지각인 점을 고려하여 지속적이고 규칙적인 운동을 실천하기 위한 전략을 개발하는 것이 중요하다.

An Exploratory study of compliance with dietary recommendations among college students majoring in health-related disciplines: application of the transtheoretical model

  • Compliance with food group and nutrient recommendations, and self-efficacy, stage of change, perceived barriers and benefits for healthy eating were assessed among a convenience sample of college students majoring in health-related disciplines. Dietary and psychosocial data were collected using three-day food records and scales, respectively. Means (SD), frequencies, and percents were calculated on all data, and logistic regressions were used to determine whether any of the psychosocial correlates predicted the stage of change for healthy eating. Noncompliance with food group recommendations ranged from 53% for the meat/meat alternates group to 93% for the vegetables/juice group, whereas noncompliance with nutrient recommendations ranged from 26% for cholesterol to 99% for potassium. A majority of students (57%) self-classified in the preaction and 40% in the action stages of change for eating healthy. The students' self-efficacy to eat healthy was highest in positive/social situations and lowest when experiencing emotional upset. The most important perceived barrier to healthy eating was that friends/roommates do not like to eat healthy foods, and the most important perceived benefit was that eating healthy foods provides the body with adequate nutrients. The difficult/inconvenient self-efficacy subscale predicted the stage of change for healthy eating. These students would benefit from interactive learning opportunities that teach how to purchase and prepare more whole grain foods, fruits, and vegetables, enhance their self-efficacy for making healthy food choices when experiencing negative emotions, and overcome perceived barriers to healthy eating.

Evaluation of a theory-based community intervention to increase fruit and vegetable intakes of women with limited incomes

  • The study objectives were to increase both the stage of readiness to eat fruit and vegetables as well as the intakes of women who participated in the Expanded Food and Nutrition Education Program (EFNEP) for families with limited incomes. The intervention was to enhance the currently used curriculum, Eating Right Is Basic III (ERIB3), with stage-specific processes based on the Trans-Theoretical Model of readiness to change. Trained EFNEP community workers taught the enhanced curriculum to 90 mothers in the experimental county and to 53 mothers in the control county. Pre- and post-intervention measures included stages of readiness to eat fruit and vegetables and to intake as assessed by 24-hour dietary recalls and staging questions. Most women recruited into EFNEP were in Action and Preparation Stages (53.5%). Fruit and vegetable intakes showed a linear trend along with the Stage of Change for fruit and vegetable. After intervention, some combination of the ERIB3 and the fruit and vegetables-enhanced ERIB3 resulted in a reported 1/2 servings/day increase in fruits and vegetables in both the control and the experimental counties, although changes were not significant. EFNEP women also moved along the Stage of Change Continuum for fruits and vegetables in both counties. The percentage of people who ate 5 or more servings of fruit and vegetables was significant, however, only in the experimental group. We encourage health professionals to apply lessons learned from this intervention and to continue to pursue theoretically based interventions to change dietary behaviors.

Modal parameter identification of in-filled RC frames with low strength concrete using ambient vibration

  • In this study, modal parameters such as natural frequencies, mode shapes and damping ratios of RC frames with low strength are determined for different construction stages using ambient vibration test. For this purpose full scaled, one bay and one story RC frames are produced and tested for plane, brick in-filled and brick in-filled with plaster conditions. Measurement time, frequency span and effective mode number are determined by considering similar studies and literature. To obtain experimental dynamic characteristics, Enhanced Frequency Domain Decomposition and Stochastic Subspace Identification techniques are used together. It is shown that the ambient vibration measurements are enough to identify the most significant modes of RC frames. The results indicate that modal parameters change significantly depending on the construction stages. In addition, Infill walls increase stiffness and change the mode shapes of the RC frame. There is a good agreement between mode shapes obtained from brick in-filled and in-filled with plaster conditions. However, some differences are seen in plane frame, like expected. Dynamic characteristics should be verified using finite element analysis. Finally, inconsistency between experimental and analytical dynamic characteristics should be minimize by finite element model updating using some uncertain parameters such as material properties, boundary condition and section properties to reflect the current behavior of the RC frames.

노인 운동행위 변화단계별 중재프로그램의 개발 및 평가 - 범이론적 모형의 적용 - (Development and Evaluation of a Stage Matched Exercise Intervention Program for Elders - Application of the Tran Theoretical Model -)

  • Objectives: This study was designed to develop and evaluate a stage matched exercise intervention program to effectively increase exercise behaviors in urban elders. Methods: The study included three phases: preliminary descriptive data collection, program development, and program evaluation. The data for the preliminary descriptive phase were collected between May and June 2001. The study participants were 89 urban elders who responded a questionnaire that included general characteristics, exercise related experiences, stage, and process of change in exercise behaviors. Data were analyzed using descriptive statistics, $x^2$-test, and content analysis. Development of the program was based on the preliminary data. and a literature review, and was guided by the tran theoretical model. It consisted of strategies to facilitate the process of changes used in each stage. Evaluation of the program was achieved from October to December 2001, using a case study method, in which eight urban female elders participated. Interviews were conducted on a weekly basis in the form of either an individual interview, or group discussion. Each elder subject received education in accordance with the program strategies and education materials. In the case that a subject's stage of change moved into another one, the scores for the process of change were re-measured. The data were analyzed using the content analysis technique. Results: The results were as follows: 1. Elders who participated in the preliminary data collection phase were over 75 years of age, and the majority of them were women. They had a higher educational level, and fewer number of illnesses than the subjects in other studies. Their stage of change was divided into pre-contemplation and maintenance. The social liberation scores were the highest across all stages of change. There was no difference between men and women on scores for processes of change in each stage. 2. The stage matched exercise intervention program that was developed in this study consisted of one counseling type program and three distinguished educational booklet materials. 3. The results of the case studies are as follows: 1) The study participants were 8 women between 75 and 87 years of age. At the first interviews, all of them were in the pre-contemplation stage. All of them reached the action stage before the 7th week. The scores for processes of change that were the focus in each stage increased more than the scores for other processes of change. During the early stages of change, experimental processes increased more than behavioral processes. However. this pattern was reversed during later stages of change. 2) Characteristics of the subjects in each stage were identical as presented at the tran theoretical model. The intervention strategies were effective in the transition occurred in any stage. 3) Barriers for exercise included unwillingness to exercise, fatigue, shortness of breath, and pain. Ways to overcome these barriers were 'learning an alternative exercise method that can be done at home', 'self-promising/ exercise-promising', and 'use of cues to exercise'. 4) The factors that affected the application of the program were consideration of age and personal preference in selecting an exercise pattern, individualized intervention, and use of education materials appropriate to elders. Women over 80 years of age preferred muscle strengthening and stretching exercise, because they can be easily done at home. They also preferred individualized interventions, materials that were easy to read, and education contents appropriate for elders. Conclusion: In conclusion, the stage matched exercise intervention program that considered the characteristics of the elders was effective to facilitate exercise behaviors of the elders.
