간호사 배치기준에 대한 정책적 함의 (Policy Implications of Nurse Staffing Legislation)

  • 급성기 병원의 간호사 배치수준은 환자안전 및 성과에 영향을 미치며, 간호사 배치기준의 법제화는 환자안전에 필요한 최소 인원을 보장하는 중요한 수단이 될 수 있다. 우리나라는 의료법에 간호사 정원기준을 제시하고 있으나, 환자안전 보장 및 간호의 질 향상을 위해서 다음의 몇 가지 사항에 대한 개선이 필요하다. 첫째 1962년 제정된 현행 의료법의 간호사 정원기준은 보건의료환경 변화에 따라 적정성에 대한 재검토가 필요하다. 둘째 의료법의 간호사 정원은 의료기관이 준수하여야할 최소 인원으로 모든 의료기관이 준수하도록 관리되어야 한다. 셋째, 간호사 배치기준은 환자의 이해를 돕고, 관리가 용이하도록 근무조당 입원환자수 대 간호사 수를 기준으로 하도록 한다. 넷째, 병원 간호단위별 근무조별 간호인력 배치정보를 공개하여야 한다.

심장외과 중환자실 일반간호사의 업무분석 (Job Analysis of the Staff Nurse in Cardiac Surgery Intensive Care Unit)

  • Purpose : This study was conducted to provide for a basic resource, which can be used to set up a efficient management system in Cardiac Surgery Intensive Care Units(CSICU). Method: 1) Questionnaires were administered and observation methods were used, to examine the nursing activities performed in the CSICU after having reviewed related literatures and a review by the experts. Thus, the nursing activities were designating 254 activities and classified into 28 categories. 2)The 22 nurses in the 2 CSICUs filled out questionnaires about nursing activities from 12 April, 2002 to 17 April, 2002. The frequency of the nursing activities in the 28 categories counted and new nursing activities added by directly observing 12 nurses by two trained research staffs for 4 day. 3)In terms of validity, the 264 nursing activities were analysed by the 25 experts. As a result, 231 nursing activities were found valid and remained as appropriate nursing activities to be used for the careful analysis of the nursing activities in CSICUs. Result: The 22 categories are as below: assessment, monitoring, respiration management, nutrition management, elimination/drainage management, mobility management, sanitation management, safety management, temperature management, specimens collection, preparation and assistance of treatment, skin/wound management, infection management, medication management, education/support, dying patient care, recording/keeping, supplies management, environment management, communications, evaluations, professional development Conclusion : The manifest job description of the staff nurse will contribute to improving the efficiency of the nursing activities and to reducing the role conflicts among the medical staffs.

경력개발시스템 경력단계별 수술실 간호사 업무수행능력 비교 (Comparison of Nursing Performance by Career Levels in a Nurse Career Ladder System)

  • Purpose: This study was to compare nursing performance by career levels in a career ladder system. Methods: This survey was conducted with 140 OR staff nurses. Ten unit managers evaluated their own staff with a performance evaluation tool composed of 51 items. Data were analyzed by frequency, t-test, one-way ANOVA, ${\chi}^2$-test, and ANCOVA. Results: In career level CN II, nursing performance showed differences by personal characteristics. However, there was no significant difference in level CN III. CN IIIs' nursing performance was significantly higher than CN IIs' in all domains. Nursing performance of CN III was significantly higher than CN IIs' in 5 to 10 years of clinical experience. Although there was no significant difference in other clinical experience groups, CN IIIs' scores were always higher than CN IIs' Conclusion: The results indicate that nursing performance improves with career ladder advancement. Organizations should encourage advancement to higher levels and eliminate obstacles.

한국형 중환자간호 분류도구 개발 (Development of Korean Patient Classification System for Critical Care Nurses)

  • Purpose: This study was performed to develop a valid and reliable Korean Patient Classification System for Critical Care Nurses (KPCSC). Methods: Tertiary and general hospitals with various levels of ICU nurse staffing were included. To verify interrater reliability, data collectors and staff nurses of 15 ICUs in 11 hospitals classified 262 patients. To verify construct validity, the staff nurses classified 457 patients according to KPCSC comparing difference by medical department and type of stay in ICU. For conversion index, 195 patients from 10 ICUs in 7 hospitals were classified and nursing time was measured by 174 nurses, 7 head nurses, 18 charge nurses, 37 nurse aids and 1 secretary. Results: The developed KPCSC has 11 categories, 82 nursing activities and 115 criterias. Reliability was found to have high agreement (r=.96). Construct validity was verified by comparing differences in medical department and type of stay in ICU. According to scores, four groups in the KPCSC were identified. One score on the KPCSC indicates 6.12 minutes of nursing time. Conclusion: The findings show that the KPCSC can be used to measure new and complex nursing demands including rehabilitation and the safety of ICU patients.

간호근무환경과 간호결과와의 관련성 및 간호사 업무 부담에 대한 내용분석 (The Relationship of the Nursing Work Environment and Nursing Outcome among it's Nurses and Content Analysis of Nurses' Workload)

  • Purpose: The aim of this study was to explore the relationship between nursing work environment and nursing outcome of clinical nurses. Also, the purpose of this study was to identify the nurses' workload. Method: The participants in this study were 246 nurses working at one hospital in a province. Data was collected using a structured questionnaire from October 2011 to November 2011. Open questions were subject to content analysis; closed questions were subject to descriptive statistics and Pearson correlation coefficient with SPSS 18.0 program were used. Result: The number of patients per nurse in day-duty and overtime work had a positive correlation. Nursing performance and the overtime work in night duty nurses had a negative correlation. The results of this study, with a much higher number of patients per nurse, showed an increase in overtime hours. In the nursing delivery system experience of nurses, 13 themes were identified and they were divided into five categories. Conclusion: Therefore, in order to improve nursing performance and increase job satisfaction, efforts must be made to reduce overtime, and nursing personnel placement to secure a substantial nursing staff.

Implementation of contextualized, emergency management cognitive aids in a periodontics clinic

  • Background: Emergencies in outpatient clinics are rare. However, potentially catastrophic events can be challenging to manage due to a variety of factors, including limited equipment and staff. The purpose of this quality improvement project was to improve the staff knowledge and familiarity with critical performance elements for emergencies encountered in the setting of a periodontics clinic. Methods: Emergency cognitive aids tailored to the clinic's resources were created for anaphylaxis, airway obstruction, and sublingual hemorrhage. The project pre-post-test repeated measures design evaluated the effectiveness of cognitive aids using a combination of hands-on simulation, written knowledge assessments, and self-efficacy surveys. Training sessions and simulations were provided to the clinic's existing care teams made up of a periodontist and two dental assistants with an anesthetist who was present for simulations involving sedation. Due to the small sample size (N = 14) and non-normal distribution, all metrics were evaluated using non-parametric statistics. Results: Significant improvements were found in knowledge assessment (-2.310, P = 0.021) and self-efficacy (-2.486, P = 0.013) scores when retention after a training session before and after the introduction of cognitive aid was compared. The mean simulation scores and times improved steadily or reached maximum scores during the project progression. Conclusion: Training sessions before and after cognitive aid introduction were effective in improving knowledge, self-efficacy, and simulation performance. Future projects should focus on validating the process for creating contextualized cognitive aids and evaluating the effectiveness of these cognitive aids in larger samples.

Factors Influencing Residents' Activities of Daily Living Related to Nursing Staff in Korean Nursing Homes using Path Analysis

  • Purpose: The purpose of this study was to empirically test a model of associations linking locations and competition among nursing homes (NHs), mediated by facility grade and registered nurse (RN) turnover, on activities of daily living (ADLs) in Korean NHs. Methods: This study used a cross-sectional design to identify causal factors on NH residents' ADLs. Data were collected from June 2017 to August 2017. A disproportionate stratified cluster sampling method of NHs across Korea was used to gain representation. The collected data consisted of location and the Herfindahl-Hirschman Index (HHI), RN turnover rate, facility grade, and NH residents' ADLs. Results: All pathways affecting ADLs were not significant, and the effect on facility grade was significant in RN turnover (β = -.59, p < .001). RN turnover associated negatively with facility grade. In other words, the higher the RN turnover, the worse the facility grade. Conclusion: This study is the first to examine the impact of location and HHIs, mediated by RN turnover rate and facility grade, on NH residents' ADLs. To improve residents' ADLs, subsequent studies are needed to identify the factors affecting ADLs utilizing other variables because this study did not identify factors that affect ADLs.

의료기관별 간호인력 활용방안-중소병원 간호사 확보를 중심으로 (A Utilization Strategy of Nursing Staff by Types of Medical Institutions - nurse staffing level of medium and small-sized hospitals)

  • 본 연구는 중소병원 간호인력 현황과 관련 정책을 분석하고 인력 확보에 영향을 주는 요인을 파악하여 중소병원 간호인력 확보를 위한 정책을 입안하고 결정하는 데에 근거를 제시하고자 실시되었다. 보건복지부와 건강보험심사평가원 등에서 제시한 통계자료를 2차 분석한 후 그 결과를 다양한 내 외적 보건의료환경에 대한 이해를 기반으로 조명하였다. 연구결과 우리나라의 활동 간호사 수는 면허간호사 수의 50% 미만으로 그 수는 해마다 감소하고 있었으며, 이는 간호대학 정원증가의 비용 대비 성과의 효율성을 재고할 필요가 있음을 의미한다. 또한 간호관리료 차등제 실시로 인해 신규간호사는 물론 간호사 인력부족이 심각한 중소병원의 경력간호사들이 상급종합병원으로 이동하게 되는 결과를 야기하였으며, 결국 중소병원 간호인력 부족 현상을 더욱 악화시키는 원인이 되었다. 따라서 입원환자 간호관리료 차등제가 중소병원 간호인력 확보 정책으로 실효성이 있는지에 대한 재검토가 필요하며, 면허간호사에 대한 추적 관리 시스템을 통해 간호인력 수급의 불균형을 해소할 수 있어야 한다.

종합병원의 수술복 이너웨어 디자인 개발에 관한 연구 (Development of the Designs of Surgical Scrubs at General Hospitals)

  • In South Korea, hospital clothes for medical staff, which require high performance and functionality, are still regarded just as uniforms. Few researches have been made into the underwear for operating room staff, which is intended to protect medical staff against possible risks happening during an operation and allow them to do their job in an easy and comfortable manner. In order to present a new design of underwear for operating room staff at general hospitals in South Korea, an investigation was made into medical staff's preference for overseas brand samples, and it was found that they preferred to wear such clothes as are blue-colored, of a V-neck line, of a box style and has good absorptiveness and breathability. After developing the primary prototype and the conventional underwear for operating room staff at Hospital A, their appearances were evaluated, and it was found that the primary prototype was more favorably evaluated. After developing the secondary prototype, their preference for the colors and shapes was surveyed, and it was found that they preferred to wear No.6 blue colors and T/C materials. In conclusion, it is considered that a systematic research need be conducted into the designs of various hospital clothes, whose roles and functions are subdivided, including the underwear for operating room staff developed in this research.

스트레스 관리 프로그램이 호텔조리사의 직무스트레스에 미치는 효과 (The Effects of a Stress Management Program on Job Stress in a Hotel Culinary Staff)

  • Purpose: This study was conducted to investigate the effects of a stress management program on Job Stress in culinary staff at a Hotel. Method: This was a quasi-experimental study using non-equivalent control group pretest-posttest design. The subjects of this study were 52 culinary staff members(head culinary. first culinary) working at a S hotel. (25 in the experimental group. 27 in the control group) The experimental group was provided with an individual counseling program for 12 weeks and 30 min/week. The control group was provided with booklets. Data were analyzed with SAS Program using repeated measures ANOVA. Results: The stress management program was effective to improve job stressors and especially to relax interpersonal conflict among the sub-factors of job stress based on the results mentioned above. Conclusion: This program was effective in reducing job stress. However. further studies are required to measure persistent and long-term effects through the application of the program. which is specifically designed for reduce to job stress. for over 12 weeks.
