Correction of the deviated tip and columella in crooked nose

  • The primary procedural components of deviated nose correction are as follows: osteotomy to correct bony deviation, septal deviation correction, manipulation of the dorsal septum to correct upper lateral cartilage deviation, and correction of functional problems (manipulation for correction of internal valve collapse and hypertrophy of the inferior turbinate). The correction of tip and nostril asymmetry cannot be overemphasized, because if tip and nostril asymmetry is not corrected, patients are unlikely to provide favorable evaluations from an aesthetic standpoint. Tip asymmetry, deviated columella, and resulting nostril asymmetry are primarily caused by lower lateral cartilage problems, which include deviation of the medial crura, discrepancy in the height of the medial crura, and asymmetry or deformity of the lateral crura. However, caudal and dorsal septal deviation, which is a more important etiology, should also be corrected. A columellar strut graft, correction of any discrepancy in the height of the medial crura, or lateral crural correction is needed to correct lower lateral cartilage deformation depending on the type. In order to correct caudal septal deviation, caudal septal shortening, repositioning, or the cut-and-suture technique are used. Surgery to correct dorsal septal deviation is performed by combining a scoring and splinting graft, a spreader graft, and/or the clocking suture technique. Moreover, when correcting a deviated nose, correction of asymmetry of the alar rim and alar base should not be overlooked to achieve tip and nostril symmetry.

카메라를 이용한 크레인 스프레더 스큐모션 계측기술에 관한 연구 (A Study on Skew Measurement Technique for the Crane Spreader using a Camera)

  • 본 논문에서는 카메라를 이용하여 크레인 스프레더의 스큐모션을 계측하는 계측기법에 대해 고찰하고 있다. 계측장치는 트롤리에 설치한 카메라와 스프레더에 설치한 두 개의 랜드마크로 구성된다. 랜드마크를 이용하여 크레인 스프레더 흔들림과 상하위치를 검출하는 기법은 저자들이 이미 제안한 기술이며 실험을 통해 그 유용성을 검증하였다. 크레인 스프레더의 스큐모션 계측기법 또한 제안된 계측기법에 기초한 것으로 두 개의 랜드마크를 검출하여 템플릿 매칭기법으로 스큐모션을 계측할 수 있다. 스큐모션은 스프레더의 회전각도를 검출하여 계측해야 하는데 계측정도와 신뢰도는 정확한 템플릿매칭의 가능여부에 의존하게 된다. 즉, 랜드마크의 회전으로 매칭이 실패할 경우에는 정확한 회전각도를 검출할 수 없는 경우가 발생할 수 있게 된다. 따라서 본 논문에서는 랜드마크 회전에 따라 템플릿을 회전시키는 방법을 도입하여 템플릿매칭의 신뢰성과 계측정도를 개선하는 방법에 대해 연구하였다. 제안된 방법을 이용할 경우 템플릿매칭이 실패하는 경우가 없음을 실험을 통해 확인하였으며, 측정범위는 ${\pm}12^{\circ}$ 이고 이것은 크레인 스프레더의 스큐모션을 파악하고 제어하는데 충분한 정도의 범위이다.

멀티셋의 크기 추정 기법에서 샘플링의 효과 (Effect of Sampling for Multi-set Cardinality Estimation)

  • 멀티셋에서 중복을 제외한 서로 다른 원소의 수를 추정하는 것은 네트워크 트래픽 측정 분야에서 매우 잘 알려진 문제이며, 많은 알고리즘들이 제안되었다. 최근에는 선형 카운팅 기법(Linear Counting)에 기반해서 매우 작은 메모리만을 이용해서 멀티셋의 크기를 추정하는 알고리즘이 개발되었다. 너무 많은 데이터를 처리하기 어려운 경우 전체 데이터를 처리하지 않고, 패킷의 일부를 샘플링해서 사용하는데, 이 샘플링은 일반적으로 정확도에 부정적인 영향을 주는 것으로 알려져있다. 하지만, 이 논문에서는 멀티셋의 크기를 추정하는데 있어서 CSE를 이용하는 경우 샘플링이 정확도와 측정 범위의 측면에서 오히려 전수조사를 하는 MCSE보다 더 좋은 결과를 낼 수 있음을 보였다. 이를 입증하기 위해 수학적 분석, 실제 데이터를 이용한 실험을 수행하고, CSE, MCSE 그리고 CSES를 비교하였다.

A Development of Composting System

  • A simple compost windrow-forming car and a compost tuner were developed at the aim of low cost manure management systems. The developed compost windrow- forming car is possible windrows formation at a short time. In order to be pulverized and mixed by upper and lower beaters at the windrow-forming, the condition of air and the mixing of materials to play a major role in composting is filled. This car can also utilized for a manure spreader by folding the windrow-forming cover. The developed compost turner is a type equipped with tractor rear-right side. Main specifications are 2.5m working width, overall length 3.3m and 45degrees conveyer inclination angle, 2 beaters for mixing and pulverization are equipped in front parts. Windrows of 2.5m width and 1.5m height are turned at the speed of 4.5-5.1m/min. In accordance with the above, by combining the compost windrow-forming car and the compost turner, a simple composting system for each farmer could be constructed by the low c st.

오버헤드셔틀시스템의 동특성해석 및 잔류진동제어 (Dynamics Analysis and Residual Vibration Control of an Overhead Shuttle System)

  • This paper discusses the dynamics and control problem of an overhead shuttle system (OSS), which is a critical part of the automated container terminal at a port. The main purpose of the OSS is efficient automated transport function of containers, which also requires high precision and safety. A major difference between the OSS and the conventional container crane is the configuration of the cables for hoisting the spreader. A mathematical model of the OSS is developed here for the first time, which results in an eight-pole system. Also, open loop control methods (trapezoidal and notch-type velocity profiles) are investigated so that the command input to the overhead shuttle produces the minimum possible sway of the payload. Simulation results show that the vibration suppression capability of the OSS is superior to the conventional overhead container crane, which is partially due to the cable configuration.

컨테이너 크레인의 흔들림 방지장치 개발에 관한 연구 (A Study on the Development of Residual Sway Motion Control System for the Container Crane)

  • The sway control problem of the pendulum motion of a container hanging on the trolly, which transports containers from a container ship to trucks, is considered in the paper. In the container crane control problem, suppressing the residual swing motion of the container at the end of acceleration or the case of that the unexpected disturbance input exists is main issue. For this problem, in general, the trolley motion control strategy is introduced and applied. But in this paper, we introduce and synthesize a new type of swing motion control system. In this control system, a small auxiliary mass is installed on the spreader. And the actuator reacts against the auxiliary mass, applying intertial control forces to the container to reduce the swing motion in the desired manner. In this paper, we apply the LMI approach and simultaneous optimization design method to design the anti-swing motion control system for the controlled plant. And the simulation result shows that the proposed control strategy is shown to be robust to disturbances like winds and initial sway motion.

컴퓨터 비젼을 이용한 컨테이너 자세 측정 (The Container Pose Measurement Using Computer Vision)

  • 본 논문은 CCD 카메라와 거리 센서를 사용하여 컨테이너의 자세 측정에 관하여 연구하였다. 특히 특징점을 추출하고 영상의 잡음을 줄이는 방법에 대하여 중점적으로 기술하였다. 가우시안 및 랜덤 노이즈를 제거하기 위하여 Euler-Lagrange 방정식을 소개하였으며 PDE(Partial Differential Equation)를 기초로 한 Euler-Lagrange 방정식을 풀기 위하여 ADI(Alternating Direction Implicit)방법을 적용하였다. 그리고 스프레더와 컨테이너의 특징점을 추출하기 위해서 기존의 황금 분할법과 이분 분할법을 이용한 방법은 지역적 최대 및 최소 값의 경우 정확한 해를 구할 수 없어서 k차 곡률 알고리즘을 이용하였다. 제안된 알고리즘은 영상의 전처리과정에서 잡음제거에 효과적이며 카메라와 거리센서를 이용한 제안 시스템은 기존시스템의 구조적 변경 없이 사용가능하기 때문에 비용이 저렴한 장점이 있다.

Central Decompressive Laminoplasty for Treatment of Lumbar Spinal Stenosis : Technique and Early Surgical Results

  • Objective : Lumbar spinal stenosis is a common degenerative spine disease that requires surgical intervention. Currently, there is interest in minimally invasive surgery and various technical modifications of decompressive lumbar laminectomy without fusion. The purpose of this study was to present the author's surgical technique and results for decompression of spinal stenosis. Methods : The author performed surgery in 57 patients with lumbar spinal stenosis between 2006 and 2010. Data were gathered retrospectively via outpatient interviews and telephone questionnaires. The operation used in this study was named central decompressive laminoplasty (CDL), which allows thorough decompression of the lumbar spinal canal and proximal two foraminal nerve roots by undercutting the lamina and facet joint. Kyphotic prone positioning on elevated curvature of the frame or occasional use of an interlaminar spreader enables sufficient interlaminar working space. Pain was measured with a visual analogue scale (VAS). Surgical outcome was analyzed with the Oswestry Disability Index (ODI). Data were analyzed preoperatively and six months postoperatively. Results : The interlaminar window provided by this technique allowed for unhindered access to the central canal, lateral recess, and upper/lower foraminal zone, with near-total sparing of the facet joint. The VAS scores and ODI were significantly improved at six-month follow-up compared to preoperative levels (p<0.001, respectively). Excellent pain relief (>75% of initial VAS score) of back/buttock and leg was observed in 75.0% and 76.2% of patients, respectively. Conclusion : CDL is easily applied, allows good field visualization and decompression, maintains stability by sparing ligament and bony structures, and shows excellent early surgical results.

네비게이션을 이용한 비밸브 기능저하의 원인 규명과 치료 1예 (A Case of Identification of the Cause Using Navigation System and Treatment in the Patient with Nasal Valve Compromise)

  • Nasal valve compromise (NVC) is a distinct cause of symptomatic nasal obstruction, yet there are several ambiguities surrounding the diagnosis and management of this disease. Understanding of nasal valve anatomy with critical assessment of the site of obstruction is essential to effective nasal valve management. Technique selection should be individualized to the type of valve dysfunction. This case report presents a 56 year old man with nasal valve dysfunction due to narrow middle vault, concave lower lateral cartilage and swollen septal body which was diagnosed by various techniques including navigation system and treated by spreader graft, alar batten graft and reduction of septal body.

Forefoot disorders and conservative treatment

  • Forefoot disorders are often seen in clinical practice. Forefoot deformity and pain can deteriorate gait function and decrease quality of life. This review presents common forefoot disorders and conservative treatment using an insole or orthosis. Metatarsalgia is a painful foot condition affecting the metatarsal (MT) region of the foot. A MT pad, MT bar, or forefoot cushion can be used to alleviate MT pain. Hallux valgus is a deformity characterized by medial deviation of the first MT and lateral deviation of the hallux. A toe spreader, valgus splint, and bunion shield are commonly applied to patients with hallux valgus. Hallux limitus and hallux rigidus refer to painful limitations of dorsiflexion of the first metatarsophalangeal joint. A kinetic wedge foot orthosis or rocker sole can help relieve symptoms from hallux limitus or rigidus. Hammer, claw, and mallet toes are sagittal plane deformities of the lesser toes. Toe sleeve or padding can be applied over high-pressure areas in the proximal or distal interphalangeal joints or under the MT heads. An MT off-loading insole can also be used to alleviate symptoms following lesser toe deformities. Morton's neuroma is a benign neuroma of an intermetatarsal plantar nerve that leads to a painful condition affecting the MT area. The MT bar, the plantar pad, or a more cushioned insole would be useful. In addition, patients with any of the above various forefoot disorders should avoid tight-fitting or high-heeled shoes. Applying an insole or orthosis and wearing proper shoes can be beneficial for managing forefoot disorders.
