Case-Control Study on Social Risk Factors of Stroke in Korea (뇌졸중의 사회적 위험요인에 대한 환자-대조군 연구)

  • The purpose of this study was done to investigate the influence of social risk factors on each stroke type. We recruited at the Stroke Medical Center in Daejeon University Oriental Medical Hospital from July 2005 to March 2007 for this study. We divided 217 patients with acute stroke within 2 weeks as the case group(Cases), 146 people without major risk factor as the healthy control group(Normals), and 160 people as the general control group(Controls). We analyzed general characteristics such as age, sex, with or without spouse, education periods, religion, psychologic stress, and the odds ratio of each social risk factors by multivariate logistic analysis. As a result of reviewing the influence of social risk factors upon each stroke type, without spouse may be risk factor of ischemic stroke, and the undereducated may be risk factor of hemorrhagic stroke and ischemic stroke. But religion and psychologic stress had no significant relation with stroke.

Predictors of life satisfaction in marriage migrant wives with school-aged children (학령기 자녀가 있는 결혼이민자 여성의 생활만족도 예측 변인)

  • This study examined the predictors of life satisfaction of marriage migrant wives with school-aged children in South Korea. For this purpose, we draw data from the 2015 National Survey on Multi-cultural Families (NSMF). The target group was marriage migrant wives who were in a first marriage and had school-aged children (N=3,004). We used OLS regression to examine the predictors of the target group's life satisfaction with the SPSS 18.0 program. The results are as follows. First, marriage migrant wives with school-aged children had maintained their marriage for at least 14 years. They were satisfied with their relationships with both their spouses and their children. Also, they were satisfied with their spouse's child care roles. They did not support the multi-cultural policy of assimilation. Their Korean proficiency was slightly higher than the middle level. More than half had jobs and had not attended a parent meeting. Second, although they were satisfied with their everyday lives, their levels of life satisfaction were less than that of migrant wives with children below 5 years of age. Third, all variables explained 38.8% of the life satisfaction experienced by marriage migrant wives with school-aged children. Satisfaction with their spouses, subjective health, satisfaction of spouse's child care role, monthly family income, satisfaction with their children, experience attending a parents meeting, and Korean proficiency were positively associated with the life satisfaction of this target group. The findings of this study are significant because they can provide certain implications for family life education and policy within a multi-cultural society.

The Effects of Climacteric Symptoms and Hardiness on Mid-Life Crisis (중년남성의 갱년기증상, 강인성이 중년 위기감에 미치는 영향)

  • This study examined the degree of climacteric symptoms, hardiness and middle-life crises. The survey period was from 25 May to 10 September, 2013 with workers in small business workplaces in Jeonnam. The data was collected by a self-questionnaire and SPSS Win 18.0 was used for data analysis. As an accounting hierarchical analysis result, middle-life crisis was affected by exercise, and the relationship with the spouse and child with an explanatory power of 25.1%. Additional input climacteric symptoms showed more explanatory power (21.1%). In addition, exercise, education, relationship with the spouse, relationship with child, income, and climacteric symptoms were predictors of a mid-life crisis with explanatory power of 42.7%. Therefore, to successfully overcome mid-life crises, a range of nursing interventions should be provided to make a positive self-control and intimate relationship with the spouse of middle-aged men.

Is There Any Spousal Concordance of Cerebrovascular Risk Factors in Stroke Patients? : A Preliminary Study (뇌졸중 환자에서 배우자 사이에 뇌혈관 질환의 위험요인 일치성이 있는가? : 예비 연구)

  • Familial concordance of vascular risk factors is well understood and there are many studies about inter-spouse concordance of the risk factors for coronary heart disease. However, there are no studies for inter-spousal relation of the risk factors in cerebrovascular disease patients. We analysed inter-spouse correlation of cerebrovascular risk factors in patients admitted our hospital due to stroke and their spouses. There was statistically significant inter-spouse concordance in the presence of hypertension(p=0.025) and carotid IMT(r=0.479, p=0.001). However, the further age-adjusted analysis revealed no significant result. Differently from the results of previous studie,, the present study shows no significant spousal concordance. Althought there are tendencies of spousal concordance in some risk factors, the age is major determinant.

Satisfaction on Marital Relationship of Husband and Wife in Old Age (노년부부가구 남편과 부인의 부부관계 만족도)

  • The prolongation of life expectancy has expanded the duration of marriage in old age. In the meanwhile, the change of life style emphasizing privacy and sociodemographic environment favoring independent living has increased the number and proportion of the elderly living with spouse only. This study, focusing on those aged 60 and over who live with spouse only the elderly, explores their marital relationship. The study pays a special attention to compare whether and how the satisfaction on the relationship with spouse would differ between husband and wife. The ordered logit model analysis is conducted based on the nationwide survey '2008 Korean National Survey of Welfare Need in the Elderly'. The results show that the marital relationship satisfaction of husbands is influenced by their own characteristics but hardly affected by his wife's. In contrast, the satisfaction of wife is influenced by husband's characteristics as well as hers. These finding reflect that women are more strongly family-centered and dependent on their husband.

Factors Influencing Medication Adherence in Patients with Hypertension: Based on the 2008 Korean National Health and Nutrition Examination Survey (고혈압 환자의 약물복용 이행에 영향을 미치는 요인: 2008년 국민건강영양조사를 이용하여)

  • Purpose: The purpose of this study is to examine factors influencing medication adherence in patients with hypertension. Methods: This study carried out a secondary analysis of data from the 2008 Korean National Health and Nutrition Examination Survey (KNHANES). Stratified sampling was used to select a participant sample that was representative of patients with hypertension throughout the country. Using the SPSS/WIN 18.0 program, data were analyzed using descriptive statistics, $x^2$ test, t-test, and logistic regression. Results: Of the patients with hypertension, 8.8% had showed non-adherence to medication. Medication adherence was associated with age, spouse, Medicare insurance, number of other diseases, and current smoking status. The cases with older age, a spouse, Medicare insurance, higher number of other diseases, and no current smoking status showed significantly high medication adherence. Conclusion: Nursing interventions and further studies are needed to achieve high levels of medication adherence based on factors influencing medication adherence such as age, spouse, Medicare insurance, number of other disease, and current smoking status.

  • In order to ensure the housing stability of homeless people who living in the information society, the Korean Housing Lease Protection Act was enacted as a special law of Civil Code in March 5, 1981, Law No. 3379. And until January 6, 2015 there were 15 times revisions. In the meantime, many issues have been resolved by legal revisions through several times however, it is true that many problems are exposed after enforcement because processing without sufficient review of legislation. Among them, at the 1st revision in 1983 the purpose of Article 9 for lease succession was admitting succession to a spouse who has no inheritance rights. Then it can protect common-law relationship and on the other hand protect the residential life of a spouse. But many questions have been raised. Therefore, in this paper, analyze the problem of lease succession carefully, proposes an improvement to contribute to the residential stable livelihood.

Living Behavioral Patterns of Single or Spouse Elderly Households and Ubiquitous Home Services for Each Household Type (노인단독가구 유형별 생활행태 특성 및 유비쿼터스 홈 서비스 요소 제안)

  • The purposes of this study were to identify living behavioral patterns in of single or couple elderly households in residential environment, to find out ubiquitous home services for the elders, and finally to suggest the ubiquitous home services according to each household's type. For this study, literature review, field works for ubiquitous home services for the elders, and in-depth interview and observation by 74 elderly participants were conducted. The major findings of the study were as follows: single or spouse elderly households were classified into 8 types according to the combination of their household types, income level, and health status. Among 8 types, 2 types were excluded for final analysis due to small sample size. Living behavioral characteristics of 6 types were analyzed and the behavioral pattern of each type was drawn. Based on this behavioral pattern of each type, ubiquitous home services which meet the needs of each type were suggested. The implications and limitations of this study were also discussed and the suggestions for further studies were recommended.

A Structural Model for Quality of Life in Women Having Hysterectomies (여성의 자궁절제술후 삶의 질 구조모형)

  • The purpose of this study was to develope and test the structural model for quality of life in women having hysterectomies. A hypothetical model was constructed on the basis of previous studies and a review of literature. The conceptual framework was built around eight constructs. Exogenous variables included in this model were marital intimacy, importance of uterus, professional support, positive coping behavior and pre-operative symptoms. Endogenous variables were spouse's support, sense of loss and quality of life. Empirical data for testing the hypothetical model was collected using a self-report questionnare from 203 women having hysterectomies at the outpatient clinics of four general hospitals and a mail survey in Pusan City. The Data was collected from December, 1997 to January, 1998. Reliability of the eight instruments was tested with Cronbach's alpha which ranged from 0.639-0.915. For the data analysis, SPSS 7.5 WIN Program and LISREL 8.12 WIN Program were used for descriptive statistics and covariance structural analysis. The results of covariance structure analysis were as follows : 1. Hypothetical model showed a good fit with the empirical data. [$\chi$$^2$=6.93(df=5, P=.23), GFI=.99, AGFI=.94, RMSR=.019, NNFI=.97, NFI=.98, CN=440, standardized residuals(-2.14-2.10)] 2. For the parsimony of model, a modified model was constructed by deleting 3 paths and adding 1 path according to the criteria of statistical significance and meaning. 3. The modified model also showed a good fit with the data. [$\chi$$^2$=5.26(df=7, P=.63), GFI=.99, AGFI=.97, RMSR=.014, NNFI=1.02, NFI=.99, CN=710, standardized residuals(-1.46-1.70)] Results of the testing of the hypothesis were as follows : 1. Marital intimacy(${\gamma}$11=.78, t=14.37) and professional support(${\gamma}$13=.12, t=2.12) had a significant direct effect on the spouse's support. 2. Pre-operative symptoms(${\gamma}$25=.32, t=3.12), importance of uterus(${\gamma}$22=.20, t=2.61) and spouse's support($\beta$2l=-.19, t=-2.43) had a significant direct effect on the sense of loss. 3. Sense of loss($\beta$32=-.66, t=-9.83) had a direct effect on the quality of life. Marital intimacy had a direct(${\gamma}$31=.19, t=3.33), indirect(${\gamma}$31=.14, t=2.52) and total effect(${\gamma}$31=.25, t=4.41) on the quality of life. Professional support had a direct effect(${\gamma}$33=.11, t=2.07) and total effect(${\gamma}$33=.13, t=2.31) on the quality of life. The direct effect of pre-operative symptoms(${\gamma}$35=-.36, t=4.02) and positive coping behavior(${\gamma}$34=.15, t=2.06) had the insignificant effect on the quality of life while, due to the idirect effect these variables had overall significant effect on the quality of life. The results of this study showed that the sense of loss had the most significant direct effect on the quality of life. Marital intimacy, pre -operative symptoms and spouse's support had a significant direct effect on this sense of loss. These four variables, the sense of loss, marital intimacy, pre-operative symptoms and spouse's support, were identified as relatively important variables. The results of this study suggested that there is needed to determine if nursing intervention would alleviate this sense of loss and promote a greater quality of life in women who have had hysterectomies.

Sex Differentials in the Utilization of Medical Services by Marital Status (배우자유무에 따른 남녀간 의료서비스 이용의 차이)

  • It has been well known that women live longer but suffer more from degenerative diseases than men do, which, in turn, results in frequent utilization of medical care services among women. Previous studies, mostly based on Western society, have discussed that women, particularly married women, are more likely to utilize medical care services than men even after considerations of different health status and demographic and socioeconomic characteristics. Social interests in women's health and socialization process of caring for health among women are known as causes. This study examines sex differentials in the medical care service utilization in Korean society, particularly focusing on marital status. Feministic perspectives in Korea have argued that sex division of labor within family puts women in a position that has to continuously provide physical and emotional services to other family members, which keeps them from taking care of their own health. This study empirically tests if this feministic perspective holds true in Korea. Results show that Korean women and those with spouse are more likely to utilize medical care services than their men and the spouse-less counterparts, net of other risk factors. However the effect of existence of spouse on the medical care utilization is much smaller among women than men. These results suggest that the feministic perspective is in part applicable to explain sex differentials on medical care service utilization in Korea.
