Impaction of mandibular first molar is relatively rare and its overall frequency has been reported to be 0.01%. The etiology of impaction are lack of eruption space, physical obstacles such as supernumerary teeth, odontomas or odontogenic tumors, hereditary factors, functional disturbances of endocrine glands and traumas. Impaction of mandibular first molar can result in a short lower facial height, formation of a follicular cyst, pericoronal inflammation, resorption of the roots of neighboring teeth and malocclusion. The treatment options available for impacted teeth include surgical exposure, orthodontic forced eruption, surgical repositioning and surgical removal of unerupted molar. This report presents two cases of distally tilted and impacted mandibular first molars which were treated by surgical exposure. In these cases, we could observe spontaneous eruption of the impacted mandibular first molars after surgical exposure.
Dentigerous cyst is an odontogenic cyst which occurs in unerupted tooth crown. After the crown formation, enamel epithelium remnants surrounded continuously proliferates and it forms effusionfluid cyst and expands due to increased internal osmotic pressure. Treatments of cysts are mainly enucleation, marsupialization and de-compression. When deciding the way of treatment, the age of a patient, the anatomical circumstances, the region of lesion and the size of cyst should be considered. Marsupialization is that some parts of internal cystic wall would be converted into oral mucosa if the cyst is large size and is concerned about neighboring anatomic structure. It can be accompanied by enucleation later and eruption of related tooth can be possible. If there is a limitation of spontaneous tooth eruption, eruption of tooth can be induced by orthodontic apparatus. There were 3 patients had dentigerous cyst and underwent marsupialization, their impacted teeth had preserved and had induced eruption, all showing satisfactory results.

Odontomas generally appear as small, solitary, or multiple radio-opaque lesions found on routine radiographic examinations. It is a comparatively common odontogenic tumor, and may lead to interfere with the eruption of its associated tooth. In general, odontomas occur more often in permanent dentition and are very rarely associated with primary teeth. This report deals with five rare cases of primary teeth impaction associated with odontomas, with spontaneous eruption occurring in all five cases after simple surgical removal of the odontoma. Impacted primary teeth may be associated with defects in development and eruption of their permanent successors, and thus long-term observation is necessary until the permanent successors erupt.

Intrusive luxation that takes approximately three percent of permanent teeth commonly occures at anterior teeth. This intrusion frequently leads to pulp necrosis, root resorption, marginal bone loss and these complications are influenced by depth of intrusion and stage of root development. Various treatment approaches have been suggested to manage of intrusive luxation. Techniques aiming to reposition the intruded tooth include an observation for spontaneous re-eruption, surgical or orthodontic repositioning. We report two cases with clinically satisfactory results for traumatically intruded maxillary central incisor. In one case which has a large open apex and mild intrusion depth, we observed for spontaneous eruption and then repositioning by forced eruption method. In other case, which has been completely intruded, was repositioned by surgical extrusion and followed by apexification.

Dilaceration is most common in maxillary central incisor. Impaction by dilacerated tooth cause orthodontic problem such as tilting of adjacent tooth, midline deviation. Dilaceration is caused by trauma, cyst, other origin and tooth shape, degree of root curvature, location in the alveolar bone, ability of spontaneous eruption should be considered whether treat or not. Labially and lingually dilacerated tooth is difficult to spontaneous eruption. Thus, Prosthetic treatment alternative with extraction, auto transplantation, orthodontic traction with surgical exposure is included within the treatment alternative. These cases are about impacted central incisor with dilacerated root. We use closed eruption technique and guide impacted tooth into normal position by orthodontic traction.

This study aimed to investigate the treatment options for the delayed eruption of mandibular premolars and identify the predictors of spontaneous eruption using panoramic radiography. The prevalence of delayed mandibular premolar eruption in this retrospective analysis, comprising 254 patients (aged 9 - 15 years), was 5.19%, with no significant difference based on gender. The mandibular second premolars were most affected (4.39%) compared to the first premolars (0.76%). No significant difference in prevalence was observed between the left and right sides. Among the treated mandibular premolars, primary molar-related lesions were identified as the leading cause (7.85%) of delayed tooth eruption. The treatment duration varied based on the Nolla stage, eruption stage, and treatment method. Teeth with Nolla stage 7 or lower had a treatment duration of 22.89 ± 11.96 months, whereas those with stage 8 or higher had a 15.02 ± 6.34 month duration. The deeper the tooth was located in the bone, the longer the treatment period became. The treatment duration varied depending on the treatment method, and statistically, there was no significant difference. The treatment durations for affected mandibular premolars increased with the depth of impaction angle of inclination. In this study, the treatment duration for delayed eruptions varied depending on the Nolla stage, eruption stage, and treatment method. Variations in the impaction depth and inclination angle across various treatment approaches, as explored in this study, might offer valuable insights into the selection of the most suitable management options for delayed tooth eruptions.

Impaction is defined as the cessation of the eruption of the tooth caused by a clinically or radiographically detectable physical barrier in the eruption path or by the ectopic position of tooth germ. Besides the third molars and the maxillary canines, the most common impacted tooth is the second premolar. The overall frequency of premolar impaction has been reported to be 0.5%. In some cases, orthodontic traction and surgical repositioning may be indicated. When impacted second premolar is involved with prolonged retained second primary molar, extraction of primary molar and space maintenance lead to eruption of second premolar. In these cases, all patients visited to department of pediatric dentistry of Kyungpook National University Hospital for the chief complaint of unerupted second premolar. Extraction of prolonged retained second primary molar and space management are tried for spontaneous eruption of impacted second premolar tooth. The results were as follows: 1. When impacted second premolar is involved with prolonged retained second primary molar, minimal treatment via elimination of primary molar leads to successful results. 2. Proper space management and periodic radiographic examination are required before eruption of second premolar. 3. Sufficient time must be allowed for confirm of tooth movement before orthodontic traction or surgical repositioning. 4. The result is more successful in incomplete root development.

The purpose of this study was to evaluate the eruption patterns of mandibular premolars associated with dentigerous cysts after marsupialization regarding the depth, angulation and root development of impacted teeth. Spontaneous eruption was found in 93% of mandibular premolars after marsupialization. Eruption speed and rate of angulation change were greater in mandibular premolars associated with a cyst than those on the non-cyst side. The group with less matured roots tended to have greater eruption speed and angulation change. The eruption speed and angulation change declined rapidly during the first 6 months after marsupialization. Based on the results of this study, it might be possible to predict the eruption pattern of mandibular premolars associated with a cyst after marsupialization, which would be useful for treatment planning.

A cyst that develops in children's jaw occasionally disturbs the eruption of the succedaneous teeth. These teeth, however, usually have the potential of eruption. So, if the obstacles to eruption are eliminated, it is possible that the teeth erupt spontaneously to their normal position. In those cases, it usually requires the management of the cyst and the eruption guidance of the displaced teeth. Many surgical procedures have been described for the elimination of cysts. When the cyst is large and displaces the permanent teeth, marsupializaion is a surgical technique that may be preferred to enucleation in treatment of cysts. In marsupializaion, if the opening is maintained properly, it may be possible to manage the cyst and guide the displaced teeth into the normal position. In these cases, the cysts were managed with marsupialization in concomittent application of acrylic obturators, and as a result the displaced permanent teeth were guided into normal position. Even though the etiologic factors of the two cases are different, the treatment was the same. And both cases show that the potential for heal ing is remarkable with spontaneous relocation of displaced tooth, provided the opening is maintained during the eruption of the permanent tooth.

When many factors involved in the eruption of the teeth act as negative effects, they can cause eruption disturbance. Periodic observation, space acquirement, surgical exposure, orthodontic traction, orthodontic traction accompanied with surgical exposure, and surgical repositioning are considered as the treatment options of an impacted tooth, which is a form of eruption disturbance. In the first case, a male patient, age 9, visited Yonsei University Dental Hospital (YUDH) with a chief complaint of ectopic impaction of the upper left lateral incisor. We extracted the upper left primary lateral incisor and primary canine, and 5 months later, a window opening procedure was executed. Eight months later, the upper left lateral incisor partially erupted, and 18 months after the extraction, the axis of the tooth improved and the tooth erupted spontaneously. In the second case, a male patient, age 10, visited YUDH with a chief complaint of ectopic impaction of the upper right first premolar. We extracted the upper right first primary molar. Ten months later, the upper right first premolar erupted partially, and 19 months after the extraction, the upper right first premolar erupted spontaneously. We reported two cases in which improvement of eruption path and spontaneous eruption of an ectopic impacted tooth was achieved by extracting the deciduous tooth which interfered with the proper eruption of it.
