Idiopathic thrombocytopenic purpura is an uncommon illness but most common form of thrombocytopenia in pregnancy. Corticosteroids, splenectomy, immunosuppressive drugs, and immunoglobulin therapy have been recommended for management. The optimal method of delivery is controversial. We have experienced a case of idiopathic thrombocytopenic purpura diagnosed previously and managed with corticosteroid and vincristine, which was followed by pregnancy, vaginal delivery and postpartum splenectomy.

This experiment was undertaken to investigate the effect of splenectomy on the hematology and maintenance of pregnant rats. Those animals that had shown regular 4 or 5 day estrous cycles for more than two cycles were used. The day after mating ws designated Day 0 of pregnancy. Spleen was removed from Day 0(early), 6(middle) and 13(late) of pregnant rat, respectively. Blood was collected on Day 1, 7, 14 and 21 of the pregnancy. 1. The total serum protein was increased significantly (P<0.05) to the progress of pregnancy in control rat. All rats that splenectomized groups were decreased significantly(P<0.05) at Day 21 of pregnancy than control rats. 2. Albumin was not significantly changed. 3. Globulin was decreased significantly(P<0.05) between contol and middle treatment at Day 7, 14 of pregnancy. 4. Glucose was increased for the due to the pregress of pregnancy. It was no significance differences among the each groups. 5. Rate of abortion was increased in groups of early and middle of splenectomy compared with control. 6. Period of pregnancy was delayed in middle and late splenectomized gruops companed with control. 7. Litter size was lowed significantly (P<0.05) in early and middle splenectomized groups compared with control.

Two 11-year-old cats, female Korean shorthair cat and male Siamese cat, with abdominal distention were presented to the local animal hospitals. Radiographic and ultrasonographic examinations revealed moderate to severe splenomegaly in both cats. In Korean shorthair cat, multiple masses were also existed on the anal and facial skin. Surgically excised whole spleens of two cats were requested for histopathologic examination. Histopathologically, numerous neoplastic round cells with cytoplasmic fine granules were widely infiltrated in the splenic parenchyma. The cytoplasmic granules were metachromatic on toluidine blue staining. These splenic masses were diagnosed as splenic mast cell tumors. Among them, Korean shorthair cat was remained healthy for at least 1 year after splenectomy. Because of no visiting of owner, we were only able to know the information for Siamese cat until 10 months after the splenectomy. To our best knowledge, this is the first detail case reports for splenic mast cell tumors in cats in Korea.

We report our experience with a case of performing laparoscopy-assisted total gastrectomy along with pancreas-preserving splenectomy for treating early gastric cancer. laparoscopy-assisted total gastrectomy was planned for a 62-year-old male patient with a double early gastric cancer located in the upper and lower third of the stomach. Five trocars were placed and we used a harmonic scalpel to dissect the greater curvature. Enlarged splenic hilar lymph node was encountered and they were proved to be metastasis by frozen section biopsy. We then performed total gastrectomy with pancreas-preserving splenectomy for the purpose of completely dissecting the lymph nodes along the splenic artery and splenic hilum. We created a 4 cm sized longitudinal mini-laparotomy below the xiphoid process to remove the specimen, and anastomosis was done via the Roux-en-Y method. The patient was discharged on the 9th postoperative days after an uneventful recovery. Our experience shows that laparoscopy-assisted total gastrectomy with pancreas-preserving splenectomy is a relatively safe procedure for treating upper third early gastric cancer with metastatic splenic hilar lymph nodes.

Idiopathic thrombocytopenic purpura(ITP) is characterized by the development of a specific anti-platelet autoantibody immune response mediating the development of thrombocytopenia. Systemic lupus erythematosus(SLE) is an autoimmune disease characterized by the production of a wide variety of autoantibodies. We experienced SLE patient whose initial symptoms were related to idiopathic thrombocytopenic purpura(ITP). She has a thrombocytopenia after Splenectomy and Steroid therapy on ITP and SLE. After she took Eight constitution Acupuncture treatment, thrombocytopenia has improved. We think Acupuncture will be effective treatment at autoimmune disease.

A 13.5kg, 15-year-old male mongrel dog with anemia, anorexia, vomiting and abdominal distension was referred to the Veterinary Medical Center of the Tokyo University. Radiographic and ultrasonographic findings indicated primary splenic tumor. The tumor was located at body of the spleen, which was surgically removed by splenectomy. Histopathological examination of the mass revealed sarcoma type tumor derived from the mesenchymal origin. The patient was in good health after surgery but suddenly showed abdominal distension on the 3 months after. Radiographic findings indicated abdominal neoplasms. The 2nd operation was performed and removed the recurrent mast but suddenly died on the 4th day. Histopathological examination of the tumor was myxosarcoma.

Splenic abscess is an uncommon lesion and may be present either as a localized area of infection in the spleen or as a part of generalized sepsis. The diagnosis is difficult because of the rather nonspecific clinical picture. Hence, splenic abscess has a high mortality rate and is after diagnosed only at autopsy. Computerized tomography(CT) offers the clinician a reliable tool for the diagnosis of intra-abdominal abscess. A successful outcome is dependent on an early diagnosis and prompt treatment by splenectomy with antibiotic cover. We experienced a care of splenic abscess which was diagnosed by CT and treated by splenectomy with antibiotics. Postoperative course was relatively uneventful.

Splenic cysts are uncommon and classified as either primary(true) or secondary(pseudo-) depending on the presence or absence of a true epithelial lining. True cysts (epidermoid cyst) of the spleen are very rare. Three cases of splenic cysts in childhood were treated at the Yeungnam University Hospital in the last eleven years(1989-1999). Two of patients were girls. The ages at diagnosis were 7, 12 and 15 years. Abdominal ultrasonography and computerized tomography were utilized for the diagnosis. Radionuclide scanning was performed in one patient. Surgical resection(one partial splenectomy and two total splenectomies) was performed. The sizes of cysts were 4, 6.5 and gem in maximum demension.
