Effect on Oriental-Western Medicine Intergrative Care after Gynecology Surgery (부인과 수술 후 한양방 협진 치료의 효과 연구)

  • Purpose: The purpose of this study is to analyze the effect of oriental-western medicine integrative care after gynecology surgery by DSOM((Diagnosis System of Oriental Medicine) and improvement of major symptoms. Methods: 66 patients belonged to the experimental group and 47 patients belonged to the control group out of 113 patients taken gynecology surgery. We studied general characteristics and difference on DSOM results between the two groups. And we had patients answer questionnaires about major symptoms such as abdominal discomfort, headache, fatigue, urinary discomfort, constipation, lumbago, edema, tingling, digestive disorder, cough, pruritus, arthralgia, cold feeling by VAS (Visual Analogue Scale) in order to analyze improvement and difference between two groups. Results: Experimental group had significantly high score on deficiency of blood, blood stasis, dryness, spleen, kidney, phlegm and high frequency on deficiency of blood, dampness, kidney, phlegm pathogenesis in DSOM. It had significantly high intensity in symptoms of abdominal discomfort, headache, fatigue, urinary discomfort, constipation, lumbago at the first time, and headache at 1 month after western hospital discharge out of symptoms associated with surgery. And it had significantly high improvement in symptoms of abdominal discomfort, headache, urinary discomfort, constipation at oriental-western integrative hospital discharge, and headache, fatigue, urinary discomfort, constipation, lumbago at 1 month after western hospital discharge out of symptoms associated with surgery. Conclusion: We recognized that oriental-western medicine integrative care after gynecology surgery has significant effect.

Study of Insam-Buja-Tang (IBT) on MRL/MpJ-Faslpr lupus-prone mice (루푸스 동물 모델에서 인삼부자탕(人蔘附子湯)이 미치는 영향)

  • Systemic Lupus Erythematosus(SLE) is an autoimmune disease invading the skin, joint, kidney, intestinal membrane, neurosystem and other organs. SLE is an autoimmune disease characterized by immune dysregulation resulting in the production of antinuclear antibodies(ANA), generation of circulating immune complexes, and activation of the complement system. In Korean medicine, lupus can be classified as acute arthritis, reddish butterfly erythema, asthenic disease, edema and so on. The cause and procedure of the diseases are flourishing noxious heat, excessive fire due to deficiency of yin, blood stasis due to stagnation of qi, internal movement of the liver-wind, congenital deficiency, exhausted vital-qi, which are treated by clearing away heat and cooling the blood, nourshing yin and extinguishing fire, treating flatulence and activating blood circulation, nourishing the blood to expel wind, invigorating the liver and kidney, invigorating qi and replenishing the blood. To experimentally examine the influence of Insam-Buja-Tang (Ginseng & Aconiti Extract, IBT) on the outbreak and development of lupus, lupus induce MRL/MpJ-Faslpr lupus-prone mice model was used. As IBT was orally administrated to a lupus model mouse, various tests such as the weight, urine protein, renal function, Lymph cell test of the spleen, Cytokine expression, histopathological analysis of kideny were performed to see the influence on the kidney and whether it work effectively on the immune function. The main purpose of this study is to evaluate the effect of IBT on MRL/MpJ-Faslpr lupus-prone mice model. The effect of IBT on MRL/MpJ-Faslpr lupus-prone mice that can have autoimmune disease similar to SLE in human was evaluated after IBT per oral in the present study.

A Case of Negative Symptoms of Schizophrenia (전증(癲證) 환자의 치험 1례)

  • Objectives : Jeon-Jeung(癲證) is one of negative symptoms from schizophrenia in Western medecine, which causes flattening of affect, emotional bluntness, and avolition. Compared with positive symptoms of schizophrenia, there is no established treatments that have been proved to be effective for negative symptoms, and since negative symptoms are chronically processed, they finally lead to devastate the mental health. Since Jeon-Jeung(癲證) is usually in set in adolescent period and tends to become chronic through life time, it is important to start getting treatments in early stage by being distinguished from other diseases, such as anxiety disorder. A patient in this case was affective blunting, general weakness, and delusion when sixteen years old. However, he refused to get Western medicine treatment and wanted oriental medicine treatment. Methods : The patient in this case had been suffered from severe stress from his family since he was young and had kept having irregular and unhealthy eating habits. Therefore, he diagnosed stagnant qi transforming into fire(氣鬱化火), heart blood deficiency(心血虛), and spleen-stomach deficiency cold(脾胃虛寒) and since then he had received several treatments including herbal treatment, acupumcture treatment, supportive therapy, and family therapy. These treatments were successful and reduced the level of symptoms. After discharged from the hospital, he had continued receiving outpatient treatment with his family for 8 months and his progress had been still observed after the discharge. Results : The symptoms of patient had been almost reduced and eliminated after he received 29days of admission treatment and the patient got better and better and now lives a normal life 8 months outpatient treatment. Conclusions : This result suggests that our oriental medical treatments and family treatments was effective on schizophrenia.

Bibliographical Review on Oriental Medicine Herbal Treatment for Uterine Carcinoma (자궁암 치료 한약물에 대한 문헌적 고찰)

  • To make a bibliographical review on the most frequently used prescriptions and herbs in treating the uterine carcinoma. 43 texts in China and 6 texts in Korea published since 1980 were reviewed for this objective. 590 Oriental Herbal Medicine formulas composed of 457 herbs were investigated. The frequency and efficacy of each herb used were statistically analyzed. The most frequently used herbal medicine formulas were Danchisoyosan in hepatic Qi stasis, Sihosogan-tang mixed with Lungeumdaebo-tang in damage of conception and thoroughfare vessel, Danchisoyosangagam in blood heat, Hoangyunhaedok-tang in pattern of retained dampness-heat, Jibapjihang-tang in iver-kidney yin deficiency pattern, Naebohyunhapsamyoungbaekchulsan in spleen-kidney yang deficiency pattern. The herbal medicines most frequently used were; Angelicae gigantis radix(126 times), Phellodendri cortex(91 times), Borneolum(87 times), Glycyrrhizae radix and Paeoniae radix alba(82 times), Realgar and Alumen(80times), Poria(77 times), Astragali radix(75 times), Moschus and Myrrha(74 times), Coicis semen and Olibanum(73 times), Codonopsis pilosulae radix and Scutellaria bardata Don(70 times) etc. The effect of the most frequently used herbs were : heat-clearing medicine(12), blood-circulation and stasis-dispelling medicine(9), tonifying and replenishing medicine(5), ointments(5), heat-clearing and blood-cooling medicine(3) and etc. The most frequently used herbs may be applied in future animal studies and clinical trials. Also this data can be used as a reference in formulating new Oriental Herbal Medicine formulas for uterine carcinoma.

Study of Insamyangyung-tang in Hyungsang Medicine (인삼양영탕의 문헌고찰과 형상의학적 치료)

  • The following conclusions are drawn, through studying of referring literature, of contents of the prescription and of clinical applications in Hyungsang Medicine. Insamyangyung-tang(인삼양영탕) was appeared first in Taepunghyemin-whajegugbang(태평혜민화제국방) which was written by Jin and others at Song Dynasty. This prescription is applied to senility and long lasting weakness, little strength of spleen and lung(비폐기허), and insufficiency of blood. The Insamyangyung-tang is composed of medical stuffs which Rhizoma Cnidii is taken out from Sipjiendaebotang and Pericarpium Citri Nobilis, Fructus Schizandrae, Radix Polygalae are added to it. This prescription is used in China up to the present. In korea, according to Donguibogam, Rhizoma Cnidii and Poria are taken out from Sipjiendaebotang and Pericarpium Citri Nobilis, Fructus Schizandrae, Radix Polygalae are added to it, and Ramulus Cinnamomi is changed to Cortex Cinnamomi, and also Radix Ledebouriellae is newly added to it. This is reorganized to fit for Korean physical conditions and to be useful for deficiency of blood, and also to be efficacious for coming and going of chillness and fever and running with sweat due to weakness of Liver. In the point of view of Hyunsang medicine, Insamyangyung-tang is applied to shapes and symptoms as follows : This prescription becomes more efficacious to women than to men. At the age of fifties when liver grows weak it effects a cure. It takes effect to the shape of going easily into the deficiency of blood, that is, to the Hyul Kwa with an oval face. It effects a cure on the man who has wrinkles on the bridge of the nose, who has marks of being choked up on the central area between two brows, or who has distinctive size of eyes and nose. It is efficacious against symptoms which are to be exhausted, to be weak, to lose flesh, to be coming and going of chillness and fever and running with sweat.

  • Purpose : We have carried out this study to investigate association of DSOM scores and women infertility in oriental medicine for systematization diagnosis and treatment of women infertility. Methods : We analyzed the results of medical records for 151 patients who were diagnosed as women infertility. We investigated the possession of symptom index of women infertility, recognition association of DSOM. Results : 1. In comparison with 151 clinical trial's data. deficiency of Yin(陰虛), coldness(寒), spleen(脾) are associated positively and deficiency of Yang(陽虛), phlegm(痰) are associated negatively. 2. In pathogenesis(病機) indicator of average, cold inside the body is associated positively and diarrhea, kidney are associated negatively. 3. In pathogenesis five score-point of value, Heat(熱), kidney(腎) are associated negatively. 4. In pathogenesis(病機) indicator. constipation(便秘) is associated positively and hardness of lower abdomen, tingly weakness of lower back and knee question are associated negatively. 5. In 140 questions, nine questions are associated positively and ten question are associated negatively.

Preliminary Study to Develop the Instrument on Pattern Identifications Knee Osteoarthritis (퇴행성무릎관절염 변증도구 개발에 관한 기초연구)

  • Objectives The aim of this study was to develop a standard tool of pattern identification for Knee Osteoarthritis, which will be applied to clinical research. Methods The advisor committee for this study was organized by 11 panel of experts (Korean Rehabilitation Medicine professors, Acupuncture and Moxibustion professors belonging to Korean Medicine colleges, Principal Researcher of Korea Institute of Oriental Medicine, Doctor of Korean medicine). The pattern identifications and symptoms for this tool were extracted from published Korean and Chinese literature. Through the discussion among internal experts and consultation from advisors, the Instrument on Pattern Identifications for Knee Osteoarthritis was developed. Results 1) Five pattern identifications (The Wind, Chill, and Moisture, The Moist-Heat, Blood Stasis, Yang Deficiency of Spleen and Kidney, Yin Deficiency of Liver and Kidney) were set for the tool. 2) The mean weights which represent the importance of each symptom and scored on a hundred-point scale was obtained. 3) The Instrument on Pattern Identifications for Knee Osteoarthritis was designed in the self-reporting format composed of 46 questions. Conclusions The Instrument on Pattern Identifications for Knee Osteoarthritis was created through this study. Though this study is not proved about validity, reliability, the instrument of pattern identification for Knee Osteoarthritis is meaningful and expected to be applied to the subsequent.

The literatural study on the oriental cognizance and acupuncture-moxibustion therapy of osteoporosis (골다공증(骨多孔症)의 한의학적(韓醫學的) 인식(認識)과 침구치료(鍼灸治療)의 문헌연구(文獻硏究))

  • Objectives and Methods: The treatments of osteoporosis in occidental medicine are need complementations for reason of low-continuation and adverse-drug-reaction. So we would have literature centered consideration both oriental medical cognition and acupuncture-moxibustion therapy in osteoporosis. Results: 1. Deficiency of bone marrow caused by heat in the kidney and deficiency of kidney Gi, is causative of osteoporosis in pathology in oriental medicine. 2. Bone flaccidity and bone impediment are similar with osteoporosis. Withered bone comes under pathogenesis of bone flaccidity. And heavy debilitating disease of the bone is exacerbation of bone impediment. 3. In acupuncture-moxibustion therapy of osteoporosis, Bladder Meridian of Foot Taeyang, Kidney Meridian of Foot Soeum, Gallbladder Meridian of Foot Soyang, Lung Meridian of Hand Taeeum, Large Intestine Meridian of Hand Yangmyeong, Spleen Meridian of Foot Taeeum, Conception Channel, Stomach Meridian of Foot Yangmyeong, and Heart Meridian of Hand Soeum are used much in the order named. 4. In acupuncture-moxibustion therapy of osteoporosis, Hyeonjong(GB39, 7times), Daejeo(BL11, 3times), Sanggwan(GB3, 2times), Sinsu(BL23, 2times), Gyeoksu(BL17, 2times), Honmun(BL47, 2times), Buryu(KI7, 2times), Taebaek(SP3, 2times), Sanggu(SP5, 2times), Sangnyeom(LI9, 2times) are used much in the order named. Conclusions: In treatment of osteoporosis, we could make full use of acupuncture-moxibustion therapy. And we need active and persistent study about osteoporosis.

A Clinical Study of the Pediatric Patients Who Visited Oriental Medical Hospital for Growth Treatment (성장 치료를 목적으로 한방병원에 내원한 환아에 대한 임상적 고찰)

  • Objectives The purpose of this study is to analyze the characteristic of pediatric patients who visited oriental medical hospital for growth treatment. Methods The study was conducted with 130 pediatric patients who visited ${\bigcirc}{\bigcirc}$ Korean medicine hospital from January 2016 to August 2017. The patient's data was classified by sex, age, pattern identification and diseases. The classified data was analyzed to find the correlations and characteristics among variables. Results There were 50.8% of boys and 49.2% of girls. 6.2% of the total patients were in 'early childhood', 50.8% were in 'late childhood' and 43.1% were in 'puberty' stage. In terms of pattern identification, 44.6% of the total patients were 'Spleen-lung Qi Deficiency', 53.8% were 'Lung-kidney Yin Deficiency' and 1.5% were 'Weakness of Heart and gallbladder'. 44.3% of the patients had various diseases including rhinitis (77.6% of 120.7%). Height percentile of the children with using the Growth chart of 2017 was lower than using the Growth chart of 2007. Predicted height based on the child's bone-age were higher than the predicted height based on the parental height, although both of the precited factors were correlated to the current child's height. Conclusions This study showed the characteristic of the pediatric patients who visited oriental medical hospital for growth treatment. It helps to determine prognosis and treatment, and to explain treatment measures.

Re-evaluation of Obesity Syndrome Differentiation Questionnaire Based on Real-world Survey Data Using Data Mining (데이터 마이닝을 이용한 한의비만변증 설문지 재평가: 실제 임상에서 수집한 설문응답 기반으로)

  • Objectives: The purpose of this study is to re-evaluate the importance of questions of obesity syndrome differentiation (OSD) questionnaire based on real-world survey and to explore the possibility of simplifying OSD types. Methods: The OSD frequency was identified, and variance threshold feature selection was performed to filter the questions. Filtered questions were clustered by K-means clustering and hierarchical clustering. After principal component analysis (PCA), the distribution patterns of the subjects were identified and the differences in the syndrome distribution were compared. Results: The frequency of OSD in spleen deficiency, phlegm (PH), and blood stasis (BS) was lower than in food retention (FR), liver qi stagnation (LS), and yang deficiency. We excluded 13 questions with low variance, 7 of which were related to BS. Filtered questions were clustered into 3 groups by K-means clustering; Cluster 1 (17 questions) mainly related to PH, BS syndromes; Cluster 2 (11 questions) related to swelling, and indigestion; Cluster 3 (11 questions) related to overeating or emotional symptoms. After PCA, significant different patterns of subjects were observed in the FR, LS, and other obesity syndromes. The questions that mainly affect the FR distribution were digestive symptoms. And emotional symptoms mainly affect the distribution of LS subjects. And other obesity syndrome was partially affected by both digestive and emotional symptoms, and also affected by symptoms related to poor circulation. Conclusions: In-depth data mining analysis identified relatively low importance questions and the potential to simplify OSD types.
