교통사고 후 응급실에 내원한 경항통 환자의 경추 만곡 연구 (The Cervical Spine Curvature of Posterior Neck Pain Patients Who Visited Emergency Room After Whiplash Injury by Traffic Accident)

  • Objectives : The purpose of this study is to investigate the cervical spine curvature after whiplash injury by traffic accident. Methods : The cervical lateral radiographs of 102 outpatients who visited emergency room in Kyung-Hee university hospital at Gangdong after whiplash injury by road traffic accident were reviewed to measure the cervical spine angle using C1-7, C2-7 Cobb method, Sagittal tangent method, Jochumsen method and the Ishihara index by two oriental medical doctors. For statistics, we used SPSS version 17.0 for windows. Results : Means of cervical angle are $37.63{\pm}11.34^{\circ}$, $12.92{\pm}9.13^{\circ}$, $16.19{\pm}10.62^{\circ}$, $1.78{\pm}3.37$ and $8.51{\pm}9.78$ by C1-C7 Cobb Method, C2-C7 Cobb Method, Sagittal tangent method, Jochumsen method and Ishihara index, respectively. Hypolordosis is most numeral in patients by C1-C7 Cobb Method(n=40; 39.22%), Sagittal tangent method(n=68; 66.67%). And Straight is the most by Jochumsen method(n=54; 52.94%), but Normal is the most by Ishihara index(n=53; 51.96%). And Female has smaller curvature in cervical spine than male significantly by C2-C7 Cobb method and Sagittal tangent method(P<0.05). Conclusions : Whiplash injury tends to make hypolordosis or straight. And female has more vulnerable curvature than male in whiplash injury.

경증 두부 외상을 가진 환자의 경추 손상을 예측할 수 있는 관련 인자 (Predictive Factors for Cervical Spine Injury in Patients with Minor Head Injury)

  • Purpose: This study aimed to determine new criteria for detecting independent factors with high sensitivity in cases of cervical spine injury. We compared the sensitivity, the specificity, and the false negative predictive value (NPV) of plain radiographs with those of computed tomography for cervical spine injury in patients with minor head injury. Methods: We retrospectively reviewed the cases of 357 patients who underwent both cervical plain radiographs and computer tomography from January 2006, to September 2008. Patients were divided into two groups: the cervical spine injury group and the no cervical spine injury group. New criteria were organized based on variables that had significant differences in the logistic regression test. Results: Among the 357 patients, 78 patients had cervical spine injuries. The average age was $43.9{\pm}15.2$ yrs old, and the male-to-female ratio was 1.90. The most common mechanism of injury was motor vehicle accidents. There was a significant difference in loss of consciousness, Glasgow Coma Scale (GCS)=14, neurologic deficit, posterior neck tenderness, and abnormality of the cervical plain radiographs between the two groups on the logistic regression test. New criteria included the above five variables. If a patient has at least variable, the area under the ROC curve of the new criteria was 0.850, and the sensitivity and the false NPV were 87.2% and 5.2%, respectively. Conclusion: New criteria included loss of consciousness, GCS=14, neurologic deficit, posterior neck tenderness, and abnormality of the cervical plain radiographs. If the patient had at least 1 variable, he or she could have a of cervical spine injury with a sensitivity of 87.2% and a false NPV of 5.2%.

두 차례의 출산 직후 골밀도 측정을 통한 임신 및 출산이 골밀도에 미치는 영향 고찰 (The effect of pregnancy and parity on bone marrow density using Dual Energy X-ray Absorptiometry (DXA) after childbirth)

  • Purpose : This study was conducted to investigate the effect of pregnancy and parity on bone marrow density using Dual Energy X-ray Absorptiometry (DXA) after parturition Methods : The observation cases who was over 20 and under 35 years old just after childbirth were admitted twice to woosuk university hospital from Aug 2000 to July 2005. During the first admission period, we measured the bone marrow density(BMD) using DXA in 13th day, and when the patient came to the hospital just after childbirth again, we followed up the BMD in 13th day. The evaluation index of this report was comparison of the T-score which was about the lumbar spine(L1-L4) BMD and femur neck BMD. Results : The continuous parturition was increased the lumbar spine BMD(P<<0.05), and decreased the femur neck BMD but it had no meaning. In the cases of the parturition interval under 24 months regarded as siblings born within a year of each other, the analysis results of BMD showed increase in lumbar spine BMD. In the cases of the parturition interval over 24 months, there was increased in lumbar BMD, and decreased in femur neck BMD. To the analysis of the weight variation, the increased BMI group has a significant increase in lumbar spine BMD, and the decreased BMI group also increase in lumbar spine BMD but there was no meaning about that. Conclusion : The continuous parturition was increased the lumbar spine BMD.

밀도검층 이격보정에 있어서의 기법 개선에 관한 연구 (Improving the Standoff Compensation in a Density Log)

  • 강원대학교 지구물리검층 보정용 시추공에서 2007, 2008년 및 2009년에 수행된 연차별 이격보정 자료를 이용하여 기존 이격보정 기법의 효과를 비교 검토하고 보다 효과적인 이격기법 마련을 위한 연구를 실시하였다. 먼저 이격보정의 두축을 이루고 있는 일반이격보정과 밀도변환이격보정 기법을 적용하고 그 보정 효과를 비교한 결과, 적용의 편의성이 돋보이는 밀도변환이격보정에서 일반이격보정보다 보정 오차가 2배 이상 높게 나타남을 확인하였다. 방사능감쇠 이론곡선을 적용하는 방법에서는 밀도변환이격보정의 경우보다도 더 큰 오차가 나타나 활용의 이점을 찾을 수 없었다. 이러한 결과 위에서 일반이격보정과 밀도변환이격보정 기법을 효과적으로 결합시켜 편의성과 정확성을 겸비한 역변환이격보정 방법을 제안하였다.

Utility of Spinal Injury Diagnosis Using C-Spine Lateral X-Ray and Chest, Abdomen and Pelvis Computed Tomography in Major Trauma Patients with Impaired Consciousness

  • Purpose: The regional emergency medical centers manage the patients with major blunt trauma according to the process appropriate to each hospital rather than standardized protocol of the major trauma centers. The primary purpose of this study is to evaluate the effectiveness and influence on prognosis of additional cervical-thoracic-lumbar-spine computed tomography (CTL-spine CT) scan in diagnosis of spinal injury from the victim of major blunt trauma with impaired consciousness. Methods: The study included patients visited the urban emergency medical center with major blunt trauma who were over 18 years of age from January 2013 to December 2016. Data were collected from retrospective review of medical records. Sensitivity, specificity, positive predictive value, and negative predictive value were measured for evaluation of the performance of diagnostic methods. Results: One hundred patients with Glasgow coma scale ${\leq}13$ underwent additional CTL-spine CT scan. Mechanism of injury was in the following order: driver, pedestrian traffic accident, fall and passenger accident. Thirty-one patients were diagnosed of spinal injury, six of them underwent surgical management. The sensitivity of chest, abdomen and pelvis CT (CAP CT) was 72%, specificity 97%, false positive rate 3%, false negative rate 28% and diagnostic accuracy 87%. Eleven patients were not diagnosed of spinal injury with CAP CT and C-spine lateral view, but all of them were diagnosed of stable fractures. Conclusions: C-spine CT scan be actively considered in the initial examination process. When CAP CT scan is performed in major blunt trauma patients with impaired consciousness, CTL-spine CT scan or simple spinal radiography has no significant effect on the prognosis of the patient and can be performed if necessary.

자장의 세기에 따른 유방 확장기 주위의 인공물 평가 (Evaluation of artifacts around the breast expander according to magnetic field strength)

  • 유방조직확장기의 Magnetic Valve는 MRI 검사 시 영상 인공물이 발생하여 MRI 검사가 제한적이다. MRI 검사가 필요한 유방조직확장기 삽입 환자에 대해 영상 인공물이 진단영역에 미치는 영향을 평가한다. 자체 제작한 팬텀과 실제 임상 조건을 이용하여 영상 인공물 측정을 실시하였다. 영상 인공물은 1.5 Tesla와 3.0 Tesla 환경에 따라 상이하게 측정되었으며, C-spine, L-spine 검사 시 영상 인공물의 영향이 T-spine에 비해 상대적으로 적었다. 유방암 전이로 인한 MRI 검사가 꼭 필요한 경우 주로 1.5 Tesla에서 Head & Neck 검사와 L-spine 아래 부위를 검사 할 수 있으나 일부 시퀀스에서는 영상 인공물로 인한 왜곡이 발생할 수 있다. 유방조직확장기 삽입 환자의 MRI 검사는 안전성 측면에서는 3.0T는 피하고 1.5T에서 조건부로 가능하다.

척추 안정화 운동과 등뼈가동술이 만성 허리통증환자의 척추 유연성에 미치는 영향 (Effects of Trunk Stability Exercises and Thoracic Manipulation on Spine Flexibility in Chronic Low Back Pain Patients)

  • PURPOSE: This study aimed to investigate the benefits of thoracic manipulation and the trunk stability exercises on spine trunk flexibility and the pain index of chronic low back pain patients. METHODS: The study included 44 patients suffering from chronic low back pain. The participants were divided into two groups: the first group was assigned for trunk stability exercises (TSE), and the second group was randomly assigned TSE and thoracic manipulation at the same time (TSE+TM). Both groups carried out each assigned treatment thrice a week for 8 weeks. The study outcome was based on assessment of spine trunk flexibility and the pain index. Spine trunk flexibility was measured by spine flexion and extension through a range of motion and thoracic cage circumference. The pain index was measured using a visual analog scale (VAS). RESULTS: Spine flexion and extension range of motion showed a significant difference within each group and between the groups before and after the treatment. The measurement of the thoracic cage circumference also showed a notable difference within each group and between the groups before and after the treatment. There was no change in the pain index. CONCLUSION: These results indicate that thoracic manipulation with the trunk stability exercises is an efficient treatment for improving the spine trunk flexibility and soothing pain for chronic low back pain patients.

Whole Spine X-ray 영상에서 척추 영역 분할을 위한 HR-Net 성능 최적화에 관한 연구 (Research on the Performance Optimization of HR-Net for Spinal Region Segmentation in Whole Spine X-ray Images)

  • This study enhances AI algorithms for extracting spinal regions from Whole Spine X-rays, aiming for higher accuracy while minimizing learning and detection times. Whole Spine X-rays, critical for diagnosing conditions such as scoliosis and kyphosis, necessitate precise differentiation of spinal contours. The conventional manual methodology encounters challenge due to the overlap of anatomical structures, prompting the integration of AI to overcome these limitations and enhance diagnostic precision. In this study, 1204 AP and 500 LAT Whole Spine X-ray images were meticulously labeled, spanning the third cervical to the fifth lumbar vertebrae. We based our efforts on the HR-Net algorithm, which exhibited the highest accuracy, and proceeded to simplify its network architecture and enhance the block structure for optimization. The optimized HR-Net algorithm demonstrates an improvement, increasing accuracy by 2.98% for the AP dataset and 1.59% for the LAT dataset compared to its original formulation. Additionally, the modification resulted in a substantial reduction in learning time by 70.06% for AP images and 68.43% for LAT images, along with a decrease in detection time by 47.18% for AP and 43.07% for LAT images. The time taken per image for detection was also reduced by 47.09% for AP and 43.07% for LAT images. We suggest that the application of the proposed HR-Net in this study can lead to more accurate and efficient extraction of spinal regions in Whole Spine X-ray images. This can become a crucial tool for medical professionals in the diagnosis and treatment of spinal-related conditions, and it will serve as a foundation for future research aimed at further improving the accuracy and speed of spinal region segmentation.

Surgical Outcomes According to Dekyphosis in Patients with Ossification of the Posterior Longitudinal Ligament in the Thoracic Spine

  • Objective : Ossification of posterior longitudinal ligament (OPLL) in the thoracic spine may cause chronic compressive myelopathy that is usually progressive, and unfavorable by conservative treatment. Although surgical intervention is often needed, the standard surgical method has not been established. Recently, it has been reported that posterior decompression with dekyphosis is effective surgical technique for favorable clinical outcome. The purpose of this study was to evaluate the surgical outcomes in patients with thoracic OPLL according to dekyphosis procedure and to identify predictive factors for the surgical results. Methods : A total of 25 patients with thoracic OPLL who underwent surgery for myelopathy from May 2004 to March 2017, were retrospectively reviewed. Patients with cervical myelopathy were excluded. We assessed the clinical outcomes according to various surgical approaches. The modified Japanese orthopedic association (JOA) scores for the thoracic spine (total, 11 points) and JOA recovery rates were used for investigating surgical outcomes. Results : Of the 25 patients, 10 patients were male and the others were female. The mean JOA score was 6.7±2.3 points preoperatively and 8.8±1.8 points postoperatively, yielding a mean recovery rate of 53.8±31.0%. The mean patients' age at surgery was 52.4 years and mean follow-up period was 40.2 months. According to surgical approaches, seven patients underwent anterior approaches, 13 patients underwent posterior approaches, five patients underwent combined approaches. There was no significant difference of the surgical outcomes related with different surgical approaches. Age (≥55 years) and high signal intensity on preoperative magnetic resonance (MR) image in the thoracic spine were significant predictors of the lower recovery rate after surgery (p<0.05). Posterior decompression with dekyphosis procedure was related to the excellent surgical outcomes (p=0.047). Dekyphosis did not affect the complication rates. Conclusion : In this study, our result elucidated that old age (≥55 years) and presence of intramedullary high signal intensity on preoperative MR images were risk factors related to poor surgical outcomes. In the meanwhile, posterior decompression with dekyphosis affected favorable clinical outcome. Posterior approach with dekyphosis procedure can be a recommendable surgical option for favorable results.

  • 요추부 척추에 기구를 시술한 유한요소 복합체 모델을 이용하여 제 4-5 요추간판에 대한 해석을 수행한 결과, 근육의 작용을 케이블과 케이블 가이드로써 단순화 모사한 follower 하중에서 수직하중인 경우보다 높은 압축력을 나타내어 더욱 안정함을 보였고 불안정성의 원인이 되는 전단력과 굽힘모멘트의 발생은 미미하였다. 장분절 고정에 의한 인접 분절에 전단력이 증가됨을 실험적으로 보였는데, 이는 장분절 고정이 퇴행성 변화 촉진에 기여할 것으로 추정된다.
