Paraplegia due to Acute Aortic Coarctation and Occlusion

  • Coarctation and occlusion of the aorta is a rare condition that typically presents with hypertension or cardiac failure. However, neuropathy or myelopathy may be the presenting features of the condition when an intraspinal subarachnoid hemorrhage has compressed the spinal cord causing ischemia. We report two cases of middle-aged males who developed acute non-traumatic paraplegia. Undiagnosed congenital abnormalities, such as aortic coarctation and occlusion, should be considered for patients presenting with nontraumatic paraplegia in the absence of other identifiable causes. Our cases suggest that spinal cord ischemia resulting from acute spinal subarachnoid hemorrhage and can cause paraplegia, and that clinicians must carefully examine patients presenting with nontraumatic paraplegia because misdiagnosis can delay initiation of the appropriate treatment.

The Effect of Distal Aortic Pressure on Spinal Cord Perfusion in Rats

  • Background: Aortic cross clamping is associated with spinal cord ischemia. This study used a rat spinal cord ischemia model to investigate the effect of distal aortic pressure on spinal cord perfusion. Materials and Methods: Male Sprague-Dawley rats (n=12) were divided into three groups. In group A (n=4), the aorta was not occluded. In groups B (n=4) and C (n=4), the aorta was occluded. In group B the distal aortic pressures dropped to around 20 mmHg. In group C, the distal aortic pressure was decreased to near zero. The carotid artery and tail artery were cannulated to monitor the proximal aortic pressure and the distal aortic pressure. Fluorescent microspheres were used to measure the regional blood flow in the spinal cord. Results: After aortic occlusion, blood flow to the cervical spinal cord showed no significant difference among the three groups. In groups B and C, the thoracic and lumbar spinal cord and renal blood flow decreased. No microspheres were detected in the thoracic and lumbar spinal cord of group C. Conclusion: The spinal cord blood flow is dependent on the distal aortic pressure after thoracic aortic occlusion.

  • Resuscitative endovascular balloon occlusion of the aorta (REBOA) is widely performed as an adjunct to resuscitation or bridge to definitive control of non-compressible torso hemorrhage in patients with hemorrhagic shock. It is a crucial adjunct for the maintenance of cerebral and coronary perfusion during resuscitation. However, in polytrauma patients with concomitant neurotrauma, such as traumatic brain injury (TBI) or spinal cord injury, the physiological effects of REBOA are unclear. In this report on REBOA performed in a clinical setting for polytrauma patients with spinal cord injury or TBI, the physiological effects of REBOA in neurotrauma are reviewed.

In Vivo Neuroprotective Effect of Histidine-Tryptophan-Ketoglutarate Solution in an Ischemia/Reperfusion Spinal Cord Injury Animal Model

  • Background: Paraplegia is a devastating complication following operations on the thoracoabdominal aorta. We investigated whether histidine-tryptophan-ketoglutarate (HTK) solution could reduce the extent of ischemia/reperfusion (IR) spinal cord injuries in a rat model using a direct delivery method. Methods: Twenty-four Sprague-Dawley male rats were randomly divided into four groups. The sham group (n=6) underwent a sham operation, the IR group (n=6) underwent only an aortic occlusion, the saline infusion group (saline group, n=6) underwent an aortic occlusion and direct infusion of cold saline into the occluded aortic segment, and the HTK infusion group (HTK group, n=6) underwent an aortic occlusion and direct infusion of cold HTK solution into the occluded aortic segment. An IR spinal cord injury was induced by transabdominal clamping of the aorta distally to the left renal artery and proximally to the aortic bifurcation for 60 minutes. A neurological evaluation of locomotor function was performed using the modified Tarlov score after 48 hours of reperfusion. The spinal cord was harvested for histopathological and immunohistochemical examinations. Results: The spinal cord IR model using direct drug delivery in rats was highly reproducible. The Tarlov score was 4.0 in the sham group, $1.17{\pm}0.75$ in the IR group, $1.33{\pm}1.03$ in the saline group, and $2.67{\pm}0.81$ in the HTK group (p=0.04). The histopathological analysis of the HTK group showed reduced neuronal cell death. Conclusion: Direct infusion of cold HTK solution into the occluded aortic segment may reduce the extent of spinal cord injuries in an IR model in rats.

A rare case of sacral epidural arteriovenous fistula with concomitant occult multiple lumbar epidural arteriovenous fistulas

  • We describe a rare case of sacral epidural arteriovenous fistulas (edAVFs) with atypical clinical course of treatment. A 78-year-old man with a history of spinal surgery presented progressive gait disturbance and urinary incontinence. Spinal angiography demonstrated a sacral spinal AVF fed by bilateral lateral sacral arteries, draining to the venous pouch with subdural drainage. The first treatment by direct interruption of a subdural drainer was incompletely finished. Postoperative reassessment by 3D imaging analysis led to the diagnosis of sacral edAVF and 3D understanding of its angioarchitecture. The second treatment by transarterial embolization (TAE) resulted in complete occlusion of a sacral edAVF. However, spinal venous congestion didn't improve, because the recruitment of occult edAVFs at the multiple lumbar levels and complex-shaped sacral ventral epidural venous plexus (VEP) were involved in the remnant of prior subdural drainage. The third treatment was performed by TAE for three occult edAVFs and the VEP compartment connecting between a patent edAVF and subdural drainage, which resulted in complete disappearance of spinal cord edema. Endovascular embolization of VEP compartment connecting to subdural drainage in addition to fistulous occlusion may be one of the treatment options for several edAVFs at the multiple spinal levels.

척추운동분절 FE모델에서 충격시간에 따른 마미 단면적의 변화 (Cross sectional area change of the dural-sac according to impact duration in a spinal motion segment FE model)

  • In this study the occlusion of dural-sac, the outer membrane of spinal cord in the lumbar region, was quantitatively analyzed using one motion segment finite element model. Occlusion was quantified by calculating cross sectional area change of dural-sac far different compressive impact duration(loading rate) due to bony fragment at the posterior wall of the cortical shell in vertebral body. Dural-sac was occluded most highly in the range of 8∼12 msec impact duration by the bony fragment intruding into the spinal canal. t=400 msec case 4% cross sectional area change was calculated, which is the same as the cross sectional area change under 6 kN of static compressive loading.

허혈성 척수 손상의 동물실험모델에서 Trimetazidine의 척수 보호효과 (Protective Effects of Trimetazidine in a Rabbit Model of Transient Spinal Cord Ischemia)

  • 배경: 흉부 및 흉복부 대동맥 수술시에 발생하는 척수의 허혈성 손상에 의한 신경학적 합병증은 발병 전의 예측이 어려울 뿐만 아니라 중증의 장애를 남기게 된다. 본 연구에서는 허혈성 심근 질환 치료에 쓰이고 있는 Trimetazidine(이하 TMZ)의 척수의 허혈성 손상에 대한 보호 효과를 동물실험모델에서 실험하였다. 대상 및 방법: 다 자란 New-Zealand White Rabbits 33마리를 대조군(Group 1, N-17)과 실험군(Group 2, N=16)으로 나누어 실험하였다. 수술은 대조군에서는 전신마취 후 정중개복하여 좌측 신동맥 기시 직하부위에서 복부대동맥을 혈관 점자로 30분간 폐쇄하여 척수 허혈을 유발하였으며, 실험군에서는 TMZ 3mg/kg을 대동맥 겸자전 투여하였다. 수술 2시간 후, 24시간 후, 48시간 후에 Modified Talrov scale에 의한 운동능력을 평가하였으며 술 후 48시간에 요천추부 척수를 적출하여 조직검사를 시행하였다. 결과: 대조군과 실험군에서 각각 7 마리(17마리 중 10마리 사망)와 11마리(16마리 중 5마리 사망)가 실험 종료가지 생존하여 척수 조직을 채취하였다. Modified Talrov scale은 대조군과 실험군에서 각각 수술 2시간 후 1.13 $\pm$ 1.25와 3.20 $\pm$ 0.77, 24시간 후 1.45 $\pm$ 1.57와 3.50 $\pm$ 0.76, 48시간 후 1.86 $\pm$ 1.86와 3.91 $\pm$ 0.30이었다(p$\leq$0.05). 척수의 조직학적 검사에서는 신경학적 결손이 큰 대조군(Group 1)의 척수조직에서 허혈성 손상이 더 심하게 일어난 것이 관찰되었다. 결론: TMZ은 동물실험에서 척수의 일과성 허혈성 손상에 대하여 통계적으로 유의한 척수 보호효과를 나타내었다.

Delayed Brain Infarction due to Bilateral Vertebral Artery Occlusion Which Occurred 5 Days after Cervical Trauma

  • Vertebral artery (VA) injuries usually accompany cervical trauma. Although these injuries are commonly asymptomatic, some result in vertebrobasilar infarction. The symptoms of VA occlusion have been reported to usually manifest within 24 hours after trauma. The symptoms of bilateral VA occlusions seem to be more severe and seem to occur with shorter latencies than those of unilateral occlusions. A 48-year-old man had a C3-4 fracture-dislocation with spinal cord compression that resulted from a traffic accident. After surgery, his initial quadriparesis gradually improved. However, he complained of sudden headache and dizziness on the 5th postoperative day. His motor weakness was abruptly aggravated. Radiologic evaluation revealed an infarction in the occipital lobe and cerebellum. Cerebral angiography revealed complete bilateral VA occlusion. We administered anticoagulation therapy. After 6 months, his weakness had only partially improved. This case demonstrates that delayed infarction due to bilateral VA occlusion can occur at latencies as long as 5 days. Thus, we recommend that patients with cervical traumas that may be accompanied by bilateral VA occlusion should be closely observed for longer than 5 days.

수직 충격률에 따른 척추 경막 단면적 변화 해석 (Analysis of Dural-sac Cross Sectional Area Changes According to Vertical Impact rate)

  • 한 개 척추 운동 분절에 대한 유한요소 모델을 이용하여 수직 충격력이 가해질 경우 발생되는 척추 경막의 교합을 해석 하였다. 교합은 수직 충격력의 지속 시간 변화에 따른 골편의 이동으로 인한 척추 경막의 단면적 변화를 계산하여 계량화 하였다. 계산 결과 8∼12ms 의 충격 지속 시간에서 골편의 이동으로 인한 가장 큰 척추 경막의 단면적 변화가 나타났으며 $\Delta$t = 400 ms의 경우에서는 4%만의 척추 경막의 단면적 변화가 나타났다, 이와 같은 해석 결과는 6kN의 수직 정 하중이 가해질 경우에 발생되는 량이 유사한 값으로 이 정도의 충격 지속 시간은 준 정 하중 상태와 유사함을 보이고 있었다.

대뇌 피질 경색으로 인한 하지 단마비 환자 한방치험 2례 (2 Cases of Lower Limb Monoplegia due to Brain Cortical Infarction)

  • Monoplegia is the paralysis of either the upper or lower limb. Monoplegia is commonly caused by an injury to the cerebral cortex; it is rarely caused by an injury to the internal capsule, brain stem, or spinal cord. Most cerebral cortex is derived from the occlusion of a brain cortex blood vessel due to thrombus or embolus. According to motor homunculus, lower limb monoplegia occurs from limited damage to the most upper part of the primary motor area(Brodmann's area 4, located in precentral gyrus). Clinically, lower limb monoplegia due to brain cortical infarction is commonly misunderstood as monoplegia due to spinal injury because the lesion is situated at the most upper part of precentral gyrus. We had many difficulties in finding lesion on brain CT, but we diagnosed two patients correctly by using an MRI, who have lower limb monoplegia due to brain cortical infarction oriental treatment.
