Alveolar Fricative Sound Errors by the Type of Morpheme in the Spontaneous Speech of 3- and 4-Year-Old Children (자발화에 나타난 형태소 유형에 따른 3-4세 아동의 치경마찰음 오류)

  • Korean alveolar fricatives are late-developing speech sounds. Most previous research on phonemes used individual words or pseudo words to produce sounds, but word-level phonological analysis does not always reflect a child's practical articulation ability. Also, there has been limited research on articulation development looking at speech production by grammatical morphemes despite its importance in Korean language. Therefore, this research examines the articulation development and phonological patterns of the /s/ phoneme in terms of morphological types produced in children's spontaneous conversational speech. The subjects were twenty-two typically developing 3- and 4-year-old Koreans. All children showed normal levels in three screening tests: hearing, vocabulary, and articulation. Spontaneous conversational samples were recorded at the children's homes. The results are as follows. The error rates decreased with increasing age in all morphological contexts. Also, error percentages within an age group were significantly lower in lexical morphemes than in grammatical morphemes. The stopping of fricative sounds was the main error pattern in all morphological contexts and reduced as age increased. This research shows that articulation performance can differ significantly by morphological contexts. The present study provides data that can be used to identify the difficult context for articulatory evaluation and therapy of alveolar fricative sounds.

Review of Clinical Researches about Korean Medicine Treatment on Language Disorder of Cerebral Palsy (뇌성마비 언어장애에 대한 한의 치료 연구 동향)

  • Objectives: The purpose of this study was to obtain some understanding about Korean medicine treatment on language disorder in cerebral palsy for future practice and the research, from the clinical studies. Methods: The literature was searched using the database-China Academic Journals (CAJ). Clinical studies of Korean medicine treatment for language disorder in cerebral palsy, including Randomized controlled trial (RCT), case control study, case series, case report were analyzed. Results: Fifteen Clinical studies met our inclusion criteria; One case study and six case series, one non-randomized controlled trial and seven RCTs. Acupuncture treatment, especially Head acupuncture, was the major treatment for language disorder of cerebral palsy in clinical studies, as it was used in fourteen studies. Acupoint massage, tuina, and acupoint injection were employed as treatment methods in the studies. Acupuncture treatment was used for language disorder of cerebral palsy combined with language therapy and other rehabilitation treatment in many studies. The effectiveness in the treatment groups, regardless of treatment methods, was higher than that of control group in all RCT studies. Conclusions: The results of this study could be used in the practice and the future study about language disorder of cerebral palsy.

Improving Cognitive Abilities for People with Alzheimer's Disease: Application and Effect of Reality Orientation Therapy (ROT) (알츠하이머병 치매 환자의 인지재활: 현실감각훈련(ROT)의 적용과 효과)

  • Healthcare providers in Korea are using conservative pharmacological treatment for Alzheimer's disease (AD) to delay the progress of the disease or to mitigate its behavioral and neurological symptoms. However, there is a growing need for interventions using practical non-pharmacologic treatment, as the effects of pharmacological treatments has faced limitations. This research provided a cognitive rehabilitation program to 3 AD patients and used a multiple baseline design across subjects to examine the effects. Performing reality orientation therapy (ROT) for 1 cycle (4 weeks) resulted in a slight increase in accuracy and responsiveness on an orientation task, mainly with patients with mild cases of AD. Also, in the sub-domain of the Korean-Mini Mental Status Examination performed to examine changes in cognitive ability, there were minimal changes in place orientation. In functional communication, however, there were no significant differences before and after the intervention. In conclusion, we found that ROT was an effective intervention for improving accuracy and responsiveness in the orientation of patients with mild cases of AD. In future studies, the effect of non-pharmacological interventions can be evaluated more reliably by examining the interaction effects of sample size, length of the intervention, outcome measurements, and pharmacological intervention.

The Effects of Changing the Respiratory Muscles and Acoustic Parameters on the Children With Spastic Cerebral Palsy (체간 조절을 통한 앉기 자세 교정이 경직형 뇌성마비 아동들의 호흡근과 음향학적 측정치들의 변화에 미치는 효과)

  • The purpose of this study was to investigate the effects postural changes on respiratory muscles and acoustic parameters of the children with spastic cerebral palsy. Nine children with spastic cerebral palsy who required assistance when walking were selected. The ages of the children ranged from 6 to 9 years old. The phonation of the sustained vowel /a/ and the voice qualities of each child such as fundamental frequency($F_0$; Hz), pitch variation (Jitter; %), amplitude variation (Shimmer; %) and noise to harmonic ratio (NHR) were analyzed by Multi-Dimensional Voice Program (MDVP). The muscle activity of three major respiratory muscles: pectoralis major muscle, upper trapezius muscle and rectus abdorminalis muscle, were measured by examining the root mean square (RMS) of the surface EMG to investigate the impact of changes in the adjusted sitting posture of each subject. However, the RMS of pectoralis major muscle showed a significant differences (p<.05). Secondly, there were no significant differences in $F_0$, Jitter and Shimmer between pre and post posture change, but there was a significant difference in NHR (p<.05). The data were collected in each individual; once prior and once after the sitting posture change. The data were analyzed by Wilcoxon signed ranks-test using SPSS version 14.0 for Windows. The findings of this study were as follows; Firstly, the RMS of upper trapezius and rectus abdorminalis muscle were not significant different between pre and post sitting posture changes. From the result, it is concluded that changes in the adjusted sitting posture decreases the abnormal respiratory patterns in the children with spastic cerebral palsy which is characterized by the hyperactivity of the respiratory muscles in breathing. Also, there is increased on the voice qualities in children with spastic cerebral palsy.

A Clinical Study on Children with Delayed Language Development - Especially Focussing on Causes - (소아 언어발달지연의 연구 - 특히 원인을 중심으로 -)

  • Delayed emergence of speech or language are frequent causes for concern about development during early childhood. Dealy in evaluation and proper management until school entry in more likely to result in frustration, anxiety and school failure. Many language disturbances and their attendant behavior disorders respond to intensive language therapy. Authors analyzed the medical recoreds of 52 children with delayed language development evaluated during 30 months from January 1986 through June 1988. The results were as follows : 1. The majority of cases were evaluated at the age of 2-2.9 year old(16 cases, 30.8%) and 3-3.9 year old(11 cases, 21.2%) 2. Male to female ratio was 3 : 1 3. The most common cause was mental retardation(53.8%), followed by developmental language disorder(23.1%) and autism(13.5%) 4. The most common associated condition was dysarticulation(17.3%), followed by strabismus(9.6%) and seizures(7.7%) 5. Special education was recommended in cases of 23(44.2%), speech therapy in 12 cases(23.1%) and consultation to child psychiatry in 7 cases (13.5%). Making diagnosis of underlying disorders is not simple because assessment of intelligence in young children is difficult and only a few tests are standardized. More detailed study on children with delayed language development and development of psychometric tests for the handicapped children are necessary, especially in Korea.

Prospect of Treatment with Herb Medicine for Developmental Delay of Language and Intelligence Quotient (어지와 지능지수에 대한 한약치료의 전망)

  • It is widely assumed that Intelligence Quotient (IQ) is determined by inherent disposition and environmental factor. IQ is estimated by age-conversion score and stabilized around age 4 and IQ of adult age can be predicted after age 10. Though children with Mental Retardation (MR) are delayed in language development since early infant period, they receive only special education including speech and language therapy, but no special medication. In traditional Korean medicine, the etiology and treatment for developmental delay of language have been handed down for a long time. Some studies on herbs and prescriptions for improving language development have been undertaken recently. We have found several cases of significant elevation of IQ in the children treated with long term medications of Korean herbal medicine for improvement of language. Analyzing these cases, especially performance IQ showed significant change. Therefore we suggest that Korean herbal medicine might improve cognition development in children with MR.

A Study on the Necessity of Home Visit Rehabilitation Therapy by Rehabilitation Professionals Working at Social Welfare Facilities (사회복지시설에 종사하는 일부 재활전문가들의 방문재활에 대한 필요성 조사)

  • Purpose: The purpose of this study was to analyze the necessity of home visit rehabilitation therapy by rehabilitation professionals working at social welfare facilities. Methods: The subjects of this study were 227 rehabilitation professionals (social worker, nurse, physical therapist, occupational therapist, speech-language therapist, special education teacher, and staff of institutions for the disabled who were working at community rehabilitation centers. The data were collected over 65 days (2008. 09. 10~11. 14). The results were analysed statistically by the Chi-square test. Results: The results are as follows: Long-term care service was established in Korea by the Ministry of Health in July, 2008. But there are limits to their being able to provide various rehabilitation services for chronic patients and old people. According to a recent survey, almost all rehabilitation professionals (N=227) stated that home visit rehabilitation therapy is necessary. Conclusion: In Korea, the long-term care service has a nursing service and a service supporting physical activities of daily living, but not physical therapy. So, home visit rehabilitation services should include physical therapy, rehabilitation exercise, and pain management.

A longitudinal analysis on interruption in preschool children who stutter during interactions with their mothers (학령전 말더듬아동과 어머니 간 상호작용 시 끼어들기 특성 종단적 분석)

  • The purpose of this study was to investigate, longitudinally, interruption behavior which children who do stutter (CWS), children who do not stutter (CWNS) and their mothers and relationship with disfluency of children showed during interactions with their mothers. Subjects for this study consisted of 2-5 year old CWS (male 2 and female 4), an age-matched group of CWNS (male 3 and female 3), and their mothers. Frequencies of normal disfluency (ND) and abnormal disfluency (AD) in children group and frequency of interruption and simultalk duration in children and mothers group were measured two times (initial visit and 12 months later) over the course of one year. As a result, no significant difference was observed in frequency of interruption and simultalk duration both between two mother groups and between two child groups at initial visit and 12 months later. However, frequency of interruption increased significantly over the course of one year in CWS group. A significant group difference was found in frequency of interruption of mothers but, no significant difference was observed in simultalk duration of mothers at initial visit. In the CWS·mothers group, no factors were related with disfluency of children at initial visit and 12 months later. These findings suggest that interruption is not just negative behavior, and that reducing interruption should be considered in child·parent interaction therapy for CWS.

Analysis on the Relative Importance and Priority in Speech Therapy Center of Parents of Children with Disabilities (장애아 부모의 언어치료실 선택속성 분석)

  • The purpose of this study is to explore the selection attributes for parents of children with language disability when choosing a clinic. The data collection was carried out in 3 steps: the preliminary survey, first open survey and second survey in AHP(Analytic Hierarchy Process). The subjects of were 252 in total. The results were as follows: First, The order of priority attributes in superior categories for parents of children with language disability when selecting a clinic were 'therapist-related attributes', 'program-related attributes' and 'physical-related attributes' in turn. The top 5 priority attributes in subcategories were 'therapist's academic background and major', 'ability to make a rapport', 'clinical experience and qualification of therapist', 'kindness and confidence' and 'counseling program for parents'. Second, The parents of preschoolers age 6 and younger chose 'clinical experience and qualification of therapist', 'counseling program for parents' and 'learning materials' for the most priority attributes, whereas the parents of students age from 7 to 12, considered 'therapist's academic background and major', 'clinical fee' and 'distance transport parking' more importantly to select a clinic. The results of this study provided preliminary data for successful planning of speech and language therapy.

Neural bases underlying Native or Foreign word production, and Language switching (모국어와 외국어의 단어산출 및 언어 간 전환에 따른 뇌 활성화 과정)

  • The neural bases underlying within or between-language picture naming was investigated by using event-related fMRI. The present suudy explorered the following two goals: The first is to compare cortical activation areas relevant to naming process in native and foreign language, and to decide whether the activation pattern of the foreign word will be the same as native words or not. The next is to find the cerebral areas involved only in alternating language switching between native and foreign language condition. Differential activation patterns were observed for language switching against one-language. Both naming tasks all activated the left inferior frontal gyrus (LIFG) as expected. However the differences in naming between languages were reflected in the activation amount of the LIFG, namely more activation in naming the native language than the foreign language. Especially, naming of the foreign word from English showed the similar area and size in activation with native language suggesting that the process of borrowed noun resembles that of native common noun. And the language switching between languages newly activated the right middle frontal gyrus as well as the left inferior frontal areas. The right middle frontal gyrus engagement in switching conditions obviously identified that right hemisphere is recruited in code switching possibly with respect to meta-cognition controlling language index at a subconscious level.
