The Relation between Hand Function and Sleep Disturbance Factors According to Quality of Sleep among Elderly Patients in Geriatric Hospitals (요양병원 노인에서 수면의 질의 따른 손 기능과 수면 장애 요인 간 관련성 연구)

  • Objective : The purpose of this study is to consider the change of hand function according to the quality of sleep and to find out a correlation among the quality of sleep and hand function, and sleep disturbing factors. Method : 23 patients who were hospitalized more than 6 months from Nov. 2017 to Jan. 2018 were tested for the quality of sleep used by sleep quality scale, for the grip strength used by a dynamometer, and for the coordination used by BBT and 9-Hole Peg test. The collected data were analyzed to implement the paired t-test and Pearson correlation coefficient. Result : In the high grade group of sleep score, the quality of sleep & BBT, and the grip strength & BBT have correlated each other; and in the medium group, only the latter has shown a correlation. In both groups, the quality of sleep and the sleep pattern have showed significant high correlation. Conclusion : When the elderly in geriatric hospitals show qualitative differences, BBT and the combination of treatment for the improvement of grip strength can improve the quality of sleep. And the sleep pattern intervention is considered to be effective to improve the quality of sleep.

The Effect of Voice Therapy in Vocal Polyp Patients (성대용종 환자의 음성치료 효과)

  • Vocal polyps are benign phonotraumatic lesions which are traditionally treated using phonomicrosurgical techniques. In the case of hyperfunctional voice use, voice therapy is effective and results in voice improvement. However, the utility of voice therapy about vocal polyp is in great demand. The purpose of this study was to evaluate the effects of voice therapy in patients with vocal polyps. The authors reviewed the medical records of 193 patients with vocal nodules or vocal polyps, and 64 patients (31 nodules and 33 polyps) were enrolled. All of the subjects had received explanation of problems, vocal hygiene education, and been treated by the $SKMVTT^{(R)}$ (Seong-Tae Kim's multiple voice therapy technique) ranging from 4 to 16 sessions (mean: 8.6 sessions). All subjects were examined by perceptual assessment, acoustic and aerodynamic measures, and VRP (voice range profile). In perceptual assessment, patients with vocal nodules had more breathy and strained voices than the vocal polyp group. Both groups significantly reduced rough, breathy voice after voice therapy. Patients with vocal polyps had worse voice quality than patients with nodules in acoustic measures. Both groups showed reduced jitter and shimmer after voice therapy. In aerodynamic measures, MPT and Psub were increased, and MFR was reduced (p<.05). Participants' frequency range and intensity range were increased after voice therapy, but only frequency range resulted in a significant difference (p<.05). In conclusion, the therapeutic effect of voice therapy in patients with vocal nodules and polyps was demonstrated perceptually and acoustically. We can suggest that voice therapy, including advice, vocal hygiene, and $SKMVTT^{(R)}$ is a useful as an initial choice of treatment for patients with vocal polyps before considering a surgical approach.

Transcoding Algorithm for AMR and EVRC Vocoders Via Direct Parameter Transformation (AMR과 EVRC 음성부호화기를 위한 파라미터 직접 변환 방식의 상호부호화 알고리듬)

  • In this paper, a novel transcoding algorithm for the Adaptive Multi Rate(AMR) and the Enhanced Variable Rate Codec(EVRC) vocoders via direct parameter transformation is proposed. In contrast to the conventional tandem transcoding algorithm, the proposed algorithm converts the parameters of one coder to the other without going through the decoding and encoding processes. The proposed algorithm consists of the parameter decoding, frame classification, mode decision, and transcoders for two frame types. The transcoders convert the parameters such as LSP, frame energy, pitch delay for the adaptive codebook, fixed codebook vector, and codebook gains. Evaluation results show that while exhibiting better computational and delay characteristics, the proposed algorithm produces equivalent speech quality to that produced by the tandem transcoding algorithm.

Speaker Adapted Real-time Dialogue Speech Recognition Considering Korean Vocal Sound System (한국어 음운체계를 고려한 화자적응 실시간 단모음인식에 관한 연구)

  • Voice Recognition technique has been developed and it has been actively applied to various information devices such as smart phones and car navigation system. But the basic research technique related the speech recognition is based on research results in English. Since the lip sync producing generally requires tedious hand work of animators and it serious affects the animation producing cost and development period to get a high quality lip animation. In this research, a real time processed automatic lip sync algorithm for virtual characters in digital contents is studied by considering Korean vocal sound system. This suggested algorithm contributes to produce a natural lip animation with the lower producing cost and the shorter development period.

Enhancement of a language model using two separate corpora of distinct characteristics

  • Language models are essential in predicting the next word in a spoken sentence, thereby enhancing the speech recognition accuracy, among other things. However, spoken language domains are too numerous, and therefore developers suffer from the lack of corpora with sufficient sizes. This paper proposes a method of combining two n-gram language models, one constructed from a very small corpus of the right domain of interest, the other constructed from a large but less adequate corpus, resulting in a significantly enhanced language model. This method is based on the observation that a small corpus from the right domain has high quality n-grams but has serious sparseness problem, while a large corpus from a different domain has more n-gram statistics but incorrectly biased. With our approach, two n-gram statistics are combined by extending the idea of Katz's backoff and therefore is called a dual-source backoff. We ran experiments with 3-gram language models constructed from newspaper corpora of several million to tens of million words together with models from smaller broadcast news corpora. The target domain was broadcast news. We obtained significant improvement (30%) by incorporating a small corpus around one thirtieth size of the newspaper corpus.

Effects of Vocalization on Upper Extremity Muscle Activity during Reaching Task in Patients with Hemiplegia (발성이 편마비 환자의 팔 뻗기 시 상지근육의 근활성도에 미치는 효과)

  • This study examined the effects of vocalization on upper extremity muscle activity during reaching task in patients with hemiplegia. Thirteen persons with right brain stroke performed reaching to a cup under four concurrent speech conditions of vocalizing the word "Ah" with hemiplegic side. These four conditions are self-vocalization, external vocalization, imaginary vocalization, and no vocalization. The muscle activity(Biceps brachii, Triceps brachii, Middle deltoid, and Upper trapezius) were measured using MP150. Muscle activity was significantly higher under self-vocalization and external vocalization conditions compared to the muscle activity under imaginary vocalization and no vocalization conditions on triceps brachii muscle.(p<0.05). Triceps brachii muscle was highly correlated with biceps brachii muscle(r=0.777, p<0.05). The results suggest that self-vocalization and external vocalization can be used in facilitating upper extremity movements in patients with stroke. When working with patients with right hemispheric stroke, therapists might explore possibilities of using patient's self-speech to enhance the quality of upper extremity movement performance.

Voice Analysis and Videostroboscopic Findings before and after Laryngomicrosurgery of Intracordal Cysts (성대낭종환자에서의 후두미세수술전후의 음성언어분석비교)

  • Background and Objectives : Dysphonia may be secondary to many different type of benign vocal cord lesions such as vocal polyp, vocal nodule, Reinke's edema, and intracordal cyst. Diagnosis and treatment of intracordal cysts are more difficult than other benign vocal cord lesions. But postoperative voice analysis of intracordal cyst have rarely been reported in the literature. The purpose of this study is to analyze aerodynamic and acoustic results and videostroboscopic findings before and after laryngomicrosurgery. Materials and Methods : We reviewed the pre and post-operative voice analysis results and videostroboscopic findings of 15 surgically treated patients of intracordal cysts at Severance hospital from Jun. 1997 to Nov. 1999 retrospectively. They were diagnosed with videostroboscopic findings, surgical findings, and pathologic reports. Their pre and post-operative speech were analyzed with MDVP(Multi Dimension Voice Analysis Program) of CSL(Computerized Speech Lab) and Aerophone II. Their pre and post-operative mucosal wave of true vocal cord was analyzed with videostroboscopy. In order to compare this results with normal group, 10 of normal persons were evaluated with same methods. Results : After the operation, mucosal wave of true vocal cord was improved in all patients. Postoperative acoustic and aerodynamic results were improved in almost parameters, but they did not reach the normal value. Conclusions : Videostroboscopy was essential in diagnosing intracordal cysts. By comparing the acoustic and aerodynamic results and video-stroboscopic findings before and after the laryngomicrosurgery, postoperative vocal function was defined more accurately and objectively. Almost parameters may be useful in assessing the quantitative changes in vocal quality before and after the laryngomicrosurgery.

The Effect of the Treatment on the Pre- and Post Respiration and the Oral Motor for Children with Cerebral Palsy by Acoustic Analysis (음향학적 분석을 통한 뇌성마비 아동의 호흡 및 구강 운동 전.후 치료 효과)

  • The purpose of this study was to find out the acoustic variation on the pre-and post respiration and oral motor for children with cerebral palsy. Five children with spastic CP at the age of 6 in average were practiced by a caregiver at home each for 25 minutes, in total, 45 times. The sustained of vowel /a/ and vowels /a/, /i/, /u/, /e/, /o/ were recorded on CSL and MDVP and analyzed by acoustic parameters. As a result, the maximum phonation time(MPT) was increased from 2.06 to 6.31 and the formant of vowels(F1, F2, F3) had significant differences in F1(/a, i/), F2(/i.u.o/), and F3(/a/) between the controls and the children with CP in pre-treatment. The total average value of vowels had significant differences between the pre-and post-treatment (p< .05). The energy of vowels had significant differences in the vowels /i, u, e, o/ and the total average value between the pre-and post-treatment(p< .001). The jitter percent, shimmer percent, and noise to harmonic ratio had significant differences between the pre-and post-treatment(p< .05). As the respiration and the oral motor improved MPT, voice quality, and articulation of vowel, and the variation of the formant(F1, F2, F3) showed the changes in the shape of lips, the place and the height of the tongue, the various development of therapy programs and the consistent intervention of treatment is needed for the children with cerebral palsy.

Efficient Harmonic-CELP Based Low Bit Rate Speech Coder (효율적인 하모닉-CELP 구조를 갖는 저 전송률 음성 부호화기)

  • This paper describes an efficient harmonic-CELP speech coder by taking advantages of harmonic and CELP coders into account. According to frame voicing decision, the proposed harmonic-CELP coder adopts the RP-VSELP coder as a fast CELP in case of an unvoiced frame, or an improved harmonic coder in case of a voiced frame. The proposed coder has main features as follows: simple pitch detection, fast harmonic estimation, variable dimension harmonic vector quantization, perceptual weighting reflecting frequency resolution, fast harmonic synthesis, naturalness control using band voicing, and multi-mode. These features make the proposed coder require very low complexity, compared with HVXC coder To demonstrate the performance of the proposed coder, a 2.4 kbps coder has been implemented and compared with reference coders. From results of informal listening tests, the proposed coder showed good quality while requiring low delay and complexity.

The Difference between Acoustic Characteristics of Acute Epiglottitis and Peritonsillar Abscess (급성 후두개염과 편도주위 농양 환자의 발화시 조음 및 음성의 차이)

  • Backgraound and Objectives : The voice change can occur in acute epiglottitis or peritonsillar abscess, and the labelings of both changes as a "muffled voice" or "hot potato voice", The aim of this study was to investigate the difference of changes in acoustic feature of voice before and after treatment in patients with acute epiglottitis or peritonsillar abscess. Subjects and Method: 13 patients with acute epiglottitis and 12 patients with peritonsillar abscess were enrolled in the study. Acoustic analysis on sustained Korean vowels /${\alpha}$/, /u/ and /i/ were performed before and after treatment. Results: In patients with acute epiglottitis, the first formant frequency (F1) of /${\alpha}$/ was increased, and the second frequency (F2) of /i/ was decreased. In patients with peritonsillar abscess, F1 and F2 of /${\alpha}$/ were decreased. F1 of /i/ and /u/ were increased, while F2 were decreased. Conclusion : The anatomical and functional changes of oropharynx and larynx by acute epiglottitis and peritonsillar abscess can cause different change in resonance and speech quality. We suggest that these changes could be the cause of 'muffled vocie' in patients of acute epiglottitis or peritonsillar abscess, but different characteristics of phonation in each disease should be distinguished.
