Effects of Wole Body Vibration Training on Transverse Abdominis Muscle Thickness and Sitting Balance in Spastic Cerebral Palsy (전신진동 자극 훈련이 경직형 뇌성마비 아동의 배가로근 두께 및 앉은 자세 균형에 미치는 영향)

  • Background: The purpose of this study was to investigate the effect of whole-body vibration stimulation training on the thickness of the transversus abdominis muscle and the balance of sitting posture in children with spastic cerebral palsy. Design: Single-subject design(A-B-A-B). Methods: The subjects of this study were 9 children with spastic cerebral palsy. The study period was 12 weeks in total, and the baseline period and the intervention period were each assigned 3 weeks. Intervention was conducted twice a week for 30 minutes. During the baseline period, trunk stabilization exercise was performed, and during the intervention period, trunk stabilization exercise and whole-body vibration stimulation training were performed. Measurements were carried out at before the experiment, baseline 1, intervention 1, baseline 2, intervention 2 and the total number of measurements was 5 times. Repeated ANOVA was performed to compare the effects of exercise according to the intervention method. Results: The thickness of the transversus abdominis muscle and the balance of the sitting posture were statistically significantly increased compared to the baseline during whole-body vibration stimulation training (p<.05). Conclusion: Therefore, it was confirmed that whole-body vibration stimulation training improved the thickness of the transversus abdominis muscle in children with spastic cerebral palsy and was an effective intervention method for improving sitting posture balance.

Effect of Whole Body Vibration Training on Proprioception and Tactile in Spastic Cerebral Palsy (전신진동 자극 훈련이 경직형 뇌성마비 아동의 고유수용감각 및 촉각에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the effects of whole-body vibration stimulation on proprioception and tactile in patients with spastic cerebral palsy. Methods: This study was conducted on 9 children diagnosed with spastic cerebral palsy. Of the single case study methods, the ABAB design was employed in this study. Out of a 12-week study period, three weeks were allocated to each of two baseline periods and two intervention periods. The exercise was performed twice a week for 30 minutes. A general trunk stabilization exercise was performed during the baseline period and a trunk stabilization exercise accompanied with whole-body vibration was performed during the intervention period. Evaluation was performed five times in total: before the experiment, after baseline 1, after intervention 1, after baseline 2, and after intervention 2. To determine the effect of the exercise method, a skin sensory evaluation tool (monofilament kit) and a trunk proprioception sensor (digital dual inclinometer) were used. To compare the effects of the exercises at baseline and after intervention, an analysis of variance on repeated measures (repeated ANOVA) was performed to analyze the data. Results: The results showed that there were statistically significant increases from baseline in the means of proprioception and tactile during the intervention period with whole-body vibration (p <.05). Conclusion: Whole-body vibration can be proposed as an effective intervention method for improving the proprioception and tactile in children with spastic cerebral palsy, and this exercise method is expected to be actively used in clinical practice.

The Effect of Weight-support Treadmill Training on the Balance and Activity of Daily Living of Children with Spastic Diplegia

  • Purpose: This is designed to study the effect of weight-support walking training through motor learning on motor functions of children with cerebral palsy, in particular their activity of daily living and balance. Methods: Thirteen children with spastic cerebral palsy, at gross motor function classification system (GMFCS) levels III~IV, underwent treadmill walking training. It used principles of weight support, 4 times a week for 7 weeks, 10 minutes at a time, before and after neurodevelopmental physical therapy. Everyday functions were measured using Functional Independence Measure for Children (Wee-FIM). The ability to keep their balance was measured using electronic measuring equipment from good balance system and the assessment was made before and after the experiment. Results: There were significant differences (p<0.05) between pre and post experiment levels of functional independence in everyday life, in self-care activities, mobility, locomotion and social cognition. With regard to changes in standing balance, there were significant differences before and after the experiment (p<0.05) in GMFCS level III. There was a reduction in the agitation velocity in the x- and y-axes which measures the left-to-right shaking; in GMFCS level IV, velocity moment was reduced. Conclusion: Walking training using a treadmill can help improve the everyday activity and balance in children with spastic cerebral palsy. It can also be served as a useful purpose as a method of intervention in pediatric care.

Comparison of Sitting Balance and Coordination in Children with Spastic Cerebral Palsy Using the Korean Version of Trunk Impairment Scale (K-TIS)

  • PURPOSE: The purpose of this study was to compare sitting balance and coordination spastic cerebral palsy in children using the Korean version of Trunk Impairment Scale (K-TIS) as well as to provide basic data about effective postural control treatment for clinicians handling these two types. METHODS: The K-TIS was measured in 29 children diagnosed with diplegic and quadriplegic cerebral palsy (18 with diplegia and 11 with quadriplegia). The average and standard scores of the children's K-TIS subscales and items of the two groups were measured. The two groups' subscales and items were analyzed by using the Mann-Whitney U test. RESULTS: Static sitting balance, dynamic sitting balance, coordination, and total score for children with diplegia were statistically high (p<.05). For all items under static sitting balance, the score for children with diplegia was higher. The first differences in the repeated items of dynamic sitting balance and coordination area that rotates between the upper and lower body were presented. CONCLUSION: The difference in balance and coordination in sitting positions is exhibited in children with diplegia and quadriplegia. For children with spastic quadriplegia, treatments should focus on static sitting balance and coordination, together with a focus on dynamic sitting balance and coordination.

The change of muscle action potential and superficial temperature of spastic upper extremity in a patient with cerebral palsy by the water temperature (수온이 뇌성마비 환자의 상지 경련근의 표재 온도와 근 활동전위에 미치는 영향)

  • The purpose of this study was to describe the alteration of muscle action potential of spastic upper extremity in a patient with cerebral palsy by the water temperature. We used seven patients with cerebral palsy. Participants classified according to each group in $29^{\circ}C,\;35^{\circ}C$ by the water temperature. All subjects participated 2 session, which at least 1 week between session. The test was measured continuously pre test, post-test by thermometer and surface EMG. The following results were obtained; 1. In changes of surface temperature, wrist flexor and extensor were significantly decreased in $29^{\circ}C$ (p<0.001) group but were significantly increased $35^{\circ}C$ group(p<0.01). 2. In changes of surface temperature, wrist flexor and extensor were significantly differenced between $29^{\circ}C$ and $35^{\circ}C$ group(p<0.001). 3. In changes of muscle action potential, wrist extension antagonist were significantly increased in $29^{\circ}C$ group(p<0.05). 4. In changes of muscle action potential, wrist flexion agonist were more significantly increased in $29^{\circ}C$ group(p<0.01). 5. In changes of muscle action potential, wrist extension antagonist were significantly differenced between $29^{\circ}C$ and $35^{\circ}C$ group(p<0.05). These results lead us to the conclusion that changes of muscle action potential of spastic upper extremity in a patient with cerebral palsy were influenced by the water temperature. Therefore, a further direction of this study will be to provide more evidence that a moderate water temperature have an effect on muscle tone in a patient with cerebral palsy.

Dual Mode Feedback-Controlled Cycling System for Upper Limb Rehabilitation of Children with Cerebral Palsy

  • Background/Objectives: This paper proposes a dual mode feedback-controlled cycling system for children with spastic cerebral palsy to rehabilitate upper extremities. Repetitive upper limb exercise in this therapy aims to both reduce and analyze the abnormal torque patterns of arm movements in three- dimensional space. Methods/Statistical analysis: We designed an exercycle robot which consists of a BLDC motor, a torque sensor, a bevel gear and bearings. Mechanical structures are customized for children of age between 7~13 years old and induces reaching and pulling task in a symmetric circulation. The shafts and external frames were designed and printed using 3D printer. While the child performs active/passive exercise, angular position, angular velocity, and relative torque of the pedal shaft are measured and displayed in real time. Findings: Experiment was designed to observe the features of a cerebral palsy child's exercise. Two children with bilateral spastic cerebral palsy participated in the experiment and conducted an active exercise at normal speed for 3 sets, 15 seconds for each. As the pedal reached 90 degrees and 270 degrees, the subject showed minimum torque, in which the child showed difficulty in the pulling task of the cycle. The passive exercise assisted the child to maintain a relatively constant torque while visually observing the movement patterns. Using two types of exercise enabled the child to overcome the abnormal torque measured in the active data by performing the passive exercise. Thus, this system has advantage not only in allowing the child to perform the difficult task, which may contribute in improving the muscle strength and endurance and reducing the spasticity but also provide customizable system according to the child's motion characteristic. Improvements/Applications: Further study is needed to observe how passive exercise influences the movement characteristics of an active motion and how customized experiment settings can optimize the effect of pediatric rehabilitation for spastic cerebral palsy.

Diadochokinetic Characters in the Persons with Spastic Cerebral Palsy in Severity ; Rate, Regularity, Accuracy and Consistency (심한정도에 따른 경직형 뇌성마비인의 교호운동 특성 ; 속도, 규칙성, 정확성 및 일관성을 중심으로)

  • The purpose of this study was to investigate the diadochokinetic characters in the patients with spastic cerebral palsy(CP) in severity. The diadochokinetic characters were measured through rate, regularity, accuracy and consistency. The subjects participated in this study included 27 persons with spastic CP(mild- 9, moderate- 9, severe- 9) and 9 normal persons who is around 11-20 years old. On the result of this study, rate in AMR was significant difference between all spastic groups and normal group, and rate in SMR was significant difference between normal and mild groups and moderate and severe groups. In regularity of diadochokinetic task, severe group had significant difference the other groups. Finally, accuracy and consistency of diadochokinetic task exhibited significant difference between all spastic groups and normal group.

The Effects of Water-based Exercise on Respiratory Function in Children with Spastic Diplegic Cerebral Palsy

  • Purpose: We investigated to evaluate the effectiveness of water-based exercise (WE) program on respiratory functions for children with spastic diplegic cerebral palsy (CP). Methods: Fourteen children with spastic diplegic CP were randomly assigned, to either the experimental group (EG, n=7), or the control group (CG, n=7). Respiratory function was measured by a spirometer, a CardioTouch 3000S ( Bionet, Seoul, Korea) at a chair-sitting posture. Forced vital capacity (FVC), forced expiratory volume at one second (FEV1), peak expiratory flow (PEF) were measured. The intervention program will last 8 weeks, with three 40 minutes sessions per week (24 training session). The usual care and the addition of a WE program, were compared in the CG and EG, respectively. Results: The EG showed a significant increase in the FVC, FEV1, PEF after training (p<0.05), whereas there was no significant difference in the CP after training. In the EG, FVC increased significantly, compared to the control group (p<0.05), but not FEV and PEF. Conclusion: These findings suggest that WE program have an effect on the respiratory function in children with spastic diplegic CP.

The Effects of Respiratory Muscle Strengthening Exercise on the Respiratory and Phonation Capacity in Spastic Cerebral Palsy Child (호흡근 강화운동이 경직형 뇌성마비 아동의 호흡능력 및 발성에 미치는 영향)

  • The purpose of this study was to evaluate the influence of respiratory capacity(forced vital capacity), EMG of rectus abdominal muscle, phonation by respiratory muscle strengthening exercise in children with spasticity cerebral palsy. 24 children with spasticity cerebral palsy was randomized in 2 groups, respiratory muscle strengthening exercise and contro group. In the exprimentral groups, respiratory muscle strengthening exercise for 30minutes duration 3 time per week for 8weeks were respectively preformed, Control group was not performed. Before and after experiments, respiratory capacity(forced vital capacity), EMG of rectus abdominal muscle and phonation was measured in all children. In comparison of difference before and after experiment, the respiratory capacity(forced vital capacity) of respiratory muscle strengthening exercise group was significantly increased than the control group(P<.05), rectus abdominal muscle EMG of the respiratory muscle strengthening exercise group was significantly increased more than the control group(P<.05) and MPT of the respiratory muscle strengthening exercise group was significantly increased more than the control group(P<.05). We found that the respiratory muscle strengthening exercise is useful to improve the respiratory capacity and phonation in children with spasticity cerebral palsy.

Correlation between Manual Ability Oassification System and Functional Evaluation in Children With Spastic Cerebral Palsy (경직형 뇌성마비 아동의 손 기능 분류 체계와 기능적 수행도 평가 간의 상관)

  • The purpose of this study was to investigate the relationship among functional evaluation systems, the Manual Ability Classification System (MACS), the Gross Motor Function Classification System (GMFCS), and the functional status (WeeFIM) in children with spastic cerebral palsy and to provide the foundation data about MACS for evaluation system of hand function in children with spastic cerebral palsy. For this, sixty children with spastic cerebral palsy were employed in this study. The sixty children were evaluated by using the MACS for their hand function and by using the GMFCS for their motor function. The functional status were assessed by using the Functional Independence Measure of Children (WeeFIM). There were a significant correlation between the MACS and the GMFCS (r =.659, p <.05). The good correlation between the MACS and WeeFIM was found (r = -.576, p <.05). The functional status according to the hand function level evaluated by using the MACS were different significantly (p <.05). The MACS in practice will provide usefulness for assessment of hand function in children with spastic cerebral palsy.
