Extensive gingival necrosis and sequestration of the alveolar bone caused by methimazole-induced neutropenia and three-year follow-up

  • Purpose: Methimazole is an anti-thyroid drug that can cause life-threatening neutropenia in rare situations. The aim of this case report is to describe a set of oral complications associated with methimazole-induced neutropenia and the healing of the gingiva after proper treatment. Methods: A 31-year-old female patient hospitalized for systemic symptoms of sore throat and fever and showing extensive gingival necrosis with pain was referred to the Department of Periodontics from the Department of Endocrinology. Methimazole-induced neutropenia was diagnosed based on blood test results and her medical history. Methimazole was discontinued and a range of treatments was administered, including the injection of granulocyte colony stimulating factor. Results: After systemic treatment, the gingiva began to heal as the neutrophil count increased. Approximately one year later, the gingiva had returned to a normal appearance. Twenty-one months after treatment, sequestra of the alveolar bone that had broken through the gingiva were removed. Periodic supportive periodontal treatment has been continued uneventfully. Conclusions: The oral manifestations of gingival necrosis and ulcerations, in combination with systemic symptoms such as fever and sore throat, are the critical signs presented in the early stages of drug-induced neutropenia. Therefore, dentists need to be aware of these oral complications in order to make an accurate diagnosis and to ensure that prompt medical intervention is provided.

Clinical characteristics of children and adolescents with croup and epiglottitis who visited 146 Emergency Departments in Korea

  • Purpose: Croup is a common pediatric respiratory illness with symptoms of varying severity. Moreover, epiglottitis is a rare disease that can rapidly progress to life-threatening airway obstruction. Although the clinical course and treatments differ between croup and epiglottitis, they are difficult to differentiate on presentation. We aimed to compare the clinical characteristics of croup and epiglottitis in Emergency Department patients. Methods: The 2012 National Emergency Department Information System database of 146 Korean Emergency Departments was used to investigate patients aged ${\leq}18years$ presenting with croup or epiglottitis. Results: We analyzed 19,374 croup patients and 236 epiglottitis patients. The male:female sex ratios were 1.9:1 and 2.3:1 and mean ages were $2.2{\pm}2.0$ and $5.6{\pm}5.8years$, respectively. The peak incidence of croup was observed in July and that of epiglottitis was observed in May. The hospitalization rate was lower in croup than in epiglottitis patients, and the proportion of patients treated in the intensive care unit was lower among croup patients. The 3 most common chief complaints in both croup and epiglottitis patients were cough, fever, and dyspnea. Epiglottitis patients experienced dyspnea, sore throat, and vomiting more often than croup patients (P<0.05). Conclusion: Both groups had similar sex ratios, arrival times, 3 most common chief complaints, and 5 most common comorbidities. Epiglottitis patients had a lower incidence rate, higher mean age of onset, and higher hospitalization rate and experienced dyspnea, sore throat, and vomiting more often than croup patients. Our results may help in the differential diagnosis of croup and epiglottitis.

  • 1. Autum-dryness syndrome(秋燥) is caused by dry-warm or dry=-cool weather condition when lower-warmer Eumfen(下焦陰分) was injured by jung hyoul hatal(精血下脫) or taking medicines of dryness in autumn. 2. The symptoms of autum-dryness syndrome are headach, fever, chilling, anhydrosis, dry cough, tinnitis, dry lip, dry skin, chest discomfort in cool-dryness syndrome(凉燥) and headach, fever, dry cough, paroxymal cough, dry skin, chest pain, polydipsia, tinnitis, eye redness, sore throat in heat-dryness syndrome(溫燥). 3. Hyangsochongsitang(香蘇蔥頭湯) chn be applied for terating cool-dryness syndrome(凉燥) and chungjogoopyetang(淸燥救肺湯) for heat-dryness syndrome(溫燥). Sang Hang Tang(桑杏湯) can be administered when the evils located in the upper-warmer, and Yukmihwan(六味丸) can be administered when located in the lower-warmer(下焦) 4. According the determination of treatment based on the differentiation of symptoms and signs. When the symptoms of Weifen syndrome(衛分證) are headach, fever, dry lip or chilling or dypsia and the treatment of weifen syndrome apply Hangsosan(行蘇散) or Sang hang tang(桑杏湯). When the symptoms of Qifen syndrome(氣分證) are tinnitis, eye redness, sore throat, chest discomfort, polydipsia, dry cough, watery diarrhea, constipation and Jibaekjihwang(知栢地黃丸) when Ohingulpitang(五仁橘皮湯) Chungjogupyetang(淸燥救肺湯) can are applied for treating Qifen syndrome(氣分證). When the symptoms of younghylfen syndrome(營血分證) are dry cough, watery diarrhea, dypsia, chest discomfort, hematemesis, epistaxis and agyohwang keum tang(阿膠湯) Oknyujun(玉女煎) can be applied for treating Younghyulfen syndrome(營血分證).

『상한론(傷寒論)』 삼음병(三陰病)에 대한 연구(硏究) - 예후 분석을 중심으로 - (A Study on the Three Yin Diseases(三陰病) in the 『Shanghanlun(傷寒論)』 -Focusing on Prognosis Analysis-)

  • Objectives : An accurate judgment of prognosis when treating diseases is crucial. While the 『Shanghanlun(傷寒論)』 deals with the prognosis of the Three Yin Diseases with great importance, full-scale studies have been lacking. This paper aims to study the Three Yin Diseases with a focus on prognosis analysis. Methods : Among the Three Yin Disease verses, those that could provide clues to prognosis were selected and analysed. Conclusions & Results : When Yang pulse patterns such as long(長脈)·floating(浮脈)·rapid(數脈) pulses and Yang symptoms such as fever, vexing heat, mild perspiration, thirst, warmth in hands and feet are present in Yin disease, it could be taken as signs of Yang Qi restoration. In these situations, Yin Cold pattern such as diarrhea and reversal cold disappear and the prognosis is positive. However, despite Yang pulse patterns and symptoms, there are cases where diarrhea happens as a result of cold dampness being eliminated due to Yang Qi restoration. Also, when Yang Qi starts communicating smoothly after its restoration in the Three Yin Diseases, perspiration can happen. When diarrhea and reversal cold, which are patterns of Yin Cold get worse, with pulse patterns such as unfelt(脈不至)·replete(實脈)·fulminating(脈暴出) pulses, false heat symptoms such as fever and hot flashes happen, accompanied with Yang Qi depleted symptoms such as inability to lie down due to agitation, continuous perspiration, sore throat, dyspnea, and exaggerated breathing happen. When fast pulse, fever, and perspiration are present due to depression and stagnation of ministerial fire, symptoms such as bloody stool with pus, purulent abscess, sore throat, and inability to lie down due to agitation show, which signal negative prognosis. In bad cases of Reverting Yin Disease, there is continuous diarrhea and bloody stool with pus, which can be due to either Kidney Yang deficiency or depression and stagnation of ministerial fire. It could also be caused by excessive heat.

일개 한의의료기관에서 COVID 19 증상 완화를 위해 한약이 투여된 환자 63례에 대한 보고 : 후향적 차트 리뷰 연구 (Reports of 63 Patients Treated with Herbal Medicine to Relieve Symptoms of COVID-19 at One Korean Medicine Clinic: A Retrospective Chart Review)

  • Objectives: The purpose of this study was to report on the cases and prescriptions of non-face-to-face care performed to alleviate COVID-19 symptoms and to present their results. Methods: This study was conducted on 63 COVID-19 patients who visited the Ilsan Hamsoa Korean Medicine Clinic from January 24 to June 11, 2022. We retrospectively analyzed all 63 patient charts and amassed COVID-19-related symptoms and their changes according to clinical characteristics and outcomes. Results: All 63 COVID-19 patients complained of symptoms, such as coughing, sore throat, sputum, fever, headache, myalgia, rhinorrhea, nasal obstruction, postnasal drip, and voice change. In total, 30 patients were administered Eunma-san, 6 patients were administered Eungyo-san plus Mahaenggamseok-tang, 10 patients were administered Galgeunhaegi-tang, 7 patients were administered Eungyo-san, 6 patients were administered Maekmundong-tang, and 4 patients were administered Jugyeopseoggo-tang. Improvements in symptoms due to herbal medicine included the disappearance of COVID-19 fever (22 cases) within 3.59±4.2 days, sore throat (32 cases) within 4.37±2.98 days, and cough (36 cases) within 4.86±3.1 days. Conclusion: Various herbal medicine prescriptions were effective COVID-19 treatments, indicating that herbal medicines may be alternative treatment options for COVID-19. However, the evidence should be supplemented with better designed prospective research on individual prescriptions.

  • Objectives: The purpose of this study is to determine the clinical features of post-acute COVID-19 syndrome and the effectiveness and safety of Korean medicine treatment at the Public health center. Methods: The study was conducted among 11 patients with post-acute COVID-19 syndrome who attended a single public health center from January to December 2022. We retrospectively analyzed the charts of 11 patients and collected clinical characteristics, previous treatments, Korean medicine treatments, outcome variables (Numeral Rating Scale (NRS), Leicester Cough Questionnaire (LCQ), Visual Analog Scale (VAS)), adverse events, etc. Results: Of the 11 patients, six were women, and the average age of all patients was 68.27±12.31 years. The most common symptom were cough(n=9, 81.82%) and sore throat(n=9, 81.82%), followed by sputum, fatigue, rhinorrhea, and loss of appetite, etc. All 11 patients were treated with herbal medicine, with Samso-eum(n=6, 54.55%), Yeonkyopaedok-san(n=5, 45.45%), and Haengso-tang(n=3, 27.27%) being the most commonly used. After herbal medicine treatment, the median cough NRS decreased from 5 to 1, and the median sore throat NRS decreased from 4 to 1, both of which were statistically significant. One patient reported adverse event of dyspepsia and heartburn, but it was mild. Conclusions: The study presented the clinical features of the post-acute COVID-19 syndrome and suggested that Korean medicine treatment at public health centers may be effective and safe in alleviating associated symptoms.

이상경상돌기증의 치험례 (A CASE OF SURGICAL TREATMENT FOR SYMPTOMATIC ELONGATED STYLOID PROCESS)

  • Styloid process is a slender pointed bone of variable length, which project downward, forward, and slightly medialward from the posteroinferior portion of the tympanic part of the temporal bone. Embryologically, the styloid process is derived from Reichert's cartilage, a structure of second branchial arch origin. Most patients with elongated styloid process remain asymptomatic, but some patients complain pain and sensation of foreign body in the throat, dysphagia, dysphonia, referred otalgia. Recently, we have experienced a case of elongated styloid process in a fifty-nine year old man. He had been suffered from Rt. side sore throat, headache and pharyngeal discomfort during head movement. Through various X-ray examination, Xeroradiography, and palpation of tonsillar fossa, elongated styloid process on Rt. side was confirmed. Under the general anesthesia, it was successfully removed out via transoral approach technique, described by Eagle. The resected styloid process was 2.5cm in it's length and he was freed from the symptoms without further complication.

Effect of Korean folk medicine 'SecSec' on inflammatory cytokine secretion in HMC-1 cells

  • 'SecSec' has been used for the purpose of prevention and treatment of throat diseases such as sore throat, cough, bronchial asthma and allergic asthma in Korea. However, its effect in experimental models remains unknown. To investigate the biological effect of SecSec, we examined cytotoxicity and secretion of inflammatory cytokines on human leukemic mast cell line, HMC-1, stimulated with phorbol 12-myristate 13-acetate (PMA) and calcium ionophore A23187. SecSec by itself had no cytotoxicity on HMC-1. When SecSec (1 mg/ml) was added, the secretion of tumor necrosis factor-alpha $(TNF-{\alpha})$, interleukin (IL)-6, and granulocyte macrophage-colony stimulating factor (GM-CSF) was significantly inhibited about 47.20%, 25.55%, and 46.43%, respectively on PMA plus A23187-stimulated HMC-1 cells. But SecSec did not inhibit IL-8 secretion. These findings may help understanding the mechanism of action of this medicine leading to control activated mast cells on allergic inflammatory condition like asthma.

  • Coronavirus disease 2019 (COVID-19) is a respiratory tract infection caused by a newly emergent coronavirus, that was first recognized in Wuhan, China, in December 2019. Genetic sequencing of the virus suggests that it is a beta coronavirus closely linked to the SARS virus. This disease has non-specific symptoms such as fever, dry cough, sore throat, and gastrointestinal symptoms. This virus can transmit via aerosol and need to droplet precaution to prevent spreading in public areas. Most people with COVID-19 develop only mild or uncomplicated illness. However, about 20% patients require hospitalization, oxygen supply, and intensive care. There is no currently effective treatment available for COVID-19 unresponsive to supportive care. This is review about the recently published epidemiologic, and clinical features, diagnosis, treatment and prevention of COVID-19.

  • Objectives : The purpose of this study is to observe the effect of Cervi Pantotrichum Cornu Pharmacopuncture at Pyesu($BL_{13}$) on Chronic Cough. Methods : 6 patients with the cough treated with Cervi Pantotrichum Cornu Pharmacopuncture at Pyesu($BL_{13}$), then self-conscious complain rates were studied. Results : There are remarkable improvement in self-conscious complain rate about cough, sputum, sore throat, runny nose, chest pain, fatigue which chronic cough patients complained. Conclusions : Cervi Pantotrichum Cornu Pharmacopuncture is effective to chronic cough.
