Investigation of Efficacy of Lidocaine Spray for Sedated Esophagogastroduodenoscopy in Children

  • Purpose: Our aim in this study is to investigate efficacy of topical lidocaine spray for sedated esophagogastroduodenoscopy (EGD) in children. Methods: The endoscopy of children aged between 3-18 years who underwent EGD in our endoscopy unit. Intravenous (IV) midazolam and ketamine were used for sedation. Prior to sedation, endoscopy nurse applied topical lidocaine 10% with pump spray at 1 mg/kg dose in group 1, and distilled water via identically scaled pump spray in group 2, in a double blinded fashion. Results: Sedation was not applied in 24.1% of the cases in topical lidocaine spray group (LS group) and in 5.7% of the cases in distilled water spray group (DS group). Gag reflex was observed in 6.5% of cases in LS group and 33.3% of cases in DS group (p=0.024), increased oral secretion was observed in 9.3% of cases in LS group and 51.7% of cases in DS group (p=0.038), sore throat was observed in 3.7% of cases in LS group and 35.6% of cases in DS group (p=0.019) and the difference was statistically significant. Conclusion: The study showed that topical pharyngeal lidocaine reduces both requirement and amount of IV sedation before EGD in children and sore throat, gag reflex and decreased oral secretion increase.

Isolated Vagus Nerve Involvement of Herpes Infection with Delayed Vocal Fold Paralysis (지연성 성대 마비를 동반한 미주신경에 국한된 Herpes 감염)

  • Vagus nerve palsy caused by herpes virus infection is rare. Here, the author presents a 65-year-old woman with acute onset of right side otalgia and sore throat, followed by delayed vocal fold paralysis on the same side. Vesicles were also found on the posterior wall of ear canal but the tympanic membrane was not involved. Laryngoscopy revealed multiple ulcerative lesions on the pharyngeal and laryngeal mucosa exclusively on the right side. One month later, she noticed dyshonia which turned out right vocal fold paralysis. Skull base to upper chest CT did not reveal local lesion. Three months after finishing the acyclovir, her symptoms were almost gone and vocal fold movement has almost completely improved. Vagus nerve involvement of herpes infection should be considered as a differential diagnosis for patients with herpes zoster oticus with sore throat.

  • Eagle syndrome is a rare condition caused by elongation of the styloid process or calcification of the stylohyoid ligament. Patients with Eagle syndrome typically present with dysphagia, dysphonia, cough, voice changes, otalgia, sore throat, facial pain, foreign body sensation, headache, vertigo, and neck pain. Here we report a case in which the patient initially presented with sore throat, left-sided facial pain, and cough. This case report provides a brief review of the diagnosis and nonsurgical management of this rare syndrome.

Acute Health Effects among Soldiers Involved in the Cleanup Operation Following the Hebei Spirit Oil Spill in Taean, Korea (허베이스피리트호 원유유출의 방제작업에 참여한 군인의 급성건강영향)

  • A collision between a barge and an oil tanker of Hebei Spirit caused the release of an estimated 12,547 tons of light crude oil into the Yellow Sea off the west coast of Taean-gun, Korea on December 7, 2007. This study was aimed to evaluate exposure conditions and acute health effects in soldiers participating in the Hebei Spirit oil spill cleanup activities and the association between these and the nature of the work and use of protection devices. The sample comprised 2,624 soldiers stratified by working area and number of working days. We divided working area into the coast of Taean-gun(highly polluted area) and other areas affected by Hebei Spirit oil spill. And we divided number of working days into 1-7 days, 8-14 days, 15-21 days and above 22 days. Data were obtained via a structured, self-administered questionnaire and included information on working area, type of working, number of working days, use of protective materials, and acute health effects. Acute health effects were classified into 5 groups: neurological, respiratory, dermatologic, ophthalmic and other symptoms. Data analysis was performed using unconditional logistic regression used to compute odds ratios and 95% confidence intervals. On logistic regression analysis, working on the coast of Taean-gun was significantly associated with increased risk of acute health problems: headache, dizziness, nausea, general fatigue, insomnia, flushed face, sore throat, dry throat, runny nose, cough, sputum, skin irritation, sore eyes, injection of conjunctiva, tear, and low back pain. Furthermore soldiers working more than 21 days were significantly associated with increased risk of acute health problems: insomnia, flushed face, sore throat, runny nose, cough, sputum, tear, low back pain and fever. Accordingly, the exposure to the oil and the subsequent cleanup efforts were suggested to inflict acute health problems on soldiers participating in the Hebei Spirit oil spill cleanup activities.

  • Melandrii Herba has been used for tympanitis, sore throat, dysentery and dehumidifying diuretic. From the BuOH fraction of methanol extracts, three ecdysteroids (ecdysterone, ecdysterone 22-acetate and inokosterone) were isolated by column chromatography using Amberlite XAD-4, ODS and Sephadex LH-20 gel and by HPLC method. The structures of these compounds were identified on the basis of spectroscopic methods.

  • Warm disease: Any of various heat disease characterizer by rapid onset and shifts, pronounced heat signs, and a tendency to form dryness and damage yin. Exterior heat sign: exterior heat patterns are characterizer by pronounced heat signs, such as a red sore pharynx and a relatively red tongue with dry fur, the pulse is floating and rapid, cough and the production of thick white or yellow phlegm. If wind-heat evil exist in weifen, it becomes exterior syndrome, and a remedy about that is dispelling wind-heat but when wind-heat evil invades in nasal and throat part so the disease occurs, you need to add relieving sore throat worsens invades in lung it makes disharmony of diffuse in lungs. So a remedy about it is diffuse the lung. disharmony of diffuse in lungs makes metabolic disorder of qi and liquid and humor malfunction therefore it occurs cough and heat-phlegm syndrome. heat from weifen invades the whole of lungs and form lung heat. So a remedy about lung heat is clearing away lung heat, this lung heat makes inevitably bleed in lungs, therefore a remedy in this case is clearing the lung to stop bleeding, or moistening the lung. Exterior heat sign means that exterior syndrome coexists with heat syndrome and it means that a remedy of this syndrome need to mix prescriptions for relieving exterior syndrome and heat-clearing prescriptions to treat this syndrome.

24-Hr Ambulatory Double-probe pH Monitoring in LPR (역류성 후두염의 증상을 가진 환자에서의 24시간 이중 탐침 식도 산도 측정)

  • The term laryngopharyngeal reflux (LPR) refers to the backflow of food or stomach acid back up into the larynx (the voice box) or the pharynx (the throat). Esophagopharyngeal reflux is suggested as an etiologic factor in laryngeal disease. To examine a possible esophageal basis for laryngopharyngeal symptoms, we studied 48 patients with persistent laryngopharyngeal symptoms, and 12 relative control subjects. Patients were evaluated for cervical symptoms by questionnaire and underwent gastrofiberoscopy, fiberoptic laryngoscopy, esophageal manometry and 24-hour ambulatory double-probe pH monitoring. We found LPR in fourteen out of 48 patients with cervical symptoms (29%). The LPR group consisted of nine men and five women. The symptoms that LPR patients complained were throat lump sensation, hoareness, sore throat, throat clearing, chronic coughing and dysphagia in order of frequency, and they were not different significantly from non-LPR patients. The laryngoscopic findings in LPR patients were posterior erythema, laryngeal edema and diffuse erythema, and there was also no significant difference between LPR group and non-LPR group. There was statistically significant correlation between LPR and gastroesophageal reflux (GER). We concluded that there is no pathognomonic symptoms or laryngoscopic findings in diagnosis of LPR, and 24-hour ambulatory double-probe pH monitoring is an essential diagnostic tool in LPR.

Analysis of the Impact on Community Health after Accidental Leak of Hydrofluoric Acid (일개 응급의료센터에서 분석한 불화수소산 대량 누출이 지역사회 건강에 미친 영향)

  • Purpose: The purpose of this study is to verify the influence of a massive hydrofluoric acid spill on community health through patients who claimed to have been exposed. Methods: We analyzed 2459 patients who visited our emergency department with the claim of exposure to hydrofluoric acid, and retrospective analyses were performed. We analyzed changes in numbers of visitors per day from the day of the accidental hydrofluoric acid spill, symptoms presented by the 1924 patients, and general characteristics. Comparisons of symptoms and hematologic characteristics were made between the initially set evacuation zone(1.3 km radius parameters from the spill) and the outer zone. Results: A total of 2,459 patients who claimed exposure visited our ED from 27 September 2012 to 23 October 2012, and there was a significant increase in the number of visiting patients from day 8 of the hydrofluoric acid spill. The most common complaints were a sore throat, 729(37.9%) and no specific symptom with health concern, 547 (28.4%). Statistically significant findings were pulmonary symptoms (p=0.001), nasal symptoms (p=0.001), diarrhea (p=0.023), and skin symptoms (p=0.007). In hematologic study, a statistically significant difference was observed in white blood cell count (p=0.018), creatine phosphokinase (p<0.001), erythrocyte sediment rate (p=0.013), and phosphorus (p<0.001). Conclusion: A significant increase in the number of patients was observed one week after the accidental spill of hydrofluoric acid. The most frequent symptoms were sore throat, headache, cough, and sputum. Statistically significant increase in creatine phosphokinase level and decrease in phosphorus level were noted in patients within the evacuation zone.

  • Background : LPRD(Laryngopharyngeal reflux disease) gives rise to inflammatory change in the pharyngolaryngeal tissue with various otolaryngologic symptoms. Ranitidine, histamine H2receptor antagonists, are currently used as therapeutic medications. However, the efficacy of Ranitidine on LPRD has not been proven yet. Objectives : We intended to analyze the efficacy of the Ranitidine on LPRD. Materials :md Methods : In 20 multicenter, 607 patients with LPR(laryngopharyngeal reflux) symptom were observed to evaluate their symptoms and laryngoscopic findings after 4 weeks, 8 weeks, and 12 weeks of treatment of Ranitidine. Results : The symptom of LPR including globus sensation, sore throat hoarseness, regurgitatioin are improved after 4 weeks $86.2\%,\;8 weeks\;91.5\%,\;12 weeks\;92.9\%$ of Ranitidine treatment and improved after 4 weeks $91.5\%,\;8 weeks\;94.5\%,\;12 weeks\; 97.2\%$ of Ranitidine combined with prokinetics. The rates of sore throat, chronic cough, globus sensation improvement at 8 weeks after treatment are $26.7\%,\;16.7\%,\;16\%$. Conclusion : In patient with LPR, Ranitidine treatment reduces LPR symptoms very effectively.
