안면비대칭자의 3차원 전산단층사진 분석에서 경$\cdot$연조직간 비대칭 정도 차이 (Comparison of asymmetric degree between maxillofacial hard and soft tissue in facial asymmetric subjects using three-dimensional computed tomography)

  • 본 연구는 안면비대칭 환자에서 경조직의 비대칭 정도와 연조직 비대칭 정도의 차이를 3차원적으로 밝히고자 시행되었다. 안면비대칭으로 보이는 성인 남녀 34명을 대상으로 두경부 전산단층사진을 촬영하고 3차원 입체영상으로 재구성한 후 기준평면에 대해 비대칭을 나타내는 5개의 계측항목을 경조직에 설정하고, 이에 대응하는 연조직 계측항목을 각각 설정한 후 3차원 계측을 시행하고 경조직과 연조직의 계측항목간 차이를 비교하였다. 이부편위측과 반대측간의 계측치 차이를 비교한 결과, 경조직과 연조직 모두에서 좌우측 계측치간에 통계적으로 유의한 차이가 관찰되었으며 경조직과 연조직의 비대칭 계측항목을 비교한 결과, 6개의 계측항목 모두에서 통계적으로 유의한 차이를 나타내었다. 이부편위를 나타내는 chin deviation, 하악지와 하악골체를 나타내는 frontal ramal inclination difference, frontal corpus inclination difference 항목은 경조직의 비대칭 정도에 비하여 연조직 비대칭 정도가 작게 나타난 반면, 입술경사를 나타내는 lip cheilion height difference, lip canting은 maxillary height difference, occlusal plane canting보다 크게 나타나 입술부위의 비대칭 정도는 하부 경조직의 비대칭 정도보다 큰 것으로 나타났다. 안면비대칭자에서 경조직과 연조직간 비대칭 정도 차이를 규명한 본 연구 결과는 안면비대칭 평가 시 경조직 외에 연조직 계측항목을 이용한 비대칭 분석도 필요함을 시사하였다.

연조직 및 골 두께가 임플란트 주위 연조직 형태에 끼치는 영향에 관한 임상추적연구 (Influence of soft tissue and bone thickness on the dimensional change of peri-implant soft tissues;A clinical follow-up study)

  • The aim of this study was to investigate the influence of peri-implant soft tissue and bone thickness on the early dimensional change of peri-implant soft tissue. Seventy-seven non-submerged implants of 39 patients which had been loaded more than 6 months were selected for the study. Following clinical parameters were measured; bucco-lingual bone width of the alveolar bone for implant placement before implant surgery; distance between implant shoulder and the first bone/implant contact at the surgery; presence of plaque, probing depth, bleeding on probing, width of keratinized mucosa, mucosa thickness, distance between implant shoulder and peri-implant mucosa, crown margin location at follow-up examination. The results showed that distance between implant shoulder and peri-implant mucosa (DIM) was correlated with probing depth and width of keratinized mucosa (p < 0.05). In addition, mucosa thickness was also correlated with probing depth (p<0.05). However, the bone width of alveolar bone and soft tissue thickness were not found to be correlated with DIM. It is important to understand the meaning of peri-implant tissue dimension in relation to dimensional changes of peri-implant soft tissue which designates appearance of implant-supported restorations. Future study is needed to elucidate the significance of the buccal bone thickness and soft tissue thickness with respect to the change of peri-implant soft tissue margin with the use of an instrument capable of measuring buccal bone thickness directly.

하악골 후방이동시 중안면부 연조직의 변화양상에 대한 임상통계학적 연구 (A Clinical Study of Soft Tissue Changes of the Midface after Mandibular Setback Surgery)

  • Prediction of the soft tissue changes following hard tissue movement is very important from the esthetic view point for patients who have orthognathic surgery. There are many cephalometric analysis of facial bone and soft tissue on the lower lip and chin region but few soft tissue analysis on the midface after mandibular setback surgery. This study was performed to obtain whether the mandibular posterior movement has influence on the midface and the predictable ratio of post-operative measurement values of the soft tissue changes following mandibular setback surgery. Fifteen patients (8 males and 7 females) who had undergone mandibular setback surgery were selected and analyzed the soft tissue movement on the upper lip and the cheek region. Post-operative changes of the soft tissue measurements after mandibular surgery were examined on pre- and post-operative cephalometrics and the ratio of changes were analyzed after drawing the reference line on the face with the barium sulfate solution. The reference lines were perpendicular to the intercanthal line from infraorbital foramen and lateral canthus. The results obtained were as follows : 1. There were tendancy of anterior movement of soft tissue adjacent the nose after mandibular surgery 2. There were incerased tendancy of the amount of anterior movement from the nasal crease to the cheek region. 3. The amount of anterior movement of the soft tissue was larger below the palatal plane compared with above the palatal plane in the cheek region. 4. The upper lip length was increased and moved posterior direction after mandibular setback surgery 5. The lower lip was moved posterior direction by posterior movement of the mandibular structure 6. Soft tissue of the midface around the nose moved anterior direction after mandibular setback surgery but there was no correlation between the amount of mandibular setback and the amount of the soft tissue changes

3차원 CT 영상을 이용한 정상교합자의 안면 연조직 계측 분석 (Facial soft tissue measuring analysis of normal occlusion using three-dimensional CT imaging)

  • 최근 들어 3차원 전산화 단층 촬영(CT, Computed Tomography) 영상을 이용한 진단기법의 개발을 위한 연구가 활발히 진행되고 있으며 여러 분야에서 3차원적인 두개악안면 분석의 필요성이 증대되고 있다. 특히 교정 치료나 악교정 수술 후의 결과에 있어서 안면 연조직의 분석은 필수적이라 할 것이다. 본 연구에서는 정상교합을 가진 성인 남자 12명, 성인 여자 11명의 CT 영상을 촬영하여 개인용 컴퓨터 상에서 V works 4.0 프로그램(Cybermed Inc., Seoul, Korea)으로 3차원 CT 연조직 영상을 재구성한 후에 soft tissue Nasion을 기준 원점으로 하는 3차원 좌표평면의 좌표계를 설정하여 정중선상의 soft tissue Nasion. Pronasale, Subnasale, Upper lip center, Lower lip center, soft tissue B. soft tissue Pogonion. soft tissue Menton 등 8개의 계측점과 양측성인 Eudocanthion, Alare lateralis, Cheilion, soft tissue Gonion, Tragus. Zygomatic point등 총 20개의 재현 가능한 안면 연조직의 계측점을 지정하였으며 V surgery 프로그램(Cybermed Inc., Seoul Korea)을 이용하여 이들 계측점의 3차원적인 좌표와 기준 원점으로부터 각 계측점까지의 Net (${\delta}=\sqrt{{X^2}+{Y^2}+{Z^2}}$) 값의 평균과 표준편차를 구하였다. 안면 연조직 분석의 3차원적인 이해를 돕기 위해 주요 계측점 간의 거리 계측을 시행하였고, 그 결과 Na'-Sn과 En (Rt)-En(Lt)를 제외한 대부분의 계측값에서 남녀간의 유의한 차이가 있었으며, 2차원적인 두부 방사선 규격사진이나 안면 사진으로는 정확한 계측이 어려웠던 Na'-Zy, Na'-Ch, Na'-Go'(facial depth) 등의 정상치도 구하였다. 이상의 자료는 부정교합 환자와 악안면 기형 환자의 3차원적인 진단 및 치료 계획에 참고자료로 사용될 수 있을 것이다.

  • Soft tissue sarcomas are malignant tumors that arise in the extraskeletal connective tissues of the body. And the clinical behavior of soft tissue sarcomas is characterized by a tendency to extensively invade surrounding soft tissues as well as early metastatic dissemination. Difficulties were encounted when soft tissue sarcomas invade the major vessels. There are a few reprots about the reconstruction of the major vessels after resection of soft tissue sarcomas. We have treated 3 cases of the soft tissue sarcomas involving the major artery. After marginal excision of the tumor mass, the involved major artery was reconstructed with Gortex artificial vessel graft and we could salvage the patient's extremity.

  • Purpose : The purpose of this study is to setup a standard treatment protocol in surgical correction of skeletal malocclusion with Angle Class III patients. We asessed the soft tissue changes according to skeletal changes of patients who have undergone orthognathic surgery with bilateral split sagittal ramus osteotomy (BSSRO) and advancing genioplasty. Materials & Methods : The soft tissue change of 9 skeletal Class III patients was assessed after BSSRO and advancing genioplasty. The patient group was skeletal Class III who was surgically treated by BSSRO & advancing genioplasty. The average follow up period is 13 months with the range of 6 and 30 months. All patients have undergone preoperative and postoperative orthodontic treatment. The assessment was devided into two groups. One was antero-posterior relationship and the other was vertical relationship of dimensional changes of soft tissue after orthognathic surgery. Results : In antero-posterior dimensional changes after surgery, the percentage of soft tissue change in comparison to hard tissue was 89%. Vertical ratio after surgery, 86% soft tissue changes were assessed.

전치부 후방이동에 따른 연조직 측모 변화의 두부방사선 계측학적 연구 (A CEPHALOMETRIC STUDY ON THE SOFT-TISSUE PROFILE CHANCES FOLLOWING THE INCISOR RETRACTION)

  • This study was carried out in order to find out the soft tissue profile changes associated with maxillary incisor retraction in Angle's class I malocclusion patients. For this study fifty two female adult patients (Maximum Retraction Group 23, Minimum Retraction Group 29) who received orthodontic treatment were chosen. Following conclusions were obtained by analysing the changes of soft tissue and hard tissue before and after treatment. 1. When considering the mean changes of soft tissue and hard tissue, UP, LIP, Ls, Li (p<0.001), Point B, Si (p<0.01), Point A, Ss (p<0.05) were significant posterior movement in Maximum Retraction Group and UIP (p<0.001), LIP, Ls, Li (p<0.01), Point B, Si (p<0.05) were significant posterior movement in Minimum Retraction Group. 2. When considering the correlations between hard tissue and soft tissue changes, greater correlations were found in Minimum Retraction Group between UIP and Ls (p<0.01), Point A and Ss, UIP and Li, Point B and Si (p<0.05) than Minimum Retraction Group. 3. Correlations (p<0.01) were found between upper incisor retraction and posterior movement of the upper and lower lip in Thin Lip-Thickness Group, whereas no significant correlations were found in Thick Lip-Thickness Group. 4. Mean changes of the soft tissue thickness subsequent to incisor retraction were increased (p<0.01) in upper lip (Ls-Ls'), whereas no changes were found in lower lip.

연부 조직 육종의 예후 인자 (Prognostic Factors of Soft Tissue Sarcomas - A Review of 94 Cases of Soft Tissue Sarcoma -)

  • Many different factors which may affect the prognosis of the soft tissue sarcomas have been reported by many authors ; Generally, tumor size, histologic type, surgical margin, and multi modality therapy therapy as the prognostic factors were reported. The objectives of this retrospective study of soft tissue sarcomas are : 1) to define more clearly prognostic variables that have significant predictive value for disease-free and overall survival ; and 2) to evaluate tumor histologic grade based upon extent of tumor necrosis as a means of stratifying more aggressive soft tissue sarcomas(grade II & III) of the extremities. We treated 94 patients who had soft tissue sarcoma of the extremities and trunk from May 1984 to September 1994(average duration of follow-up was 5 years ranging from 2 months to 10 years) and evaluated the prognostic factors of the soft tissue sarcomas; age, sex, depth, size, location, histologic type and grade, stage, therapy modality, surgical margin, local recurrence and distant metastasis. The results were as follows. 1. The patients with poorer prognosis were over the age of fifty, whose mass was deeply located, size of the mass was over 10cm in diameter, grade III in histology, who had local recurrence, metastasis, and received only surgery. 2. Among these prognostic factors, the most significant prognostic factor was histologic grade base upon extent of tumor necrosis.

Relations between Airway Narrowing and Prevertebral Soft Tissue Swelling after Anterior Cervical Spine Surgery : The Value of Lateral Neck Radiographs

  • Objective : The authors attempted to evaluate the pattern of the airway narrowing due to prevertebral soft tissue swelling after surgery of the anterior cervical spine and their clinical significances using plain cervical X-ray images. Methods : Twenty-four patients undergoing anterior cervical spine surgery were reviewed from January 2004 to December 2005. Postoperatively, in daily basis, lateral radiograph of the neck was obtained in neutral position. We measured the upper airway diameter above and below the epiglottis level and prevertebral soft tissue diameter every day for a week and finally 2 weeks after surgery using their simple lateral cervical X-ray films. Results : Both airway narrowing and prevertebral soft tissue swelling were maximum in postoperative 2 days, and decreased rapidly in postoperative 7 days. Airway narrowing was aggravated postoperatively but slowly decreased as prevertebral soft tissue swelling diminished. But, the severity of airway narrowing showed no clinical correlations with clinical symptom and radiologic severity. Conclusion : Not all patients who show severe airway narrowing and prevertebral soft tissue swelling on their plain cervical X-ray film complain respiratory insufficiency. But, the patients with undergoing anterior cervical spine surgery should be monitored carefully for respiratory insufficiency, especially during several days following operation because both airway narrowing and prevertebral soft tissue swelling become peak at postoperative 2-3 days.

하악전돌증 환자의 악교정 수술후 시간경과에 따른 안모 연조직 변화에 관한 연구 (A STUDY ON CHANGE OF THE SOFT TISSUE FACIAL PROFILE AFTER ORTHOGNATHIC SURGERY IN PATIENTS WITH THE MANDIBULAR PROGNATHISM)

  • This study was performed to evaluate the change of the soft tissue facial profile after mandibular set back surgery during time intervals. For this study, 33 patient, 8 males and 25 females, were selected and their lateral cephalograms were taken and analyzed periodically. Hard and soft tissue changes during postoperative time intervals, correlation between surgical skeletal changes and postoperative soft tissue changes, and prediction for long-term soft tissue changes were established through varying statistical methods. The results were as follow : 1. There were meaningful changes of anteroposterior skeletal position at 6 months and 2 years after mandibular set back by mandibular ramus osteotomy. Two years postoperatively, there was 30%, 32%, 29% relapse on B point, pogonion, menton each. 2. Two years after the mandibular ramus osteotomy, the relative changes of the soft tissue to their osseous counterparts showed 76% on the lower lip and 91% on the pogonion. 3. The movements of the mandibular landmarks in correlation to anteroposterior position of the lower lip and soft tissue of the chin showed to be effective on a long-term basis. 4. Using surgical changes of pogonion, prediction of changes in soft and hard tissue pogonion was useful and the coefficient of determination was 0.46 each and their reliability decreased 2 years postoperatively. 5. The upper lip position after the mandibular set back surgery was somewhat anterior 2 years postoperatively, but that has no statistical meanings.
