The Influential Factors on Parenting Guilt of Mothers using a Day-care Center (보육시설 이용 어머니의 양육죄책감 영향요인 연구)

  • The purpose of this study was to examine the influential factors on parenting guilt of mothers who use a day-care center. To achieve these research goals, the present study 1) analysed the differences in the mother's parenting guilt as the socio-demographic variables of the mother and child, and 2) examined the relative effect size of influences on the mother's parenting guilt with their parenting efficacy, satisfaction on the day-care center, emotional and informational support from teachers, and mother's active interaction with day-care teachers. The participants of the study were 350 mothers whose infants and toddlers were attending day-care centers in Gwangju and Jeollanamdo. The results of this study are as follows. First, the mothers with infants who were younger than 3 years old reported more parenting guilt than the mothers with toddlers. Second, the mothers whose children were the first child reported more parenting guilt than the mothers with children who were the second or the third child. Third, the emotional support from the day-care teacher was the most influential factor on the mother's parenting guilt. This study suggests that the mutual cooperation between mother and day-care teacher are very important to reduce the mother's parenting guilt.

Impact of Registration Program after Hypertensive or Diabetic Patient Detection through Community Partnership on Compliance and Blood Pressure or Blood Sugar Control (고혈압·당뇨병 신규 환자 발견 이후 지역사회 협력을 통한 등록관리가 치료순응도 및 혈압 및 혈당 조절에 미치는 영향)

  • = ABSTRACT = Objectives : Incheon metropolitan city has set a model program of community partnership for hypertensive or diabetic patients detection and follow-up since 2005. This study examines the effects of the model on the compliance and the control of blood pressure or blood sugar level Methods : Telephone-surveys were done for 140 persons selected from 408 patients who were detected newly as hypertensive or diabetic patients at Gangwha-Gun and Seo-Gu in Incheon between January 1st and August 31th in 2006, and finally completed in 110 patients(78.6%). Survey questionnaires included socio-demographic(age, gender, and educational level), health behavioral (smoking, drinking, physical activity, and diet), and therapy-related (registration state, compliance, blood pressure or blood sugar control) variables. Odds ratio and 95% CI were derived from logistic regression model. Results : Registered group exhibited high compliance and well managed blood pressure or blood sugar level. The odds ratio of registration were 5.55(95% CI:1.83~16.89) for compliance and 3.78(95% CI:1.43~9.99) for blood pressure or blood sugar control after adjusting for age, gender, disease, and area Conclusions : It is independently related to compliance and blood pressure or blood sugar control whether the patient is registered or not. To control hypertension or diabetes mellitus in terms of compliance and control state, a community-based registration program through community partnership could be a powerful tool.

The Relationship between the Serum Cytokine and Clinical Improvement in Major Depressive Disorder (주요 우울증에서 혈중 Cytokine과 임상적 호전과의 관계)

  • Object : Currently, the alteration of cytokine system has been known to play an important role in regard to depressive symptom. We focused on the relationship between immunological parameters and clinical improvement in major depressive disorder. Method : Data were collected on 26 patients with major depressive disorder using a 8-week prospective follow-up design. After 8-week treatment period with fluoxetine, patients were classified into a response group and a non-response group according to their psychopathological outcome as evaluated by Hamilton Depression Rating Scale. The differences of the immunological parameters between pre-treatment phase and post-treatment phase were compared among patients. The difference of those was also compared within each phase among them. The relationship between socio-demographic variables, depression, cytokine, mononuclear cells was examined by correlation analysis. Multiple regression analyses were performed to explore the predictors of clinical improvement of major depressive disorder. Result : Pre-treatment levels of IL-$1{\beta}$ in the response group were significantly higher than those in the non-response group. Pre-treatment levels of IL-$1{\beta}$ of all patients and in the response group were positively correlated with pre-treatment monocyte counts. Patients with subsequent remission showed significantly lower IL-6 values at baseline than those with non-response. Post-treatment values of IL-6 did not differ significantly among the patients. The correlation test showed more frequent relations among cytokines and mononuclear cells in the response group than in the non-responder group. Especially, serum level of IL-6 in pre-treatment phase was only significantly correlated with HAMD score after 8-week treatement phase, while other cytokines and mononuclear cells were not. Pretreatment level of IL-6 was of paramount importance in predicting clinical improvement of depressive symptom. Conclusion : The immune system of major depressive disorder patients might dichotomize the patients into subsequent responders and non-responders. Immune system might be of great influence on the clinical improvement of major depressive disorder. The mode of interaction between depression and cellular immune function and the mediators responsible for the cytokine production need to be studied further.

Clinical Characteristic of Anxiety in People with Epilepsy (간질 환자에서 보이는 불안의 임상적 특징)

  • Objectives : Anxiety is a common co-morbid symptom in patients with epilepsy, which can affect the treatment and prognosis of epilepsy patients. This study is designed to compare the characteristics of anxiety between epilepsy patients and normal controls and also analyze them in epilepsy patients by examining both seizure-related and socio-demographic variables. Methods : As cross-sectional study, 80 epilepsy patients were enrolled from January to July 2008. The State-Trait Anxiety Inventory-(STAI) was used to assess the characteristics of anxiety. STAI is composed of transitory episodes of anxiety-(state subscale of STAI ; STAI-S) and stable personality features presenting chronic levels of anxiety-(trait subscale of STAI ; STAI-T). As controls, 113 healthy age-and sex-matched people were included. Results : The mean score of STAI-S and STAI-T were not different in both groups(STAI-S ; p=0.998, STAIT ; p=0.343). Within patients, patients without occupational engagement showed higher STAI-S(p<0.001) and tendency to higher STAI-T(p=0.052). Patients with depression showed higher score in both modalities(STAIS and STAI-T ; p<0.001). Patients with aura showed higher STAI-T(p=0.031). Conclusions : STAI-S and STAI-T was not significantly different between patients and controls. Of 3 factors related to anxiety, higher STAI-T in patients with aura is likely to represent misunderstanding internal and external changes as an aura and worrying about impending seizure. Occupational engagement and depression had relation to both STAI-S and STAI-T and more concern is needed to evaluate the risk of anxiety and manage it appropriately.

Factors affecting on Health-Related Quality Of Life Among Cancer Survivors: Focusing on Gender Difference (암생존자의 건강관련 삶의 질에 대한 영향 요인 -성차를 중심으로)

  • Objectives: The purpose of this article was to evaluate the health related quality of life (HRQoL) of cancer survivors and to identify its predictors according to gender. Methods: The research was conducted with the data for 203 (cancer survivors?) taken from the 6th wave of the Korea National Health and Nutrition Examination Survey and the EQ-5D index score was used for the measurement of the health-related quality of life (HRQoL). The independent variables inluded socio-demographic data, health related factors (survival duration, disability, subjective health recognition), and psychosocial factors (stress, unmet medical needs). The data were analyzed by the t-test, ANOVA, and hierarchical multiple regression analysis. Results: the HRQoL of the female cancer survivors was significantly worse than that of the males in terms of their mobility, usual activities, pain/discomfort and anxiety/depression quality of life. The only statistically significant factor affecting the HRQoL of the male cancer survivors was their subjective health recognition. In the case of the female cancer survivors, the statistically significant factors were their age, subjective health recognition and unmet medical needs. Conclusions: the results of this study showed a different pattern of predictors according to the gender of the cancer survivors. Therefore, gender should be considered when assessing and addressing the individual care needs of cancer survivors, in order to obtain optimal treatment outcomes.

The Relationship between Insomnia and Suicidal Idea Through Resilience (회복탄력성을 통한 주관적 불면의 심각도와 자살사고와의 관계)

  • Objectives : Insomnia may be one of the risk factor for suicidal ideation, but little is known about the mechanism by which sleep disturbances confer risk for suicide. The aim of this study was to investigate examine whether insomnia severity would be associated with resilience and suicidal ideation, and whether resilience would mediate the relationship between insomnia and suicidal ideation. Methods : A total of 432 community-dwelling adults(227 male, 205 female,) completed the self-report questionnaire that covered basic socio-demographic data. To assess the psychological variables, the following instruments were applied: Insomnia Severity Index(ISI), Korean Version of the Connor-Davidson Resilience Scale(K-CD-RISC), Beck Hopelessness Scale(BHOP) and Scale for Suicidal Ideation(SSI-Beck). People with an ISI score of 8 or higher were defined as insomnia. Results : Greater insomnia symptom severity was significantly associated with higher level of suicidal ideation and lower level of resilience, adjusting for hopelessness, age, sex, presence of family members living together, and household income. Additional analysis revealed that disturbance of sleep initiation and disturbance of sleep maintenance were significantly associated with suicidal ideation. Mediation analyses revealed that resilience significantly accounted for the relationship between insomnia symptom severity and suicidal ideation. Conclusions : These findings suggest that the evaluation and control of insomnia and resilience may be needed to reduce the risk of suicide.

Analysis of Area Type Classification of Seoul Using Geodemographics Methods (Geodemographics의 연구기법을 활용한 서울시 지역유형 분석 연구)

  • Geodemographics(GD) can be defined as an analytical approach of socio-economic and behavioral data about people to investigate geographical patterns. GD is based on the assumptions that demographical and behavioral characteristics of people who live in the same neighborhood are similar and then the neighborhoods can be categorized with spatial classifications with the geographical classifications. Thus, this paper, in order to identify the applicability of the geographical classification of the GD, explores the concepts of the geodemographics into Seoul city areas with Korea census data sets that contain key characteristics of demographic profiles in the area. Then, this paper attempt to explain each area classification profile by using clustering techniques with Ward's and k-means statistical methods. For this as as as, this paper employs 2005 Census dataset released by Korea National Statistics Office and the neighborhood unit is based on Dong level, the smallest administrative boundary unit in Korea. After selecting and standardizing variables, several areas are categorized by the cluster techniques into 13, this paps as distinctive cluster profiles. These cluster profiles are used to cthite a short description and expand on the cluster names. Finally, the results of the classification propose a reasonable judgement for target area types which benefits for the people who make a spatial decision for their spatial problem-solving.

A Study on the Relationships between Occupational Therapists' Burn-out, Professional Self-concept, and Self-efficacy (작업치료사의 소진과 전문직 자아개념 및 자기효능감 간의 관계)

  • The purpose of this study was to examine the degree of occupational therapists' burn-out, professional self-concept, and self-efficacy and also their socio-demographic characteristics and to verify the effects of professional self-concept and self-efficacy on their burn-out. The subjects of this survey were 138 occupational therapists who perform occupational therapy. From October 13th till November 2nd, 2011, a structuralized questionnaire was used to collect data. The results of this study were follows: First, the occupational therapists' mean score of burn-out was 2.12(range: 0-6), and the mean score of professional self-concept and self-efficacy were 2.64(range: 1-4) and 3.56(range: 1-5). Second, according to the general characteristics of the occupational therapists, burn-out showed significant difference by treatment hours per day, duty to work on Saturdays, aptitude and religion. Third, professional self-concept had positive correlation with self-efficacy. And professional self-concept and self-efficacy showed negative correlation with burn-out. Fourth, chief variables affecting occupational therapists' burn-out were professional self-concept and religion. Therefore it is necessary to reduce occupational therapists' burn-out and improve the services of their occupational therapy qualitatively by developing programs to enhance their professional self-concept.

The Effects of Interpersonal Relationships on Job Satisfaction of Home-visiting Care Helpers (방문 요양보호사의 대인관계가 직무만족에 미치는 영향)

  • This study was to investigate the effects of interpersonal relationships on job satisfaction of home-visiting care helpers and the category of interpersonal relationships was subdivided into elderly clients, guardians, colleagues and directors considering the person-to-person nature of home-visiting care helper's job characteristics. A group of 336 care helpers who engaged in home-visiting care service providing facilities in Daejeon City was selected as a research subject, and the data were analyzed with SPSS 22.0 program using reliability, frequency, correlation and hierarchical multiple regression. The results for the final analysis revealed that the effect of socio-demographic features was not significant while, among characteristics of job performances, work priority, turnover experience, and career choice reasons turned out to have significant impacts on job satisfaction. In particular, interpersonal relationships except the one with guardians were found to be the most influential variables and the effects of those relationships on job satisfaction were significant. The influencing power was stronger in relationships with director, elderly clients, and colleagues in order, and the job satisfaction level was higher as they were in better relationships with director, elderly clients, and colleagues. The results of this research shows that it is necessary to set an atmosphere where harmonious interpersonal relationships can be formed inside and outside of facilities in order to improve job satisfaction of home-visiting care helpers.

An indicator for managing the regional variations in approval rates of long-term care (LTC) service (지역별 장기요양 인정의 차이 관리지표 개발)

  • This study develops an indicator to manage regional variations of approval rates for long-term care (LTC) service. We used LTC insurance data for grade assessment that include 433,155 applicants from 227 LTC centers across Korea in 2015. The approval rate for each center was defined as the proportion of the numbers of approved applicants out of all applicants. We assumed that the approval rates depended on the characteristics of applicants. We estimated the 'standard' approval rates from a multiple linear regression analysis using the characteristics of applicants as independent variables. The difference between the observed and the standard rates was then defined as an indicator for deviation. A center having a large difference could be considered as a center with a potential error in grade assessment. We also examined if the characteristics of investigators affected the approval rates. We found that the socio-demographic characteristics of applicants and reapplication rate for LTC grade were independent factors affecting the approval rates. Centers having the management indicator values falling outside the middle 95% of the distribution were identified as centers with an error in grading. We expect that this study will contribute to enhancing reliability and equity in LTC grading.
