Intervention Mapping 설계를 통한 중학생 대상 흡연음주예방 교육프로그램 개발 (Development of a Smoking and Drinking Prevention Program for Adolescents using Intervention Mapping)

  • Objectives: We describe the development of a smoking and drinking prevention program for adolescents, using intervention mapping. Methods: The study sample consisted of 1,000 high school second-grade students from 6 high schools in Seoul. The PRECEDE model was applied for the needs assessment. We carried out a social diagnosis by assessing the factors such as the quality of life, happiness level, and satisfaction with school life; an epidemiological diagnosis on the perceived health status, stress levels, and priority of health issues; a behavioral diagnosis on the smoking and drinking rate and the intention to smoke and drink; and an educational diagnosis on knowledge, beliefs, attitudes, self-efficacy, outcome expectations, social norms and life skills. Results: The development process included a needs assessment, identifying factors that influence smoking and drinking among adolescents. Intention, knowledge, perceived norms, perceived benefit, perceived cost, perceived susceptibility, self-efficacy, and life skills were identified as determinants. Three performance objectives were formulated to describe what an individual needs to do in order to avoid smoking and drinking. Subsequently, we constructed an intervention matrix by crossing the performance objectives with the selected determinants. Each cell describes the learning objectives of the smoking and drinking prevention program. The program used methods from the transtheoretical model, such as consciousness raising, outcome expectations, self-reevaluation, self-liberation, counterconditioning, environmental reevaluation, and stimulus control. The program deals with the effects of smoking and drinking, self-improvement, decision making, understanding advertisements, communication skills, social relationships, and assertiveness. Conclusions: By using the process of intervention mapping, the program developer was able to ensure a systematical incorporation of empirical and new data and theories to guide the intervention design. Programs targeting other health-related behavior and other methods or strategies can also be developed using this intervention mapping process.

뇌졸중 기능손상 노인의 가족수발자에 대한 사회복지실천 개입의 효과에 관한 연구 (A Study on the Effectiveness of Social Work Intervention for the Family Caregivers of Older Persons with Stroke)

  • 본 연구는 뇌졸중 노인의 가족 수발자들을 대상으로 집단 프로그램과 개별상담을 제공하고 이러한 사회복지실천에 입각한 개입이 간병 부담을 완화하는 효과가 있는 지 검증하였다. 12명의 가족 수발자들로 실험집단과 통제집단을 설정하고 사전 검사 후 실험집단에 대해서만 8회에 걸쳐 개입을 실시하고 다시 두 집단에 대하여 사후검사를 실시하여 개입 효과를 확인하였다. 실험집단에 대해서는 3개월 후에 추후검사를 실시하여 개입 효과의 지속 여부를 검증하였다. 통계분석은 비모수통계방법인 Mann Whitney test, Wilcoxon test를 사용하였다. 연구결과는 다음과 같다. 첫째, 본 연구에서 실시한 개입은 수발자의 간병 부담, 소외감을 완화시켜 주는 효과가 있었다. 간병자의 자존감과 간병에 대안 자진감, 노인과의 관계, 정서적 지지도 향상되었다. 그러나 간병자의 우울, 전반적인 삶의 만족, 공적 서비스의 사용은 변화가 없었다. 둘째, 추후검사를 통해 개입 효과의 추이를 확인한 결과 간병부담, 자존감, 간병에 대한 자신감, 정서적 지지에 대한 개입효과는 유지된 것으로 나타났다. 그러나 노인과의 관계는 악화되었으며 소외감도 증가한 것으로 조사되었다. 셋째, 본 연구에서 실시한 사회복지실천 개입에 대한 참여자들의 만족도, 유용하다고 느끼는 정도는 매우 높았다. 참여자들은 특히 지지적 세션에 의해 많은 도움을 받은 것으로 보인다. 이러한 연구 결과를 바탕으로 사회복지실천 개입의 확대, 자조집단의 조성, 지지적 세션을 잘 이끌어갈 것, 간병 받는 노인의 프로그램 참여에 대한 배려, 수발자들의 내재된 분노나 죄의식을 다루어 줄 것 등의 실천적 함의가 제시되었다.

Social Intelligence Counseling Intervention to Reduce Bullying Behaviors Among Thai Lower Secondary School Students: A Mixed-method Study

  • Objectives: To develop and investigate the effectiveness of an integrative counseling intervention for enhancing social intelligence and reducing bullying behaviors among lower secondary school students in Bangkok, Thailand. Methods: An interventional mixed-method design was employed in 2 phases. Phase 1 involved the development of a qualitative method-based integrative counseling program from key informants using the eclecticism technique. In phase 2, a randomized controlled trial with a wait-list control was conducted and qualitative research was performed with students who demonstrated bullying behaviors. Demographic data, Social Intelligence Scale (SIS) scores, and Bullying-Behavior Scale (BBS) scores were collected at baseline. Changes in SIS scores and qualitative findings obtained from in-depth interviews were examined after counseling ended, and BBS scores were collected again 1 month later. Results: The developed social intelligence counseling program included eight 1-hour weekly sessions consisting of 3 components: (1) social awareness, (2) social information processing, and (3) social skills. After receiving this intervention, scores for the SIS overall (p<0.001) and all of its components (p<0.05) were significantly enhanced in the experimental group compared to the control group. Moreover, the mean BBS scores in the experimental group significantly decreased 1 month after counseling (p=0.001). With regard to the qualitative research results, the experimental students demonstrated improvements in all components of social intelligence. Conclusions: The results indicated that a preventive counseling program may enhance social intelligence, decrease bullying behaviors among lower secondary school students, and prevent further incidents of school violence. However, further studies in various population subgroups should also be performed.

보건소 중심의 방문보건.재가복지 통합시범사업 성과 (Outcome Assessment of a Demonstration Project on Integration of Home Visiting Health Care and Social Welfare Services)

  • Purpose: The objectives of this study was to measure the outcomes of interventions on the health and social welfare of the elderly in a rural community in Korea. The project involved integrating services of one public health center with that of one social welfare agency, which were under different administrative structures. Method: A single group pretest-posttest design was used for this research. Seventy-five elderly residents living alone in a rural community participated in the study. All of them had coverage of free basic medical care and social welfare services by the government. Major activities for the intervention included: developing partnerships among community leaders/institutes; forming committees of community residents; educating care providers and volunteers; developing 8 integrated service programs and instruments; and organizing the networks. The 20-month intervention was care-managed by a public health nurse whom collaborated with social worker, and was assisted by volunteers. The t-test was utilized to analyze the outcome variables including the elder's health, social welfare and quality of life. A major limitation of this study was the lack of a control group. Results: The outcome of the intervention was shown by improved elder's health, social welfare needs, and quality of life. Integrating the services of public health centers with those of social welfare agencies is an effective way to improve the health of the elderly in the community. Conclusion: Developing community capacity with such integrated services will pay an important role in improving the health of the elderly who live alone.

취약계층 노인의 우울예방을 위한 지역사회기반의 통합프로그램 개발 및 효과검증 (A Community-Based Integrated Preventive Program of Depression and Its Effectiveness in Caring for Vulnerable Elderly)

  • The purpose of the study was to develop an integrated prevention program to strengthen elders self-care capability and to examine its effectiveness on their psychological condition. This study used one group pre- and post-test design. Subjects were 85 elderly residents (over 65 years of age) who lived alone, and received free basic medical care and social welfare services in a rural community in Korea. Subject eligibility criteria for this study were to an elders who 1) is not currently taking any anti-depressant medication 2) is able to communicate, and 3) agrees to participate in this study. The integrated program was composed of horticulture, reminiscence, and friendship activities. Twelve sessions were provided for 12 weeks in community-based partnerships to achieve better outcomes. The intervention was case-managed by a public health nurse and aided by six volunteers. The main outcome variable was depression, which was assessed by using 15 items selected from the Geriatric Depression Scale-short form Korean version. Socio-demographic characteristics, functional status, and satisfaction with social support were used as covariates. Results showed that there was a significant intervention effect at post-intervention time point compared to pre-intervention time point(E.S. 0.94). Multiple linear regression analysis showed significant interaction effects between intervention and satisfaction with social support. These findings must be interpreted within the context that an effects of an integrated program could be more synergistically increased when social support factor is considered in the program. A community-based integrated prevention program of depression is effective for vulnerable rural elderly. It is suggested that randomized controlled trials within community setting for better methodological strength as well as multi-level outcomes on community need to be conducted in future.

발표상황에 대한 가상현실노출과 웹기반 인지적 개입의 통합 프로그램 효과 검증 (Effect of Virtual Reality Exposure and Web-based Cognitive Intervention Integrated Program on Social Anxiety Disorder)

  • 본 연구에서는 웹기반 인지재구조화 교육과 결합된 VR 노출 프로그램이 사회 불안 감소에 미치는 영향을 확인하였다. 실험 집단(n=12)은 웹기반 인지 재구조화 교육과 가상현실노출 프로그램(VRET)에 4회기 참석하였고, 비교집단(n=15)은 동일한 가상현실노출 프로그램(VRET) 이외에 연설기법에 대한 교육을 받았다. 두 집단 모두 프로그램 참여 전,후에 자기보고식 증상심각도 척도와 암묵적 해석 편향을 측정하였으며, 4주 후에 추후평가를 실시하였다. 프로그램 직후 실험집단과 비교집단 모두 사회적 불안 수준이 감소하였으나, 4주가 지난 후 실험집단에서만 긍정적인 해석 편향이 증가하였고, 부정적인 해석 편향이 감소하였으며, 감소된 사회불안 수준이 유지되었다. 이러한 결과는 VRET 치료효과를 증진시키기 위해서 자신의 생각에 대한 객관적 점검을 시도해 볼 수 있는 인지적 개입이 결합될 필요가 있음을 시사한다.

Empowering Rural Housewives in Iran: Utilizing the Transtheoretical Model to Increase Physical Activity

  • Objectives: Rural housewives are integral to household management and family care, yet their sedentary lifestyles present significant health risks. This study used the transtheoretical model (TTM) to investigate strategies that encourage and maintain regular exercise habits among rural housewives. Methods: A semi-experimental study was conducted in 2021 with 114 housewives aged 30 to 59 who attended rural health centers in Gorgan, Iran. Participants were randomly assigned to 1 of 2 groups. Data collection involved a validated questionnaire that gathered demographic information and constructs of the TTM. The intervention group participated in a comprehensive educational program, which included four 60-minute sessions. Data were collected again 6 months post-intervention and analyzed using descriptive and inferential statistics in SPSS version 21. Results: The study encompassed women with an average age of 39.75±6.05 years, the majority of whom had educational levels below a diploma, and over 90% were married. We observed strong correlations between the processes of change, self-efficacy, and decisional balance. At the outset, there were no significant differences in demographics or model structures between the 2 groups. However, 6 months post-intervention, the intervention group exhibited statistically significant differences in the mean scores of model structures, stages of change, and body mass index (p<0.05). Conclusions: This study highlights the importance of physical activity training for rural housewives. The findings suggest that the educational intervention, which utilized the TTM, significantly impacted the participants' model structures and their stages of change.

대사증후군 상태 개선을 위한 생활습관 중재프로그램의 프로그램 이론 평가 (Program Theory Evaluation of a Lifestyle Intervention Program for the Prevention and Treatment of Metabolic Syndrome)

  • Objectives: The purpose of this study is to evaluate the program theory of a lifestyle intervention program for the prevention and treatment of metabolic syndrome. Methods: The program evaluated is a tailored intervention for multiple health behavior associated with metabolic syndrome which is informed by theoretical constructs from the Intervention Mapping and Transtheoretical model. The program components include one-to-one health counseling, a self-management handbook, and a health diary. To evaluate program impact theory we examined the logic of program goals and objectives, intervention methods and strategies, and the theoretical constructs of program materials through document review and matrix building. Results: This evaluation has found that the intervention program applied social cognitive theory constructs to design intervention methods and strategies in addition to the Transtheoretical model: self-monitoring for goal setting and monitoring skill, outcome expectation for the benefits of health behavior change, and interaction with environment for observational learning through modeling. While the intervention addresses multiple determinants and behaviors, it is limited to an individual level and lacks social and environmental approaches. Following the Transtheoretical framework, the contents of the intervention materials were developed utilizing consciousness raising as a main strategy for earlier stages of change, and counterconditioning and stimulus control for later stages of change. Conclusion: Program theory evaluation can be a process of enhancing program validity. It would also be necessary for providing basis for efficient program implementation. When comparisons of program theory between similar programs are possible, program theory and validity will be strengthened when comparisons of program theories between similar programs are possible.

영아기 자녀를 둔 어머니를 위한 양육지지 집단 프로그램 개발 및 평가 (Development and Evaluation of the Parenting Support Group Program for Mothers with Infants)

  • Purpose: The purpose of this study is to evaluate the changes in parenting competency, parenting environment, and parenting stress by developing and applying parenting support group program based on the social support intervention model for mothers with infants. Methods: In this experimental study, mothers with infants of 8~12 weeks old were recruited. 23 mothers were selected for analysis (11 in the experimental group and 12 in the control group). The parenting support group program was provided to the experimental group for seven sessions with 120 minutes per session. Results: The experimental group showed that the parenting competency and parenting environment were significantly increased and the parenting stress was significantly decreased immediately after intervention. In the case of parenting environment, it was found to have a lasting effect until the point of further investigation. Conclusion: The results indicate that the program as a social support intervention strategy for mothers rearing up infants is effective intervention in a universal context in the community.
