백서 미세 혈관 문합 방법에 따른 동맥류 발생에 관한 연구 (DEVELOPMENT OF ANEURYSM AFTER MICROVASCULAR ANASTOMOSIS ON RAT SMALL VESSELS WITH DIFFERENT METHODS)

  • Purpose : This study was undertaken to observe the occurrence of the vessel aneurysms according to several different methods of microvascular anastomosis. Mterials & methods : Forty Sprague-Dawley rats, weighing $180{\sim}200$ grams, were used for this experiment. The rats were divided into 4 groups. Group 1 (10 rats): The adventitia was trimmed off 5mm from the cut edge each and 20 arterial anastomoses were performed using 8 to 10 interrupted 9-0 polypropylene ($Prolene^{TM}$, Ethicon, U.K.) suture. Group 2 (10 rats): The adventitia was trimmed off as in group 1. Twenty arterial anastomoses were performed using continuous 9-0 polypropylene($Prolene^{TM}$, Ethicon, U.K.) suture. Group 3 (10 rats): The adventitia was stripped only 1mm from the cut edge each but not removed,. Twenty arterial anastomoses were performed using 8 to 10 interrupted 9-0 polypropylene($Prolene^{TM}$, Ethicon, U.K.) suture. Group 4 (10 rats): The adventitia was handled as in group 3. Twenty arterial anastomoses were performed using 9-0 polypropylene($Prolene^{TM}$, Ethicon, U.K.) suture. The arteries of the animals in all groups were explored at 28th days. We examined patency, presence of an aneurysm, other vascular abnormalities and microscopically observed the aneurysms with H&E and Van-Gieson stains. Result : 1. Patency rate was 80% in group 1, 95% in group 2, 85% in group 3 and 90% in group 4, respectively. 2. Aneurysm occurred 20% in group 1, 5% in group 2, 5% in group 3 and 5% in group 4, respectively. 3. There was no other vascular abnormalities in each group. 4. Infection rate was 5% in group 1, 0% in group 2, 20% in group 3 and 15% in group 4, respectively. 5. In the histopathological findings, we observed partially necrotic changes, loss and fragmentation of outer elastic lamella of smooth muscle in media and the proliferation of hyperplastic subintima. A lot of inflammatory cells were infiltrated in hyperplastic intima. Conclusions : On the basis of these observation, we could state that there were little differences in the occurrence of aneurysms according to different anastomotic suture methods.

위와 소장에 생긴 위장관 간질종양의 임상병리학적 비교 (Comparative Study of the Clinicopathologic Characteristics between Gastrointestinal Stromal Tumors Arising from the Stomach and Small Bowel)

  • 목적: 위와 소장에 발생한 위장관 간질종양의 임상병리학적 특징을 비교하여 예후 인자 및 적절한 치료의 방침을 알아보고자 하였다. 대상 및 방법: 대진의료재단 분당제생병원에서 1998년 8월부터 2006년 5월까지 위장관 간질종양으로 진단된 38명의 환자 중 분석 및 추적 조사가 가능했던 29명의 환자에서 임상 양상을 조사하고, 면역조직화학적 염색을 시행하였으며, NIH 합의안에 따라 위험도를 분류하여 각각 위와 소장에서 발생한 위장관 간질종양을 비교하였다. 결과: 위와 소장에 발생한 위장관 간질종양의 임상병리학 적 차이 및 재발 양상의 차이는 없었으며, NIH 위험도 분류에 따라 나눈 고위험군과 저위험군 간에 재발의 차이는 있었다(P=0.030). 결론: 위장관 간질종양에서 원발 부위인 위와 소장간에 임상병리학적 양상이나 예후에 통계학적으로 유의한 차이는 없었으나, NIH 분류에 의한 고위험군에서는 재발 가능성이 높으므로 치료 지침에 따른 적절한 추적관찰이 필요하며, 앞으로 여러 기관의 예를 통합한 큰 모집단을 대상으로 지속적인 연구를 시행하여 국내 실정에 맞는 정확한 진단 기준 및 치료지침이 만들어져야 될 것으로 사료된다.

The Significance of SDF-1α-CXCR4 Axis in in vivo Angiogenic Ability of Human Periodontal Ligament Stem Cells

  • Periodontal ligament stem cells (PDLSCs) are multipotent stem cells derived from periodontium and have mesenchymal stem cell (MSC)-like characteristics. Recently, the perivascular region was recognized as the developmental origin of MSCs, which suggests the in vivo angiogenic potential of PDLSCs. In this study, we investigated whether PDLSCs could be a potential source of perivascular cells, which could contribute to in vivo angiogenesis. PDLSCs exhibited typical MSC-like characteristics such as the expression pattern of surface markers (CD29, CD44, CD73, and CD105) and differentiation potentials (osteogenic and adipogenic differentiation). Moreover, PDLSCs expressed perivascular cell markers such as NG2, ${\alpha}-smooth$ muscle actin, platelet-derived growth factor receptor ${\beta}$, and CD146. We conducted an in vivo Matrigel plug assay to confirm the in vivo angiogenic potential of PDLSCs. We could not observe significant vessel-like structures with PDLSCs alone or human umbilical vein endothelial cells (HUVECs) alone at day 7 after injection. However, when PDLSCs and HUVECs were co-injected, there were vessel-like structures containing red blood cells in the lumens, which suggested that anastomosis occurred between newly formed vessels and host circulatory system. To block the $SDF-1{\alpha}$ and CXCR4 axis between PDLSCs and HUVECs, AMD3100, a CXCR4 antagonist, was added into the Matrigel plug. After day 3 and day 7 after injection, there were no significant vessel-like structures. In conclusion, we demonstrated the perivascular characteristics of PDLSCs and their contribution to in vivo angiogenesis, which might imply potential application of PDLSCs into the neovascularization of tissue engineering and vascular diseases.

심방 이뇨호르몬의 분비조절에서 Ca2+이 protein kinase C 활성화에 미치는 영향 (Effects of Ca2+ on protein kinase C activation in atrial natriuretic peptide regulation)

  • Atrial natriuretic peptide(ANP) is a hormone with potent natriuretic, diuretic and relaxing properties on vascular smooth muscle. Specific chemical modulator in response for the ANP secretion has not been found yet. Therefore, we have investigated the role of $Ca^{2+}$ responsible for the regulation of ANP induced by protein kinase C(PKC) on mechanically stretch-induced ANP secretion in the rat atria. The results obtained were as follows ; 1. ANP secretion and ANP concentration were increased to more in $Ca^{2+}$-free buffer than in the Kreb-Henseleit buffer on mechanically stretch-induced ANP secretion(p < 0.05), but extracellular fluid translocation(ECF) was not significant. Phorbol 12-myristate 13-acetate(PMA, $10^{-7}M$) induced ANP secretion and ANP concentration in $Ca^{2+}$-free buffer shown to more accentuate on mechanically stretch-induced ANP secretion than in the $Ca^{2+}$-free buffer(p < 0.05), but ECF translocation was not significant. 2. In the presence of ryanodine($3{\times}10^{-6}M$), PMA($10^{-7}M$) induced ANP secretion and ANP concentration in the Kreb-Henseleit buffer were shown to more increase on mechanically stretch-induced ANP secretion than in the ryanodine($3{\times}10^{-6}M$) with the Kreb-Henseleit buffer(p < 0.05), but ECF translocation was not significant. 3. In the presence of ryanodine($3{\times}10^{-6}M$), PMA($10^{-7}M$) induced ANP secretion and ANP concentration in the $Ca^{2+}$-free buffer was shown to more increase on mechanically stretch-induced ANP secretion than in the ryanodine($3{\times}10^{-6}M$) with the $Ca^{2+}$-free buffer on mechanically induced ANP secretion(p < 0.05), but ECF translocation was not significant. 당뇨병 백서의 복부 대동맥 운동성에 대한 Vit C 의 보호효과 (The Protective Effects of Ascorbic Acid on the Vascular Motilities in Streptozotocin- induced Diabetic Rat)

  • 배경: 당뇨병 환자에서 사망률과 이환률의 원인은 70%이상 혈관계의 합병증에 기인한다. 이러한 합병증은 혈관 내피세포 이완 작용 이상과 연관되어 있으며 이는 oxygen free radical의 직접적인 독성으로 추정되어 본 연구는 당뇨를 유발시킨 백서 복부 대동맥 운동성에 대한 Vit C의 보호효과를 연구 목적으로 한다. 대상 및 방법: 백서 60마리를 실험군(n=33)과 대조군(n=27)으로 나누고 실험 군은 streptozotocin을 투여하여 당뇨를 유발시켰다. 각각 실험 군과 대조군을 ascorbic acid를 투여한 군과 투여하지 않은 군으로 세분한 후 ascorbic acid투여 직후, 6주, 9주, 12주후의 복부 대동맥 혈관근육의 운동성을 측정하였다. 결과: 대조군의 경우 6주째 복부 대동맥 절편에서 acetylcholine투여 후 정상적인 이완반응이 나타났으나 실험군의 경우 현저히 저하됨이 관찰되었다. 9, 12주 째 절편에서는 실험군 중 ascorbic acid투여군에서 acetylcholine에 의한 이완반응이 거의 대조군에서의 결과와 일치할 정도로 회복되었다. 결론: 이상의 결과로 당뇨병을 유발한 백서에서 내피세포 의존적인 장애가 나타남을 확인할 수 있었으며 이러한 장애는 ascorbic acid의 투여로 회복됨을 알 수 있었으며 그 효과는 항산화 작용에서 비롯된 것으로 생각되므로 ascorbic acid가 당뇨환자의 혈관성 질환에 대해 보호적 효과를 보일 수 있을 것으로 사료된다.

상·하향 계단보행을 위한 근전도 신호 기반 보행단계 인식 (Gait Phase Recognition based on EMG Signal for Stairs Ascending and Stairs Descending)

  • 동력의족은 하지 절단 환자나 다리근력이 부족한 사람들의 보행 보조를 위해 사용된다. 동력의족의 자연스러운 구동을 위해 선 보행단계가 잘 분류되어야 한다. 물리센서를 이용하여 보행단계를 분류하는 기존 연구는 동력의족이 사전에 훈련된 보행속도로만 재현되는 단점이 있다. 따라서 본 논문에서는 물리센서를 사용하지 않고, 근전도 신호만을 이용하여 오르막, 내리막 계단보행을 각각 4단계로 분류하는 방법을 제안한다. 근전도 신호를 RMS, VAR, MAV, SSC, ZC, WAMP 특징으로 산출하여 LDA(Linear Discriminant Analysis) 분류기를 통해 보행단계를 인식한다. 훈련 단계에서는 AHRS센서를 이용하여 무릎각도 변화에 따른 보행단계 범위를 생성한다. 실험 결과, 선행 연구의 경우 오르막 보행에서 평균 58.5%, 내리막 보행에서 35.3%의 정확도를 보인다. 반면, 제안하는 방법은 오르막 보행에서 평균 85.6%, 내리막 보행에서 69.5%의 인식률을 보인다. 또한, 본 연구를 통해 개별 근육 별 보행단계 평균 인식률을 분석하였다.

Lack of Influence of the ACE1 Gene I/D Polymorphism on the Formation and Growth of Benign Uterine Leiomyoma in Turkish Patients

  • Uterine leiomyomas (ULM), are benign tumors of the smooth muscle cells of the myometrium. They represent a common health problem and are estimated to be present in 30-70% of clinically reproductive women. Abnormal angiogenesis and vascular-related growth factors have been suggested to be associated with ULM growth. The angiotensin-I converting enzyme (ACE) is related with several tumors. The aim of this study was to identify possible correlation between ULM and the ACE I/D polymorphism, to evaluate whether the ACE I/D polymorphism could be a marker for early diagnosis and prognosis. ACE I/D was amplified with specific primer sets recognizing genomic DNA from ULM (n=72) and control (n=83) volunteers and amplicons were separated on agarose gels. The observed genotype frequencies were in agreement with Hardy-Weinberg equilibrium ($x^2=2.162$, p=0.339). There was no association between allele frequencies and study groups ($x^2=0.623$; p=0.430 for ACE I allele, $x^2=0.995$; p=0.339 for ACE D allele). In addition, there were no significant differences between ACE I/D polymorphism genotype frequencies and ULM range in size and number ($X^2=1.760;$ p=0.415 for fibroid size, $X^2=0.342;$ p=0.843 for fibroid number). We conclude that the ACE gene I/D polymorphism is not related with the size or number of ULM fibroids in Turkish women. Thus it cannot be regarded as an early diagnostic parameter nor as a risk estimate for ULM predisposition.

배(이(梨)) 추출물과 고경(枯梗), 행인(杏仁) 배합제제의 용량 및 기간별 투여가 Ovalbumin으로 유발된 천식 생쥐에 미치는 영향 (The Effects of Pear Phenolic Compound and Herbal Drugs According to the dose and Duration on the Respiratory System of Asthma Mice Induced by Ovalbumin)

  • Objectives: Oriental pear was used as traditional remedies for respiratory diseases like cough and asthma etc. The purpose of this study was to observe the effects of preventing asthma by the combination of phenolic compound extracted from pear and herbal drugs. Methods: In order to study the effects of preventing asthma by the combination of phenolic compound extracted from pear and herbal drugs(Platycodon grandiflorum, Prunus armeniaca) on allergic asthma, mice were pre-treated by oral administration of the solution before antigen sensitization for 10 days and 20days. And 2 days later, mice were actively sensitized with a subcutaneous injection of ovalbumin and 13 days later, they were provoked with ovalbumin aerosols. The experimental groups were divided 6groups(10d1P, 10d2P, 10d4P, 20d1P, 20d2P and 20d4P) by meditation quantity and period. We measured isometric contractile responses to acetylcholine(ACh) and KCl in the isolated tracheal smooth muscle(TSM), IL-4, eosinophil and lymphocyte in bronchoalveolar lavage fluid(BALF), IgE in serum, WBC, RBC and hemoglobin in blood. Results: Contractile responses of TSM to ACh, the sensitivity of TSM to Ach and the maximal contractile response of TSM to KCl were decreased by direct proportion of meditation quantity. Eosinophil and IL-4 level in BALF were more significantly decreased, and IgE level in serum was more significantly increased in 10d4P and 20d4P group than the control group. Conclusions: Based on the above results, it is assumed that oral administration of the combination of phenolic compound extracted from pear and herbal drugs(Platycodon grandiflorum, Prunus armeniaca) can help the preventing effects of allergic asthma.

복부 MDCT 영상으로부터 간혈관 자동 추출 알고리즘 (Auto-Segmentation Algorithm For Liver-Vessel From Abdominal MDCT Image)

  • 간이식 수술을 함에 있어서 간 내부의 혈관의 형태를 알고 시작하는 것이 수술의 성공률을 매우 높일 수 있다. 본 논문은 조영제를 투여한 정상 환자의 복부 MDCT를 이용하여 얻어진 영상을 다른 여러 장기부분은 제거하고 간 영상만을 추출한 후 간 내의 혈관들의 기본형태를 파악하여 몇몇 구조단위들을 만들고 Morphological filtering을 이용하여 주요 혈관인 좌, 우, 중간정맥을 찾아낸다. 중간정맥을 기준으로 간 실질을 절단하여 절단된 부분의 크기를 예측하고 수술전에 전체 상황을 파악하기 위한 연구이다. 간의 추출 방법은 명암값의 범위와 분포 샘플링 과정에 의한 명암값 분포비율을 가지고 배경과 근육층을 제거하였다. 간의 대략적인 위치 정보와 몸통의 위치정보를 이용하여 단위 매쉬영상과 일치되는 영상을 찾은 후 결과 영상을 조합하고 8방향 연결성을 이용하여 확장하고 화소간의 채우기 과정을 거쳐 최종적인 간영상을 추출하였다. 추출된 간 영상에서 간 영역의 특징적인 명암값과 다양한 구조단위를 가지고 Morpological Filtering을 수행 한 후 나타난 결과들을 조합하여 만들어진 영상에서 각 슬라이스 별로 크기순으로 큰 부분들을 남겨두어 굵은 혈관만을 추출하였다. 추출된 영상들을 3D로 구성 시 자연스럽게 보여지도록 인터폴레이션을 수행한 후 3D Reconstruction 을 수행하여 3D 형태의 간 혈관을 보고 중간 정맥을 파악하여 간 실질의 절단 위치를 예측하게 된다. 절단되어진 간 실질의 크기를 확인하고 계산에 의하여 수술 성공 가능성을 파악할 수 있다.

  • Endothelin (ET) is a 21 amino acid peptide with multifunctional effects on the vasculature as well as a variety of other cell types such as respiratory, gastrointestinal, urogenital, endocrine, central nervous systems, and others. Endothelin has emerged as a modulator by autocrine and paracrine actions for many cellular activities, including vasoconstriction, cell proliferation, hormone production, neurotransmitter and/or neuromodulator. The endothelin family consists of three closely related peptides, ET-1, ET-2, and ET-3 derived from separate genes, such as chromosome 6, 1, and 20, respectively. ET-1 is the predominant isoform produced in the cardiovascular system and about which most is known. Endothelin receptors are seven-transmembrane GTP-binding protein-coupled receptors, which are classified into endothelin-A (ETA) and endothelin-B (ETB) receptors. Interestingly, recent evidence is accumulating to suggest that ET -1 may contribute to a variety of pain states such as allodynia and hyperalgesia in animals and humans. Therefore, in this review the biological characteristics and contraction-related mechanism of endothelin-1 in mammalian cells will be summarized. Especially, we focus on multifunctional roles for ET-1 in noxious stimulation-induced pain for the study of pain specialized physical therapy.
