Objective: This case report is to report the effect of Korean traditional treatment for idiopathic ninth, tenth, and twelfth cranial nerve palsy with ipsilateral headache. Methods: The medical history and imaging and laboratory test of a 39-year-old man with cranial palsy were tested to identify the cause of disease. A 0.2-mL dosage of Hwangyeonhaedoktang pharmacopuncture was administered at CV23 and CV17, respectively. Acupuncture was applied at P06, Li05, TE05, and G37 on the right side of the body. Zhuapiandutongbang (左偏頭痛方) was administered at 30 minutes to 1 hour after mealtime three times a day. The symptoms were investigated using Visual Analogue Scale (VAS). Results: The results of magnetic resonance imaging (MRI), computed tomography (CT), and laboratory tests were normal. The medical history showed no trauma, other illnesses, family history of diseases, medications, smoking, drinking and so on. All symptoms disappeared at the 10th day of treatment. Conclusion: Korean traditional treatment such as acupuncture, pharmcopuncture, and herbal medicine for the treatment of ninth, tenth, and twelfth cranial nerve palsy of unknown origin is suggested to be effective even though this conclusion is based on a single.
Purpose: The purposes of this study were 1) to compare the contribution of demographic-behavioral variables and psychological variables in explaining the variance of depression, 2) identify the most important predictors of depression for Korean female adolescents. Method: The participants were 840 female adolescents. Data was collected through self-report questionnaires, which were constructed to include demographic-behavioral factors, self-esteem, hostility, hopelessness, and depression. Data was analyzed using the SPSS program. Result: Female adolescents' demographic-behavioral variables explained 17% of the variance in depression, and perceived physical health status, history of physical abuse, smoking, satisfaction of body weight, parental alcohol abuse, parental divorce, and history of suicidal attempt were the significant predictors of depression for female adolescents. Psychological variables explained 50% of the variance in depression, and self-esteem, hostility, and hopelessness were the significant predictors of depression for female adolescents. The significant predictors of depression among female adolescents' demographic-behavioral variables and psychological variables were self-esteem, hostility, hopelessness, perceived physical health status, parental alcohol problem, and history of physical abuse, explaining 52% of the variance in depression. Conclusion: In order to reduce depression in female adolescents, it is necessary to design an intervention program that emphasizes improving self-esteem while reducing hostility and hopelessness.
Objectives : This study was designed to investigate the differences of effects in smoking cessation after acupuncture treatment and we hypothesized that the discrepancies might be caused by individual genetic differences. Methods : Acupuncture treatment was given to the subjects three times a week for the 231 healthy male Korean smokers without personal or familial history of psychiatric or neurological illness. We evaluated for differentiate responder and non-responder who showed more than 50% decrease in the cigarette consumption or the desire for smoking were regarded as responder, and less than 25% decrease in the cigarette consumption or the desire for smoking were regarded as non-responder, respectively. Allele and genotype frequencies of the Taq1 A polymorphism of dopamine D2 receptor (DRD2) gene were compared in 231male smokers. Chai-square analyses were performed to test for an interactive effect between the DRD2 Taq1 A allele. Results : The allele frequencies and genotype distributions of DRD2 gene among the smokers (n = 231) showed significant the differences in their genotype distributions. The responder and non-responder showed the difference in genotype distribution with a prevalence of A1 allele. A slightly positive association of DRD2 Taq1 A1 genotypes with smoking was observed. Conclusions : This experiment results indicate that the present of DRD2 allele genotype showing significant difference in the genotype distributions between responders and non-responders could be explained by the difference in the genetic effect of DRD2 A1 allele.
Background : While the incidence of lung cancer is gradually leveling off in developed countries, it is continuing to rise in Korea. With the rapid increase in the prevalence of smoking among women and the young, the incidence of lung cancer is also expected to increase within the next three and four decades. The aims of the present study are to analyze the smoking habits in patients with lung cancer and to evaluate of the relative risk of lung cancer according to patients' smoking habits. Method : This investigation was a hospital-based, case control study, which included data of 93 case subjects with lung cancer and 1132 controls with disease unrelated to smoking, obtained through a smoking history questionnaire by a direct personal interview. Result : Compared with non-smokers, those who smoked for more than 50 years had an odds ratio for lung cancer of 8.8(1.8-20.7). The odds ratios was 8.5(3.5-20.7) for those who smoke more than a total of 40 cigarettes per day and 5.5(2.3-13.3) for men who started habitual smoking under the age of 20. The risk increased for men with more than of cigarette smoking(OR : 5.5, 95% CIa : 2.6-11.9). Odds ratios associated with cigarette smoking were 2.5(1.1-5.8) and 5.1(2.6-10.4) for ex-smokers and current smokers, respectively and 2.2(0.0-4.6) for non-filter smokers of more than 16 years. Conclusion : There was a clear dose-response relationship between the risk of lung cancer and smoking. It can be concluded that dose is an important risk factor for lung cancer, as well as smoking habits.

Here, we present a case of lung cancer in a 48-year-old male horse trainer. To the best of our knowledge, this is the first such case report to include an exposure assessment of respirable crystalline silica (RCS) as a quartz. The trainer had no family history of lung cancer. Although he had a 15 pack/year cigarette-smoking history, he had stopped smoking 12 years prior to his diagnosis. For the past 23 years, he had performed longeing, and trained 7-12 horses per day on longeing arena surfaces covered by recycled sands, the same surfaces used in race tracks. We investigated his workplace RCS exposure, and found it to be the likely cause of his lung cancer. The 8-hour time weight average range of RCS was 0.020 to $0.086mg/m^3$ in the longeing arena. Horse trainers are exposed to RCS from the sand in longeing arenas, and the exposure level is high enough to have epidemiological ramifications for the occupational risk of lung cancer.
Background: Tumor recurrence is the most common cause of treatment failure, even after complete resection of early-stage non-small cell lung cancer (NSCLC). In this study, we investigated the prognosis of patients with early recurrence in order to identify independent risk factors related to early recurrence. Methods: Between February 1995 and December 2012, 242 patients who underwent surgical resection for stage I NSCLC at Dong-A University Hospital were reviewed. The factors predicting overall survival (OS) and early recurrence were investigated. We also investigated the relationship between the patterns and period of recurrence and clinicopathological factors. Results: For patients with stage IA and IB NSCLC, the 5-year OS rate was 75.7% and 57.3% (p=0.006), respectively. A multivariate Cox proportional hazards model demonstrated that gender (p=0.004), comorbidity number (p=0.038), resection type (p=0.002), and tumor size (p=0.022) were the statistically significant predictors of OS. Moreover, the multivariate analysis revealed that smoking history (p=0.023) and histologic grade (p=0.012) were the independent predictors of early recurrence. Additionally, only histologic grade (poor differentiation) was found to be significantly associated with a higher frequency of distant metastasis; there was no relationship between the patterns and period of recurrence and clinicopathological factors. Conclusion: The present study demonstrated that smoking history and histologic grade were independent prognostic factors for early recurrence within two years in patients with early-stage NSCLC. Patients with these predictive factors may be good candidates for adjuvant therapy.
Purpose: The aim of this study was to investigate the association between job stress and metabolic syndrome among male workers. Methods: A total of 396 male workers employed in an electronic company, located in 'P' city, Gyeonggi Province, who had taken medical examination at the 'D' hospital, volunteered in this study. All the data used in this study were obtained from 'D' hospital after getting consent and permission from the workers. General and occupational characteristics, medical history, and health-related behaviors of the subjects were obtained by self-administered questionnaire. Results: The highest occupational stress by sub-sector was reported in the order of insufficient job control, organizational system, lack of reward, job demand, job insecurity, physical environment, job culture, and relationship conflict. The prevalence rate of metabolic syndrome of the participants was 19.9%. The results of the multiple logistic regression analysis on metabolic syndrome of the subjects indicated that the syndrome was significantly higher among those with family history, smoking habit and ex-smoking, working duration of 3 years and over, and with higher total score of occupational stress. It was significantly more prevalent, when the degree of stress was high in the sub-sectors of occupational stress; job autonomy, job insecurity, lack of reward, and job culture. Conclusion: In conclusion, it is necessary to implement a plan to efficiently manage the job stress of these male workers, as the probability of metabolic syndrome increased with the increase of occupational stress. Furthermore, considering the highest occupational stress was found to be job autonomy among its sub-sectors, it is necessary to prepare various measures to enhance the autonomy of such employees.
Objectives : High-normal blood pressure' is a factor influencing decision to initiate targeted intensive intervention strategy in westernized populations. JNC-VI offered the vigorous lifestyle modification for persons with 'high-normal blood pressure', who could be early detected. As a hypertension seems to be the result of multiple genetic factors operating in concert with associated environmental factors, it will be necessary to identify the high-normal blood pressure as a risk factor of hypertension for applying primary prevention strategy in Korean people. Methods : Although cohort study design might be adequate to recruit incidence cases, to keep time sequence of events, and to prevent information bias, nested case-control study was chosen for avoiding measurement errors because hypertension is a benign disease. Source population was the 'Seoul Cohort' participants and follow-up was done by using Korea Medical Insurance Corporation's database on the utilization of health services from 1 Jan93 to 30Jun97. Incidence cases were ascertained through the chart review, telephone contacts, and direct blood pressure measurements. Controls included the pairing of 4 individuals to each case on the basis of age. Results : As 75% of 247 incident cases had high-normal blood pressure, the crude odds ratio for hypertension was 2.04 (95% CI 1.47-2.83). Another statistically significant risk factors of hypertension were body mass index, dietary fiber, alcohol consumption, weekly activity and history of quitting smoking. The multivariate odds ratio of high-normal blood pressure adjusted for all risk factors was 1.84 (95% CI 1.31-2.56). Among high-normal blood pressure group, body mass index, weekly ethanol amounts, weekly physical activity, and dietary fiber except history of quitting smoking were still risk factors of hypertension. Conclusion : 'High-normal blood pressure' is a risk factor for hypertension in Korean middle-aged men, which represents that the vigorous lifestyle modification for persons with 'high-normal blood pressure' is need.

Objectives: The purpose of this study was to clarify the association between serum carcinoembryonic antigen (CEA) and type of work in the shipbuilding industry. Methods: 1,072 final study subjects were admitted to a general hospital from April through July 2010 for the purpose of medical examination. Data on general characteristics such as age, smoking history, alcohol history and exercise habits was gathered through structured self-administered questionnaires. Information on job factors was collected from a medical examination, by interview and through company personnel data. Serum CEA levels were measured after eight hours' fasting and were analyzed by a radioimmunoassay. Results: On univariate analysis, the mean serum CEA level was significantly higher among married (p=0.02), older age (p<0.01), longer work time (p<0.01), smokers (p<0.01), lower education (p<0.01), and indirect and direct exposure groups (p<0.01). On multiple regression analysis, serum CEA level was influenced by smoking (p=0.001), duration of work (p=0.019), and direct exposure group (p<0.001). However, among the direct exposure group, serum CEA level was not significantly different between welding, mounting, electro-device constructive work, grinding and cleaning, and painting. Conclusions: The goal of this research was to determine if there were differences between serum CEA levels according to occupational role among shipyard workers. The direct exposure group of shipyard workers had a relatively higher level of serum CEA than did the indirect exposure group and office workers, most likely due to occupational exposure.
Background: The aim of this study was to assess clinical factors associated with Helicobacter pylori positivity and to evaluate the incidence of gastric carcinoma in first-degree family members of infected patients. A total of 580 patients (mean age:$38{\pm}17$) with gastrointestinal complaints underwent C-14 urea breath test (UBT). Patients were grouped as: Group-1, untreated patients (n:384); and Group-2, patients who previously treated with eradication triple therapy (n:196). C-14 UBT was performed 1-2 months after the completion of eradication therapy. Associations of H pylori positivity with age, gender, ABO and Rhesus groups, smoking, dietary habits, and history of gastric cancer in first-degree family members were evaluated. The frequency of H pylori positivity was significantly higher in group-1 (58%) compared to group-2 (20%), p=0.001. There were no correlations between H pylori positivity and age, gender, ABO groups, Rhesus subgroups, smoking and dietary habits in both patient groups. The frequency of gastric cancer in family members was significantly higher in patients with H pylori infection among group-1, compared to infected patients among group-2 (56% vs. 28.6% respectively, p=0.03). We observed a significant association between H pylori positivity and the presence of gastric cancer in first-degree relatives of group-1 patients. Our results provide some confirmation of the presence of a link between gastric cancer development and H pylori. C-14 UBT is a sensitive, reliable and a widely recommended test for the detection of H pylori infection and recurrence. We suggest that detection and eradication of H pylori may contribute to a reduced risk of gastric cancer in the family members of infected patients.
