Association between Cigarette Smoking and RASSF1A Gene Promoter Hypermethylation in Lung Cancer Patients: a Meta-analysis

  • Objectives: Epidemiological studies have shown that molecular mechanisms underlying the development of lung cancers differ between smokers and unsmokers. Aberrant promoter methylation in some tumor suppressor genes is frequent in lung tumors from smokers but rare in those from non-smokers. Recently, many studies have investigated the association between cigarette smoking and RASSF1A gene promoter hypermethylation in lung cancer patients, but a unanimous conclusion could not be reached. We therefore performed this meta-analysis to derive a more precise estimation of any association. Study Design: An electronic search of PubMed and Chinese Biomedicine databases was conducted to select studies. A total of 19 case-control studies were chosen, and odds ratios (ORs) with confidence intervals (CIs) were used to assess the strength of associations. Results: The case-control studies covered 2, 287 lung cancer patients: 63.4%(1449) of the patients were smokers, 36.6% (838) were unsmokers. The overall results suggested that smokers with lung cancer had a 1.297-fold (95% CI: 1.066~1.580, p=0.010, p=0.087) higher risk for RASSF1A gene hypermethylation than the non-smokers. In the stratified analysis, an increased risk of RASSF1A gene hypermethylation in smokers than in non-smokers was found in Asian (OR=1.481, 95%CI: 1.179~1.861, p=0.001, p=0.186). Conclusions: This meta-analysis supports the idea that RASSF1A gene hypermethylation is associated with cigarette smoking-induced lung cancer.

우리나라 성인 흡연자의 전자담배 이용경험과 금연행태의 관련성 (The Relationship between E-Cigarette Experience and Smoking Cessation Behavior among Current Smokers)

  • Background: Electronic cigarette (e-cigarette) has rapidly increased. Even though e-cigarettes are actually not smoking cessation alternative supplies, some people perceive them as smoking cessation alternative supplies. The purpose of this study is to identify the relationship between e-cigarette experiences and smoking cessation behaviors among Korean adults. Methods: Cross-sectional data was collected from 7,550 participants of Korea National Health and Nutrition Examination Survey in 2014. A total of 867 participants who were current smokers were enrolled in this study. Multi-variable logistic regression analysis were performed to identify the relationship between the smoking cessation behavior and the e-cigarette experience. Results: The e-cigarette experience rate of current smokers was 22.1%. The e-cigarette experience rate was higher in groups with the higher income, the higher education level, and the white collar jobs. Smokers with experience of smoking cessation (odds ratio [OR], 3.03; 95% confidence interval [CI], 1.67 to 5.76) and with intention to smoking cessation in the future (OR, 1.59; 95% CI, 1.05 to 2.43) were more likely to experience the e-cigarette. Especially, smokers who used smoking cessation alternative supplies were more likely to experience the e-cigarette than smokers who did not use it (OR, 2.87; 95% CI, 1.59 to 5.18). Conclusion: We confirmed that e-cigarette experiences were associated with smoking cessation experience, smoking cessation plan, and use of smoking cessation aid among smokers in Korea. Additional studies should be continued to confirm whether e-cigarette are continuously influencing the smoking cessation of current smokers in South Korea as one of the smoking cessation alternative supplies.

구강관리용품의 사용에 따른 흡연자의 구강 환경 변화 (Analysis of Changes in Oral Care Index of Smokers Using Oral Care Products)

  • 본 연구는 흡연자의 구강 내 환경을 관찰하여 흡연자의 구강관리용품 사용에 대한 중요성과 구강 환경 개선을 위해 지속적이고 체계적인 구강관리용품 사용의 필요성을 확인하기 위해 강원도 지역의 대학생들을 대상으로 흡연자군 12명, 비흡연자군 12명에게 설문조사 및 실험조사를 실시하였다. 조사결과 두 집단 모두 구강관리용품 사용율은 75%로 나타났으며, 그 중 치실 70.7%으로 구강관리용품 중 치실을 가장 많이 사용하는 것으로 나타났다. 비흡연자군보다 흡연자군이 구강관리용품 사용 후 PHP Index, Streptococcus.mutans 수, snyder 활성도가 감소한 것으로 나타났고, 비흡연자군보다 흡연자군이 구강관리용품 사용 후 pH가 증가한 것으로 나타났다. 본 연구조사 결과 구강관리용품의 사용이 전반적으로 흡연자의 구강건강을 증진시키고 구강 환경을 개선할 수 있다고 나타났다. 구강관리용품에 대한 교육을 실시하면 흡연자들의 동기부여에 도움이 될 것으로 사료된다.

한국인에서 흡연과 우울증상의 연관성 (The relationship between smoking and depressive symptoms among Korean adults)

  • Objectives: This study investigated the relationship between smoking status and depressive symptoms among Korean adults. Methods: We used the data of 17,871 adults aged 19 years or older from the fifth Korea National Health and Nutrition Examination Survey(2010-2012). The subjects were classified as nonsmokers, former smokers, and current smokers by gender. Logistic regression analysis was used to calculate the odds ratios(ORs) and the 95% confidence intervals(CIs) for the effects of smoking status on depressive symptoms, and depression diagnosed by a doctor. Results: Compared to nonsmokers, the ORs of depressive symptoms for current smokers were 1.11(95% CI, 1.11-1.12) among males, and 1.64(95% CI, 1.63-1.64) among females. Compared to former smokers, the ORs of depressive symptoms for current smokers were 1.05(95% CI, 1.05-1.05) among males, and 1.89(95% CI 1.88-1.90) among females. Compared to nonsmokers, the ORs of depression for current smokers were 0.94(95% CI, 0.94-0.95) among males, and 1.40(95% CI, 1.39-1.41) among females. Compared to former smokers, the ORs of depression for current smokers were 1.09(95% CI, 1.09-1.10) among males, and 0.99(95% CI, 0.99-1.00) among females. Conclusions: Smoking is associated with depressive symptoms among Korean adults. Therefore, it is necessary to consider depressive symptoms with the management of tobacco control policies.

고등학교 남학생들의 흡연 정도가 식습관, 음주습관 및 생활만족도에 미치는 영향 (A Study on Relationship between Smoking Habits, Eating Habits and Lifes Satisfaction of the High School Male Students)

  • The purpose of this study was to examine the relationship between the smoking and eating habits of high school students. We also presented the basic data for the effective smoking cessation and smoking prevention programs, and proper nutrition education programs. We surveyed 304 high school male students in Sokcho city. The results were as follows ; a total of 23.7% of the subjects were smokers, the smokers spent more pocket money than the non-smokers and they also spent more time on the internet or smartphone. The smokers had lower awareness of the dangers of smoking than that of non-smokers. They started drinking alcohol earlier than the non-smokers. Their water intake was higher and they preferred consumption of high-protein foods like fried chicken, but were not inclined to vegetables and sour tasting foods. These results imply that smoking habits of the subjects affected their eating and drinking habits. A matter of concern was the low intake of vegetables and sour tasting foods, which could lead to a deficiency of nutrients such as vitamins, minerals and dietary fibers. The smokers were less satisfied with their school life than the non-smokers. There was a negative correlation with the degree of awareness of the dangers that smoking can cause. Conversely, the amount of smoking and drinking habits were positively correlated. To reach a healthy adulthood, it is crucial to quit smoking and participate in smoking prevention education along with nutrition education and abstinence education for the adolescents.

흡연이 남자 청소년들의 일부 항산화 관련 효소체계에 미치는 영향 (The Effects of Smoking on Antioxidative Enzyme Activities in Male Adolescents)

  • Smoking can increase oxidative stress and thereby change the antioxidant defense system in the body. To investigate the relationship between male adolescent smoking and antioxidant status, we surveyed the eating habits and dietary intake of 82 smokers and 44 nonsmokers recruited from a male technical high school. In addition, antioxidant enzyme activity and lipid peroxide values were determined in both the plasma and the erythrocytes. Although the frequency of food intake was not significantly different, most nutrient intake was unexpectedly higher in smokers than in nonsmokers. In comparison with the Korean RDA, especially the average intake of Ca, Fe and vitamin $B_2$ didn t reach 75% of the Korean RDA in either smokers or nonsmokers. An analysis of antioxidant enzyme activity showed that plasma catalase. superoxide dismutase (SOD), glutathione peroxidase (GSH-px), erythrocyte catalase and GSH-px activities showed no significant difference between smokers and nonsmokers. However, the erythrocyte SOD activity of smokers (1.57 unit/mgHb) was significantly lower than that of nonsmokers (2.00 unit/mg Hb). In addition, the plasma ceruloplasmin concentration of smokers (28.68 mg/$d\ell$) was significantly higher than that of nonsmokers (26.30 mg/$d\ell$), whereas the specific ceruloplasmin ferroxidase activity of smokers (0.31 unit/mg) was lower than that of nonsmokers (0.35 unit/mg). The plasma and erythrocyte thlobarbituric acid reactive substance (TBARS) of smokers (2.57 $\mu$mol/L, 0.32 $\mu$mol/gHb) were also significantly higher than those of nonsmokers (2.25 $\mu$mol/L, 0.27 $\mu$mol/gHb). The overall data indicate that adolescent smoking might decrease the antioxidant capacity of the body, in part, by lowering the erythrocyte SOD activity and the specific ceruloplasmin ferroxidase activity.

  • Purpose : The purpose of present study was to investigate pulmonary function among smokers and non-smokers, and effect on respiratory muscle training in smoker. Methods : Twenty participants were allocated into smokers group(n = 10) and non-smokers group(n = 10). Pulmonary function was measured by spirometry(Pony FX, COSMED Inc., Italy). The smoker group was compared pulmonary function before and after respiratory muscle training. Results : The results were as follows; There was significant difference on PEF, FEV1/FVC%, FEF25~75%, MEF75% and MEF50% among smoker and non-smokers(p<.05). But, there was not significantly difference after intervention in smokers. Conclusion : The present study found that smoker decreased pulmonary function than non-smokers.

남자 고등학생 흡연자의 영양상태 판정 및 흡연관련 요인분석 -II. 흡연과 관련된 사회심리적 요인에 대한 연구- (Assessment of Nutritional Status and Factors Related to Smoking in Adolescent Males -II. Psychosocial Factors Influencing Smoking among Male High School Students-)

  • The study purpose was to investigate psychosocial factors related to smoking among adolescent boys. The Theory of Planned Behavior provide the basis for the study. Twenty-five attitudinal beliefs, 9 normative beliefs and 20control beliefs were identified through questionnaire development. The data were analyzed using t-test and χ2-test. Thirty-three percent of 300 students were smokers. Most of the beliefs examined were significantly different between smokers(n=92) and nonsmokers(n=92). With respect to attitudinal beliefs, smokers responded less negatively on the items of bad health effects of smoking such as sore throat, headache, chest pain, risk of cancer and bad blood circulation(p<0.001), and decreased physical strength(p<0.05). Smokers believed less negatively on the items that smoking leads to bothering others, bad breath, yellow teeth and making them spend money(p<0.001). In contrast, smokers felt more positively on smoking as a means of stress management, relaxing, helping digestion(P<0.001). Smokers felt less pressure for not smoking from significant others. Especially mother, siblings, friends, girl friends, seniors of school were important sources of influence regarding subject's smoking. Smokers felt less confident in controlling the urge to smoke in several situations including; when they were with friends or asked to smoke by friends; after the meal, or drinking; when they were bored or stressed, upset, and when they felt unstable(p<0.001). Smokers also scored lower on specific skills to quit or control the urge to smoke as well as overall perceived control, compared to nonsmokers(p<0.001). These results suggest that interventions for adolescents incorporate diverse strategies to increase the perceived control over smoking in specific situations as well as overall perceived control, to help them realize and modify attitudinal beliefs, and to elicit support from significant others for not smoking. Uninary Nicotine and Cotinine Levels in Smokers and Nonsmokers Related to Smoking Habit in Korea

  • This study was conducted to determine the urinary nicotine and cotinine concentration in 126 smokers and 143 nonsmokers. While urine samples were being collected, personal characteristics related to smoking habit such as sex, age, number of years since a person has been a smoker, average number of cigarettes consumed per day, and number of smokers in the family were surveyed. Urinary nicotine and cotinine concentration were analyzed by GC/NPD. The smokers smoked an average 17.0 cigarettes per day and the average concentration of nicotine and cotinine was 3.88 $\mu\textrm{g}$/ml and 3.64 $\mu\textrm{g}$/ml, respectively. The average number of smokers in the family was 0.72 persons and the average concentration of nicotine and cotinine were 0.11 $\mu\textrm{g}$/ml and 0.02 $\mu\textrm{g}$/ml in the urine of non-smokers, respectively. The concentration of nicotine and cotnine in smoker\`s urine was dependent on the number of cigarettes smoked per day(p<0.01). The number of years since a person had been a smoker, and the number of smokers in the family were not associated with the concentration of nicotine and cotinine. Also there was no significant effects of passive smoking on the family of smokers by the level of nicotine and cotinine concentration. We describe the relationship between smoking habit as assessed by urinary nicotine and cotinine excretion. This study provides an evidence for the assessment of active and passive exposure to tobacco smoke.

여고생 흡연자의 영양소 섭취 실태 및 흡연관련 사회심리적 요인에 관한 연구 (A Study of Nutrient Intakes and Psychosocial Factors Associated with Smoking among Female High School Students)

  • This study was done to compare nutrient intakes, anthropometric indices, and psychosocial factors related to smoking by smoking status among adolescent girls in Seoul. Subjects were high school students, and smokers were 17.6%. Nutrient intakes were analyzed by convenient method, and anthropometric measurements were done by Inbody 2.0. Psychosocial factors of smoking were examined based on the Theory of Planned Behavior : 26 attitudinal, 9 normative, and 17 control beliefs were used. Smokers smoked 5-7 cigarettes a day on average, with 1.26 pack-years of smoking, There were no significant differences in anthropometric characteristics, except fat distribution, between smoking(n=92) and nonsmoking(n=92) and nonsmoking(n=94) group. The percentage of alcohol groups. Especially, caloric intake (63.6% of RDA) were much below the recommended level. With respect to psychosocial factors, 18 out of 26 attitudinal beliefs were significantly different between smokers and nonsmokers. Smokers responded less negatively on the items of bad health effects of relax(p<0.001). In addition, smokers were more convinced of advantages of smoking (test, etc.)but responded less negatively on the disadvantages of smoking(bothering others, yellow teeth, bad breath, etc.). Smokers expressed less pressure for not smoking from siblings, friends, school seniors than nonsmokers. In addition, smokers, expressed less confidence in controlling the urge to smoker or quitting smoking in several situations, such as 'after a meal', 'feel bored', 'with friends', 'when angry' offered a cigarette by friends', 'drinking coffee or tea', 'drinking alcohol', 'seeing others smoke'(p<0.001). Smokers also felt less developing in applying specific skills for controlling the urge to smoker or quitting smoking. These findings suggest the need for developing smoking cessation programs for adolescent females, focusing on specific beliefs identified in this study.
